Robert Singer, MD CHAIRMAN OF THE BOARD
Take the very-buzzing, although not totally accurate, term of “plastic surgery reversals”—a hot-button search item that has more than a few misconceptions associated with it. Patients and prospective patients have been led to believe reversing a procedure they have had can be as easy as pushing a button.
The bottom line: From body-focused surgeries like BBLs and breast procedures to facelifts, no matter what you have done, you cannot get back to square one—even when you’re in the hands of the best surgeon.
As we dip our toes back in, there are plenty of high-powered, hydrating and highly recommended options: Check out our stories on supercharged facials (page 32), post-summer fixes for hyperpigmentation (page 46) and our take on TikTok trends for your smile (page 42).
For those of you who are out and about, there are many amazing new face and body treatments worth trying, and for those of you who are still more comfortable bringing the spa home to your very own private sanctuaries, fear not, there are options a plenty!
Don’t call it a comeback!
There is a difference between a revision, which may be beneficial in a small percentage of patients, and a “reversal,” which is rarely performed. While all statistics point to the fact that the majority of people who have plastic surgery are glad they had it done, a large part of that satisfaction comes back to the provider: Make sure that, no matter which aesthetic treatment you receive, it’s from a boardcertified surgeon, practicing in scope of their training, who properly screens patients and explains what is realistic and unrealistic—a true “secret” for success.
For many of us, life has seemingly returned to normal (whatever that means). Everything beauty has had a major resurgence and, whether it be skin care or self-care, the need to invest in ourselves has become more of a necessity than a luxury.

As quickly as trends come in, trends go out, and in no arena is that quite as true as the world of aesthetics. Mix in the power of social media and its close cousin of “the filter,” and we’ve seen a few interesting years of an overall message push that makes plastic surgery look “easy.” Of course, it’s not. And when a fleeting trend comes into play (think more fox-eye looks and fully plumped lips, less vibrant hair colors that can change with the season), the question of “what happens when the trend goes away” comes into play.
Internationally renowned aesthetic plastic surgeon and noted educator Robert Singer, MD of Prime Plastic Surgery in La Jolla, CA is a diplomate and prior examiner of the American Board of Plastic Surgery and has served as president and chairman of the Board of Trustees of the prestigious Aesthetic Society, chairman of the Board of Trustees of the American Society of Plastic Surgeons (ASPS), president of the American Association for Accreditation of Ambulatory Surgery Facilities, and president of the Aesthetic Surgery Education and Research Foundation (ASERF), an organization that bestowed on him the highest honor in aesthetic surgery, The Career Achievement Award, to recognize his contributions to the field.

I spent a good portion of the summer on the California Coast and treated myself to some over-the-top treatments and procedures. From a much-needed facial at Facial Lounge in Costa Mesa (their cryo globes are a must) to a relaxing overnight at Paséa Hotel & Spa in Huntington Beach, where I dipped more than just a toe, I concluded my tour of Southern California at Rancho Valencia Resort & Spa, with a relaxing Balancing GemStone massage (I may or may not have fallen asleep in the relaxation room)! A little bit closer to home, I also discovered a new personal favorite, the CryoFacial at The Spa at Carillon Miami. And, in between, I spent time enjoying the outdoors (covered in SPF) and visited a few of our experts across the country for some innovative in-office treatments.

“The first step to a healed scar is an incision that traces along the natural skin lines. Postoperatively, or postinjury, massage the scar daily to promote elasticity. While extremely di icult for the patients, patience is key. Rushing to treat a scar can often worsen the problem.”

“Early intervention is extremely important for optimizing scars, but avoiding excess sun exposure is number one. Sun exposure can lead to darkening of the scar, which can cause hyperpigmentation and make it harder to erase.”
“For dark scars, lightening products plus a laser for brown pigment, such as Picosure or BBL, are effective. For red, vascular scars, a VBeam laser can be used. To treat elevated scars, we inject a steroid solution and use silicone products.

“We start laser therapy on scars no later than six weeks after the injury. The Cutera Excel V is great for treatment. Then, depending on how the scar presents itself after treatment, we could finish with a pico laser, like PicoLO, or microneedling. The same treatments work for scars that have been around longer.”

BODYBUZZ 2022FALL 034
PEOPLEIMAGES/GETTYIMAGES
Treating patients from around the world at his iconic Harley St. clinic, London plastic surgeon Yannis Alexandrides, MD keeps a solid pulse on aesthetic trends. The latest buzz coming from all corners of the globe? A spotlight on body care, which is precisely why his celebrity-adored skincare line, 111SKIN, chose now to launch its first-ever commercial body product.
Topicals for below the neck isn’t the only trend experts are sp otting. Fort Lauderdale, FL dermatologist Dr. Matthew Elias says body treatments are currently more popular than ever in his practice, and predicts the wave to continue to rise. This shift, he says, is an expected one: “Body treatments like Coolsculpting, CoolTone, Emsculpt NEO, and radiofrequency microneedling have been trending over the past few years, so now there is a focus by skin-care manufacturers on pairing safe and effective skin-care topicals with these treatments to yield synergistic effects.” Ahead, a guide to the potent, active-infused topicals filling our beauty aisles.
BODYBUZZ
For example, Dr. Alexandrides says the skin on the body not only requires rehydration, but also its own antioxidants, especially if it’s exposed to the sun. “The skin of the body is thicker than on the face, although there are exceptions to that rule, and it is usually less sensitive than skin on the face and neck. However, it still needs hydration, vitamin A and all of the ingredients to stay healthy.”
BasicsBody
We’ve learned not to neglect the skin on our chest, hands or neck in our daily routines, so why would we abandon our body? This new wave of skin-care launches points back to a bottom line of having a more holistic approach to skin care, says Dr. Alexandrides. “We don’t want to be in a situation where we have kept our face and neck in pristine condition, but our body looks older and dehydrated.”
Ingredient Intel Similar to your facial skin-care routine, the ingredients to look for in your body care will vary depending on your skin’s needs. Generally speaking, “the best products for the body will contain a blend of peptides, antioxidants and caffeine,” says Dr. Elias. While some products will spotlight these ingredients alone, you might catch a name of a proprietary complex instead—NAC Y² in the case of 111SKIN, or SkinMedica’s Firm & Tone Technology—which often bundles hard-hitting ingredients in a balanced, clinically proven manner, explains Dr. Elias.

Skin is the body’s largest organ, and Dr. Alexandrides reminds us that its main function is to act as a barrier to the environment and pollutants. But, certain areas of the skin (read: the face, scalp and body) have different sets of requirements needed to thrive.

When it comes to Brazilian Butt Lift procedures, the news out of Miami can be grim. Since 2016, BBL-associated deaths in Florida have reached double digits. Some experts believe that heavy daily caseloads have contributed to do ctor fatigue, leading to a risky, sometimes fatal complication called fat embolism, which happens when the fat placed into the butt is inadvertently injected into an artery. In response to the fatalities, the Florida Board of Me dicine has set new rules that limit the number of BBL cases a surgeon can perform to three a day and requiring practitioners to use ultrasound technology during the procedure.
Citing doctor fatigue as a factor toward fatal risks, Florida becomes the first state to set rules for BBL surgery.

Fort Myers, FL plastic surgeon Ralph R. Garramone, MD says the re st of the country should follow: “Florida may be progressive with re gard to this ruling, but it should be a national standard established by the American Society of Plastic Surgeons or The Aesthetic Society.”
FACT TWO Recent Safety Measures In the last four years, a specialized multisociety plastic surgery task force was created with esteemed surgeons from across the country to research BBL risks and provide surgeons with risk-mitigating re commendations. The task force’s findings led to the recommendation that surgeons inject the fat above the muscle. Other advisory groups re commended using ultrasound te chnology during the procedure to avoid puncturing an artery. He eding suggestions from members of these groups, the Florida Board of Medicine has now made it a re quirement for surgeons to use ultrasound technology when reinjecting fat into the buttocks. However, not everyone agrees that ultrasound technology makes the surgery Louisville, safer.KYplastic surgeon M. Bradley Calobrace, MD says Florida’s ruling is a good step forward in addressing the fatal risks associated with this popular surgery. “Our role as plastic surgeons is to provide treatments but err on the side of safety. We can’t put our head in the sand. It is right to look at the things we can do to minimize risks while still being able to provide the de sired result. Deaths have occurred from BBLs, and something has to change if we want to continue providing these treatments.”

Eugene, OR plastic surgeon Mark Jewell, MD disagrees: “Real-time imaging has been shown to be useful in determining gluteal muscle depth to avoid injecting in the wrong place.”
Miami plastic surgeon Daniel Careaga, MD says it impedes on his normal protocol: “I have always used my nondominant hand as a guide. If I feel the tip of the cannula, I am superficial to the muscle therefore there is no chance of a fat embolism. The probe is now in the way and there is no real evidence showing that it is safer.”
FACT THREE FACT FOUR Turn to page 145 to find one near you.
Tucson, AZ plastic surgeon Raman Mahabir, MD says, “At best, using ultrasound may be helpful; at worst, it may create a false sense of safety.”
The ‘Surgical Mill’ Issue La Jolla, CA plastic surgeon Robert Singer, MD says the guidelines made in the last few years have greatly improved the safety of the procedure, and the Florida ruling is a reasonable progression. “The other aspects that need to be looked at are where the fatalities are occurring. Many of these centers are in strip malls without good oversight of the facilities,” he adds. “Patients need to avoid surgical mills where multiple procedures are done on these patients day and night. The issues come down to physician training, ethics, ability, and most importantly, patient safety.” Miami plastic surgeon Sean Simon, MD, who has safely performed thousands of BBL surgeries, also believes the Florida B oard of Medicine should take a closer look at these facilities where repeated complications have occurred.
Vero Beach, FL plastic surgeon Alan Durkin, MD adds that Florida surgeons are eager to elevate patient safety standards; however, this particular surgery requires the greatest elevation in the shortest period of time: “In my opinion, a blanket ruling is widely punishing the surgeons who are doing it correctly. We need to identify the surgeons that are doing it wrong, many of whom are not even real plastic surgeons, to prevent them from doing it wrong in the future.”
“If there is one takeaway for me, it’s that the Board should investigate the specific surgery centers where this is happening. The one thing that can save lives would be to remove the Florida Department of Health as the group that accredits these surgical centers and require that the operating room accreditation come from the American Association for Accreditation of Ambulatory Surgery Facilities.”
The Ultrasound Debate Weeks after the ruling, a group of Florida plastic surgeons formed an organization called Surgeons for Safety to appeal it, but that appeal was denied. They contend there is no hard data pointing to either the doctor fatigue factor or proof that ultrasound technology increases safety. “Making the procedure safer is our primary concern,” says group president and Miami plastic surgeon Constantino Mendieta, MD. “However, this decision was not made from evidence-based research. The other issue is that 98 percent of us have never been trained to use ultrasound in this manner.”
“Our specialty is one of the last to use ultrasound, but we’re making strides,” says Grand Rapids, MI plastic surgeon Bradley Bengtson, MD. “Everyone may not be trained yet, but training is available to increase proficiency.”
“This is just one way to make it safer,” notes Dr. Singer. “If other technology comes along that can make it safer, we should embrace that, too.”

