Word Choice: Clients & Consumers

Our Response Thank you for your letter, Pat! At the Training Partnership, we are often asked about why we use the term “Consumer.” The word comes from scholars and advocates of people receiving care who prefer the term Consumer, rather than patient, client, customer, employer, or service-user. Their thinking is that “Consumer” emphasizes the Consumer’s self-determination: their conscious choice to receive services, as well as their control over them. However, we know the word also brings up images of “consumption” and department store checkout lines. We recommend always to ask those around you what they prefer to be called – but consider Consumer a safe default. We also do our best to reflect that choice in the stories in this magazine.

just read the latest InSight Magazine and appreciate the many and varied stories of other caregivers and their challenges. It has been the most fulfilling of all of my experiences (formerly an ESL teacher). People helping people with both professional and personal connections is, to me, what means the most in this life. My only concern is that of the changed terminology: “Recipients” (of care) became “clients” and are now to be referred to as “Consumer.” Where did we lose the human touch and personal dignity of the special people that we serve? 6 Put two fingers under the cuff of the second glove and pull the glove down so that it comes off inside out. Twist the top of the glove as your remove it. Both gloves should be inside out after you remove them. CLEANUP - Place the

soiled glove in the trash and wash your hands.

Breathe In As a Home Care Aide, it can be difficult to carve out time for yourself. You spend your days caring for others, and the stress of the work can build up over time. Try taking a few minutes in the morning or at the top of your day to practice this simple breathing exercise. It can have a big impact on reducing stress and the exercise acts as a natural tranquilizer for your nervous system to ease tension. The best part is that it’s free, simple, and can be done anywhere.

4-7-8 BREATH RELAXATION EXERCISE

1. Exhale completely through your mouth, making a whoosh sound. 2. Close your mouth and inhale through your nose, mentally counting to 4. 3. Hold your breath for a count of 7. 4. Exhale completely through your mouth for a count of 8. 5. Repeat the cycle again for three more breaths.

hen the phone rings at Full Life Care, identical twin sisters Dana Beckwith and Danielle Aumoeualogo have to be ready to answer any number of questions from Home Care Aides in the field. Beckwith, an HCA and home care administrative assistant, does “telephone triage” and remembers a particularly intense call. “I had a Home Care Aide call and her client was responsive. But he was in a state where he needed medical attention and she didn’t know whether she should call (911) or not,” Beckwith says. “I was able to just talk her down, calm the situation, give her some practical advice and we got him to the hospital in time.” Beckwith said if she had not been in the field herself as an HCA, she might not have known how to handle the situation. Both Beckwith and Aumoeualogo began as Home Care Aides and now draw on that knowledge and expertise to provide administrative support to other HCAs. Aumoeualogo, a field supervisor, prepares new caregivers for what the job is really going to be like during the interview and orientation. After a new hire was upset about the state of the building her Consumer was living in, she turned to Aumoeualogo for advice. “I was able to sit down and tell her, ‘OK, you’re in home care. So you got to be on your guard, but you also got to expect the unexpected and be prepared for it,’” Aumoeualogo says. By the time they finished their conversation, she was able to encourage her to give it another shot and return the next day. “It is kind of scary when you start and you go out there on your own, into the client’s home,” she says. “I know that everything I’ve gone through as a

Home Care Aide and the position that I’m in now, I can actually help. And the caregivers that I’m helping and that I’m speaking with, they know, ‘OK, I’m not out here by myself.’” It took some trial and error for the sisters to find their groove in home care. Coming from a family with multiple nurses, Beckwith and Aumoeualogo initially started down the path of nurse and medical assistant programs and worked at nursing home facilities. Beckwith says, “You don’t really get to find out about patients’ lives and who they are as people. You’re there for a couple of hours and then you’re gone.” Aumoeualogo added. “It was almost like a factory; people coming in, people coming out. After a year doing that, it just wasn’t satisfying.” Beckwith introduced her sister to Full Life, where working as Home Care Aides provided the connection with Consumers the sisters were looking for. Aumoeualogo still warmly remembers her first Consumer, Eileen. “I got to really learn about her and her family and her culture and traditions,” she says. “And she taught me how to make homemade tortillas.” Beckwith continues to work with a Consumer on the weekend on top of being a peer mentor to new HCAs. As for building on her career, she says, “I

could see myself moving further in the company. This is home care and every day you learn something new. That’s a plus in my book.” For Aumoeualogo, the experience of connecting HCAs with resources and helping problem solve has opened up a possible new career pathway. “Since I’ve been here in the office, it’s got me thinking about doing some social work. So I’m thinking about going back to school for social services.” It was news to Beckwith, who turned with surprise to her sister, “I had no clue! I’m so happy for you!” As the sisters laughed together over the news, Aumoeualogo said, “It’s been something I’ve been thinking about. With this field supervisor position, I get to go out and assess the home and the client. So I’m excited about that experience.” For new Home Care Aides, the sisters recommend patience and problem-solving above all. “You got to be able to do a lot of problem-solving and finding solutions,” Beckwith said. “And to realize that the clients have lives before we got there. And some are flexible and some it just depends on the level of care they need.” Aumoeualogo advises, “Even when it’s your hardest day, just remember

AFTER THE

TRAUMA Home Care Aide Tamara shares her expertise in caring for a Consumer with post-traumatic stress disorder.

