OnOctober10,2019,IattendedmyfirsteverNorthernCountiesHealthCareAnnualMeetingatBurke Mountain.IwassixmonthsintomyserviceasCEO,stillthenewguy.Icouldnotimagine,onthatfinefallday, thatitwouldbe1,100daysuntilwemetagainasafullteam.

Assomeofyoumayknow,myfulltimedayjobishelpingtoleadpublichealtheffortsservingthenorthern halfoftheNortheastKingdom.SowithmanyVermontersfullyvaccinated,Iwasfullofhopeandoptimism duringtheearlydaysofthesummerof2021.However,asmanyexperienced,Iwasinsteadpresentedwitha lessonofresilienceasweheadedstraightintotheDeltaandthenOmicronCOVIDsurges.Thoughcompletely dedicated,Ineededamomenttoregroup,recenter,andfocusonmyownwellbeingsoIcouldagainpickup thegauntlettocombatthisglobalpandemic.Withsomedaysoffandwithlotsofsummerhikesundermy belt,Ifeltabitmoreresilientandabletofocusonthepositiveimpactpublichealthandhealthcarehad duringtheheightofthepandemic.ThoughIknewtheroadaheadwasgoingtobelongandwinding,thislittle bitofselfcarereinvigoratedmeaswegearedupformoreresponseactivitiesduringthefallof2021.
IlistenedtoMichaelandtheNorthernCountiesHealthCare(NCHC)Administrationshareabouttheimpactsof COVIDonstaffmoraleandresilienceandthedescriptionsresonated exhaustionandburnoutarereal,even forthemostdedicatedhealthcareworkersamongus.Ihadmyburnout,andmyworkinpublichealthwasat leastoneortwostepsawayfromyourfrontlinehealthcare.
ConversationsanddiscussionsofhowtosupportstaffacrosstheagencygalvanizedmysupportofNCHCand alltheteammembersworkingtoachieveourmission:Iwantedtosupportthosegivingmorethan100%in respondingtoCOVIDandcompletingtheeverydaytasksofprovidinghighqualityprimarycare,dentalcare, andhomehealthservices.AlloftheBoardmemberswantedtodoeverythinginourpowertosupportstaff andtheorganizationthroughthisfinalpushoftheCOVIDpandemic
Andatthesametime,Iwasheartenedbythestoriesofourteamssupportingeachotherthroughgooddays andbaddays,andinparticularwhendayswerehard.Thisistrueforourpatients,ourstaff,andour communities weinspiredeachothertobethebestwecouldbeineachmomentandweprovidedcomfort andreliefwherewecould!MostBoardmeetingssincehavefocusedontheneedsofstaffandhowour Administrationdoesitsverybesttorespond witheveryBoardMembersupportinganyeffortstoprovide reliefandsupport.

