Types of Facilities, Care Options A number of broad categories of care options and facilities are included in this guide. Terminology may vary from facility to facility. Residential Living is for individuals who are responsible for their own basic needs. Nursing services are not available on site nor are residential communities affiliated with nursing care providers. No meal service or activity programs are offered. Active Adult Communities are residential-type communities for mature adults only. Individuals are responsible for their own basic needs. Nursing services are generally not available on site. Meal service is not provided. Outdoor maintenance service may be provided. Continuing Care Retirement Communities/Life Plan Communities To portray the “vibrant, forward-looking nature of the contemporary senior living community,” some continuing care retirement communities have chosen to describe themselves as life plan communities. A life plan community follows the definition of a CCRC set by The National Investment Center: Age-restricted properties that include a combination of independent living, assisted living, and skilled nursing services (or independent living and skilled nursing) available to residents all on one campus. Resident payment plans vary and

include entrance fee, condo/co-op, and rental programs. Retirement Communities are planned for those who are able to care for their own basic needs (or almost so) and choose to live in a community with other seniors. Organized social programs, meal service, transportation, recreational activities, and access to health and shopping are typical amenities. Nursing care is not usually provided. Some do offer assisted living/personal care designed for individuals who function mainly on their own but may need help with medications, bathing, or dressing. Services offered are similar to those provided by assisted living/personal care facilities. Retirement communities can be freestanding or affiliated with a complex offering nursing care services. Personal Care Homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services of a licensed long-term care facility but do require assistance or supervision in activities of daily living (ADL). Some ADL provided are transferring in and out of a bed or chair, toileting, bladder and bowel management, personal hygiene, securing and managing healthcare, selfadministering medication, and proper turning and positioning in a bed or chair. 50plus Living 2024


Assisted Living Residences provide an environment that combines housing and supportive services to allow residents to “age in place” without having to move to a licensed long-term care facility when their care needs increase. ALRs have living units with kitchen capacity, private bathrooms, and only one person per unit. An ALR may provide services that would be provided in a longterm care facility (personal care homes may not). As of January 2011, changed regulations regarding licensure for assisted living communities became effective. Nursing/Rehab Facilities provide nursing care and related medical or other personal health services 24 hours a day, seven days a week, to individuals who require full-time care or supervision but do not need more intensive, hospital-based care. Nursing/rehab facilities may be independent or part of another community. Individuals may be temporarily placed for rehabilitation following an illness, injury, or surgery or reside long term due to illness or aging. Nursing facilities offer skilled medical care under the supervision of licensed nurses, and at least one registered nurse must be on duty during the day. Custodial care is also offered and includes services that assist in the activities of daily living, which include bathing, dressing, using the bathroom, or transferring from a bed to a chair. Nursing centers are licensed by the State Department of Health. Acute Care Facilities are licensed as hospitals. They provide the highest level of care under a physician. Some services provided at an acute care facility include emergency services, critical care, medical/ surgical services, and maternal/childbirth services. Dementia/Alzheimer’s services offer assistance and support to those living with memory issues. These units/communities www.onlinepub.com

enable the person living with memory issues to be as independent as possible while enhancing freedom of movement in a safe, secured environment. Adult Day Centers offer programs in facilities or function as independent organizations. The services may include but are not limited to personal care; assistance with eating, using the toilet, and taking medication; and social assistance. Centers offer a protective, supervised setting and generally operate during normal business hours. Respite Care provides caregiving opportunities on a short-term basis. Care may range from personal to nursing care. Home Care Services may be provided in a residential setting or as ancillary services, such as in personal care, assisted living, or nursing care environments. Services include skilled nursing services, therapies, and changing and reinforcing simple wound dressing, as well as nonmedical services, such as light housekeeping, transportation to doctor visits, shopping, respite, and more. Hospice Care is for families living and coping with life-limiting illnesses and provides professional treatment of pain and symptom management with support and counseling for medical, psychological, and spiritual needs. Palliative Care also focuses on providing relief from the symptoms and stress of serious illness but, unlike hospice, can be provided along with curative treatment. Living Independence for the Elderly (LIFE) is a managed care program that provides a comprehensive, all-inclusive package of medical and supportive services. The program is known nationally as the Program of All-Inclusive Care for the Elderly (PACE). All PACE providers in Pennsylvania have “LIFE” in their name.

Susquehanna Valley Profile Weather Average high temperatures are the 80s in the summer months and the 30s in the winter. The warmest month is July, with an average maximum temperature of 85 degrees. The coldest month is January, with an average minimum temperature of 20-23 degrees. Average annual precipitation is 41-47 inches.