H

ome Care Aide Tamara works with a Consumer who has never served a day in the military, but her experience with post-traumatic stress disorder (PTSD) is no less intense. Tamara knows all too well. For the last year, she’s worked as a caregiver for a survivor of domestic abuse (name withheld for privacy), who was diagnosed with a condition commonly associated with military veterans. “She used to be on heavy medication,” Tamara says on a sunny spring Friday. “But she’s growing, and she’s getting healthier.” The Mayo Clinic defines PTSD as “a mental health condition that’s triggered by either witnessing or experiencing a terrifying event.” The initial trauma can include anything from sexual assault to traffic accidents to experiences related to war. As relayed in countless movies and television shows, symptoms include nightmares, drastic mood swings, and flashbacks. A condition that affects the memory and psyche, long-term effects of PTSD can include personality disorders and identity alteration. For Tamara’s Consumer, it’s been years of “growing and getting healthier.” But it was only in 1987 that the official causes of PTSD were expanded to include a host of experienced, as well as witnessed traumas, such as domestic violence. “She acquired PTSD at a young age,” Tamara says. “Where is the help? How do you get out of a situation like that and never get back into it?” Tamara describes all the ways in which her Consumer’s condition has affected her life. She has difficulty trusting the people around her. Building a support system for Consumers like Tamara’s can be difficult, since every interaction is a potential trigger of their past. “People who talk about their own situations can trigger her in thinking about her own safety,” Tamara says. “She can be very paranoid and her senses are very heightened.” The eldest daughter in a family of nine, Tamara’s father passed away when she was just 10 years old. She credits her childhood—as well as her Oglala

Post-traumatic stress disorder can occur after you have been through a traumatic life event. A traumatic event is something terrifying that you see, hear about, or that happens to you.

I’ve just always seen how people have to take care of each other. -Home Care Aide Tamara Sioux ancestry and her background in livestock management—for her patience as a caregiver. Tamara says. “Through my life, I’ve always been a nutrition-conscious, healthy person. I worked 10 years as a machinist, another 20 years in construction, and I got my livestock master’s. I’ve just always seen how people have to take care of each other. Even in construction, you have health and safety concerns, and you need teamwork. You have to be

sensitive to who can’t deal with heights— with who might get motion sickness.” In Washington state, caregivers like Tamara are not alone in supporting Consumers with PTSD. The array of treatments for the condition are as diverse as the range of PTSD triggers; some of which are related to war-time experiences, while many aren’t. In Washington, traditional psychiatric treatment—often facilitated by mental health professionals—exists alongside novel methods such as medicinal marijuana. HelpGuide.org provides extensive online resources for caregivers supporting Consumers with PTSD. They recommend patience, self-education on the condition, and not pressuring your Consumer into seeking help or talking about their experiences. According to their guide, they recommend framing professional treatment in a positive light. “Emphasize the benefits. For example, therapy can help them become more independent and in control. Or it can help reduce the anxiety and avoidance that is keeping

them from doing the things they want to do.” Located in Seattle’s Central District, The Samarya Center is pioneering a form of yoga treatment known as Integrated Movement Therapy. Molly Lannon Kenny is the center’s founder and director, and she advocates for a holistic method of healing for patients with conditions stemming from PTSD. “At Samarya Center, we provide a sense of wholeness, and we take judgment out of the equation,” Kenny says. “The most healing thing for people is to acknowledge that they’re valuable, and to let them know that nothing they’ve experienced or seen can take that away from them.” Kenny reasons that care is a two-way street, and insists that her instructors take an active, receptive attitude to caring for anyone who comes to The Samarya Center for treatment. “Our job as helpers,” she continues, “is to keep our minds open to complexity, and to keep our hearts free of judging others for situations that may’ve led them to acquire PTSD.” Kenny points out that,

PTSD can occur after a traumatic event that you see, hear about or that happens to you, such as: • Sexual or physical violence • Serious accidents, such as car wrecks • Combat exposure • Natural disasters • Physical injury • Emotional Abuse

“Getting better” means different things for different people. But treatment such as counseling, and sometimes medication, can help you cope with symptoms from interfering with everyday life.

