Sun related issues in children ACuTe Sun DAmAge (SunbuRn) Definition: A sunburn is an acute, delayed, and transient erythema of the skin following exposure to ultraviolet radiation (UVR) obtained from sunlight. Clinical features: Sunburn is characterised by erythema, and, if severe, by vesicles, oedema, tenderness, and pain. Age: Infants have an increased susceptibility to sunburn. Phototype: Sunburn is most frequently seen in individuals who have lighter skin and a limited capacity to develop facultative melanin pigmentation (tanning) following exposure to UVR. Fitzpatrick Classification of Skin Types There are 6 different Skin Phenotypes (SPT) classifying the different capacity to develop tanning in persons with white skin: 1. Almost always sunburns, never tans. Pale white skin, blue/green eyes, blond/red hair, freckles. 2. Usually sunburns, tans with difficulty. Fair skin, blue eyes. 3. Sometimes sunburns, can tan gradually. White skin, any colour hair, any colour eyes.

4. Occasional sunburns, tans easily. White or brown skin, dark hair, dark eyes. 5. Rarely sunburns, tans well. Brown skin, dark hair, dark eyes. 6. Almost never sunburns, tans well. Dark brown or black skin, dark hair, dark eyes. Race: White skin types sunburn the most. Persons with brown and black skin colour can sunburn following long exposures. History: Mild reactions to sunlight begin 6 to 12 hours after onset of exposure, peak at 24 hours and fade at 3 – 5 days. Sunburns have a similar time course with blister formation on day 2 and desquamation on resolution. Symptoms: The skin usually is pruritic, painful, and warm to touch. Severe sunburn over large surfaces creates fever, chills,

malaise, and even prostration. The skin lesions are typically confined to areas of exposure. Course and prognosis: Sunburns self resolve over 2 – 4 weeks and scarring rarely results. management Treatment is syptomatic with: 1. Cold, wet compresses. 2. Baths with colloidal oatmeal. 3. Emollients. 4. Medicated ointments or creams (topical steroids). 5. Analgesics to help alleviate the discomfort.

Severe cases may require systemic steroids therapy. Consult your Paediatrician, Doctor or Pharmacist for advise.

Breastfeeding is best for babies. You should always seek the advice of a doctor, midwife, health visitor, public health nurse, dietician or pharmacist on the need for and proper method of use of infant formulae and on all matters of infant feeding. Good maternal nutrition is important for the preparation and maintenance of breastfeeding. Introducing partial bottle-feeding may have negative effect on breastfeeding and reversing a decision not to breastfeed is difficult. Social and financial implications should be considered when selecting a method of infant feeding. Infant Formulae should always be prepared and used as directed. Inappropriate foods or feeding methods, or improper use of infant formula, may present a health hazard.


Lactose intolerance in babies

L

actose is the sugar which naturally occurs in milk and other dairy products. In our body, it is the job of the small intestine, the organ carrying out most of the digestion and nutrient absorption which releases the digestive enzyme called lactase.1 Lactase is responsible for the breakdown of lactose into its two simple sugar constituents; glucose and galactose. The latter are more easily absorbed by the body directly through the bloodstream.2 Lactose intolerance takes place when the body does not produce enough lactase and hence, lactose remains undigested in the intestine leading to the manifestation of the following symptoms: abdominal bloating, pain, excessive wind, diarrhoea as well as nappy rash in the case of infants.1,3 These symptoms usually mainfest themselves after 1 to 3 hours after the consumption of food containing lactose.2 It is common for most babies to become temporarily intolerant to lactose after an episode of severe gastroenteritis infections or after a long course of antibiotics.1,3 Untreated coeliac disease, Crohn’s disease or other intestinal bowel diseases may cause a transient lactase deficiency too. This is known as secondary lactose

intolerance which may take more weeks to resolve (to 6 – 8 weeks). The lining of the small intestine called the ‘brush border’ which constitutes the site of lactase production becomes damaged leading to a deficiency in the lactase enzyme.1,3 In babies, the most common indicator of secondary lactose intolerance following gastroenteritis is persistence of diarrhea or loose stools. There is also congenital lactose intolerance but it’ s very rare. It still may affect newborn babies who are either born without the lactase enzyme or else their body produces very low amounts of the enzyme due to an inherited genetic mutation in the gene coding for the production of the lactase enzyme.2,4