Taxes Sales tax is 6%. Sales tax exemptions include groceries, health-related items, medical services, prescription drugs, and groceries. State income tax is 3.07%; retirement income is tax-exempt. Property tax is determined by the local government according to school districts and municipalities.

State Lottery Benefits The Pennsylvania Lottery contributes to programs such as Property Tax/Rent Rebate; the Mass Transit Program; PACE, PACENET, and PACE Plus Medicare; long-term care services administered by the Department of Public Welfare; and the 52 Area Agencies on Aging.

PACE To be eligible for PACE, you must be 65 years of age or older and a Pennsylvania resident at least 90 days before the date of application. You cannot be enrolled in the Department of Public Welfare’s Medicaid prescription benefit. PACE and PACENET eligibility is determined by your previous calendar year’s income. Effective in 2014, Social Security Medicare Part B premiums are

excluded from income. For a single person, your total income must be $14,500 or less. For a married couple, your combined total income must be $17,700 or less. Once you are enrolled in the PACE program, a benefit card will be sent to you, and you will pay no more than $6 for each generic prescription medication and no more than $9 for each brand-name prescription at your pharmacy. These copayments are based on a 30-day supply. Effective Jan. 1, 2022, the PACE Program encourages cardholders to enroll in a Part D plan because it may provide additional benefits for members. Effective August 2021, PACE reinstated prescription limits when PACE is the primary payer. PACE legislation limits prescriptions to a 30-day supply or 100 units, whichever is less. Enrollees must use 75% of their supply before refills will be reimbursed.

PACENET To be eligible for PACENET, you must be 65 years of age or older, a Pennsylvania resident at least 90 days before the date of application, and you must not be enrolled in the Medicaid prescription benefit program. PACENET income limits increased by $6,000 in 2022. Residents who were not eligible in the past may now be eligible with the new income limits in place. A single person’s total income must be between $14,500 and $33,500. A couple’s combined total income must be between $17,700 and $41,500. PACENET cardholders who do not enroll in a Part D plan will pay a nominal deductible each month at the pharmacy, which will be calculated through the cost of their medications. In addition, the individual will pay no more than $8 for each generic prescription medication and no more than $15 for each brand name. PACENET cardholders enrolled in one of the program’s partner Part D plans will pay the Part D premium at the pharmacy each month, which will be calculated through the cost of the medications. PACENET cardholders enrolled in a Part D plan that is not one of the program’s partner plans will pay the Part D premium directly to the Part D plan. In addition, they will pay no more than the PACENET co-payments of $8 for each generic prescription medication and $15 for each brand name. Effective Jan. 1, 2022, PACENET cardholders not enrolled in a Part D plan pay a $40.74 premium at the pharmacy each month they use their PACENET benefit. PACE Plus Medicare Under PACE Plus Medicare, PACE/ PACENET coverage is supplemented by federal Medicare Part D prescription coverage and offers older Pennsylvanians the best benefits of both programs. Older adults continue to receive the same prescription benefits while, in many cases, saving more money.

Transportation Pennsylvania Free Transit Program – Provides free transit service on bus routes, trolley, and rapid-transit lines. Individuals 65 and older may ride free on fixed-route www.onlinepub.com

services at all regular operating times on weekdays, weekends, and holidays. You will need a senior citizen transit identification card to ride free. The identification card is available free of charge from participating local transit operators. Shared-Ride Program – Provides reduced-fare transportation services to persons 65 and older utilizing shared-ride transportation services. Reduced-fare transportation is provided to medical appointments, senior community centers, and adult daycare centers. Mature Driver Safety Program – There is a mandated auto insurance discount for older drivers who complete a mature driver improvement course. Courses are offered through AARP, AAA, and the National Safety Council. It is intended for drivers 55 and older. Low-Vision License – Persons who do not meet the vision standards for a driver’s license may qualify for a restricted (lowvision) license. A restricted license allows the driver to drive during daylight hours on roadways other than freeways. A person with visual acuity of less than 20/70 combined vision, but at least 20/100 combined vision with best correction and 120 degrees’ field vision in the horizontal meridian, may qualify with a recommendation from an optometrist or physician. Retired Status Registration – Customers who qualify for retired status registration pay a $10 per year processing fee instead of the full $38 registration renewal fee. Total income must not exceed $19,200 and vehicles must weigh less than 9,000 lbs.