1. Reliving the event You may have bad memories, nightmares, or feel like you’re going through the event again. This is called a flashback. 2. Avoiding the situations that remind you of the event You may try to avoid situations or people that trigger memories of the trauma. You may avoid talking or thinking about the event. 3. Negative changes in beliefs and feelings You may feel fear, guilt or shame. Or you may not be interested in activities you once enjoyed. 4. Feeling keyed up (or hyperarousal) You may feel jittery, always alert and on the lookout for danger. Or you may have trouble sleeping or concentrating. “The thing about a PTSD diagnosis we often don’t talk about is that it isn’t just about something you’ve experienced, like war or sexual trauma. It can also be the result of something you witnessed.” Kenny believes that as a society and as individuals, our self-worth is constantly in danger. It is one reason why mental health is as important for Home Care Aides as it is for the Consumers they care for. Tamara recommends that HCAs caring for Consumers with PTSD look into resources for their own self-care. “Many Consumers with PTSD are on medication and not in good physical health,” she says. “When they’re stressed, you get stressed too.” The U.S. Department of Veteran Affairs has an extensive online PTSD Caregiver Support library at www.Caregiver.VA.gov. The VA also encourages HCAs to create space for themselves and acknowledge how caring for someone with PTSD can affect them. own boundaries, and about what matters to you.” Taking time to pay attention to those feelings through journaling, meditation, talking to a friend, or therapy can help HCAs process their work as caregivers. With imaginations weaned on cinematic spectacles that celebrate the veterans’ valor, so much of our nation’s attention when discussing PTSD is focused on wartime experiences. And rightfully so. But trauma can also be tied to verbal abuse, sexual assault, and poverty, which are equally important causes of PTSD. Caregivers like Tamara are doing the work at home to make sure all citizens have a quality of life worth defending. Tamara says, “This isn’t just about PTSD. As a Home Care Aide, you know the importance of staying healthy. You know about preventive health visits, dental care and the importance of good nutrition and being active. But when you think about staying healthy, is your mental health part of the equation? Mental health is just as important to your overall health as physical health, although many people don’t think of it that way. In fact, according to the federal Office of Disease Prevention and Health Promotion, “Mental health plays a major role in people’s ability to maintain good physical health. Illnesses, such as depression and anxiety, affect people’s ability to participate in health-promoting behaviors. In turn, problems with physical health, such as chronic diseases, can have a serious impact on mental health and decrease a person’s ability to participate in treatment and recovery.” So why do so few of us take our mental health seriously? While stigma and lack of knowledge can be part of it, many people don’t know that help is available to them. Mental health support and treatment is now required by all insurance carriers, due to changes brought by the Affordable Care Act (ACA). For those receiving care through Apple Health or the Washington Health Benefit Exchange, Sound Mental Health is one of the largest mental health providers in the Puget Sound region. Steve McLean, director of communications, says, “Because of the ACA and Apple Health, many who could not afford quality mental health care previously, now have many resources available.” For members of the Health Benefits Trust, mental health care and treatment is a key part of the benefits that members receive.

For Health Benefits Trust members, mental health care and treatment can look like a number of things: • One-on-one therapy with a mental health counselor • Medication • Psychiatric care • Group therapy • Support from your Primary Care Provider Often, the first step to getting care is talking to your Primary Care Provider. Let them know you would like to access your mental health benefits and that you are aware of the different options available to you. With your input and preferences, they can help guide you to the care that is best for you. Sarah Stuckey, a Group Health behavioral health expert, says one of the biggest challenges for anyone seeking out mental health help is the stigma or stereotypes they perceive as being associated with it. “[People worry about] how people are going to see them, especially when the issues are around substance abuse.” The second biggest hurdle is trust – they are hesitant to share personal feelings with people they don’t know. “They say, ‘People don’t know me, they’re not going to understand,’” Stuckey says. Stuckey says that changes in how people behave and trouble sleeping are a couple of the most common signs that something might be amiss and that it may be time to seek a mental health expert to talk with. Other signs include increased irritability, more anger than usual, and decreased hunger or overeating.

D

id you know that agency-based Home Care Aides are 450 percent more likely to report a workplace injury than Individual Provider HCAs? When you provide care to a family member, friend, or Consumers, it can be difficult to know when an injury happens “at work.” A good rule to go by: If you are injured performing any tasks on your Consumer’s Care Plan, you can file for worker’s compensation through the Washington State Department of Labor & Industries (L&I). Here are the most common injuries HCAs reported in 2014. n

Overexertion Back injuries or other types of muscle strains from lifting or transferring a Consumer are easily the most common HCA injuries. Practicing proper body mechanics learned in Basic Training will go a long way in preventing injuries. But when you do overexert yourself, an L&I claim can help you get the care you need to prevent further injury.

Assaults by Animals While a dog or pet can provide valuable companionship to a Consumer, he can also be an occupational hazard for many Home Care Aides. If you are on the clock in your Consumer’s home, a dog bite, tripping over a pet or other type of animal-related injury can be covered by an L&I claim.

