A letter from the Director.. Welcome to our 2018/19 Winter/Spring program brochure! I recently had a conversation with someone who gave me some simple yet powerful advice. The message is something that I have heard many times over, but it never gets old. Simply put, he told me to live in the moment. When your kids are at PLAY, be present. When friends join together to play in one of our adult sport leagues, enjoy the here and now. This lesson should be taught to our children at an early age as well. The next time they partake in one of our enrichment classes, encourage them to immerse themselves in the subject matter and LEARN as much as they can. This lesson is more important than ever especially in the age of cell phone distractions. Life can pull you in many different directions, but when we focus on the present we can see what life is truly about, LIVING in the moment! In this new year our team will be striving to offer the community with great opportunities for people of all ages to PLAY more, LEARN as much as possible, and to LIVE in the moment! General Information Cancelation Policy: A full refund will be issued if a

program is canceled by the LMRD due to low enrollment or a facility usage conflict.

Self-cancelation Policy: Before the start of program-

Participants may request to drop out within 2 weeks of a program before the start date of a program, including practices and games. Those who drop under 2 weeks of the start date may be charged additional fees which are listed below. 1) To receive a check refund, participants will be charged a $3 administrative fee. Additionally participants may be asked to cover the cost of any supplies or equipment that were ordered in advance of the program. 2) Participants may also choose to roll over their entire fee in program credit. This credit may be applied towards their next program.

Refund Policy: In the event that an adult or child partici-

pant requests to drop out from a program or league at any time prior to the start of a program the following refund procedures will occur.  During the program- Individuals and in Non team activities who request to drop-out after the start of a program but before the halfway mark will forfeit 50% of their registration fee. Any adult team that chooses to drop out after the start of a program forfeit 100% of their registration fee.  No Refund will be offered after the halfway point of any program.  Medical Withdraw- If a participant can no longer continue to participate due to a medical emergency, that participant will receive a prorated refund. The individual must provide a signed doctors letter in order to receive a refund.  Relocation Refund- If a participant or the participants family moves out of any time before the start of a program will receive a refund minus the cost of supplies if ordered already. If the participant moves after the start of a program they will be issued a prorated refund.

Weather Cancelations: In the case that school may be

canceled due to bad weather we will be canceling all recreation department activities. Weekend cancelations will be done via email, Facebook posts or contact from your child’s coach.

Fees/Late Registration: Fees are based off of the

minimum amount of participants needed to successfully run a program we will do our very best to anticipate needs for now and the needs for the future of our programs so that we may set our prices accordingly. To ensure proper planning we may set deadlines for our programs. Those who register after a set deadline will have to pay an additional $10 late fee and may also run the risk of not being able to participate if the program is full. .

Residency: For program enrollment residents are those

who reside within the Lake Mills Area School District boundaries, open enroll to LMASD, or work within the school district. Residents pay the fee dir ectly befor e the “R”. The second fee listed is for non-residents “NR”. Non-Residents are those who do not reside within the Lake Mills Area School District or attend school at LMASD. Residents support the school district through tax-dollars, while non-residents pay to participant through higher registration fees. Non-Residents will be responsible for paying a minimum of an additional $10 per activity and participant they register.

Concussion information: In 2011 the State of

Wisconsin passed Act 172. Which requires any child under 19, who participates in an organized athletic game or competition, to receive information about concussions and their symptoms. Annually parents and the athletes must return a signed copy of the “Parent and Athlete Agreement” prior to their participation in a rec sport.

Insurance: The Lake Mills Area School District and Lake

Mills Recreation Department do not provide medical or accident insurance for any program participants both residents or non- residents. Participants are strongly encouraged to obtain their own insurance coverage prior to participating in programs.

Age/Grade Requirements: All youth program age

restrictions will be based on the first day of the program unless otherwise specified. All grade restrictions will be based on the 2018-19 school year. Summer will officially mark the new school year. Age and grade requirements are set to benefit all children and make instruction easier for the program leader.