Premature babies might also suffer from a temporary developmental lactose intolerance due to the fact that their small intestine is still immature. However, premature babies might not remain lactose intolerant as they get older as their gut eventually becomes fully developed.2,4 In case of secondary lactose intolerance, formula fed babies can benefit from a lactose-free formula which is nutritionally complete and specifically designed to manage this condition.4 Babies who are fed with a lactose-free formula show regular growth patterns and the absence of lactose allows recovery from the functional alterations of the intestinal mucosa.5,6,7 In the case of a baby who is already in the weaning stage, parents must follow healthcare professional advice and ensure that the diet of their baby is managed carefully by eliminating lactose from the baby’s diet and making sure that the baby is obtaining all the nutrients required without hindering his or her development.5,6 IMPORTANT NOTICE: Seek the advice of a GP or paediatrician if suspecting that the baby is sufferring from lactose intolerance.

Board games help children’s social skills improve such as: Taking turns. for young children, this is often a difficult social skill to learn. Offering children plenty of opportunities to take turns helps them to develop this vital life skill.

The Plasmon Oasi adventure continues as we head towards the beautiful city of Verona, the city most famous for being the setting of Romeo & Juliet. The team plans to visit the Plasmon fields and apple orchards in the small village of Zevio, which is 14 kilometres southeast of Verona. We met Francesco, a dedicated farmer who for the past 25 years has been growing apple trees for the production of baby food. He speaks with great passion and dedication about the way fruit is harvested and cultivated. We hop on his truck and he drives around his 100 acres of land of apples. While walking through the orchards, the Plasmon agronomist, Michele Rossi, explains that not every farm can enter the Plasmon SRI programme. The SRI programme or as famously known as Oasi programme takes root around a set of rigorous rules and criteria that ensure the best methods of cultivation are used. The programme offers a unique guarantee of total quality from the raw material to the finished product. Francesco shares the Plasmon Oasi philosophy as he maintains and follows the cultivation agreement designed by Plasmon. The apple orchards are situated away from sources of pollution, away from motorways and industrial areas. Impressed by the magnificent green scenery, Francesco and Michele, tell us that there are 3 varieties of apples grown at Francesco’s fields. These are the golden, red and green apples loaded with vitamins, minerals and goodness. All apples are picked by hand and gently packed into crates. These are then sorted into groups, checked and tested. Apples are ground into the perfect puree consistency, suitable for our baby’s early nutrition. The puree is then transported to the Plasmon plant for bottling and packaging. All the fruit is completely traceable from the fields to the finished product. Babies and their delicate little tummies remain at the heart of the team, as they prepare a nutritionally complete product suitable for their early nutrition. Our visit at the apple orchards came to the end and we drove back to the historic citycentre of Verona. We took a stroll through the picturesque streets and finally reached the worldfamous Juliet’s house, with its famous balcony and Juliet’s statue. Astonished by its simple elegance, the square was an on-going party. The following day, the team planned to take an internal flight from Verona to Rome to visit the Plasmon plant. The Oasi adventure continues…

Re a To Play With P Everyone loves play dough! It is so much fun and provides plenty of learning opportunities, and the possibilities of what it can be moulded into are endless! 1. Play dough Improves Fine Motor Skills Squishing, squeezing, stretching and moulding play dough helps to build muscle strength in the fingers and hands. This in turn makes it easier to complete fine motor activities like holding a pencil and cutting with scissors later on. Providing small items like buttons, sequins, pasta, pebbles and shells with the play dough will also encourage children to use their fine motor skills when picking up and placing these objects into their play dough creations. It is a great idea to put out some shape cutters, a grater, blunt plastic knives and tweezers which can all be used to mould and transform the play dough.