The categories below are common types of injuries you can file on an L&I claim. However, even when you cannot name your injury, your doctor and L&I can help you file your claim.

Falls and Tripping Loose cords and rugs are common tripping hazards in a Consumer’s home. Discuss moving these items with your Consumer so they are out of the way or secured to prevent falls. Struck by Object Objects can often fall from a shelf or be bumped into accidentally. For regularly used items such as cooking oils, try moving them to a more accessible spot in the kitchen versus a high shelf above the stove. It is also common for HCAs to be hit by objects thrown or knocked over by Consumers. It is important to report your L&I claim so that your medical bills can be covered and you stay safe.

Repetitive Motion These are tissue injuries that occur from doing everyday tasks over and over, such as scrubbing floors and ironing, and reaching, lifting, and pulling objects. Symptoms can include pain, tightness, dull ache, and throbbing. Your doctor can help diagnose this type of injury and help you file the appropriate L&I claim.

Bodily Reaction Similar to overexertion, bodily reaction is an injury from bending, pinching a body part, reaching, or standing without necessarily falling. For example, an injury could occur when lifting a heavy laundry basket or trying to catch a Consumer mid-fall.

Car Accident If you are in a car accident while transporting your Consumer or if you are on your way to or from providing care, you can qualify for worker’s compensation. Even though you may not be on the clock, an injury during your commute can be covered by L&I.

Steps to Reporting Your Injury When you are injured at your Consumer’s home or your own home as a family caregiver, report your injury to Washington State Department of Labor & Industries (L&I). If your claim is accepted, you will receive worker’s compensation to pay for medical care directly. If you are unable to work following your injury, you may be eligible for a portion of your lost wages.

Step 1: Injured at Work

Claims can be rejected if the doctor cannot certify your medical condition is related to something specific that happened at work or an occupational disease.

Get first aid. Many falls, cuts, and sprains can become serious injuries if they aren’t treated right away. If you are injured at work, go to your doctor, nearest Urgent Care center or, for severe injuries, the Emergency Room. Let them know you were injured on the job and they will assist you in filing the worker’s compensation claim. Let your employer know right away about your injury.

For IPs: Ask your doctor for the “Report of Injury or Occupational Disease” form. List your employer as: HCQA Negotiated Contract 601 Union St., Suite 3500, Seattle, WA 98101.

Step 2: File a Claim You can file an accident report for your injury at your doctor’s office, over the phone to the L&I office at 1-877-561-FILE (business hours only), or online at www.LNI.WA.gov. Remember to: • Complete all required paperwork and keep copies of everything for your records. • Read all paperwork carefully and respond promptly. • Put your claim number on your paperwork and have it ready when you call L&I and your employer. For IPs: A company called Sedwick CMS will manage your claim. Contact them toll free at 1-866-897-0386.

Step 3A: Approved Claim L&I will approve your claim if your doctor certifies that you were injured at a specific time and place at work, or if you have a disease or disorder caused by your work. Benefits cover medical bills. They also may include wage replacement, return-towork help, and disability for the severely injured.

You and/or your doctor have the right to protest any decision made about your claim online at www.lni.wa.gov. Appeals can be made directly to the Board of Industrial Insurance Appeals.

Step 4: Get Back to Work Some injured workers miss days of work while they recover. However, many can return to work gradually, while still receiving medical benefits. Returning to work as quickly as possible is a team effort between you, your doctor, and your employer. Stay in touch with them. L&I will provide assistance when you need it.

Step 5: Close Claim Claims are closed when: • Your doctor certifies that further treatment won’t improve your condition. • L&I has no new information showing you need further assistance. • Your treatment was successful! (In these cases, L&I accepts and closes your claim at the same time in a “Notice of Decision.”) HBT Members: Your insurance coverage through the Health Benefits Trust will NOT be affected by reporting your injury to L&I. You can continue to use your current benefits while collecting additional benefits from L&I.

