S&T were established in the town of Shaffhausen in Switzerland in 1966. S&T’s founders were at the forefront of microsurgery instrument design. With a reputation for high quality and precision workmanship, adapting forceps used in the watchmakers industry to fine instruments for use by the pioneers of early reconstructive micro surgery.
S&T offer a high level technical capability in combination with innovation and quality. Which are corner stones of the S&T philosophy and practiced during the manufacturer of every instrument. S&T Microsurgery Instruments are now well known to micro surgeons throughout the world and are globally recognised as the leaders in the field of microsurgery instruments.
Mercian have been their exclusive distribution partners in both the UK and Ireland for over 35 years offering their instruments to our hospitals.
STANDARDS
Mercian is certified to meet standard ISO 1348 5: 2016. Recognised as a mark of excellence applicable to our management systems, processes and procedures. We are subject to continual assessment of these procedures to ensure standards are maintained by an independent outside notified body.
Annual surveillance ensures our compliance with ISO 13485: 2016, MDD 93/42/EEC Annex II and MDR 2017/745.
WARRANTY
To ensure our instruments perform as expected, all Mercian instruments are covered by a 12 month warranty covering any manufacturing/material defect. Any instrument failing these standards will be repaired or replaced without charges.

“A” or “V” Pattern?
The “V” pattern is the all-purpose clamp, used on all veins, and almost all arteries. The “A” pattern, available in all sizes, is designed for exceptionally thick-walled vessels and slippery arteries. The slight incurve of the tips of the “A” pattern jaw provides extra security against slippage.

PRESSURE TESTER GAUGE
ACLAND CLAMP PRESSURE TESTER GAUGE
Ensuring your Acland Micro Vessel Clamps are applying the correct pressure onto delicate vessels is an essential consideration for successful anastomosis.
Each size of vessel clamp has its own validated pressure parameters carefully calibrated to the vessel size the clamp is designed to be used on.
This pressure can often vary depending on the age of the clamp and the amount of use.
Vessels can suffer damage if the clamp is applying too much pressure.
Equally if not sufficient pressure is applied then the vessel is not completely occluded resulting in a difficult anastomosis of the vessel with the vessel leaking blood obscuring the operating field.
An annual audit of Acland Micro clamps is recommended to test the pressures thus preventing incidents with non-conforming clamps considered good practice for Microsurgery sets.
Clamp is applied between two sensors on the Clamp Gauge where the applied pressure/force can then be accurately calculated.

TO ASSIST IN VISUALISING MICRO VESSELS DURING ANASTOMOSIS
Description: Silicon strip with 1mm grid on one side and plain on reverse side, available in blue, green and yellow colours. Packed in boxes of 12 pieces.
Application: For use in micro surgical anastomosis of micro vessels under a microscope to allow the surgeon to better visualise the detail of the vessels, being joined.
The Visibility Material can be used either with the grid side up to give the surgeon a scale of the vessel, alternatively the reverse plain side can also be used.

SUPERFINE MICRO INSTRUMENT SET

ARTERIOTOMY CLAMP
The Acland-Banis Arteriotomy Clamp was developed to ovecome the difficulties of creating a hole in the side of a vessel, in an end - to - side anastomosis. The device enables the surgeon to make an arteriotomy with an instrument that cuts the vessel the best -- a knife. The arteriotomy clamp picks up and holds the piece of the vessel wall that is to be excised. Then, with a single curving sweep of a straight blade that is held close against the clamp tip, the ellipse is excised. The result is a perfect arteriotomy, every time.
The clamp is a precision- made, cross - action forceps with a heavy closing pressure and a tapered, rounded, fenestrated tip. The hole in the tip is important because the tissue that is picked up can squeeze into it and be held firmly. There are three sizes of clamps, with tips 1.2, 1.8 and 2.4mm in diameter. Each clamp can cut a different size hole, depending on the depth of the tissue being picked up. Using the set of three clamps, holes can be cut with mean diameter ranging from 1 to 5mm.
USING THE CLAMP
1. Mobilise the vessel so that it can be rotated freely.
2. Apply occluding clamps, keeping the vessel at its natural state of resting tension.
3. Empty the occluded segment by finger pressure while momentarily releasing one clamp.
4. Remove adventitia thoroughly and widely around the planned arteriotomy site.
5. Rotate the vessel so that the site of arteriotomy is seen in clear profile.
6. Have the small vessel in view, with its end prepared and dilated, in order to judge its size.
7. With an arteriotomy clamp of appropriate size, carefully pick up a piece of vessel wall which is half as long and just as wide as the desired arteriotomy.