Notice of Non-Discrimination: No person shall, on the

basis of sex, race, national origin, ancestry, creed, marital or parental status, age or disability, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity, including employment.

No News is Good News!!: After signing up for a program

you may not receive a confirmation. Consider yourself registered for that program. Please be sure to mark your calendar with the dates for each program. If a program requires specific information we may send out an informational sheet via email. You may also check your rSchoolToday account for a list of all past registrations from your account.

Typo, Human Error, We Goofed…!?!

Occasionally there may be an error in days, times, registration requirements or fees in the brochure. When such errors occur, our staff will do everything possible to correct the situation promptly. We thank you for your patience and understanding when these situations arise. For the most up to date info check our web page.

Photo Policy: Participants or their parents (if participant is

under 18) permit the taking of photos, audio and videotapes during Recreation Department activities for publication and use as the Recreation Department deems necessary. If you do not want a picture or video taken of yourself, please inform the director or instructor prior to the start of the program.

Online registration with our NEW registration site will open, Tuesday April, 3. Blue Sombrero will no longer be used to register for any programs. Please follow the instructions below:

Registration forms and computer access are available in our office during our hours of operation, Monday–Friday, 8:00A-4:30P. Cash, check or credit card are accepted. Please bring exact change.

Browse the website! Classes are divided up into categories (adult, youth, older adult) and from there , subcategories (athletics, athletic camps, summer playground program...etc).

Once you have selected the appropriate class you would like to register for, click ‘Add to Cart’. You may add several classes to your cart. By clicking ‘Back to Catalog’ you can continue to search for classes.

Once you have selected the class(es) you would like to register for, click ‘Check Out’.

If you have not registered for a Rec Dept. program on the new site, please click the ‘Create New Account’ button. There will be a ‘Student Participant Information’ Page, followed by a ‘Payer Information’ Page. If you ar e both the participant AND the payer, check the ‘Student/ Participant is also the Payer’ box on the ‘Payer Information’ Page, and it will automatically autofill. This is where you will create your User Name and Password to log in to your family account. Click Continue.

To finish enrolling, assign a participant by clicking ‘Assign a Person’ to the right of the class name. You may add any class specific notes by clicking ‘Add’ under the ‘Notes’ column. You may also delete a class from your cart by checking the box under the ‘Del’ column. If Resident and Non-Resident pricing apply, this is when you will choose the correct fee(s). Click ‘Check Out’ to proceed.

Confirm the classes you wish to sign up for, followed by ‘Payment Information’. You may chose to securely store Credit Card information if you wish for future payments.

Click ‘Submit Payment’ to finish enrollment.

9.

IF YOU ARE A RETURNING USER when you click ‘Check Out’ click the ‘Login to My Family Account’ button and log in using your User Name and Password.

10. To view your Account, use the ‘View My Account’ Tab at the top of your screen. From here you can see Reservation and Payment History and edit Family Members Info.

In consideration of being allowed to participate in the Lake Mills Recreation Program, the undersigned parent(s) or legal guardian(s) of the player named above: 1. (a) Agree that prior to participating, they will inspect the facilities and the equipment to be used, and if they believe that anything is unsafe, they will advise their coach or supervisor of such condition(s) and refuse to allow their children to participate. Further, there may be other risks unknown to the Lake Mills Recreation Program and/or it’s coaches, sponsors and agents not reasonably foreseeable at this time and assume all forgoing risks and accept personal responsibility for the damages following such injury, permanent disability, or death. 3. Intending to be legally bound, hereby release, waive, discharge, or covenant not to sue the Lake Mills Recreation Program, the Lake Mills School District, the City of Lake Mills, Sponsors, Supervisors, Coaches, Volunteers, Officials, or the other agents, all of which are referred to as “releases” from all liability from the undersigned, his or her heirs and next of kin for any claims, demands, losses, or damages on account of injury including death or damage to property, caused or alleged to be caused in whole or part by the negligence of the “releases” or otherwise in connection with association or participation in and/or arising out of my travel to participate in a Lake Mills Recreation Program. 4. In the event that the player named below sustains injury or illness while participating in a Lake Mills Youth event, I/we hereby authorize any emergency first aid, medication, medical treatment, or surgery deemed necessary by licensed medical personnel. I/we also give my permission for attending medical personnel to execute on my behalf permission forms regarding treatment of my minor participant or other necessary medical documents and to act in my/our behalf if I/we am not immediately available to do so. 5. My/our child has completed a physical exam, given by licensed medical personnel in the last 24 months, and is physically fit to compete in activities related to the Lake Mills Recreation Department. Adult Athletics & Open Gyms ADULT BASKETBALL OPEN GYMS These programs are open to all adults interested in playing pick-up basketball. Ongoing program locations are subject to change. All participants must fill out a waiver prior to participation, if under 18 a parent must sign waiver.