But healthcare professionals are starting to think twice about that and Home Care Aides (HCAs) should, too. New data show that the tendency to choose the ER over local Urgent Care centers costs patients and insurance networks millions of dollars. More importantly, many conditions may also be treated better and faster at your Primary Care Provider or Urgent Care. Less personal care and higher costs to patients are just some of the pitfalls of an unnecessary ER visit. Headaches, common colds, back pain, and urinary tract infections are the most common reasons patients go to the ER when an Urgent Care or doctor visit would be a better option, and less expensive. ER for something that could be better treated elsewhere, the Health Benefits Trust sends the member a postcard reminding them of other options. Brooks reminds us that people with threatening and debilitating issues such as compound fractures, severe burns, and uncontrollable bleeding are always advised to visit the ER. But Urgent Care or Primary Care Providers can best treat allergies, migraines, and sore throats, where the insurance copay will only be $15 (Group Health) or $30 (Kaiser Permanente), rather than $200 for the ER. There is also a 24-hour nurse consultant line that HCAs can call for advice about what action to take. “This is an issue that costs members millions of dollars in unnecessary claims,” Brooks says. “And it costs the HCA $200 outof-pocket to go to the ER, when the majority of the symptoms that lead people to go there can be treated elsewhere.” But if we find the best places to get care instead of the ER, it could result in a savings of at least $13 million annually in the Puget Sound region and bring costs down for all HCAs in our community. You said you had to sneak out of El Salvador when you left. How did you do that? Yes, I did sneak out. I hired a man to get us out. I paid him $3,000 for each person. My mom, my two daughters, my baby son, along with my sister-in-law and her children, and I left after dark. The man I hired drove us to Tijuana. We had to walk to the United States from there. We went through a tunnel and into the mountains where men with guns robbed us. They took $600 from us but thankfully they let us go and didn’t hurt my children or me. We made it across the border and into San Diego. But then we got caught by immigration officers. We were all arrested and spent the night in jail. We were so frightened we would be sent back to El Salvador. Because we asked for political asylum, they let my brother bail us out the next day, but it cost him a lot of money. We were so thankful they let us go. We got work visas and jobs in Texas. I took the Fundamentals of Care Giving course, but caregiving was such a low paid job in Texas I had to find another way to make a living. So I got a job in a dry cleaner. My grandmother had taught me to sew when I was young and I could do alterations for the dry cleaner. Were you granted asylum in the U.S. at that time? No. Immigration wanted more proof that I was in danger in El Salvador. I told my story

but I had no one to back it up. There was no one left in El Salvador for Immigration to contact about me. So political asylum was denied. When my immigration officer said my work visa was going to expire, I was afraid of being deported. That scared me but I kept this from my children. At that time the church had a program to help people from Central America apply for political asylum in Canada. So the church helped us fill out all the paperwork and Canada gave us permanent resident status. We lived in Canada for six years. What was Canada like for you? Canada was good. They had programs to help immigrants get established. For example, we could have gone to school in Canada to learn English and the government would support us for a year. But we thought since we were there and we were safe, we should provide for our family and not take support from Canada. So we worked very hard at two jobs and we didn’t have time to go to the English classes. I did take the Home Care Attendant course there. It was six months of training. But now we think we made a mistake. We should have taken the English classes too. Our grandchildren tease us now because we speak part Spanish and part English. We call that Spanglish. Do your children and grandchildren speak both languages?

My children do, and my younger grandchildren, too. But the older grandchildren, no. They only speak English. Their father was American and he thought the children should speak English. Now we realize that knowing both languages would be valuable to them. My granddaughter works as a manager and she is learning to speak Spanish now because it helps her in her job. It is good to be bilingual. How did you get started in healthcare and caregiving? I had three sisters-in-law who were nurses. When I was only 13, one of them took me to the hospital with her. She taught me how to give injections and how to draw blood, which you could do at that time without a license. I spent a lot of time in the hospital visiting my sister-in-law and learning from her about nursing. After I started nursing school, my father had five strokes and got sick. I had to take care of him and that is when I decided the kind of work I wanted to do was caring for others. How does your experience as an HCA compare to working in a residential facility? In the residential facility there are schedules for most things, like showers, time to get up, mealtime, and giving medication. We take them to the dining room and back to their rooms and help get them ready for bed. We do their laundry and help them


dress. I do those same things for my Consumer at home, but in the facility we have to do it according to the schedule. At home, they get to decide when they want to have a shower, get up in the morning, or eat. It is more flexible. At the facility we don’t take the residents shopping, to doctor appointments, or go out for walks. In home care, we do those things for our Consumer. We also cook and clean for them. I think there is more interaction with them at home. I can suggest things to do since I don’t have to keep a certain schedule. I can take my home client outside for a walk or to enjoy the beautiful day or take her to the mall if she wants to go. The HCA has a more personal relationship with their clients because of being in their home on a daily basis. What is the hardest job you have to do for your residents? The hardest thing I have to do is wake them up in the morning. Some of them don’t like to get up early but they have to get up even if they don’t want to. I have to help them get dressed, wash and do their teeth, and get them to breakfast. I have to keep their schedule. What techniques do you use to connect with your Consumers? When you open the door to the clients you have to leave all your personal prob-