Visiting a chiropractor for muscles, bones and joints can be extremely helpful for issues such as neck or back pain, but Boca Raton, FL cosmetic dentist Clive Rosenbusch, DDS is hesitant to send his patients to a chiropractor for TMJ treatment. “I am not a believer that this joint should be manipulated,” he says. Instead, he recommends visiting a dentist to diagnose the cause. “Once the reason for pain is determined, I like to use laser treatments to eliminate the pain, and they are very successful.”
CHIROPRACTORTMJ
“While whitening the teeth with ingredients you can find in your kitchen may provide mild stain removal, the destructive effects of these ingredients outweigh the small benefits,” says Prospect, KY cosmetic dentist Sara Cummins, DDS. “The combination of kiwi and lemon juice provides a significant amount of acid, which is detrimental to the teeth because it dissolves the mineral content of enamel, making it weaker.” Dr. Cummins also notes that adding baking soda to enamel is far too abrasive. For stronger, safer and more effective whitening products, she recommends visiting a professional or purchasing an over-the counter whitening product from a reputable company.
“Think of Newton’s Law: For every action, there’s a reaction,” says Los Altos, CA cosmetic dentist Joseph Field, DDS. “When you put a rubber band around a gap in the teeth, they will move, but that opens up spaces elsewhere in an uncontrolled manner, which alters the bite. So, it’s a very short-sighted solution and the results can be very dangerous.” Instead of going the DIY route, he suggests opting for a clear aligner like Invisalign.
“Gummy smile surgery involves lessening the gum tissue showing during smiling,” says Atlanta cosmetic dentist Ronald Goldstein, DDS. “This can be done by performing surgery to the gums around the teeth or removing some of the bone around the teeth.” Adding that this surgery can make a difference for people who have a “horsey” look, Dr. Goldstein says the best patients for the procedure are those who have a lot of gum tissue showing when they smile, or those who are embarrassed about their smile aesthetics.
RINSEPEROXIDEHYDROGEN
Dr. Rosenbusch explains that rinsing with hydrogen peroxide does have its advantages. “It can decrease anaerobic bacteria, which helps lower the chances of developing periodontal disease. It can also wash out debris from the oral cavity.” But, if the hydrogen peroxide is above a concentration of 3 percent and is not mixed with water before rinsing, it can cause damage to the digestive tract. “For this reason, I don’t advise it.”

Houston cosmetic dentist Guy Lewis, DDS says no-prep veneers can quickly conceal dental problems like teeth that are discolored, chipped, cracked, misshapen, or separated by gaps. However, their biggest benefit is that they do not require reduction of the tooth, which preserves the natural tooth structure. Turn to page 145 to find one near you.
“While tooth gems [gems and/or diamonds that are bonded to the teeth] can be a simple procedure, there are important factors to consider,” says Dr. Cummins. “Taller or larger gems have the potential to irritate the surrounding soft tissue, such as the inside of the lips. They can also recess into the enamel of the tooth, which would be an irreversible injury.” The location of the gem should also be considered. “The bite should feel normal and natural after placement. Even the slightest disruption to the bite could have long-term effects.”
New York cosmetic dentist Timothy Chase, DMD says there are no reliable studies to prove that oil pulling reduces cavities, whitens teeth or improves oral health and well-being. Instead, he advises patients to brush their teeth twice a day for two minutes with fluoride toothpaste, floss once a day and avoid tobacco.
Noticing decay between your teeth? This TikTok trend might be on your favorite’s list. “Using toothpaste on your floss pick cannot whiten your teeth like an in-office treatment can, but it’s great for preventing decay,” says Dr. Goldstein, who adds that it also works for patients who have decalcification (softening) between the teeth. “To do this, make sure you are using a thick floss and that the fluoride concentration in your toothpaste is high enough. If done every night, you’ll notice a difference in decay.”

“I have had to fix countless smile makeovers after patients have gone overseas to seek a more inexpensive dentist,” says Dr. Field. “While there are some great clinicians in other countries, many patients go to Mexico or Turkey because the procedures there are a fraction of the cost, but they get what they pay for. Many of them end up with aggressive reductions or root canals, which are even more expensive to repair.” His advice: Go to a doctor in the U.S. who offers financing options. Los Altos, CA cosmetic dentist Karl T. Supal, DDS adds that a great way to find a safe doctor is to look at some of the cases they’ve done within the last six months and read reviews.

Four proven ways to put inhyperpigmentationpost-summerreverse
After the summer months, especially during a high-heat year like this one, hyperpigmentation issues like dark spots, age spots and melasma show up more prominently in the fall than in any other season. Even if you’ve done your due diligence shielding your skin from UV rays, hyperpigmentation can ramp up from being outdoors, in front of screens, relaxing in saunas, and even taking long, hot showers. Before throwing in the towel on brighter, eventoned skin, try these expert-approved solutions.
.
VIVANITY/GETTYIMAGES
Using Senté’s Cysteamine HSA Pigment & Tone Corrector for 15 minutes a day over 16 weeks, this 53-year-old woman saw stubborn dark spots on her face lighten and she achieved a more even-toned complexion.
Mischievous Melanocytes “There are many causes of hyperpigmentation,” says Aventura, FL dermatologist Bertha Baum, MD. “Melanin production can be overstimulated by exposure to UV radiation, as well as genetics, hormonal changes and any inflammatory condition.” While overactive melanocytes are the widely known culprits for increased darkening, there is also a vascular component, adds New York dermatologist Orit Markowitz, MD. “Recent studies have shown that hyperpigmentation is very much associated with an increase in vasculature. In terms of treatment, we’re not only treating the pigment; we’re also targeting swollen blood vessels. Therefore, it’s not unreasonable to think that long, hot showers and other types of heat exposure can exacerbate hyperpigmentation, but again, the biggest factor is UV exposure.”
Celebrity aesthetician Tammy Fender says the best offense is a good defense. Even though summer is over, protecting your skin from further damage is the best thing you can do to decrease the appearance of dark spots. “The more physical blocks the better,” she notes. “Any SPF that has some form of a mineral, physical block will work, but to be doubly sure, add a little bit of micronized zinc oxide to your daily cream.” After treating her own hyperpigmentation issues for years, Fender’s first recommendation to clients is to choose a mineral, not chemical, sunblock. “I follow this with treatments that remineralize the skin and slough off dead surface cells in order to constantly create new cell renewal.”
While vitamin C, arbutin, hydroquinone and acid-based products are known dark spot correctors, cysteamine has become a superstar ingredient for safe, effective brightening on all skin types. “It inhibits the enzyme pathway responsible for melanin production,” explains Denver dermatologist Joel Cohen, MD. Cysteamine can target dark spots and patches without the potential carcinogenic risks associated with hydroquinone. “Senté’s Cysteamine Corrector has a more stable form of vitamin C called THD ascorbate, as well as niacinamide and Heparan Sulfate Analog to increase moisture and reduce inflammation and redness,” he adds. Other options featuring the lightening ingredient include Cyspera Intensive System and Urban Skin Rx Hypercorrect Intense Fading Cream. One derm-approved approach is using compounded, medicated creams that contain tranexamic acid, which also slows the production of melanin, to boost results. “Its anti-melanogenic properties target hyperpigmenation, which helps reduce symptoms,” says Dr. Markowitz. Dr. Baum says she uses the powerful ingredient three different ways: “Injected via AquaGold microinfusions; topically in compounded creams I prepare; or taken orally for severe melasma cases that are hard to treat.”
“Radio-frequency microneedling has shown promising results with treating melasma,” notes Dr. Markowitz. “The combination of energy delivery with the microneedles really helps renew the skin. It also allows the topicals we use in conjunction to absorb much better and faster.”
Photoacoustic Energy A new modality that uses sound waves rather than heat has also seen promising results. “Lasers are classically used to treat any sort of pigment concerns, but one of the issues with melasma is that too much heat and thermal energy can actually make it darker,” says Dr. Markowitz. “This uses sound energies to vibrate the skin, causing pigment resorption with a lot less heat.”
UltraClear is another laser solution that reduces uneven skin tone and the effects of sun damage in a quick, 10-minute treatment. “We’ve seen good results in patients with darker skin, like Asian patients and East Indian patients,” says Dr. Cohen. “From a safety and efficacy standpoint, it’s nice to be able to address darker skin tones without fear, whereas with CO2 lasers, we really sort of back away.” UltraClear delivers fractional cold ablation and requires no topical numbing. “It’s really the only laser I use that I would call a ‘lunchtime laser.’”
Newer Laser Options Lasers with lower wavelengths, like Clear + Brilliant Permea, are the go-to resurfacers for slow and careful pigment correction. New to the toolbox are TIXEL and Aerolase Neo Elite, which have shown promise for evening skin tone and “bringing up the pigment in a few visits,” says medical aesthetician Amy Peterson. At her Miami medspa, she uses both modalities and has seen impressive results in a short timespan. “Neo Elite goes deep and treats all kinds of contributing factors at a vascular level and breaking up deeper pigment,” she explains. “I often combine it with TIXEL and have seen incredible results.”
Avoiding HyperpigmentationPost-Inflammatory
Targeted in-office treatments help skin regenerate, causing dark spots to lessen over time. Chemical peels, Intense Pulse Light (IPL) and pigmenttargeting devices like the PICO laser are often recommended to lighten stubborn spots, which can feel like a totally untreatable concern.

New York dermatologist Michelle Henry, MD says anyone with melaninrich skin should proceed with caution when it comes to treatments involving heat, as post-inflammatory hyperpigmentation (PIH) can occur when the approach is too intense. “I tell everyone, ‘Your first treatment won’t be your most aggressive,’” she says. “It’s slow and steady. We start low to keep you safe. Best practice, especially for those who are just starting out with a resurfacing treatment, is to ease your way into it. If I’m going to use a laser that targets pigment, for the most part, I’m going to do a test spot first to ensure we don’t do further damage.”

“At the beginning of my career, I thought I needed makeup to feel beautiful and I didn't know much about skin care—I used body wash on my face. My makeup artist and bestie Yolonda was the voice of reason and told me I should take care of my skin. But I’ve never really been sure which products to use, and that’s one of the reasons I created OAM—it stands for “On a Mission” because I always say I’m a woman of ambition on a mission. I’ve put my heart and soul into this brand, down to every little detail. And being a woman of color, I’m all about inclusivity, and these formulas allow everyone to have that radiant glow—these are what you need to level up your skin-care regimen.”

“I use just a wash of beautyblender’s Foundation—I’m shade Tan Sienna. It’s lightweight but gives great coverage, and it’s easy to apply with my fingers if I’m getting ready on the go. I don’t use concealer every day, but when I do, I like just a touch of beautyblender’s Concealer in Tan Golden. I dab it onto a few spots and under my eyes to add some brightness after a long night.”
07 Body Icon “Kiehl’s Creme de Corps is a staple! I apply it after I shower, and it leaves my skin feeling soft and moisturized. I love that it’s thick, but it absorbs well and isn’t sticky. I’ve also been using Aquaphor for years, on both myself and my babies. I swear by it! It’s a great moisturizer for my lips and hands, but I can use it pretty much anywhere on my body and it makes my skin better.”

“My bathroom is my little oasis. It’s essentially where I go to start and end the day. I know I’m truly done for the day when I’m in there, taking off my makeup. It’s also where I spend the most time focused on me and self-care, like soaking in a bath or doing my skincare routine. I love white flowers and I try to always have some around—it adds life and brightens the space.”

“Pinky-nude shades are my go-to for everyday wear, and Pat McGrath’s Lip Fetish Colour Balm in Blow Up is one of my favorites. The sheer color looks great by itself or over liner, and it also makes my lips feel hydrated and juicy.”