ADULT & HS ULTIMATE FRISBEE — INDOOR PICK UP GAMES Have you heard of Ultimate Frisbee? Imagine combining the football field with goals at each end, the constant movement of soccer and a Frisbee! This is a non-contact sport that easy to learn and fun for all ages. All games will be co-ed and teams will be divided evenly each week. High School Players must be 16 or older. All participants must fill out a waiver prior to participation, if under 18 a parent must sign the waiver. PICKLEBALL OPEN GYM

Pickleball is one of the fastest growing sport in America. Pickleball combines elements of tennis, badminton and table tennis. Come try your hand at Pickleball at no cost to you! Paddle recommended but not required.

ADULT MENS BASKETBALL LEAGUE Sign up your own team for Men’s Winter League basketball! This eight week league is for both recreational players and seasoned veterans alike. Game times will be based off of the number of teams that sign up. For more league information and a downloadable roster visit our website and click on the Recreation Department tab. Minimum 4 teams are needed to run and a maximum of 8 teams will be accepted. Community based teams will be given registration priority. Registration Deadline: January 18

Rise and Sweat*

This boot camp style class uses high reps to build long, lean muscles. You will complete different dynamic strength exercises with cardiovascular work in between.

This will be a circuit style class with a positive upbeat environment where you can work out and feel awesome afterwards! In this circuit workout you are free to challenge yourself. Youth Athletics – Open Gyms YOUTH BASEBALL & SOFTBALL OPEN GYMS Dust off your glove and warm up that throwing arm. This is a great chance to get yourself warmed up for softball before the weather starts warm up! Pre-registration not required. But all participants must fill out a waiver prior to participation. Waiver s will be available to pr int online or at each open gym. Volunteer coaches are needed to provide a safe and productive atmosphere. Only indoor balls can be used in the gyms.

Recreation/Optimist Co-ed Basketball Intro to Game Play

LMHS Youth Hip Hop Dance Clinic NEW!

This co-ed program is designed to follow the Optimist/Rec Skills and Drills program. Each week participants will brush up on their fundamental skills, as well as learn new skills specific to playing in a basketball game. Each week players will work on game skills in a small group setting, eventually working their way up to a 3v3 or 5v5 game setting. This program includes a t-shirt.

There will be a coaches meeting 15 minutes prior to the start of the program you are coaching; Grades 1-2 @ 8:15 am, Grade 3 @9:15 am.

Come join the LMHS Varsity Dance Team for the NEW Hip Hop Dance Clinic! This camp will be led by the 2018-19 Varsity Dance Team and Coach. All dancers will learn a hip hop routine. Participants will get to show their skills during halftime of the LMHS Boys Varsity Basketball game on Tuesday, January 15. Program includes a t-shirt and instruction. Participants should wear comfortable clothes and dance or tennis shoes. No one will be allowed to dance in wet shoes or socks. Bring water or Gatorade for drink breaks.

Optimist Tri Star Competition It’s back! The annual Optimist Tri Star basketball skills competition is run by the Lake Mills Optimist Club. Participants will show off their skills while passing, shooting and dribbling their way through the competition. The top participant in each age bracket will qualify for the State Competition! No preregistration is required.