lems outside and bring a big smile to them. All the worries in my life, they don’t need to know. They are expecting support from me. Sometimes they are depressed or sad and they don’t need to hear the problems I have with my husband or children. They have the same kind of feelings for us as we have for them. And our problems make them worry and sometimes they feel like they have to help. We are there to help them not the other way around. What do you miss most about El Salvador? A lot of things. Good things. Authentic food. Visiting family and celebrating with them and going to the beautiful beach we have. And I miss the weather. The weather is so beautiful there. We celebrated Christmas and New Year’s and Good Friday with a big party at the beach. What traditional recipes do you like to make? Pupusas! It is a very famous and traditional food and everybody likes them but it’s not so easy to make. It’s made with masa harina (corn flour) and it’s like tortillas but thicker and softer and filled with chicharron (pork). The chicharron is the hardest part but you can make them with cheese and beans. Or it can be made with different fillings, like bell peppers or even zucchini. We have a dessert called Arroz con Leche (rice and milk), and it’s

good and sweet. Yuca (cassava) is a salty dish. You can boil it or fry it and put it with coleslaw and salsa. The skin is hard to peel but you can buy it at a Mexican store already peeled. What is the most rewarding part of being a Home Care Aide? When I make someone laugh with my crazy sense of humor. I like to joke around with them. I say, “Hey, it is shower day and it is raining outside, so let’s go out and get your shower.” That makes them laugh. I have one tiny lady who is 98 and she has to take half a tiny pill, so I tease her. I say, “I have a tiny pill for a tiny lady. Who’s this tiny pill for?” She gets so excited and says, “Me, me!” and she laughs. What advice do you have for other HCAs? This job (home care or residential care) is to help us to survive but it is a job that requires feelings. If you don’t have the feelings (compassion), don’t do this job. I’ve seen both sides of good and bad in coworkers. I heard one coworker say that this elder lady is crazy. She is not crazy. She has dementia. Don’t get stressed if your client repeats the same story every day. Just enjoy them. I really love this job. I even recruited my husband. He is now a caregiver too. Pupusas PUPUSAS DE QUESO CON ESPINACA (SPINACH & CHEESE PUPUSAS) Servings: 8 pupusas Total time: 30 min Cost: $3.50 (based on avg. food prices) Ingredients: • 2 cups masa harina (often in the Mexican food section of most grocery stores) • 1 cup warm water • 1/2 cup chopped spinach • 1/2 cup shredded white melty cheese (Monterey jack, mozzarella) • 1 tomato, diced • 1/4 cup onion, diced • 1/2 tsp salt • Olive or vegetable oil 28

Directions: 1. In a large bowl, combine masa, water and salt. With lightly oiled hands, mix until dough comes together into a moist but firm ball. 2. Separate dough into 8 equal pieces. Roll each into a ball with your hands, adding water if cracks form. 3. Use your thumb to create a small pocket in the center of the ball. 4. Mix together spinach, cheese, tomato, onion and salt in a small bowl.

5. Fill about 1 1/2 tbsp of the mixture into each ball. Pinch the edges of the dough together to close the hole. 6. Flatten your ball of masa by gently shaping the ball with your thumbs and palms. 7. Heat about 1 tbsp of oil in a skillet over medium-high heat. Place a couple pupusas at a time in the skillet and cook both sides about 2-3 minutes each. You are looking for a nicely toasted, light-brown crispiness. 8. Serve pupusas warm with salsa, curtido (pickled cabbage), sour cream or your favorite toppings.

T

hough Mexican-Americans are the largest and most visible immigrant community in the United States, nearby El Salvador has a unique and lesserknown history and culture. The country is the smallest and most densely populated in Central America – home to 6.3 million people. Imagine the entire Washington state population living just on the Olympic Peninsula. Salvadoran cuisine features rice, tropical fruits, coffee, sugarcane, and a Central American staple: maize, or corn. With these ingredients, El Salvador has become widely known for its signature dish, pupusas. (Try your hand with the delicious recipe on the left.) The majority of El Salvador’s population is Christian, largely divided between Evangelicals and Roman Catholics. Many of the country’s iconic figures have been beloved religious leaders, such as Archbishop Oscar Romero, who was assassinated after speaking out for farm workers and for justice. In the wake of a bloody civil war, a slew of natural disasters, and a recent exodus of unaccompanied minors, there are now over 2 million Salvadoran immigrants currently living in the United States. After independence from Spain in 1821, El Salvador built up an elite oligarchy that centered around the main agricultural exports: indigo and coffee. The Salvadoran Civil War in 1979 sparked a surge of refugees fleeing violence

and economic turmoil. The conflict began between the militaryled government of El Salvador and the guerilla groups of the Farabundo Martí National Liberation Front (FMLN). The war saw nearly 12 years of violence using tactics such as death squads, kidnappings, and child soldiers. Over 75,000 were killed, and thousands disappeared. By the end of the war, nearly 30 percent of Salvadorans had fled their country, nearly half of them to the United States. The United States played a large role in the conflict, seeing the war as an arm of the Cold War with Communism to defeat. As a result, U.S. presidents continued funneling aid to a Salvadoran army that kidnapped and murdered thousands and trained the army at the School of the Americas. At the end of the Cold War, the United States saw El Salvador as less of a strategic interest and began to cut military aid, and finally put pressure on the government to negotiate a treaty to end the war. Today, Salvadoran communities in the U.S., like Home Care Aide Isabel Palma and her family (page 25), come largely from this time period. The conflict left a generation of children marked by war, nearly 2,500 of who were sent to the U.S. as orphans. Many young men came as refugees, hoping to escape inevitable draft into the military or as guerilla fighters. Researchers are now looking at the psychological impacts this has had