As the eternally youthful-looking Ralph Macchio preps for a new memoir, Waxing On, and Season 5 of Cobra Kai, the 60-year-old Karate Kid star shares his all-time favorite skin-care, SPF and shaving products, all while defending his longtime love for the Mets. Liz Ritter

On-Set Staples The Braun MobileShave M90 travel razor is a staple. With the 5 a.m. wake-up calls, I do a less-than-stellar job of shaving with my eyes half-closed, so we keep this around for clean-ups and missed spots in the makeup trailer—I think I have three of them in various backpacks and desk drawers. Cococare Cocoa Butter Lip Balm is another constant. For some reason, this has become the on-set favorite for me and many of the cast who love the cocoa-butter smell. During Season 4, when we were filming during winter in Atlanta, I went through more than a few of these. Scan for more
Some of my favorite staples for skin care have been these two Kiehl’s products: Facial Fuel and Creme de Corps. I use both of these often, especially in the winter when everything is so dry.
Razor’s Edge I love the Cade Shaving Cream from Homme.L’Occitane I picked it up about seven years ago, as I was drawn to the name ItheJohnny“Cade.”Cadewascharacterportrayedin
SPF Star I’ve recently been experimenting with Supergoop! Unseen Sunscreen. It has become quite popular, and I wanted to see what it was all about. It’s different from a lot of the sport lotion sunscreens I have used in the past. I definitely see what the appeal is, and it does provide very good invisible protection. I’m on board. Holy Grail For years, I have consistently used L’Oréal Paris Elnett Satin hairspray. Most of the hairstylists I have worked with use it, and I have accustomedgrowntoit.
The Outsiders, so that inspired the purchase and it has stuck around ever since. Stay gold!

There are a few things for me to be excited about this fall. First off, Cobra Kai Season 5 drops on Netflix. What an amazing ride that continues to be. And then, in October, I have my first-ever book coming out, a memoir entitled Waxing On. The book dives into my personal journey with The Karate Kid universe from the earliest days of making the original film through its undying relevance today. The launch of these two projects should keep me pretty busy—however, I will definitely find some time to check out the MLB postseason. Fingers crossed the NY Mets have a decent run; I’ve been a die-hard fan since grade school!

A D V I C E C O L U M N : 2022FALL 062
Like the promise of fall leaves changing, less-humid days and “Thriller” on repeat, tis the time for seasonal classics—only, this year, there’s a twist on a favorite with the release of Hocus Pocus 2, a sequel to the original that, after almost 30 years, brings the beloved trio of Bette Midler, Sarah Jessica Parker and Kathy Najimy (aka, “the infamous Sanderson Sisters”) back to the big screen. We recently caught up with 65-year-old Najimy, who shared a personal review of her career, as well as her thoughts on what makes this Halloween cult-classic click so much with audiences.
The most difficult thing about my career: “Feeling bad about telling agents or managers that I’m not interested or passing on a project. I had a child, who is now 25 years old, and I wanted to spend every minute with her that I could while she was growing up.”
The most rewarding thing about it: “My career has granted me a large platform that has allowed me to talk about the causes and issues I believe in. And some projects are really fun!” What I would tell a young Kathy: “I wish I could tell my younger self that every moment you’ve experienced—whether positive or hurtful—is going to benefit you beautifully for the rest of your life.” What I wish I did more of: “I’d like to work on paying more serious attention to my health, which is wildly uninteresting to me. That, and getting my backflips perfected!”
On Hocus Pocus becoming a Halloween staple: “It feels surprising, but great! It ’s fun to be part of something that has made such an impact on people’s childhoods!” My approach to aging: “A couple things: I have my mother’s great Lebanese skin! I eat pretty healthily—in fact, I’m eating edamame and hummus as we speak. But, full-disclosure, I’m addicted to bubble tea. I’d also like to say, we all know women are made to feel horrible about something they have no control over, and that is aging. The good news is that if you are aging, that means you are still alive!”

“I went through a very difficult time with fertility, and I came to it late—not as late as some—but I had some troubles. When I was 36, I met a partner [Watts has two teenage children with actor Liev Schreiber and is now in a long-term relationship with Billy Crudup] who I wanted to have children with, and I discovered that I wasn’t able to get pregnant as easily as I thought I might.
“We are taught in this industry t o keep things private and be mysterious. I think we all have this idea that if we can stay secretive, we’ll be better at our jobs.”
I did try some other things, multiple interventions, and nothing was working.
You were secretly reading books under the mattress?
When I was told that it was looking like I was going to go into menopause early too, it sent me into a rather shocking state of mind; I felt completely panicked. Plus, I was told that I wasn’t an IVF candidate.
Eventually, long story short, I tried all the alternate routes—the smuggling in bags full of herbal teas from China and buying all of the wheatgrass from Whole Foods.”
“I was all kinds of things! And I had a lot of shame around it…like I was not a full woman if I couldn’t bear children. Of course that’s a ridiculous notion, but society labels you as such. At that time, it was all very secretive and particularly taboo in my industry— Hollywood prides itself on well-known people being private and mysterious; I think we all have this idea that if we can stay secretive, we’ll be better at our jobs— that there’s less onus on the audience to suspend their disbelief.”
It was all in very vague terms.
+ The launch of Stripes will include skin care, hair care and supplements, but Watts says that’s “just to get started.”
I had a bunch of tests and conversations with my OB-GYN, and all of it just didn’t sound so great. The word ‘menopause’ was mentioned very early on; I don’t even know if there was such a word as perimenopause used then. I certainly hadn’t heard of it, but I did know that my mother had gone into menopause early at 45. I didn’t know anything else around that; I just knew that she had her last period when she was 45.
0102
I first met Naomi Watts some 10-plus years ago in the penthouse of The Plaza Hotel—a two-story, glitzy, goldencrusted space floating high above the New York City and Central Park–adjacent skyline—that is, by its very nature, incredibly opulent and over-the-top. Today is not necessarily that same day. With her daughter, Kai—the most practical, about-to-enter-high-school teenager, who is uninterested in ordering the iced matcha tea, but is very good at helping run everything on-set (maybe only for the fact that she’s ready to head back home after being at work with her mom all-day) in tow—Watts has driven herself out to our more laid-back Montauk beach location, a peaceful place that, as we later find out, the paparazzi has decided to set up s hop early, hiding behind a nearby surf camp to get a quick shot of the A-list actress. As the sun starts to come out of the clouds on this unexpectedly overcast day, Naomi shows up early with her brother, Ben, waiting on-set, ready to photograph her. She has no requests, but when parking passes for the notoriously tough-about-parking beach community come up, she’s first to raise her hand. “Of course Naomi needs her pass!” her brother shouts in a moment of playful sibling rivalry. “It’s Naomi! It’s all about Naomi!” And, at the age of 54, it really is. On the cusp of launching her labor-of-love, very-personal project of Stripes, a brand that she set out to help “erase the stigma of menopause,” Naomi admits that she’s excited, yet nervously awaiting a bit of backlash for it. “Most Hollywood careers are made on the fact that actors are somewhat mysterious,” she says. “But the whole thing around this line is making some taboo subjects not so mysterious. It’s time.”
Why did you decide to make the move into this second-career of skin care?
The least amount of information you can put out there, the better…
With the loss of your hormones—with them dipping down, especially estrogen—you lose hydration. That goes from your scalp to your vag. How can we support with good ingredients in products that are needed for all of those places—your skin, your hair, your vagina, your body? Have a campaign that’s really targeting menopausal women, so that they can feel better knowing there’s a community surrounding them, and at the same time, that’s hopefully solving a problem? This is why we created Stripes as a wellness brand—one that intends to address beauty needs and more.”
“It all worked out in my brain as, ‘Well, no one must be experiencing the same things as I am.’”
As soon as I had one baby, my body seemed to know how to do it, and then I had a second baby. Then, pretty soon after at age 43, I went into perimenopause. But I still did not know what that term ‘perimenopause’ actually meant at the time. There’s no one test, because if you do a blood panel and read your hormones, they could read one way on one day and completely different the next day. It’s really not a perfect indicator. All the things were happening, but I only knew of maybe one or two symptoms.” And, like you said, it’s not necessarily something everyone talks about.
“I was really out there on my own. Yes, I tested the waters with friends: I made jokes about menopause and hormone-estrogen dips and things like that, and they were always met with nervous laughter, which worked out in my brain as, ‘Well, no one must be experiencing the same things as I am, yet.’ I knew about the classic symptoms, like hot flashes and night sweats, but I was having multiple night sweats and no one seemed to want to talk about it—you can actually have night sweats while breastfeeding. Of course, when I say you have, I don’t mean everyone. It doesn’t manifest for everyone in the same way. I also started having chronic migraines. I had itchy skin and a multitude of symptoms. I quickly dealt with it because I was very in touch with my body, so I made sure I got the right kind of treatment. But back to why this brand was born…I didn’t want to have to walk through that feeling again, of being completely alone and shrouded in secrecy and shame, nor should anyone feel like that. It just a terrible feeling, and wherever there’s stigma, it should be broken down because you need a supportive network. You need a team, whether it’s your family or hopefully doctors, to provide you with that care that you need, or you’re going to think that you’re losing yourself. You’re definitely thinking you’re losing your mind as you go through those ups and downs. It was chaos. As soon as you understand it better and have the knowledge on the table, you can be your best advocate. You are always your best advocate.”
“We’re at the beginning, which is why we’re starting with the beauty space to ease into it, but it’s more than that. Menopause can hit every part of your body in a different way. It’s all-encompassing. For starters, I wanted to reach women who feel underserved and under-supported, and not necessarily reflected in the ways they want to be. We don’t want to look at packaging or ad campaigns of women in their 20s or even 30s. That’s never going to be the story I can adapt to. I’m never going to look like that again. Why not try to embrace this time and make it as positive as possible? The only way to do that is to arm yourself with the knowledge, the community and the products to support you.
You’re still in the moment of launching it, but what was the most difficult part about taking the idea from pen to paper?
2022FALL 070
“The stigma. That part is still ahead of me, especially in my industry. Part of my job—part of acting—is that we are taught to keep things private and be mysterious, so we can immerse in the characters that we play. People want to get lost in that story, not your own story. But this endeavor is all another version of storytelling to me. I’m in the business of hoping that people see themselves through the telling of whatever that story is—through the character I’m playing and whatever movie I may be in. The goal is to, hopefully, help the audience tap into a part of themselves that makes them think and dig deeper and feel less alone in the world. This is the same thing, only it’s a bit more specific and a whole lot more personal. Every day I get closer to products on shelves, or another Instagram post, or another conversation 04 05 03
It sounds like the goal is to fill some voids in the beauty space. Do you see Stripes as more of a self-care or sexual wellness brand—or something else?
“Yes, I had a tough time for a couple of years. I ended up falling pregnant naturally, and I think it had a large part to do with all these alternative methods and remedies I put in place, and then backed off of it...in retrospect, it seemed like.