You Can Teach Little Children All About Numbers And Letters when Using Play Dough Using play dough to teach numbers and letters is fantastic, particularly for tactile learners, as they are able to mould and move the dough into the correct shapes. Children may like to make their name, write a secret message to a friend or make the numbers to 10. There is HeAPS of research out there that shows that children learn much more effectively when they are actively involved in their learning in a handson manner. A Journey oF TAsTEs & TEXTUREs

Up until now your baby has been content with breast or formula milk whilst dabbling in the delights of pureed fruits and root veggies, but now’s the time to let them discover a world of tastes and textures Between six and nine months, once first tastes have been accepted and your baby’s digestive system is maturing, you can start to increase the amount and variety of food you give your baby. Babies are developing quite rapidly at this stage, so this is a window of opportunity to help them master the art of chewing. The key to introducing tastes and textures successfully is to take it slow and follow your baby’s pace – after all the experience can be quite daunting for them. There’s no point in pushing them when they’re not ready as they’ll just kick up a fuss and it will be harder next time around. You simply have to be patient and persistent. TACKILING TEXTURE So you’re a first foods pro but now tackling texture? Don’t worry if you’re unsure where to start, just follow my top steps to introducing texture.

MAKE pUREEs ThICKER I would start by making the consistency of the puree thicker. Try mashing a portion of their food then adding it to the puree; gradually increasing the ratio of mashed the pureed food. Once she’s open to a thicker texture then you can slowly stop pureeing and simply just mash the food by hand. FINELy Chop ThE vEGGIEs Dicing vegetables finely is also a good way to introduce texture. Start by softening small cubes of carrots or apple which will allow their gums to squash them when they try to chew. Be careful when you do this as surprise chunks can really startle your baby and put them off. Making your own baby food will allow you to control the size of the lumps and in turn, you can gradually increase their size.

Add ChUNKIER Foods With my son Nicholas I soon realised that if I added baby pasta shapes to his favourite puree then he’s been more receptive because it was a flavour he was familiar with. Give rice or couscous a go too - you might be surprised at what gets wolfed down. For a sweeter option, try cereal or porridge with a little cow’s milk which can now be introduced when your baby is 6 months plus.

Fish like salmon or cod provide the important fatty acids needed to help aid your baby’s brain, nervous system and vision development. Try mixing fish with stronger tastes such as carrots, tomato and grated cheddar and always check for bones before serving. IMpoRTANT IRoN Babies are born with a store of iron inherited from their mother. At six months this begins to run low so introduce foods rich in iron such as meat, leafy green veggies and pulses. Vitamin C can help aid iron absorption so give foods such as broccoli or citrus fruits.

BY ANNABEL KARMEL CHICKEN, PEAR, PARSNIP AND CARROT Makes 5 portions Ingredients • A knob of butter • 1 large leek, trimmed, washed and chopped • 125g carrots, peeled and diced • 100g pears, peeled, cored and diced • 100g parsnips, peeled and diced • 100g boneless, skinless chicken breast, chopped • 300ml milk

PEACH, APPLE, STRAWBEERY PUREE Makes 2 portions Ingredients • 1 large apple, peeled, cored and chopped • 1 large peach, peeled, stoned and chopped • 75g strawberries, halved • 1 tbsp baby rice Method 1. Steam the apple for about 4 minutes. 2. Add the peach and strawberries to the steamer and continue to cook for about 3 minutes. 3. Blend the fruits to a smooth puree and stir in the baby rice.

Thicken up this puree by stirring in some baby rice or crushed rusk.

dURING THE SUMMER MONTHS

Time for that trip to the beach, a picnic with the family or a walk under our beautiful Maltese blue sky. Summer gives us many good reasons to head out of the house – and most of us do. It has been estimated that 80% of our lifetime sun exposure occurs before the age of 18 since children like to spend time outdoors. The summer months also bring with them those annoying mosquitoes which will quickly target a toddler’s tender skin. So here are some quick tips to care for your toddler’s skin during the summer months.

sUn Care

* excessive exposure to the sun’s harmful rays will predispose your toddler for skin cancer – also known as melanoma – in the future. A toddler’s skin is delicate and will get more easily sunburnt. You may have gotten sunburnt before so you know it is not a nice experience. So the following advice should help you prevent this.

* There is no easy way of putting this: sun exposure needs to be avoided between 11am and 4pm when the sun is at its highest intensity. At least seek shade if you cannot stay indoors. * Use a good, child-friendly sunscreen. Always searching for creams that are protective against UVA and UVB rays and make sure the Sun Protection factor is 50+. Apply the cream 30 mins before leaving the house and reapply approximately every 2 hours. * Get your toddler used to wearing hats (ideally with a broad brim) and a good pair of sunglasses. Cover as much skin as possible with loose cotton clothing keeping in mind not to overheat your child.