on a generation of Salvadorans with posttraumatic stress disorder. (See our article on PTSD on page 13.) On Sept. 1, 2013, the Salvadoran Attorney General’s office announced it would open investigations into key wartime massacres. That same year, the University of Washington’s Center for Human Rights launched an ongoing project, Unfinished Sentences, to bring international awareness to the wartime crimes. Director Angelina Snodgrass Godoy is currently working with Salvadoran groups to reunite children of war who grew up in the United States, with their families in El Salvador. “Some of them have been searching for nearly 30 years,” Godoy says, who noted that though Americans adopted children out of blind faith, many were forcibly removed from families as agents of war. “It’s an honor to be just a small part of that.” n INSIGHT MAGAZINE

Dept. of Health Certification Follow the Steps for Success

In an effort to help workers be successful through the process, partner organizations such as the Department of Social and Health Services and the Training Partnership are working to help share information about the DOH exam with workers. If you are required to become certified, the Training Partnership is currently recommending the following timeline to allow ample time to work through the certification steps. The steps below outline the SUGGESTED timeline for the process.

STEP 2: Register for Training Individual Providers:

Watch the Safety & Orientation DVD that was provided to you at time of hire. Call to confirm completion 1-866483-1397. You will need your confirmation number and the last 4 digits of your Social Security number. Complete prior to providing care. All workers are required to complete a name and date of birth background check before you can begin work. You will work with the contracting staff to complete this. After the name and date of birth background check is complete, you are required to schedule a fingerprint appointment. Talk with your employer for directions on how to complete this.

STEP 1: Submit DOH Certification Application DOH requires that you submit your application to become a Home Care Aide within 14 days of hire.

NOTE: If you are limited English proficient (LEP) you must indicate this on your initial application. This means if your ability to read, write or speak English is limited you may be issued a provisional certification allowing up to 60 additional days to become a certified HCA.

The Department of Health (DOH) is responsible for the certification process. This is the WA state agency responsible for issuing the Home Care Aide credential (HCA). DOH contracts with a testing company named Prometric to implement the HCA written and skills test.

You should allow four weeks for your application to be processed. Download the forms at www.prometric.com/WADOH.

STEP 4: Target Completion of Basic Training Take your training as soon as possible to ensure best access to classes in your area and your preferred language.

STEP 5: Self Study and Prepare for Exam To prepare for the exam, review “Exam Preparation Materials” found on the Prometric website for the HCA Exam. • There you will find practice questions, skills checklists, and instructions that will help you be successful in the exam. • In addition, review the Practice Exam. www.prometric.com/WADOH

STEP 6: Take Exam Report to test location prepared to follow testing guidelines in the Candidate Information Booklet and General Instructions. www.prometric.com/WADOH

STEP 7: Get Certified Your results are reported to DOH and they will send you a letter confirming you are officially certified. • You can check the DOH website listed in Step 2 to see if your certification is Active. • If you are Pending it means that DOH does not have all the information they need to complete certification. • Make sure you have fully completed the application, completed the background check, fully explained any personal history that could affect your ability to get certified and you have successfully passed your written and skills examination through Prometric.

Frequently Asked Questions Q: What happens if I don’t apply to the Department of Health (DOH) / What happens if I don’t send in my application to DOH? A: You must submit applications to the Department of Health (DOH) within 14 days of the date approved. This is important because the Department of Health issues the candidate ID number or HM # needed to apply to Prometric. If there is no application with DOH, there is no data to match and payment cannot be confirmed between the Training Partnership and DOH. Q: When can I send my application to Prometric? A: Send your application to Prometric as soon as you know your training completion date. This is important because sending applications through the mail can take a couple of weeks to reach Prometric. (Applications submitted online are preferred.) If there is no application with Prometric, there is no data for the automated systems to match between DOH and Prometric.

Q: Does my information need to be the same on the DOH application and the Prometric application? A: You must give the exact same first name, last name, birthdate, and Social Security number on both the DOH and Prometric application. Make sure you use the exact HM number given to you by the Department of Health to put on the Prometric application.

For example, you cannot use Robert on one application and Bob on another application. Do not include hypens in your name. Also, your birthdate must match exactly in all applications. If any of your data doesn’t match, Prometric will not get confirmation of payment and you will not be able to schedule your examination (until the data is corrected).