I wasn’t equipped; I wasn’t prepared. It’s mindboggling that people don’t understand it. When you think about it, it’s really the bookend to puberty. We had that whole story spelled out for us in Sex Ed in fourth or fifth grade, but no one gave us the ending, which is ridiculous. It’s like the story just stopped. It feels like a secret code of silence that we all somehow agreed to. We need to stop staying silent about it.”
with a journalist, and I get a little bit more scared. We’re getting closer to launch now, and I love my day job. I don’t want to give up my day job. I love being an actor, and I love that this is also something that is helping women and hopefully solving a problem, and yes, helping them feel supported. Again, telling a story that could be positive.”
I know you don’t want to give up your day job, but Hollywood has not always been kind to the 50-something woman, or even the 40-something one. Do you think it is getting any better?
++++ Role Play: ”I hate answering the question about my favorite role…because there’s a couple. I’ve had such good times on so many different movies. Early on, the David Lynch experience [Mulholland Drive] was really a standout, especially because it originated as a TV series, and then the doors closed because ABC didn’t want to make it a TV series. But then it became a movie two years later when the French producers came back and said, ‘Let’s not let that sit there, doing nothing. Let’s make a film out of it.’ It was very, very lucky for me—it could have been a project that just sat on the shelf, that no one got to see. That was just a wonderful experience, and then, of course, King Kong and 21 Grams and The Impossible because it was such a gut-wrenching story, and shooting it there with people who had experienced the tsunami was life-changing for me.”
the list:
Co-Parent Tip: ”Have good communication, have good, shared decency, and always have the kids’ needs in mind. That is always at the top of the list in how you negotiate, and we’ve always seemed to achieve it. Not to say that there won’t be bumps along the way, but we’ve always had that goal in mind. We just want the best for our kids.”
Basic Instinct: ”I wish I had been not so caught up with what everyone else needed and wanted from me when I was younger, and that I got more into what I felt was good for me. I’m definitely a people-pleaser and I still am, but I’m getting better. But it took a long time to get to that place. Just trusting my instincts, I would say, is the best piece of advice I would give to my younger self.”
I want to tell the stories. I want to collect everyone’s stories, have everyone share them, and hopefully, bolster each other through this time.” Shake the stigma.
“Yes! I think it’s as simple as, wherever there’s stigma means that we’re lacking collective support. We just want to find the alignment and make it work.”
Next Up: ”I’m going to be filming a new project in the fall and it’ll be shooting in New York. It’s called Capote’s Women I’m playing Babe Paley, so I’m super excited about that.”you’re in your 50s, you have accumulated your ups and downs.”
“Yes, but I don’t want to be reduced to menopause. I just want to own it and say, ‘Hey, this is OK.’ We’ve had a whole lot of experiences, good and bad at this point. We should feel proud of this moment. We’ve earned our stripes. We should be able to hold our heads high and feel a certain level of status. I know status is such a pejorative term, but in this case, if you’re in your 50s, you have accumulated your ups and downs. We know how to recover. We’ve earned the right to feel good about ourselves. Now, let’s just make that happen and enjoy this part of our lives. We’re living so much longer now! We’ll spend 40- or 50-percent of our lives postmenopausal. Why wouldn’t you want to enjoy that? Why wouldn’t you want to set yourself up better in a medical way, in a community way, in a fun way?
090708 “If
I think it’s fair to say you’ve prepped people…your social media posts have made it clear you think that “midlife and menopause” are both a liberating time.
“I do. It’s still risky doing this because, like I said, it is deeply personal. But I don’t care, and I don’t mind being part of the changemaker movement. I think every conversation is opening up now. We’re seeing it in multiple arenas. People in menopause today have a different approach to it because we have the internet. We’re the first generation that has been able to access information through the internet. We are the generation of changemakers. We should be throwing down the ladders to the younger generation, so that they’re better equipped.

“Hot stones can also be used in conjunction with body treatments to remove creams or scrubs, helping to further stimulate the skin surface and circulation,” she adds.
Hot Springs Hot springs are earth’s answer to a heated wellness experience. They’re similar to thermal pools found at spas, but created by Mother Nature. Labreque explains that these springs are born from geothermal heat, and the water features a variety of minerals, like calcium and magnesium, that are beneficial to our bodies. The heat of the water helps the nutrients seep more readily into the skin, adds Sarfati. Other benefits include a boost in circulation and oxygen flow.
“We all love that feeling of a warm blanket wrapped around us, and that is because heat encourages relaxation and sleep, improves blood circulation, reduces pain, and eliminates toxins in our bodies,” explains Lana Labreque, spa director at The Spa at Four Seasons Hotel One Dalton Street Boston. Additionally, heat can help “wake up” muscles, promoting more flexibility and movement in tight areas, says Kimberly J. Holmes-Cardona, lead massage therapist at Chillhouse. “When vessels are dilated, they act like a super highway and will deliver more blood to the area,” says Beverly Hills, CA dermatologist Ava Shamban, MD. Heat causes vasodilation—a widening of the blood vessels—in an effort to cool itself down, resulting in increased circulation that helps clear out toxins and allows the skin to metabolize more effectively, explains Campbell, CA dermatologist Amelia K. Hausauer, MD.
“Through improved circulation, heat on the body can help ensure vital processes function properly, and that muscles, organs and tissues get adequate nutrients and oxygen,” explain Lauren Berlingeri and Katie Kaps, cofounders and CEOs of HigherDOSE. “Heat triggers the body’s detoxification process through sweating and lymphatic drainage, helping to dispose of toxins through sweat.” These benefits can culminate to produce glowing skin.
Founder and CEO of Repêchage, Lydia Sarfati, says the stones are generally 125 to 135 degrees Fahrenheit, and the heat eases muscle tension, relieves stress, reduces body aches, and aids in better sleep.
Sauna Holmes-Cardona says a sauna is “traditionally a wood-paneled space that has a high-heat environment to promote body detoxification via higher perspiration, relaxation and pain relief by increasing heart rate, blood flow and circulation.” The heat generally comes from infrared technology to heat the body’s core temperature. However, Dr. Hausauer notes that pigmentary conditions, particularly melasma, may worsen in a sauna.
“Sweating it out in a sauna also has amazing mental health benefits, as it can help return the body to ‘rest and digest,’ aka the parasympathetic nervous system,” explain Berlingeri and Kaps. Returning to this resting state can also improve sleep.
Hot Stone Massage “A hot stone massage involves several heated stones being placed or moved along the body for pain relief, relaxation and/or therapy,” explains Labreque.
“Cold by causing vasoconstriction will reduce the delivery of inflammatory cytokines to a localized area,” explains Dr. Shamban. Chilly temperatures can also cause constriction in lymphatic vessels, which can help reduce puffiness, as you’re not retaining as much fluid in your tissue. “This is where the old trick of putting cold spoons on the under eyes comes from,” adds Dr. SarfatiHausauer.saysstudies indicate that cold can have a positive effect on the “reduction of inflammation, and stress after injuries.” When it comes to skin care, she explains that icy temps “can help temporarily reduce the appearance of pore size, firm the appearance of the skin and improve elasticity.” Labrecque says cold can also increase circulation, promote lymphatic drainage and help boost metabolism.
Cryotherapy has expanded vastly in popularity throughout the globe, but Yannis Alexandrides, MD, plastic surgeon and founder of 111CRYO/Heat and 111 Harley St, says he was the first to introduce the practice to the UK market.
“Whole-body cryotherapy is the use of extremely low temperatures to incite vasoconstriction, which leads to a vast array of physical and mental benefits,” he explains. According to Sarfati, the cryotherapy found in today’s spas involves entering a chamber that emits liquid nitrogen, with temperatures reaching as low as 200 degrees below freezing, where you stay for two to three minutes. For a more localized treatment, Dr. Hausauer gives a nod to cryo facials and Glacial Rx, an in-office treatment that uses cooling technology called cryomodulation to remove age spots and other kinds of excess pigment.
Dr. Alexandrides claims cryotherapy can promote reduced inflammation and pain relief, even in those with arthritis and tendonitis. He believes the practice can assist in weight loss, too, with some people burning up to 800 calories in one session. Additionally, he claims the elevated oxygen uptake can increase energy and boost collagen in the skin.
Before there were cortisone creams and lasers, there was cold. 01
“Before there were cortisone creams and lasers to reduce inflammation, there was cold,” says Dr. Shamban, noting that cold is especially beneficial for people with eczema. Dr. Hausauer adds that it’s also great for those with cystic acne, rosacea or other inflammatory skin conditions.

Cold Plunges
CoolSculpting
Cold plunges are trending right now as celebrities post videos partaking and luxury resorts advertise the experience, but they’ve been around for centuries with roots in a variety of different cultures. “A cold plunge is either when you submerge your body in ice cold water or have it fall over you,” explains Labrecque. “Your body benefits from this because the cold increases circulation, reduces inflammation and helps to boost your metabolism.” Ideally, the temperature of the water is between 45 and 55 degrees Fahrenheit, but don’t worry, you only have to tolerate it for two to three minutes to reap the benefits.
CoolSculpting is a technology that uses specific temperatures, generally between - 3 and -5 degrees Fahrenheit, to eliminate stubborn fat, explains Dr. Hausauer. “At those temperatures, your fat cells, which are more sensitive to the cold than muscle, skin or nerve, start to go through what we call a programmed cell death. Your body processes those cells out naturally and clears them from the body so they’re permanently gone.” While the modality can be effective for the right candidate, Dr. Hausauer notes that there is a risk of the “rare side effect” paradoxical adipose hyperplasia (0.025 percent), so talk with a board-certified doctor about the risks and benefits and decide if this treatment is right for you.

Another major difference between the skin and scalp microbiome: “The inside of each hair follicle on the scalp has its own microbiome,” says Omaha, NE dermatologist Joel Schlessinger, MD. Gaunitz explains that, typically, the scalp microbiome will remain healthy on its own as long as there have been few disruptions. “Just like the active microorganisms living in your gut, there needs to be a balance of healthy microorganisms on the scalp to ensure peaceful coexistence. If a high level of unhealthy microorganisms begins to take hold, then symptoms can occur.”
OUR BODIES ARE MADE UP OF DIFFERENT MICROBIOMES THAT STICK WITH US FROM BIRTH. We have the gut microbiome, which exists in the intestines and is known for having the largest count of microbes in the entire body; we have the skin microbiome, which is the most widespanning microbiome; we have the oral microbiome, which is often considered the gateway to whole-body health; and then we have the scalp microbiome, which some experts say may be the most unique of them all.
Life on the Scalp Look Out for Symptoms U.S.,theInthereare4.8K monthsearchesaverageevery serum.”“scalptheforterm
When our microbiome becomes imbalanced—also known as scalp dysbiosis—Anabel Kingsley, brand president and consultant trichologist at Philip Kingsley, says scalp issues like dandruff and seborrhoeic dermatitis can flare. “Yeasts live naturally on the scalp and usually do not cause any problems. However, when a certain species of yeast known as malassezia yeast begin to overgrow, it can cause skin cells to divide too rapidly, leading to flakes and itching.” Gaunitz adds that an out-of-balance scalp microbiome has a slew of other symptoms as well: “Look out for itching, burning, flaking, excessive oil, dryness, redness, and general inflammation.” “An unbalanced microbiome can also cause hair loss, but just as different scalps have different microbiomes, not all imbalances will directly result in this,” says Hezekiah, who adds that if you are losing hair for this reason, it is typically diagnosed as androgenetic alopecia. “This occurs due to DHT or dihydrotestosterone, which is a molecule created by testosterone. DHT triggers miniaturization, which impacts the microbiome and results in hair loss.” Androgenetic alopecia is one form of an impacted microbiome, but there are many other contributions that can result in hair loss. While these imbalances are a bit more difficult to prevent, a rapidly deteriorating microbiome due to poor hygiene is avoidable. “Neglecting your scalp health and picking at your scalp irritations can lead to scalp folliculitis or breakouts of pimples on the scalp with slow-healing scabs, which is another common expression of microbiome disruption.” Davie, FL dermatologist Marianna BlyuminKarasik, MD notes that changes in temperature or emotional stress can also be blamed for scalp imbalances.
According to trichologist William Gaunitz, the scalp microbiome is a collection of bacteria, fungus and microorganisms that coexist on the skin of the scalp. While they are very similar to that of the skin, the microbes on the scalp are a little extra unique. “The scalp’s microbiome is distinctive because of the concentration of sebaceous glands within a humid environment,” says Chuck Hezekiah, educator and brand ambassador at René Furterer. “This type of crowded environment is not found anywhere else on the body, which results in an increase of fungi, bacteria, yeast and other organisms, and as a result, excess flaking, irritation, inflammation, dandruff, and seborrheic dermatitis are common.”
“Just like there are different skin types, scalps also have different microbiomes,” says Hezekiah, who adds that you must tailor the ingredients you use on the scalp to specific microbiome concerns. “For someone with an excessive amount of sebaceous production or an oily scalp, ingredients like orange and lavender oils can help exfoliate and tone while breaking down excess sebum. On the other hand, sensitive scalps benefit from ingredients such as peppermint, eucalyptus and echinacea to help soothe and calm.
To treat this, Dr. Schlessinger often suggests an over-the-counter product called CLn shampoo. “It treats pores on the scalp and reduces bacterial colonization with sodium hypochlorite.”
Before trying treatments of your own or if the all-natural route isn’t working, Dr. Schlessinger recommends visiting your board-certified dermatologist to rid signs of irritation or discomfort on your scalp. “Many patients are misdiagnosing themselves: There are many people who are dealing with staph aureus and other infections that are being mistaken for dandruff and irritation.”

