The publication design process is complex and multidimensional. It requires professional knowledge, plentiful experience and ability to make decisions in uncertain situations. Although trial and error is an important part of the process, it is both possible and advisable to avoid common mistakes that might cause significant loss of time and money. The following is about 4 types of common mistakes and misconceptions often made by different characters during the product development and design process.

inserting bleeding in every single pages is a must. While exporting as .PDF via InDesign or Illustrator, there is a setting that can let the software put the bleeding automatically.

Photos and Fonts play the most important roles as they are the main spirits of the book.

n u m b e r / 1 0 NO mistakes should be occurred and they must be in high quality in order to let people read in a comfortable way. .eds can make the keyer effect become path, do not use .psd to save the file because it causes error as always due the version factor. It is better to keep the resolution between 304-350 dpi. Otherwise, the images will become blurry and fuzzy.Text should be created outlinebecause your font may not be supported in other computers. All the fonts and image files should be submitted together to the publishing side in case something errors may happen.

When the file is .pdf, it should be made sure the text has been created outline, even though in most cases different fonts can be shown in any computer when the file is exported as PDF. Do not forget to take the ‘high-quality printing’ option while exporting the file as PDF in Illustrator. Setting high resolution for low quality images does not mean those images will be clearer. The size will be larger but they are still blurry.

Apart from visual design, designer is responsible to hand in a complete soft copy file to the publisher. Before the printing process, a good communication between the designer and printing plant should also be established. The color mode of the file should be CMYK, not RGB, as the first one is for printing and the second one is for monitor. Creating outline before submitting the files because the publisher cannot see the prefered font if they do not have the font. Communicating with the publisher about the exporting size and the bleeding size is important before printing the outcome. Publisher can help the calculation of the spine of a booklet. In order to make the printing process to become convenient and no mistake is occurred, the file format should be output in .ai, .pdf, .id or other vector file. Designer should not use the .psd and .jpg file, or the words will become indistinct and difficult to modify. It is better to put all the related files into the same folder so as to prevent the publisher cannot find the linked files.

Electronic error-reporting systems can possibly make the time required to report shorter, shorten the time for correcting unsafe conditions, and alert providers to emerging unsafe patterns. Some systems can also facilitate quality improvement initiatives through enhanced error-reporting systems. The benefits of Web-based health care reporting systems that clinicians find easy to use and see the effects of their reporting in changes to systems might ultimately reduce the incidence of serious errors, and significantly improve the safety and quality of health care afforded patients.

p a g e

To improve safety, error-reporting strategies should include identifying errors, admitting mistakes, correcting unsafe conditions, and reporting systems improvements to stakeholders. The greater the number of actual errors and near misses reported, the more reliable a health care system could be, from a safety viewpoint, when systems improvements are consistent with error patterns. Clinicians appreciate seeing the results of the reports they submitted transformed into systems improvements. Understanding and communicating the root causes of errors and near misses can decrease the risk of future errors, and support the concept that health care errors are often systemic and multifactorial.