Q: What should I do if Prometric tells me they have not received payment for the exam? A: If Prometric tells you that they did not receive your payment (and it’s two weeks after the training completion date), contact the Department of Health at 360-236-2700 immediately. DOH staff will then research the issue and get it corrected as soon as possible. If you do not need an interpreter, you will receive a testing date the day your issue is resolved.

Q: What happens if I don’t get an examination date within two weeks of completing basic training? A: If you do not receive notification of an examination date within two weeks from the training completion date, you need to contact Prometric at 1-800-324-4689. Verification of payment should not take longer than two weeks from the training completion

Provides care to a Consumer living in his or her home. Employed by a private, Medicaid homecare agency or DSHS.

HCA Credentialed

A worker who has successfully passed a test and been credentialed by the Department of Health as a Home Care Aide.

Agency Provider (AP)

Home Care Aide (HCA) employed by a private, Medicaid homecare agency.

Individual Provider (IP)

Home Care Aide (HCA) whose employer of record is DSHS.

Standard HCA

Home Care Aide who does not work with their own parent or child. Works more than 20 hours a month or has more than one Consumer.

Parent Individual Provider (HCS/ AAA)

This is an IP who provides care to his/her own adult child and is contracted through Home and Community Services (HCS) and/or an Area Agency on Aging (AAA). This is often referred to as a non-DDD Parent Provider.

Parent DD Individual Provider (DDA)

This is an IP who provides care to his/her own adult child with a developmental disability and is contracted through the Developmental Disability Administration.

Как сообщить о травме на работе Если Вы получили травму дома у своего клиента или у себя дома при осуществлении ухода за членом семьи, сообщите о своей травме в Департамент труда и промышленности штата Вашингтон (L&I). Если Ваше заявление принято, Вы получите компенсацию, выплачиваемую работникам, получившим травму на работе, непосредственно для оплаты медицинских услуг. Если вследствие травмы Вы не можете работать, возможно, Вы сможете претендовать на часть своей утраченной зарплаты.

Шаг 1: Производственные травмы • •

Получение первой помощи Часто падения, порезы, растяжения и вывихи могут привести к серьезным травмам, если их немедленно не обработать. При необходимости обратитесь к врачу. Если Вы получили травму на работе, обратитесь к врачу, в ближайший центр скорой или неотложной помощи (при серьезных травмах, потере конечностей и т.д.). Вам помогут заполнить заявление на получение компенсации, выплачиваемой работникам, получившим травму на работе. Сообщите безотлагательно в агентство или Департамент социальных услуг и здравоохранения (DSHS). Паботодатель должен знать о травмах, чтобы при получении документов от L&I помочь Вам спланировать возвращение на работу.

Шаг 2: Подача заявления Вы можете сообщить о несчастном случае в кабинете своего врача, обратившись в L&I по телефону1-877-561-FILE (исключительно в часы работы) или в онлайн-режиме www.LNI.WA.gov. Шаг 3: Одобренное заявление L&I может одобрить Ваше заявление, если обслуживающий Вас врач подтвердит получение Вами травмы в определенное время и в определенном месте на работе, или заболевание или нарушение, вызванные Вашей работой. Выплаты покрывают расходы на медицинское обслуживание. Кроме того, они могут включать в себя компенсацию заработной платы, помощь при возвращении на работу и пособие по инвалидности при серьезных травмах.

Шаг 3: Отклонение заявления Заявления могут быть отклонены, если врач не может подтвердить связь медицинского состояния пациента с конкретной ситуацией, имевшей место на работе, или с профессиональным заболеванием. Вы или Ваш врач имеете право опротестовать любое решение, принятое в отношении Вашей претензии, в онлайн-режиме. Апелляции можно направлять непосредственно в Апелляционную комиссию по производственному страхованию.

Шаг 4: Возвращение на работу Некоторые получившие травму работники в период выздоравливания скучают по работе. При этом многие могут постепенно возвращаться к работе, в то же время, продолжая получать пособие по болезни. Cómo reportar su lesión

Cuando se lesiona en el hogar de su cliente o en su propio hogar como cuidador familiar, reporte su lesión al Departamento de Labor e Industrias (L&I) del Estado de Washington. Cách Thức Báo Cáo Chấn Thương Tại Nơi Làm Việc của Quý Vị Khi quý vị bị thương tại nhà Khách Hàng của quý vị hoặc nhà của chính quý vị trong vai trò là người chăm sóc gia đình, vui lòng báo cáo chấn thương của quý vị cho Sở Lao Động & Công Nghiệp Tiểu Bang Washington (L&I). Nếu yêu cầu bồi thường của quý vị được chấp nhận, quý vị sẽ nhận được bồi thường cho người lao động để thanh toán trực tiếp dịch vụ chăm sóc y tế. Bước 1: Bị thương tại nơi làm việc • •