While TikTok might have you going a week without washing your hair, New York trichologist Shab Reslan says the most common way to throw off the microbiome is by washing too infrequently or insufficiently. “Removing sebum, debris, water and product residue from our scalp is the number-one priority we should maintain for the betterment and health of our hair,” she explains. “Neglecting the cleanliness of our scalp can cause excess buildup that will suffocate the follicle opening of the hair, impede strong hair growth and even influence hair thinning.” To ensure that you’re keeping your scalp squeaky-clean, she recommends using an exfoliating scrub before shampooing once a week. “Shampooing would then be followed by a gentle and hydrating scalp conditioner that does not leave behind any harsh debris that microorganisms would feed on,” Gaunitz adds.

Prioritize the wash cycle The term “you are what you eat” also applies here. “Certain foods like full-fat dairy products and ultra-sugary snacks can worsen scalp issues,” says Kingsley. “Eat foods that contain anti-inflammatory omega-3s, like salmon, sardines and flax seeds, to keep your microbiome at ease and healthy.” For Gaunitz, it’s all about getting the right level of nutrients, “including proper levels of iron, vitamin D3 and zinc.” To get them all in at once, add a daily multivitamin to your diet. If you don’t know where to start, Kingsley says keeping a food diary to track the relationship between what you’re eating and how your scalp is reacting, is a smart move. Over the summer, it’s easy to give our hair a break opting for air-drying methods or quick and easy updos, but as the cold creeps back in, we’re once again relying on our hot tools. Before picking up the blowdryer, straightener or curler, read up on these tips experts swear by for hot-tool hair safety.

CURLY While it’s beneficial to use a heat protectant on any hair type, Lavoy says it’s essential for curly hair. “Less heat means less chance of damage,” he notes. Abergel recommends using diffusers on curly hair in tandem with a hydrating and defining product. “Focus the heat close to the roots while scrunching the length with the diffuser,” he advises.
COILY “I love wrapping coiled hair around a tiny barrel iron to define the curl, giving it a more bouncy, shiny appearance,” says Lavoy. For really coily hair, celebrity hairstylist Kim Kimble suggests using a press comb followed by a ceramic thermal straightener.
HOT TIPS FOR DIFFERENT HAIR TYPES
“To enhance a wave, twist the hair before wrapping it around a curling wand,” Lavoy adds.
STRAIGHT When it comes to using hot tools on straight hair, celebrity hairstylist Adir Abergel says “light product and low heat” is the name of the game. Celebrity hairstylist Johnny Lavoy advises using a stying iron with temperature control and avoiding keeping the hair on the heat for longer than 10 seconds at a time.
If you have easy-to-straighten or heavily processed hair, stick to ceramic, “as it heats the hair shaft from the inside out, preserving moisture and causing less damage on the outer layer,” explains Abergel. Kimble notes that ceramic is also the best bet if you want to maintain your natural curls. “Titanium is very hot, and it may start to straighten it so much that it can change your curl pattern,” she warns. However, if you have coarse or frizzy hair, Abergel says it can handle higher temperatures and will need them to achieve a smooth look. “Titanium reaches hotter temperatures to penetrate the outer layer and reflect shine,” he notes.

BEST HEAT PROTECTANTS…
Hair-care expert Josh Rosebrook advises always keeping the blowdryer moving. “Never hold it static on the hair shaft, especially on a high-heat setting,” he says. “This will cause the most damage very quickly.” Also, avoid touching the nozzle to the hair shaft. Kimble suggests always using a nozzle, as “it directs heat when straightening or smoothing hair out.” Lavoy says a nozzle provides a more precise airflow for quicker drying; it also protects hair from direct heat. For curly hair styling, Kimble recommends opting for a diffuser. Blowdryer brushes, which have been trending big-time over the past couple years, are also great for anyone who struggles to manipulate a dryer and brush simultaneously, especially those with a lot of hair.
COLOR By BurhopBrittanyFallon 2022FALL 100
For many of us, our hair color is a key piece of our identity. I’ve been many shades of blonde throughout my life and never strayed to “the dark side,” though I’ve o en envied my friends who take a more chameleon approach and make drastic changes depending on their mood or the season. The world of hair color is vast and complex, and the list of salon services continues to grow as trends evolve. Here’s what to know before your next appointment.STOCK_COLORS/GETTYIMAGES
BasicsBalayage
“It’s good for those who want their natural color in between their highlights to appear lighter, or those who want to warm up their natural hair.”
Processvs.ProcessSingleDouble
There’s also a service for color correction, which takes the double process a step further. “In situations where someone is going from dark hair to very light hair, they may actually undergo two back-to-back applications of bleach, which is a multi-hour appointment and usually more expensive than a double process,” Perkins says. Celebrity colorist Chad Kenyon adds, “When going from dark to light, it can take an unexpected journey to get to the desired results, aka multiple color sessions. I always explain this to my clients during their initial consultation, which is super important. I ask them to show me inspiration photos of precisely what they want to achieve, and what they don’t want.”

What is a base break?
A French technique used to lighten the hair, balayage means “sweeping,” and tends to blend more naturally with the hair’s base color. “It got its name from sweeping strokes, or lightly freehand painting highlights, and though it’s best on lighter hair, it can be done on any color,” says celebrity colorist Rita Hazan. The roots are left alone and color is applied from the mid-lengths down, creating a sun-kissed, multidimensional effect, adds Lee. As a balayage artist, Kenyon paints hair freehand all day. “This means I do not use foils when creating the highlights,” he says. “Every balayage artist paints with their own technique: some use cotton and plastic wrap to separate the painted pieces with lightener on them from the hair they don’t want to lighten; others freehand like I do. I use clay-based lightener instead of bleach, which forms a protective shell on the outside of the hair. The clay also creates the perfect consistency with which to paint. I love customizing clients’ hair color so that it looks unique and individual.” If balayage is done properly, the hair should grow out seamlessly with no hard lines of demarcation, and as a result, only two to three salon appointments per year will be needed to freshen it up.
For this reason, balayage has a reputation for being lower maintenance than traditional highlights, but Hazan says that’s not always true. “It depends on the color and technique,” she explains. “I would not recommend it for textured hair. I also don’t think you should go into a salon asking for balayage specifically— I think you should show pictures of the color you like and then let the colorist decide which technique is best. Balayage is a technique, not a color.”
According to celebrity colorist Nikki Lee, a base break is when a root color is used to slightly lighten your natural hair color—it breaks up darker roots to help fade the line of demarcation.
One of the most requested color services, “a single process is one color service during one session, and the color can be done all over the hair, or just a root touch-up,” explains celebrity colorist Tracey Cunningham. “A double process is when two color services are required in a session to achieve the client’s desired look, such as highlights with toning, which is very common.” For example, blondes who bleach their roots to get super platinum and then need another 20 to 30 minutes of toning after the lightening process, adds Shvonne Perkins, master colorist for Madison Reed.

Nowadays, there are several options available for covering gray hair, from full-coverage permanent color that generally lasts four to six weeks, to demi-permanent color that blends grays without fully covering them and lasts up to 24 shampoos. Though full-coverage remains the most common choice, “some clients prefer the demi-permanent method because it makes the grow-out process more gradual and softer,” Perkins says.
“However, not all gray hair accepts color easily, and most hair-color products have shades that are specifically designed for maximum coverage to support hard-to-cover hairs. Our Madison Reed Radiant Hair Color Kit offers ammonia- and PPDfree neutral and warm shades with additional pigment for this reason.”

What foilyage?is
Gray Matters
Though not technically balayage, foilyage involves using the same sweeping technique to apply lightener or bleach to the hair, but then each section is wrapped in foil, like traditional highlights, rather than cotton or plastic wrap. “Using foils will allow for more lift because the foils create heat,” says Lee.
Style Smart Excessive heat styling can ruin the molecules in hair color, causing it to dull and fade. “Avoid using hot tools when possible, and always use a heat protectant,” says Lee. “In Common Magic Myst 4-in-1 Elixir is my favorite.”
“You don’t want to spend time and money on gorgeous color and then have your shampoo strip your hair shaft and make your color go dull sooner than it should,” Cunningham says, noting that sulfates are the key ingredient to avoid. Though sulfatefree shampoos get a bad rap for not lathering well, Kenyon likes Olaplex No.4 and says “the lather is luxurious.”
Cunningham says the “cashmere blonde”—a soft blonde that mixes cool and warm hues—and “expensive brunette” trends that went viral last winter are still very popular. “Expensive brunette appears like it is all one shade of brown, but it’s babylights around the face, down the hairline and through the part, and then a lighter brown over them,” she explains. “It looks multidimensional and beautiful.”
Gloss Boss
Kenyon, who always does a gloss after highlights and root or base color touch-ups for clients, says that “even if the overall tonality of the hair looks great, a clear gloss will give the hair super shine. A salon gloss is always the way to go—leave glossing to the pros.” Is a glazethe same as a gloss? Though they may sound different, Kenyon says glosses and glazes are essentially the same thing. “I tend to hear gloss more in Los Angeles and glaze more in New York City,” he notes. “Generally, they sit on top of the cuticle to tone, refresh and add shine. Some brands also make demi glosses that ‘semi-penetrate’ the hair and can even blend grays.”

Lee adds that it’s best to only wash color-treated hair two times a week if you can manage, and to wash with lukewarm or cool water, as hot water can dry out the hair and fade color.

Toning shampoos can be helpful to eliminate brassiness. For those with blonde and gray hair, look for purple formulas, and for brunettes, blue shampoo can banish orange tones.
Glosses can be either transparent or tinted with demi-permanent pigment (not dye), and can be mixed with developer to adjust your specific hair color. They help to refresh the tone of the color, as well as add shine, dimension and silkiness—think of them like a topcoat over nail polish. They last about a month, slowly fading with each shampoo. “The great thing about glosses is that they don’t open the hair cuticle—they preserve the health of the hair and don’t add porosity,” adds Perkins. “So, you’re getting the benefit of the shine and tone without overpowering and drying out the mid-lengths and ends. Gloss is not always automatically included in color treatments, but it is always recommended, so be sure to ask your colorist. A gloss after a root touch-up is the best way to make your hair look salon-fresh.”
“Color treatments can damage and break the disulfide bonds in the hair, so you want to use bond-building products like Olaplex No.3—once a week—that help relink the bonds to repair the hair,” says Cunningham.