* A common complaint in a paediatrician’s clinic is heat rash. In this type of rash, small red bumps start appearing in the toddler’s

skin. This usually occurs in sweaty areas like the face, neck, armpits, abdomen and thighs. This type of rash is not serious and usually lasts around a week but talk to your paediatrician if you see white pus filling the bumps. To prevent heat rash make sure your toddler is not overdressed and use a good moisturising wash and cream to help the skin stay healthy.

* Mosquito bites may make children miserable and the best way for prevention is by using insect repellent. Search for products that contain no more than 30% DeeT or natural based ingredients. Spray on exposed skin areas (but not the face) and on clothing. I hope this simple advice will enable you to enjoy summer more, while taking care of your toddler’s gentle skin.

Summer It is the season for our winged friends to wreak their bloody habit again. Mosquitoes are nasty fellows, and there are plenty in Malta at this time of year. It is important that we take measures in our homes to prevent the proliferation of these mosquitoes. Mosquitoes do not feed on blood. The female mosquito requires a blood meal for development of her eggs and not for nutritional purposes. When they are 10 feet away they use extremely sensitive thermal receptors on the tip of their antenna to locate blood near the surface of our skin. The range of these receptors increases threefold when the humidity is high. That is one of the reasons why we get bitten more in summer! The average life span of a female mosquito is 3 to 100 days while the male lives only 10 to 20 days. Here are a few facts and tips that will help you keep these little pests away: • Remove any water-filled containers such as flower plates, old buckets, food containers and tyres; • Drain puddles, inlets to showers and drainage systems and do not allow water to stagnate for more than three days, alternatively, treat with chlorine; • Keep gutters free from leaves and other debris so that water does not collect in them; • Standing water in pools, catchment basins, etc, that cannot be drained or dumped should be periodically treated with properly labelled insecticide; • Swimming pools should be kept empty if not in use or regularly maintained with chlorine; • Keep water tanks, wells and reservoirs covered.

Here are some suggestions to prevent being bitten: • ensure that door and window screens fit tightly and are in good state; • Wear socks, shoes, long trousers and a longsleeved shirt when outdoors for long periods of time, and when mosquitoes are most active. Clothing should be made of tightly woven materials to keep mosquitoes away from the skin; • Use mosquito netting when sleeping outdoors or in an unscreened structure, and to protect infants when outdoors; • When it is necessary to be outdoors, insect repellent should be applied regularly as indicated on the product’s label. • Use fans – mosquitoes are weak fliers and a strong wind produced by a fan not only keeps them from flying but diffuses chemical cues they use to locate blood meals.

Burns and scalds are damage to the skin caused By heat. a Burn is usually caused By dry heat, like fire, a hot iron, or the sun. a scald is caused By wet heat, like steam or a hot cup of tea. You need to be extra careful when treating burns. The longer the burning goes on, the more severe the injury will be, and the longer it may take to heal. So you need to cool the burn as soon as possible. If someone has a severe burn or scald they are likely to suffer from shock (is a life-threatening condition, not to be confused with emotional shock) because of the fluid loss, so they will need urgent hospital treatment.

What to look oUt for If you think someone has a burn or scald, there are five key things to look for: • Red skin • Swelling • Blisters may form on the skin later on • The skin may peel • The skin may be white or scorched


What yoU need to do

Stop the burning from getting worse, by moving the casualty away from the source of heat.

If it is serious, call 112 for emergency medical help.

Remove any jewellery or clothing near the burn (unless it is stuck to the skin).

6 Start cooling the burn as quickly as possible. Run it under cool water for at least ten minutes or until the pain feels better. Don’t use ice, creams or gels – they can damage tissues and increase risk of infection.

Assess how bad the burn is. It is serious if it is: • larger than the size of the casualty’s hand • on the face, hands or feet, or • a deep burn

After the burn has been cooled; cover the burned area with kitchen cling film or another clean, non-fluffy material, like a clean plastic bag. This will protect from infection.