Note that because of the pressure which the tip of the clamp exerts, this step is irreversible.
8. With a CBS-35 blade held in metal - to- metal contact with the clamp tip, cut around the tip with a steady sweeping action.
Important!
With practiced use, the size of arteriotomy results is highly predictable. However, we strongly recommend a brief initial practice period on animal or fresh cadaver vessels. These vessels should be stretched out to their natural length and tension to simulate real conditions. Remember that the arteriotomy will be twice as long and just as wide as the piece which is picked up and excised.

CARE OF MICRO INSTRUMENTS
Instructions for reprocessing Micro Instruments
Micro Instruments are delicate in nature and require careful handling to avoid damage.
Precautions:
• Use a container to protect micro instruments from damage during transport and reprocessing.
• New Microsurgery Instruments should be processed 3 times prior to first use, to protect the vulnerable surface of new Instruments.
• Avoid use of mineral acids and harsh, abrasive cleaning agents.
• Do not use saline to clean instruments in Theatres, use sterile water.
• Ensure Instruments are processed following surgery within 3 hours.
• Instruments can be damaged by high alkaline detergents. pH range 8.0–11.0 are preferred.
Instructions From Point of Use:
Do not allow blood, debris or bodily fluids to dry on Microsurgery Instruments. Reprocess as soon as possible after use and within 3 hours of use.
If they cannot be reprocessed immediately, use an enzymatic foam spray cleaner to prevent protein’s from drying on surface.
Cleaning - Automated washer- disinfector machines:
Our Microsurgery Instruments are suitable to be processed through automated washer-disinfector machines which are CE marked and validated to ISO 15883-2:2024
Adhering to: HTM 01-01: Management and decontamination of surgical instruments: Part D - Washer-disinfectors
1. Place Microsurgery Instruments securely in a Mesh Basket/container with hedgehog matting to prevent damage to tips.
2. Use a soft brush to remove debris and blood from Micro Instruments, particularly the Acland Vessel Clamps.
3. Ensure that soft, high purity water which is controlled for bacterial endotoxins is used in the final rinse stage.

Magnetism in Micro Instruments
Surgical Instruments of Excellence
Magnetism of micro instruments is common occurrence, but a frustrating aspect of Microsurgery for Surgeons. It is not a reflection on the quality of the Instrument.
What is magnetism
Magnets work because of tiny particles (electrons).
These electrons spin and create small magnetic fields. In most things, these fields cancel out. However, in certain materials, like stainless steel, the tiny magnetic fields align in the same direction.
How do instruments become magnetised
Microsurgical instruments become magnetized over time due to various reasons.
1. Simple friction, contact between instruments against each other.
2. Contact with magnetic mats, used in Operating Theatres to hold instruments.
3. Exposure to magnetic fields in Theatres such as MRI scanners.
Why it's a problem in microsurgery
During microsurgery procedures, where instruments are extremely small, even a small amount of magnetism can cause micro needles to jump onto a needle holder and stick to Jaws of micro needle holders making release difficult. Surgery therefore becomes more difficult prolonging operating time.
Solution:
To demagnetize your micro instruments with a demagnetizer. Demagnetizers work by using electromagnets to create intense, high frequency, alternating current (AC) magnetic fields. This shifts the magnetic field randomly in the instrument so that electrons no longer align in the same direction.
Switch on the demagnetiser. Pass the instrument across the surface of the demagnetiser surface ensuring that you clear the end of the surface by a least a foot on both passes. This should be done repeatedly (perhaps 5 to 10 times) to ensure it is effective. Test on a needle to prove it is successful.