By the mid-century, the lifeline for wounded soldiers became a Hollywood staple. Marilyn Monroe, Elizabeth Taylor and Hedy Lamarr were said to have surgically perfected their noses. While pop culture contributed to the rhinoplasty boom, Scottsdale, AZ facial plastic surgeon Kelly Bomer, MD says a good result should “look as if the nose was born to the face in proper proportions, it’s best not to seek another person’s nose.“ By the ‘70s, closed rhinoplasty, where the incision is done within the nostrils, becomes a common technique. “Open rhinoplasty, where the incision is made along the Dr.thepopularnotindescribedwhilecolumella,originally‘20sdidbecomeuntil‘80s,”saysOvchinsky.
Ancient Indians and Egyptians are credited as the first to rebuild noses. In Egypt, thieves often had their noses cut off as punishment. “Ancient inscriptions described a ‘plastic repair of a broken nose,” says Short Hills, NJ facial plastic surgeon Alexander Ovchinsky, MD. In 600 B.C., ancient Indian surgeon Sushruta described a skin graft for nasal reconstruction, says New York facial plastic surgeon Edward S. Kwak, MD: “Detailed descriptions of blood supply preservation and wound care were quite advanced for the time.” The American Academy of Facial Plastic and Reconstructive Surgery counts rhinoplasty as the top facial surgery for women. Here, a glance at our obsession with nose jobs through the ages. —Tatiana Bido Cosmetic rhinoplasty emerged at this time on both sides of the pond by American otolayrngologist Joe Orlando Roe and German facial plastic surgeon Jacques Joseph, who are considered fathers of rhinoplasty and facial plastic surgery. These surgeons refined the basic techniques still used today. Many advances were also made during times of war in the early 1900s to reconstruct facial injuries sustained by soldiers on the battlefield. “Advanced reconstructive techniques were developed by Sir Harold Gilles during World War I, including the use of skin flaps along with cartilage grafts to reconstruct nasal deformities,” notes Dr. Kwak.

The basics of nose reshaping have remained the same over time, however, various variations like ultrasound, revision and ethnic rhinoplasty are popular today.

“Current emphasizetechniquespreserving the nasal anatomy and adding structural support through the use of cartilage grafts,” Dr. Kwak explains. Dr. Bomer adds that results have evolved greatly: “Before, more tissues were removed, which compromised functionality and reduced airflow; today, you can achieve beautiful results and maintain functionality.”
By the mid-80s, nonsurgical nose jobs using bovine collagen and silicone injections were done for mild reshaping and were eventually replaced with safer fillers like hyaluronic acid and calcium hydroxyapatite.
“With this, we flatten the nose rather than cut off the top, which gives a very natural result.“ La Jolla, CA plastic surgeon Robert Singer, MD adds: “More important than the technique, technology or approach used, it is the training and artistry of the surgeon that matters.” Dr. Kwak also stresses that facial balance is key: “The nose is the centerpiece that harmonizes the face; it should not be the focal point of the face.”
“Filler can only correct mild irregularities,” says Stanford, CA facial plastic surgeon Sam P. Most, MD. “You can hide a bump, but you can’t make the nose smaller or lift the tip.” On the surgical front, results like Jennifer Grey’s, which she says left her unrecognizable, ushered in a move toward more conservative results.

“Preservation rhinoplasty is also hot right now,” shares Dr. Most.
Citing breathing issues, deviated septums and broken noses, celebs like Jennifer Aniston, Cameron Diaz and Tyra Banks have attributed their surgeries to improving nasal function.

There are plenty of ingredients that are good to use any time of the day, but not all ingredients are alike. Some simply function better when used at a certain time, while others can be risky if used at the wrong time. Align the following with the sun or moon for the best skin all day long. —Allie Hogan Vitamin C’s antioxidant properties make it ideal for layering beneath sunscreen before facing the day. “Vitamin C is one of the strongest antioxidants we have,” New York dermatologist Julie Russak, MD says. It helps combat free radicals in the environment that our skin is bound to encounter during the day, including smog, smoke, radiation from the sun, and pollution, explains Dr. Blyumin-Karasik. Be cause exposure to these sources is more prevalent during the daytime, vitamin C is re commended for use in the morning. If you use it at night, you’ll be missing this key perk, but you can still reap its brightening and skinimproving benefits. 02 VITAMIN C+ Similar to vitamin C, vitamin E also boasts excellent antioxidant properties. Dr. Allenby notes that vitamin E “inhibits acute and chronic ultraviolet damage, such as photoaging and skin cancer.” She adds that applying vitamin E in the morning rather than the evening “helps bolster these defenses.” Plus, vitamins C and E work synergistically, amplifying the potency of one another, explains Dr Blyumin-Karasik, noting that you’ll likely se e these two ingredients combined in products, which can help streamline a morning routine.

VITAMIN E+ The point of SPF is to prevent the damaging effects of ultraviolet radiation on the skin, explains Davie, FL dermatologist Marianna Blyumin-Karasik, MD, which makes it a non-negotiable in a morning routine. However, not only is wearing SPF at night “superfluous and wasteful,” according to Dr. Blyumin-Karasik, but it can also have adverse effects. Delray Beach, FL dermatologist Dr. Janet Allenby warns that “sleeping in SPF can clog pores and dry out the skin.” In the morning and throughout the day, use a product that has SPF 30 or higher and is broad-spectrum to protect against skin damage, sunburn and skin cancer. 01 SPF+ Day

Sun exposure and retinol are like oil and water— they just don’t mix well. Retinol is known to make skin more sensitive to the sun, says New York dermatologist Marina Peredo, MD. “If you are using retinol daily, your skin will be sensitive to the sun no matter what time you apply it,” so it’s essential to apply sunscreen in the morning even if you used retinol at night. Dr. Allenby warns that applying retinol in the morning without wearing sunscreen can result in increased redness, itching, peeling, burning, and dryness. Background: ”The cause may be a combination of genetics, the immune system, allergies, environmental triggers, and stress,” explains Miami dermatologist Dr. Deborah Longwill. Other triggers include weather changes, environmental allergens, certain foods (like peanuts or eggs), nickel, smoking, and strong scents or perfumes, says Montclair, NJ dermatologist Jeanine Downie, MD. “While it is thought to be caused by genetics and a weakened immune system, it’s multifactorial,” she adds. “We don’t know exactly what causes it.”

Treatments: “Conventional treatments often begin with gentle cleansing and rich emollient moisturization,” says Bloomfield Hills, MI dermatologist Linda C. Honet, MD. “Patients will improve over the years into adulthood, requiring only daily hydration, but some may continue to have flareups and will require persistent treatment and fastidious skin care.”
Description: Rosacea is characterized by reddening of the face. In the early stages, it may look like blushing or a sunburn.
Symptoms: Red, sometimes swollen skin around the middle of the face, forehead, cheeks, and nose Types: Rosacea is categorized according to four subtypes: facial redness, acne-like breakouts, thickening of the skin and eye disorders.
Symptoms: “Psoriasis can present with red, flaky, scaling patches and plaques that cover the knees, scalp, ears, and nails,” says Eagan, MN dermatologist Charles Crutchfield III, MD. “These areas are common, but it can occur anywhere on the body.”
Doctor’s Note: As with all skin concerns mentioned, sunscreen is a must. “OTC topical skin-care ingredients containing extracts of green tea, licorice and daisy can be soothing for their anti-inflammatory effects,” adds Dr. Honet. “Other anti-inflammatory ingredients to look for are niacinamide and azelaic acid.” 03

Medications: “Luckily, we’ve entered a new generation of treatments,” says Dr. Crutchfield, who typically designs a bespoke plan of medications for his patients. “By combining topical and systemic medications—including oral and injectables—and biologic agents, we’re able to achieve 95- to 100percent clearing with most patients.” Phototherapy may also be used to decrease pain and itchiness.

Doctor’s Note: According to Dr. Downie, an anti-inflammatory diet, exercising and decreasing stress will help with these skin concerns. Whatever you do, don’t scratch she says: “Scratching, picking or aggravating your symptoms will make it worse.”
Background: Dr. Longwill says that like psoriasis, we do not know the actual cause of rosacea. “We believe it is due to an overactive immune system, genetics, environmental factors, or a combination of these things,” she notes. “Sun exposure, alcohol, hot weather, stress, wind, freezing temperatures, hot baths, and certain skin-care products or foods can also trigger flare-ups.” Dr. Crutchfield says there aren’t many great at-home solutions other than maintaining an anti-inflammatory diet and avoiding triggers, even though the list is quite long. “Most people only have a few things they need to avoid.”
Treatments: “If the psoriasis is mild and localized to small areas, over-the-counter (OTC) topical medications, such as emollients and creams, lotions and/or shampoos containing salicylic acid, tar, or a mild corticosteroid, may help control flare-ups,” says Dr. Honet.

You may feel like you’re the only person on the planet with facial (or body) features that don’t match, but you’re not. Most of us have some degree of asymmetry somewhere on our bodies, and it is totally normal. But a few millimeters difference can be enough to make some people feel like their face or body is askew. When that is the case, a temporary fix or even surgery may be what the doctor orders.
OccurAsymmetriesWhy
—Dr. Singer Turn to page 145 to find one near you.
2022FALL 122
To understand why asymmetries happen, let’s take it back to human biology 101 for a minute. In utero, two plates fuse together to form the face. However, rarely is there a perfect fit, and often, barely visible asymmetries form. Development continues during puberty, which is why some teenagers notice inconsistencies in their nose or eyes. From there, aging takes its course. Bone density and muscle tone are lost, and fat shifts down the face, leading to more obvious imbalances. “Asymmetries can develop from muscles that are slightly stronger on one side of the face than the other,” explains New York facial plastic surgeon Konstantin Vasyukevich, MD. In addition, injuries, trauma, environment and even lifestyle choices can cause a disproportionate look. “Sleeping on one side of the face, chewing more on one side, weak supporting ligaments, and dental loss can enhance asymmetries, too,” explains Palo Alto, CA facial plastic surgeon Jill Hessler, MD. Poorly done fillers, thread lifts and surgery can also elevate one area more than another. A minor disparity doesn’t mean the face or body is unappealing. “Asymmetries are a part of the beauty of human beings,” says Miami plastic surgeon George Varkarakis, MD. Yet, personal perception determines what we see as asymmetrical, which differs for each part of the face. “The eyes are the most noticeable, even with the slightest difference. However, the cheeks can be quite asymmetrical, yet seen as perfectly fine,” he adds. “Welcome to the town of symmetrywherethe population is zero. Patients expect to be perfectly symmetrical, butthat’susuallynotpossible.”
“The closer we get to the concept of balanced harmony, the more aesthetically pleasing a patient looks,” Dr. Singer explains.

Exist?SymmetryPerfectDoes
So, if we all show some extent of asymmetry, why is it more evident on some than others?
Some procedures can improve facial balance, but minor unevenness will prevail after correction. Attempting to make any feature a replica of itself leads to a contrived look. So, while patients strive for perfection, most plastic surgeons choose to retain subtle asymmetries for natural results.
Almost everyone bears some sign of asymmetry, further solidifying that perfect symmetry does not exist. But, that doesn’t stop individuals bothered by off-kilter features from fixing them.
La Jolla, CA plastic surgeon Robert Singer, MD tells his patients who wish to be more symmetrical: “Welcome to the town of symmetry where the population is zero. Patients expect to be perfectly symmetrical, and while significant asymmetries may be improved, perfect symmetry is usually not possible.”
Beverly Hills, CA facial plastic surgeon Kimberly Lee, MD says asymmetries closer to the midline of the face are less noticeable than those farther to the side. “These discrepancies can become bothersome when the eye notices them as being off, which is usually more than 1 millimeter.”