How to Choose an Assisted Living/Personal Care Facility

By Jim Miller Dear Savvy Senior, What’s the best way to go about choosing an assisted living facility for my 86year-old father? Since Mom died last year, his health has declined to the point that he can’t live at home anymore but isn’t ready for a nursing home either. – Searching Susan Dear Susan, If your dad needs help with things like bathing, dressing, preparing meals, managing his medications, or just getting around, assisted living facilities and personal care homes are definitely good options to consider. 10

In Pennsylvania, personal care homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services of a licensed long-term care facility but do require assistance or supervision in activities of daily living. Assisted living residences provide an environment that combines housing and supportive services to allow residents to “age in place” without having to move to a licensed long-term care facility when their care needs increase. An ALR may provide services that would be provided in a long-term care facility (personal care homes may not). There are nearly 29,000 assisted living/personal care communities (also called board-and-care, supportive-care, or residential-care facilities) in the U.S. today, some of which are part of a retirement community or nursing home. Most facilities have anywhere between 10 and 100 suites, varying in size from a single room to a full apartment. And some even offer special memory care units for residents with dementia. To help you choose a good assisted living/personal care facility for your dad, here are some steps to follow. Make a list: There are several sources you can turn to for referrals for assisted living/personal care communities in your area, including your dad’s doctor or nearby hospital discharge planner; friends or neighbors who’ve had a loved one in assisted living/personal care; or your county office of aging. Do some research: To research the communities on your list, put a call in to your long-term care ombudsman. This is a government official who investigates long-term care facility complaints and advocates for residents and their families. This person can help you find the latest health inspection reports on specific assisted living/personal care communities and can tell you which ones have had complaints or other problems in the past. To find your local ombudsman, visit ltcombudsman.org. Call the facilities: Once you’ve identified a few good assisted living/personal care communities, call them to see if they have any vacancies, what they charge, and if they provide the types of services your father needs. www.onlinepub.com

Tour your top choices: During your visit, notice the cleanliness and smell of the facility. Is it homey and inviting? Does the staff seem responsive and kind to its residents? Also be sure to taste the food, and talk to the residents and their family members, if available. It’s also a good idea to visit several times at different times of the day and different days of the week to get a broader perspective. On your campus visit, get a copy of the admissions contract and the residence rules that outline the fees (and any extra charges), services and residents’ rights, and explanations for when a resident might be asked to leave because their condition has worsened and they require more care than the facility can provide. Also, find out their staff turnover rate, COVID infection-control procedures, and if and when medical professionals are on site. How to pay: Monthly costs for assisted living range anywhere from $2,500 to $6,000 or more, depending on where you live, the facility you choose, and the services provided. Since Medicare does not cover assisted living, most residents pay out-ofpocket from their own personal funds, and some have long-term care insurance policies. If your dad has limited financial resources and can’t afford this, most states now have Medicaid waiver programs that help pay for assisted living/personal care. Or, if he’s a veteran, he may be able to get funds through the VA’s Aid and Attendance benefit. By Kimberly Blaker When you’re finally able to retire, a new and exciting chapter in life begins. The idea of relocating is an exciting way to embrace your new life. But it’s also a big decision you may want to consider carefully, especially if it means leaving family behind. Living Where You Love or Someplace More Affordable There are many reasons retirees choose to relocate. Most often, they want to live in a place that offers them a better way of life. A significant factor retirees consider is choosing an area where they’d love to live. Maybe you live in a suburban area but really enjoy nature and hiking. Or perhaps you’ve lived and worked in a crowded city for years but would rather spend your time relaxing by the beach. After you retire, you’re better able to prioritize your personal preferences when deciding where to live. Think about what you enjoy and the environment that makes you feel your best. Another important factor to consider is affordability. If you’re thinking about moving after retirement, you may want to consider downsizing. If all your kids are grown and gone, you probably don’t need as much space. Plus, you may have different needs that are better served with a smaller home. 24