StaffCompensation:NCHCwasabletoprovideemployeeswithcostoflivingadjustments,an organizationwidebonusrelatedtotheextraordinaryworkcompletedduringCOVID,andanadditional organizationalwidebonustocombatthesignificantimpactofinflationarypressuresstaffatalllevelswere experiencing.
OURMISSION
CommunityHealthNeedsAssessment:TheBoardStrategicPlanningCommitteeinitiatedaCommunity HealthNeedsAssessment(CHNA)thatwouldassesscommunityhealthneedsbothwithinourpatient populationandacrossourhealthservicearea.TheCHNAincludeddatafromqualitativeinterviewsand focusgroups,communityandyouthsurveysthatreachedalmost1200people,andincludedbothpatient utilizationandpubliclyavailabledataandrepresentedacomprehensivelookatthecommunityneedsin theregion.
StrategicPlanning:UsingthedataandresultsfromtheCHNA,theStrategicPlanningCommitteestarteda strategicplanningprocessthatisnearingcompletion.Areasoffocusincludeprimarycareaccess,mental health/substanceusetreatmentaccess,staffrecruitment&retention,financialhealth,anddiversityand inclusionTheBoardandAdministrationhopetoshareideaswithallofNCHCandthecommunityatlarge intheupcomingyear.
Governance:OurBoardhascontinueditsstudysessionsmodeltohelpprovidebothBoardeducationand additionalopportunitiestodivedeepintocriticaldecisionsbeforecomingtotheBoardforavote.Thishas helpedincreaseBoardcapacityaswellasimprovedBoarddecisionmaking.Additionally,theExecutive Committee,withallnewofficers,beganmeetingwiththeCEObetweenBoardMeetingstoprovideinput andsupport,getupdates,andplanforthenextmeeting.ThishasbeenbothavaluablesupporttoMichael aswellasprovidingmoreopportunitiestocultivateadditionalBoardleadershipwhichwillhelpensure successfulBoardsuccession.
OUROPERATIONS
Facilities:AttheendofOctober2021,thefullyrenovatedConcordHealthCenterreopenedwithsignificant upgradesandanexpansion.Thissummer,theDanvilleHealthCentertemporarilymovedtotheSt. Johnsburylocationwhiletheirfacilityunderwentupgradesandtheteamwilltransitionbacktothenewly renovatedandexpandedhealthcenterinNovember2022Finally,thefacilitiesteamdidanamazingjob coordinatingalltheworkneededtolaunchanewNorthernExpressCareinNewport,Vermont.
IT&Cybersecurity:TheITteamsupportedthetransitionofanewElectronicMedicalRecord(EMR)system fortheHomeHealth&Hospiceteamandareintheprocessofworkingwithprimarycaretoidentifyanew EMRforprimarycareservices.Thisworksupportsimprovedoperationsandtheincreasedabilityto documentservices,whichisneededtodrawdowngovernmentfunds.Additionallythiswork complementsalltheworkofincreasingoverallcybersecuritywithintheorganization makingfor seamlessoperationsandincreasedprotectionsrelatedtopatientinformation.
NorthernExpressCare:Thisyear,inpartnershipwithNorthCountryHospital,NCHCopenedNorthern ExpressCare-Newport.Thepracticehasopenedwithexcellentfeedbackandhighdemand,showingjust howimportantthisadditionalprimarycareaccessistotheNewportcommunityTheStJohnsbury locationiscontinuingtohelpmeettheneedforwalkincareandhasalsobeenexperiencinghigh utilization.Bothlocationsareprovidingexceptionallinkagestoprimarycare.

FinancialHealth:TheNCHC2022financialauditdemonstratedstrongperformanceoverthepastyear wherefinancialhealthincreased,despitepressuresofCOVID.OurCFOhasworkedactivelywithour BoardFinanceCommitteetomoveorganizationalinvestmentstomaximizereturnsoninvestment. Additionally,theBoardandAdministrationwillbeworkingonlongtermplanningtoensurethatNCHC’s financialhealthremainsstrong.

SocialDeterminantsofHealth&HealthEquity:NCHCcommitteditselftoworkingonpopulationhealth andcreatedabrandnewleadershippositiontoaddressthisinourcommunities.NCHCtookon leadershiproleswithinNEKProsper!andhelpedtoestablishVibrantONE(OrleansNorthernEssex) two groupsintheNortheastKingdomdedicatedtousingcollectiveimpactmodelstopositivelyimpactthe socialdeterminantsofhealthaswellastoaddresshealthequity.NCHChasprovidedvaluableleadership ineachofthesegroupsandmostrecently,inpartnershipwiththeVermontDepartmentofHealth, providedover$300,000ingrantfundstolocalorganizationsproposingprojectstoaddresshealthequity andimprovethelivesofNortheastKingdomresidents.
ARPAFunds:NCHCisutilizingAmericanRescuePlanAct(ARPA)fundstoexpandcommunitybased dentalservicesandprimarycarebypurchasingmobiledentalandprimarycaretrailers whichwill createnewandexcitingopportunitiestoincreaseaccesstocarethroughouttheservicearea.