08 The Lips Dr. Varkarakis says the right side of the midface is usually shorter than the left side, which affects the lips’ position. “The left side of the lip may look more curved than the right side.”
A few filler injections can temporarily correct the pink part of the lips. When filler doesn’t cut it, a lip lift may help. “It provides long-lasting change and corrects the imbalance of the length of the lip,” Dr. Hessler adds. From quick fixes to surgical solutions, the best approach to improving asymmetries depends on where they exist and their size and cause.
Dr. Rayham injects filler into the soft tissue of the face to make certain features look similar to the opposite side. “For example, I fix lip asymmetries by placing more filler in one side of the lip versus the other.” Neuromodulators can also help balance out uneven eyebrows and wrinkles. If you’re more one-and-done, a fatstimulator like Renuva (a matrix injected like filler) can reinstate volume for up to 10 years in areas that inherently contain fat, like the face and breasts. “This is a promising technology, but we still need data of long-term efficacy,” says Dr. Singer.
“Every person decides for themself when inquiring about corrective procedures,” says Brooklyn, NY plastic surgeon Roman Rayham, MD. These are the tried-and-true options to choose from.
Dr. Lee explains if one nostril is lower than the other, raising or lowering one side surgically can improve the asymmetry.

The strategic application of makeup can create the illusion of a balanced face. Celebrity makeup artist Monika Blunder says darker shades recess or push back facial features while lighter shades bring them forward. Using cream concealer, contour and highlighting creams or powders, or foundation, apply makeup to either highlight or downplay those features. If one eye is shorter or droopier than the other, Blunder says eyeliner can help. “Exaggerate the flick on the smaller eye and go lighter on the eye you’re trying to match.”
03 Fat Fat lasts longer than any filler or injectable. Harvesting a patient’s fat and injecting it into a deficient area eliminates inconsistencies in the stomach or thighs while smoothing out areas where it is placed, like under the eyes or in the cheeks. Using fat in the breasts can make them more symmetrical and give a slight boost in volume and size.
02 Injectables + Fillers
Dr. Vasyukevich performed a full-facial rejuvenation on this 55-year old patient, including a facelift, necklift, and upper and lower blepharoplasty. Images simulated to reflect how results would look if symmetrical.
One of the goals of plastic surgery is to create better proportions. In the case of facial asymmetry, Dr. Lee says it’s essential to take photos of the patient and create a 3-D avatar of their face. “Using cutting-edge technology, I split the face in half and put two right sides of the face together and two left sides together to create two mirror images, which makes any asymmetry more obvious. Often, the asymmetry is only off by a couple of millimeters, so when the risk outweighs the benefits, I advise patients of that.” While surgery aims to create as much balance as possible, Dr. Lee never promises perfect symmetry, because she says that, in most cases, it’s nearly impossible to achieve.
Blepharoplasty is the gold standard for correcting eyelid asymmetries. “Removing different amounts of skin and fat from the eyes results in a more balanced appearance,” says Dr. Hessler.
Structural differences, like a deviated septum in one side of the nose, can cause the external to look uneven.
06 The Eyes
Hyaluronic acid fillers are a fast fix for a minor discrepancy in the nose, cheeks, lips, and jawline.
07
The implant route corrects significant facial asymmetries in the jaw, cheeks and chin, but Dr. Lee says the only way to correct asymmetries using implants (usually manufactured to be symmetric) is to use different-sized ones that can accommodate disparities.
Think of the eyebrows like cousins, not twins—they’re not perfect replicas of one another. “In some, patients, the right side of the forehead has more surface area than the left, affecting the position of the eyebrows. Most patients I see have a higher left eyebrow because the frontalis muscle is smaller on that side, so there is more muscle activity,” explains Dr. NeuromodulatorsVarkarakis. can lift the brows to make them appear more even, and a noninvasive treatment like Ultherapy is also an option. “We can use it with varying levels to create lift,” Dr. Hessler says. A browlift may be the best solution for more severe inconsistencies that crowd the upper eyelids.
11
12
The Hips, Flanks + Butt Indifferences in the lower body can make one side appear more prominent. “When patients have asymmetries in the size of their hips or flanks, we can use liposuction to take out fat,” says Dr. Singer. “We can add fat to areas where there are depressions, too.”
New Technology
The first-of-its-kind EmFace treatment employs radio frequency and HighIntensity Facial Electrical Stimulation (HIFES), which Denver dermatologist Joel L. Cohen, says “tones and lifts various areas of the face by enhancing facial musculature.”

“Sometimes, we need to lift one side or add an implant, or use differentsized implants or fat to even out the breasts,” Dr. Singer explains.
The Breasts
It’s not uncommon for one breast to be larger, perkier, rounder or fuller than the other, and even the nipples may not match up. The rules of breast asymmetry are anything but standard, which is why various procedures may be combined to achieve a more harmonious look.
•Routine screening mammography for breast cancer will be more difficult, and implants may rupture during the procedure. Perform self-examination every month for cancer screening and ask your surgeon to help you distinguish the implant from your breast tissue. Lumps, persistent pain, swelling, hardening, or changes in implant shape should be reported to your surgeon and possibly evaluated with imaging
Who can get breast implants?
•Women with active infection anywhere in their body
•Autoimmune diseases (eg, lupus and scleroderma)
•Planned chemotherapy or radiation therapy following breast implant placement
•Rupture of a silicone-filled breast implant is most often silent. Even if you have no symptoms, you should have your first ultrasound or MRI at 5 to 6 years after your initial implant surgery and then every 2 to 3 years thereafter regardless of whether your implants are for augmentation or reconstruction. If you have symptoms of or uncertain ultrasound results for breast implant rupture, an MRI is recommended.

Unlike in our younger years when our metabolisms were working overtime, we can’t wake up on a Monday, hoping for a quick five-pound weight loss by Friday. “As we age, the biggest change we see is a decrease in metabolism, especially approaching our mid-40s,” says Pittsburgh plastic surgeon Jeffrey Antimarino, MD. As our metabolism stalls, it becomes more difficult to lose excess fat. “Hormonal changes are also at play,” he explains. “Women, especially in their late 40s, start becoming perimenopausal and menopausal. This is when estrogen dips and they start to store fat in areas they’ve never had before, mostly in the midsection and upper part of the abdominal wall.”

While we love good transformation stories, ones like Kim Kardashian’s rapid weight loss for the Met Gala draw ire for what some believe is an unhealthy approach. The star boasted dropping more than 16 pounds in less than a month. “The goal is to sustain the weight loss over time, so crash dieting is not a good idea,” says nutritionist Jennifer Hanway. “No one wants to hear it, but if you’ve been gaining weight for the last two years, you are not going to miraculously lose it in two weeks.” Hanway notes that “quick fixes” typically have an opposite effect: “Dropping your caloric intake too much reduces lean muscle mass and lowers your metabolism. It can also lower thyroid function, both of which can slow your weight-loss results.” Here, pro tips for slow and steady weight loss that wins the race. by Tatiana Bido
Cut It Out Reducing or eliminating highly processed foods is the best starting point. “Highly processed foods—candy, chips, cereal, baked goods and even some ‘healthy’ energy or protein bars—are full of sugar, refined carbs and unhealthy fats that can lead to blood sugar imbalances, weight gain and increased inflammation,” says Hanway. “Replace them with whole food options like proteins, vegetables, nuts, seeds, and cellular carbohydrates.”
High + Low A low-carb, high-protein diet with healthy fats and vegetables helps to jump-start weight loss. “It balances blood sugar, boosts metabolism and switches the body into fat-burning mode, even when nothing else has worked before,” Hanway says.

FDA-cleared as a medical device, this twice-a-day pill is made from naturally derived modified cellulose and citric acid. “It’s a really cool concept,” says Dr. Shah. “It works as a fiber matrix, and when you take it before a meal, it expands to about 100 times its size, mimicking the effect of eating two extra cups of food.” However, side effects may include diarrhea, distended abdomen, infrequent bowel movements and flatulence.

Possible Results: “The average weight loss achieved in the clinical studies was more than 20 percent of total body weight, which is comparable to what we see in bariatric surgeries,” says Dr. Shah.
Possible Results: In a clinical study, 6 out of 10 participants lost at least 5 percent of their total bodyweight. Dr. Korner notes the average weight loss is around 6.5 percent. For those struggling to maintain a healthy weight, New York endocrinologist Judith Korner, MD says these new medications have proven to be more effective than anything we’ve had before.
Although both drugs are made from the same manufacturer and contain semaglutide, they’re licensed to treat two different conditions: Ozempic was created to treat Type 2 diabetes, and Wegovy was created for weight management once weight-loss results were noted. Both are administered as a once-a-week injection in the abdomen, thigh or upper arm, and possible side effects include nausea, diarrhea, indigestion, inflammation of the pancreas, and thyroid tumors. “Semaglutide has been around for several years,” says Dr. Korner. “It was developed as a diabetes drug and mimics a hormone our body makes when it senses there is food in the intestine, helping the pancreas make insulin when glucose levels are high. It was later found to slow down gastrointestinal motility, helping us feel full and acting on appetite centers in the brain.”

Possible Results: “In a trial, about 50 percent of people treated with semaglutide lost at least 15 percent of their body weight,” said Dr. Korner. Mounjaro (Tirzepatide) FDA-approved this spring, Mounjaro is made of tirzepatide, and like semaglutide, it mimics hormones that stimulate insulin release and regulate blood sugar; it also signals the brain that you’re full. It has the same list of possible side effects as semaglutide, too. Chicago, IL bariatric and lifestyle specialist Neha Shah, MD says this weekly injection is a game-changer.

While nonsurgical solutions can be beneficial for the appropriate patient, results can be temporary. The gold standard for long-lasting fat reduction is liposuction. “To eliminate larger pockets of fat along the waistline, back, arms or legs, I combine ultrasonic VASER lipo with power-assisted liposuction, which has a rapidly vibrating cannula,” says Dr. Antimarino.
Radio-frequency treatments like BodyTite, EON and SculpSure deliver focused heat that can help reduce fat. “They are good for targeted volume reduction and mild skin tightening. Most patients will need more than one session,” says Richmond, VA plastic surgeon John Zinsser, MD. Other treatments, like Emsculpt NEO and truBody, add muscle building to boost strength while targeting fat.
OFEBRE
Once a healthy weight has been achieved, these noninvasive and surgical options help reshape the body even further.
Despite leading a healthy lifestyle, this 50-year-old patient was unable to lose stored fat around her waist. Dr. Antimarino performed a tummy tuck for a tighter, flatter look.
New York plastic surgeon Daniel Y. Maman, MD says wait a year after maintaining a stable weight and cautions not to treat the arms and thighs at the same time: “Typically I’ll combine surgeries, but treating the arms and legs together reduces mobility during recovery.”
New York plastic surgeon Roman Rayham, MD says patients can address skin laxity after weight loss with breast lifts, tummy tucks, bodylifts, and arm and thigh lifts. “Surgery works best to create a more sculpted look by eliminating fat and excising sagging skin.”

