5 Registration This may be a provisional or permanent registration, however, the owner may wish to proceed with permanent registration from the outset providing all applicable requirements are met. 5.1 Commercial Provisional Registration (CDVR-6001)

Provisional Registration is valid for up to 3 months. Compliance with the following is required: • Application for Official Yacht Number and Call Sign • Proof of Ownership • Dominica IBC Application or Foreign Maritime Entity (FME) Application • Evidence of Authority to Sign Registration Applications (usually a Power of Attorney) • Tonnage Certificate • Payment of Registration Fees • Application for Ship Station License (Required for all yachts with radio station on board) • Application for Minimum Safe Manning Certificate • Copy of Previous Declaration of Maritime Labor Compliance (DMLC) Part II • Request for Issuance of DMLC Part I • Load Line Certificate

5.2 Commercial Permanent Registration (CDVR-6001)

Compliance with the following is required: • Deletion Certificate from Previous Flag • Completed Carving and Markings Note • Vessel and Owner Contact Sheet • Copy of Classification Certificates (if applicable) • Proof of Liability Insurance (P&I Cover Note) 5.3 Private Provisional Registration (CDVR-6002)

Provisional Registration is valid for up to 3 months. Compliance with the following is Required: • Application for Official Yacht Number and Call Sign • Application for Ship Station License • Declaration of Private Use CDP 502 YACHT CODE Rev01