Retirement means you likely have less income than you did before. So having a smaller mortgage or rent payments, lower property taxes and insurance, less maintenance, and fewer repairs can save you a bundle. If you have equity in your home or home values in your area have risen since you purchased your home, you might even make a profit from selling it. Do you currently live in an area with a high cost of living? If so, you may be able to find an area you’d enjoy with a much lower cost of living, thereby offering you multiple benefits. The Pros and Cons of Relocating Deciding to move away from family and friends after retirement is a big decision. Creating a list of personal pros and cons is a helpful tool to help you process all the factors. The ones below can help you get started. But don’t forget to add your own. Pros: • Leaving behind obligations, old drama, or bad memories • Getting a fresh start • Finding a more appropriate place for your stage of life • Finding a new community with whom you have more in common • Leaving an area that has a younger population and a family focus • Saving money by downsizing or living in a less expensive area Cons: • Being away from familiar and special places • Having to develop new routines • Not getting to see family and friends regularly • Starting over new takes a lot of effort • Needing to make new friends and find new social outlets • Moving can be difficult and stressful How to Manage Relationships if You Move Away One of the biggest hesitations retirees have about relocating is that it’ll take them away from their kids and grandchildren. You may feel relocating is right for you, yet you’re still worried about living so far away from your loved ones. Moving away from family and friends is easier than ever before because of all the technology now available for keeping your relationships close through virtual connection. Gone are the days of delayed communication through limited means. Through social media, you can see regular updates, pictures, and videos of important events happening in their lives. You can have direct communication at any time using text messaging and phone or video calls, which can give you the feeling you’re right there with your family. In some ways, living away from your family can make seeing each other even better. When you live nearby, you may not put as much effort into the quality of your time together. If you live farther away, the times you spend together will be more focused, special, and memorable. You can travel to each other’s locations or meet for vacations together for a fun change of pace. How to Make the Most of Your New Life If you do decide to relocate, the best thing you can do is go into it prepared. Figure out precisely what you want out of your new home, town, and life to narrow down the places that make the most sense for you. Even if you already have a dream location in mind, know the reasons you want to live there and make sure it actually meets your expectations for retired life. Visit any new places you’re seriously considering relocating and spend time there. Check out the city or town, including the more mundane aspects of it, like places where you’ll run errands. Talk to locals, also, particularly those at a similar stage of life, and get their perspective. Realtors and librarians are both excellent resources for getting more information about what your potential new hometown has to offer. Once you’ve relocated, look for ways to get involved and become a part of your new community. Look for group classes that align with your interests or offer the opportunity to try something new. Both the local library and city recreation department are helpful resources for finding these classes and groups. Retirement is a time of change that can be both wonderful and daunting. Simple Home Safety Solutions for Aging-in-Place

By Jim Miller Dear Savvy Senior, Falls and fires are the two things I worry most about for my elderly father, who lives alone. Do you have any suggestions on what we can do to help keep him safe and keep an eye on him from afar? – Concerned Daughter Dear Concerned, Of course! There are a number of small adjustments and modifications you can make to your dad’s home to help protect him from falls and fires, both of which cause thousands of injuries and deaths each year. Here are some tips to get you started. www.onlinepub.com

Eliminate tripping hazards: Since falls are the leading cause of home injury among seniors, a good place to start is to pack away your dad’s throw rugs, which are common tripping hazards, or use carpet tacks or double-sided tape to secure them. You may also need to adjust your dad’s furniture so there are clear pathways to walk through and position any electrical or phone cords along walls and out of the way. For hardwood steps, consider attaching a nonslip tread to each one to provide traction and help him see the edge. And for added protection in the bathroom, buy some nonskid rugs for the floors and use adhesive nonslip treads or a mat with rubber suction inside his tub or shower stall. Improve his lighting: Good lighting is essential for safe aging-in-place, so check the wattage ratings on your dad’s lamps and light fixtures. Install the brightest bulbs allowed, and add supplementary lighting if necessary. You should also purchase some dusk-to-dawn nightlights for the bathroom and in the hallways that light up when the sun goes down. And mount some motion-sensor lights outside the front and back doors and in the driveway that automatically come on when he comes and goes after dark. Get grab bars: These can significantly reduce his risk of bathroom falls. Install them where he enters the shower or tub and on a wall inside the stall, but don’t use grab bars that attach with suction. Instead, have wall-mounted bars put in by someone who can affix them to the wall studs. It’s also best to choose bars whose surfaces are slightly textured and easier to grip. Ensure railings are stable: Wherever he has steps — stairways, entryways, or basements — he needs sturdy railings. Ideally, they should be on both sides of the steps. Prevent cooking fires: There are several affordable products you can purchase to help your dad prevent home cooking fires, like BurnerAlert discs that attach to a stove’s knob and will continuously blink or beep after the stove has been in use for a preset amount of time. 30

Ome smart knobs can control a stove’s heating settings from an app. Or you can invest in a more expensive iGuardStove sensor that shuts the stove off when it doesn’t detect motion for five minutes. Install smoke alarms: Install a smart smoke alarm in your dad’s house (buy one for each floor) that will alert him when smoke or carbon monoxide is detected. These smart devices will also send alerts to your phone to let you know when a problem is detected. Google Nest and First Alert both make smart smoke and carbon monoxide detectors. Add fire extinguishers: Get portable, multipurpose fire extinguishers for each level of your dad’s house and in the garage. Consider a medical alert: To ensure your dad’s safety and provide you some peace of mind, consider getting him a medical alert device that comes with a wearable SOS button that would allow him to call for help if he were to fall or need assistance.