Filler Fix Diet Do INSIDE TIP: Listening to pregnancy-themed podcasts proved incredibly helpful when I had questions about all of the topics mentioned here. Three I enjoyed: The Birth Hour, What to Expect and Mommy Labor Nurse. to the but also improves the of skin-care
What the expert says: “While it’s important to take prenatal vitamins no matter how healthy you’re eating, Daily Harvest and other like-minded subscription services are great options to help pregnant women consume more of the necessary vitamins and minerals—like calcium, folate, protein, iron and DHA—through real fruits and vegetables,” says New York–based nutritionist Amy Shapiro, noting that many women don’t crave vegetables during pregnancy, so this is an easy way to get them in. “It’s convenient because they do all the heavy lifting for you, and the food is frozen, so it always on hand when you’re hungry.” If you love your neurotoxin and filler (we don’t blame you!), it’s advised not to “load up” while you’re trying to conceive with the thought that you’re prepping your skin for the 10 injection-free months that could follow. Believe me, this thought crossed my mind: “I wish I’d gotten a ‘last round’ of line-smoothing before I saw my positive pregnancy test.” However, I’m glad I didn’t because there are risks involved. What the expert says: “Due to the fact that neurotoxins and fillers have not been studied in pregnant women— I would not recommend getting either one while trying to conceive,” says New York dermatologist Dendy Engelman, MD. “But, if you do happen to get them before your pregnancy is confirmed, there is no cause for panic. The dosing of products used for aesthetic purposes are minute. Additionally, studies in the past looking at actual botulinum toxin have found that it doesn’t cross the placenta barrier. However, we always err on the side of caution, so for those who are trying to conceive but still want a more toned look, I suggest using an at-home contouring device like Conture Kinetic Skin Toning Device, which is safe to use while pregnant and trying to conceive. It not only helps restore radiance and firmness to the skin, but also improves the absorption of skin-care products.”
By the third trimester, shaving becomes pretty difficult, as it’s tough to reach your calves and bikini line with a big baby belly in the way. Had I realized this prior to becoming pregnant, I would have opted for laser hair removal. (I had heard from several friends how painful bikini waxes are during pregnancy—one NewBeauty staffer actually left mid-wax because it was too painful to continue.) If you’re not yet pregnant and you have time to fit in a series of laser hair removal sessions—four to six are typically needed—I’d go for it. However, if body hair doesn’t bother you, then by all means, let it be.
What the expert says: “Due to hormonal changes, a pregnant person’s skin can become more sensitive, and plucking or waxing hair can be more painful,” says Rochester, NY dermatologist Lesley Loss, MD, adding that side effects like increased redness, sensitivity and swelling may also occur. “While bikini waxing is generally considered safe, I suggest my patients double check with their OBGYN first, and do not attempt to do it at home. Regarding laser hair removal, if a patient is pregnant, I advise them to stop all laser treatments due an increased risk for hyperpigmentation. I strongly recommend that my patients who are planning to or trying to conceive complete their laser series before there is any possibility of pregnancy.” If you’re not the best fruit and veggie eater, make it easier on yourself by having someone else do the work for you. I craved smoothies throughout all trimesters, and I’ve heard many pregnant women crave cold things because they don’t have a strong smell. MyDaily Harvest subscription was an amazing addition to my daily routine—I whipped up a different flavor smoothie almost every morning—and I felt good knowing I was getting in solid nutrition and “eating the rainbow” before the day really started.

2022FALL 140 “The tendency to get stretch marks may be genetic in some cases...”
You don’t have to wait until you have a “belly” to use belly butter. I felt silly using products like that until I had a visible belly (around 20 weeks or so), but in hindsight, I should have used them from day one to keep my skin super hydrated and promote better elasticity for what was to come. Three products I couldn’t get enough of: Mama Mio Tummy Rub Butter, Kopari Tropical Coconut Melt (it smells like vacation) and HATCH Belly Oil. However, it’s important to note that while it’s essential to keep stretched skin hydrated, unfortunately stretch marks are often genetic, so there’s no magicbullet product to keep them away if your genes suggest otherwise. What the expert says: “The tendency to get stretch marks may be genetic in some cases,” Reno, NV plastic surgeon Tiffany McCormack, MD says. “It may also depend on the size of the baby and the length of the pregnancy. I don't personally believe creams and lotions can entirely prevent them—some products will help improve the overall quality of your skin and may be worth considering, but I would not count on them to prevent stretch marks altogether. If you do get stretch marks and want to minimize their appearance after your delivery, inoffice treatments such as lasers and microneedling can typically help. And in some cases, a surgical tummy tuck can completely remove the skin where most stretch marks remain.”

Your eyes may get really dry, and your vision may actually change, too. Both things happened to me, and I found my contacts to be pretty uncomfortable for several weeks in my third trimester. If you wear contacts or glasses, your eye doctor will probably advise that you don’t come in for your annual appointment during pregnancy, but rather hold off until baby is born (when your eyes will readjust).

Dry Spell
What the expert says: “It is very common for a woman’s eyes and vision to change during pregnancy,” says Oakland, NJ ophthalmologist Diane Hilal-Campo, MD, founder of twenty/twenty beauty. “The surge in hormones causes fluid shifts, which can change the shape of the cornea and lens temporarily, blurring vision. Women who already use glasses or contacts may notice they’re not as effective as they were pre-pregnancy, while those with healthy vision may observe weaker or blurry vision. However, large changes in vision, flashing lights or light sensitivity could be symptoms of pre-eclampsia and you should let your doctor know immediately. Dry eyes are also common during this time, making your eyes red and irritated. Preservativefree artificial tears—popular ones are Refresh and Systane, available over the counter—are the safest to use, and you can use them as frequently as you like, with and without contact lenses.”
However, one time I curled up “the wrong way” and my belly button poked out like a little tent, which startled me enough to keep from doing that again.

What the expert says: “You should not activate your rectus abdominis muscle during pregnancy, especially as your belly expands!” says Silicon Valley, CA plastic surgeon Kamakshi R. Zeidler, MD. “The muscles start to pull to the side around the expanding baby bump, which worsens their separation. After you have your baby, this separation can persist and lead to a potbelly look and feel. Sometimes there is no getting around this effect, but minimizing the amount of strain and separation during pregnancy definitely helps. If the diastasis is significant, a tummy tuck can cinch the waistline and flatten the abdomen. And if you aren’t ready for the commitment of surgery, a muscle stimulator like Emsculpt can help strengthen your core, though it won’t tighten loose skin. ” Though often reserved for long flights, compression socks can make a huge difference during pregnancy after a salty meal, or really anytime your feet and legs feel puffy, or you’re standing or sitting too much. I sit a lot throughout the workday, so I started wearing NUFABRX Maternity Compression Socks around the seven-month mark. Whereas some other brands I tried felt scratchy or itchy, these are infused with shea butter that releases throughout the day for moisturizing benefits. I also liked the brand’s compression leggings with full-belly coverage for extra support during workouts.
You don’t have to go crazy throwing out all of your skin-care products the moment you find out you’re pregnant. It’s true that some ingredients are general no-nos—retinol and retinoids are vitamin A derivatives that are proven to cause fetal deformities in high doses; plant-based bakuchiol is a safe alternative—but some are a little less black and white. I learned this early on about alphahydroxy and betahydroxy acids, as well as chemical sunscreen. What the expert says: “While it is always best to err on the side of caution and talk to your doctor about any specific concerns, many ingredients are completely fine to use in moderation,” Dr. Engelman says. “I do not recommend using salicylic acid while pregnant, though some women do opt to use it in moderation. One method might be to use a cleanser that contains a low percentage of salicylic acid—less than 2 percent—rather than applying it as a leave-on product. However, an alternative would be to use glycolic acid, which is generally safe during pregnancy and can help reduce pore-clogging associated with acne. Another no-no is benzoyl peroxide. When it comes to sunscreen, it’s not the end of the world if you use a chemical sunscreen here and there— active ingredients include octisalate, octinoxate, avobenzone, and oxybenzone—but these formulas are absorbed by the skin and can enter the bloodstream. Therefore, I prefer mineral sunscreens made with zinc or titanium dioxide during pregnancy because they sit on the surface of the skin and do not get absorbed.” I learned later in my pregnancy that I should avoid sit-up motions, like curling up from bed to rearrange my massive pregnancy pillow, to prevent a separation—or worsening of the separation—of my abdominal muscles called diastasis recti. Apparently, it’s better to roll onto your side and get up that way, which isn’t convenient, especially in the middle of the night.
INSIDE TIP: Do a patch test on your inner arm before using any new skin-care product during pregnancy. I was surprised at how my skin reacted—redness and irritation—to a few tried-andtrue ingredients (mainly acids) I had used for years prior.
What the expert says: “Research suggests that nearly two in three women experience mild-to-moderate swelling during pregnancy, which is caused by a growing need for fluid, as well as the growing uterus putting pressure on veins and impairing blood flow. It tends to become most noticeable around the fifth month,” says Mike Drzewinkski, chief science officer for NUFABRX. “Scientific evidence supports the use of lighterpressure compression socks to reduce the swelling, and we’ve taken the concept a step further by adding shea butter to moisturize the skin and mitigate skin issues caused by the swelling. We’re the only company with compression socks that are able to maintain the shea butter inside for up to 25 or more washes through a patented process we call HealthWear.”

Your gums can bleed more often during pregnancy, and because of this, it’s more important than ever to floss regularly. You can also develop little tumors on your gums—they can resemble a raspberry—and though this didn’t happen to me, a friend of mine was freaked out when she discovered one.
You may smell like cold slaw, but there could be worse things.”
10
The “4th Trimester” No One Talks About Be prepared for breast changes.
What the expert says: “Many women experience bleeding and swollen gums during pregnancy, which is referred to as pregnancy gingivitis,” explains Los Altos, CA cosmetic dentist Marlana Shile, DDS. “It is the same as normal gingivitis in that the gums swell, bleed and are more sensitive, but it differs in that during pregnancy, the increase in hormones makes the gums more sensitive to plaque and tartar, so they can get inflamed much easier. Some pregnant women also develop little swollen areas on their gums that look like raspberries and bleed easily. These little swellings are caused by the same irritants that cause the pregnancy gingivitis, and their treatment is the same: The most important thing to do is brush and floss thoroughly to remove any plaque and food debris. It is helpful to inform your dental hygienist that you are pregnant, and even consider having your gums and teeth cleaned more often during your pregnancy to help prevent buildup and the subsequent gum inflammation.”

If you decide not to breastfeed, your milk will still come in, so you may want to have some tight sports bras handy. One trick that helps with breast engorgement as milk comes in is cold cabbage leaves. You simply place them cupped around your breasts, and it is bizarrely soothing.
Don’t be ashamed of pelvic floor issues. They affect thousands of women, and there are experts who can help. “One of my missions is to make pelvic floor therapy part of routine OBGYN care, and to encourage women to rebuild their abdominal and pelvic floor muscles as soon as possible,” says Dr. DeLucia. “It can minimize urinary incontinence, a sense of looseness and other changes that occur after birth. Doing kegel exercises during pregnancy can also help prep the pelvic floor for childbirth.”
“If you are breastfeeding, you may want to get soothing cream or balm for your nipples and nursing pads to avoid leaking,” Dr. DeLucia advises.
In the few months following childbirth, your hormones will go into overdrive and you may feel completely out of whack as you adjust to life with your new addition. These three tips from New York OB-GYN Carolyn DeLucia, MD can help make things a little easier. Create a postpartum kit for your bathroom. “Within the first couple of days after your baby is born, you will feel sore and experience vaginal bleeding,” says Dr. DeLucia, noting that the bleeding is the same after a cesarean birth. “The consistency and color of the bleeding changes over several weeks, which is due to the shrinking of the uterus down to its normal size again. If you had a vaginal birth, you may have stitches, and you will need ice packs or ‘padsicles’—Google this for several DIY recipes. A peri bottle filled with witch hazel is also helpful to squirt while urinating so it does not sting, and it’s a nice way to feel fresh.”
“Nursing bras are also very helpful.
