If you received DACA from ICE, select Number 2 in part 1, but answer all other questions, not just those for renewal, and submit all supporting documents as if applying for the first time.
If applying to renew DACA, make sure to have a copy of your initial I-821D form to ensure that the information you include in your renewal request is consistent with the information you originally submitted.
Include expedited removals at the border. However, voluntary returns are not removal proceedings and do not count for this section.
Check this box if you received DACA and want to renew your request.
You can find this date on the work permit or I-797, Notice of Action. Name as it appears on birth certificate or most recent legal document (i.e. marriage certificate, etc.). Make sure this matches the name you write on forms I-765 and I-765WS.
Check “other� if unsure of immigration history. Explain in part 8 that you do not know what happened.
Make sure you are using the newest version of the form. The date should be 06/04/14.
All renewal applicants will have an A#. It is the 9-digit number written on your employment authorization card as “USCIS#.� You can also find this number on your receipt or approval notice as A#. Initial applicants may not have an A# if have never come into contact with immigration authorities.

If can't remember exact dates, can handwrite month and year (e.g. May/2007) or handwrite “approx.� next to the date.
Renewal applicants need only provide addresses since their previous DACA application.
If traveled without advance parole since receiving DACA, then DACA is automatically terminated. If traveled with advance parole, then mark "No."

Renewal applicants who have already submitted documents of their criminal history with their first DACA application do not need to re-submit that evidence. Can indicate in Part 8 that USCIS already has criminal history documents. Provide all documentation for any new incidents.
