The Path Points to Home W

e are exceedingly proud of the communitybased MJHS Road to Recovery orthopedic program. It showcases our commitment to providing innovative healthcare solutions to address the needs of our patients in the comforts of their own homes. Last May, the MJHS Road to Recovery program expanded to include targeted rehabilitation services that could be offered to patients recovering from hip and knee replacements. Intensive PT services start within 24 hours of returning home—and are typically provided up to seven days a week. This unprecedented continuity of care coupled with a

high level of skilled services makes it possible for most hip and knee patients to skip a post-acute stay in a rehab facility. To date, MJHS has treated hundreds of joint replacements with this aggressive physical therapy program. The results demonstrate that we are making a difference. Rehospitalization rates are lower than the national average and clinical outcomes exceed national benchmarks. Perhaps even more importantly, our patients are happy they can receive the therapy they need while at home and are pleased with the results.

I

t is increasingly common to know someone with chronic pain, depressed mood, cognitive decline, sleep problems or mobility difficulties. Work being done by Helena Knotkova PhD, Director of Clinical Research and Analytics at the MJHS Institute for Innovation in Palliative Care, may help improve the lives of people who are living with these conditions. In collaboration with a research team at the City University of New York, Dr. Knotkova has created a new type of transcranial direct current stimulation (tDCS) unit that involves the delivery of a mild electrical current to the outside of the head.

As part of the evaluation process, Dr. Knotkova and her colleagues have initiated two innovative research studies. One will determine whether very ill patients who have distressing symptoms can use the new tDCS device effectively with the help of a family member. The other will collect data about the effect of the device on a broad array of symptoms, ranging from lack of sleep or concentration to pain, worrying and feeling sad. This research may introduce an important new tool in the nonpharmacologic management of many problems related to advanced illness. Prior to Dr. Knotkova’s work, tDCS was difficult to implement in chronically ill patients because treatment required repeated visits to a facility.