SKI CLUB

NASP Archery Intro

Ski Club is open to all students’ grades 6th through 12th. The 4 dates listed above are confirmed, additional make up days may be scheduled on March 8. All participants are required to have a signed liability waiver to each completed for each trip, NO EXCEPTIONS. Please arrive at the school by 3:45 at the very latest. Students are responsible for all money or possessions they bring on this trip. Lockers are available for rent on site, quarters are required. Minimum of 20 participants are needed to run this trip.

Bull’s-eye! Nearly everyone can enjoy the success in archery. In this program participants will learn how to determine their dominant eye, make a string bow, and the eleven steps to achieving archery success. We will provide Genesis bows and arrows to be used by all students. FIRST

The Medal Volleyball League is open to all girls 5 th through 8th Grade. This league is designed to help develop strong confident volleyball players. The serving line and net height have been altered to help give players a higher chance to succeed. Teams at any given grade level may field multiple teams. Participants will play against teams from surrounding communities. Volunteer coaches will set practice schedules. League T-shirt included.

Your child will love this introduction to the game! In this class, participants will go over the proper techniques to SET them up for success. Spring Rec Soccer

T-ball League

Have a ball with this recreational based program for children in 5K- 5 grade. Players will have fun while learning the fundamentals, rules and skills of the game. Practices will start 2 weeks before the season. The games will be held at LMHS Practice fields off of E. Lake Park Place on Saturday mornings. Participants will be contacted by their coach with league schedules & team information.

This is an instructional baseball league. Participants will work on the proper techniques of throwing, batting, base running, catching and fielding. Rubber T-Balls are used and will be hit off a tee. Volunteer coaches are needed to help run this program. Coaches will contact their players with a practice schedule.

There will be a coaches meeting and equipment hand out from 6:00 - 6:30 pm in the LMES Cafeteria on March 14. ALL coaches training from 6:30 - 7:30 in the LMES Gym to follow.

The focus of this league is on skill development and sportsmanship over winning. No scores will be kept, and both coaches will assist in making any calls on the field. Little League Baseball

L-Cats Baseball Camp

LMRD Little League Baseball program is open to both boys and girls who are currently in grades 3 - 6. Teams will participate in the Eastern Suburban Youth Baseball League which consists of surrounding communities. Transportation is required. More information about the ESYBL can be found at www.esybl.org. There will be a coaches and advisory committee meeting on, February 5 at 5:30 pm in the LMMS Cafeteria. Par ents interested in coaching, assistant coaching, fundraising, concessions coordination should attend.

Week 1- Par ticipants will gain the foundation in hitting skills. Each kid will get to hit in the batting cage and have one on one instruction from LMHS players. The goal of the camp is for the kids to leave understanding basic form of hitting and fielding. Please have your child bring a glove. If they have their own bat they are allowed to bring that as well or one will be provided. Week 2- We will wor k on pitching development and defensive skills. The kids will learn the basic steps of pitching form and will be able to throw to a varsity catcher for a live bullpen session. The goal of the camp is for all the kids to understand basic mechanics of pitching and learn the proper defensive skills.

Little League Spring Training Day LMRD will be hosting a MANDATORY “Training Day” for all kids interested in playing in American or National League baseball. SOFTBALL

LMRD Pony & Teener Baseball program both participate in Travel Leagues (parent transportation required). Players will be divided into teams based on their grade level. There will be a coaches and advisory committee meeting on Tuesday, February 5 at 6:30 pm in the LMMS Cafeteria. The Teener program with help from the coaches will use a FLEX player schedule. This schedule will maximize playing time for the kids. Coaches will also be able to pull players up or down to make sure all games can be played with a full roster. Teeners participate in a higher level competitive league. Team practices and league schedule will be set by the coaches. Commitment, attitude, skill level and sportsmanship will be considered when determining playing time. The Pony Baseball program participates in a 7th grade only Baseball League. This is a transitional league, games are played on a modified field with 80 ft bases and 54 ft pitching rubber. Home games will be played at Wallace Park or Campus Field. Player uniforms will be provided for all leagues.

Beginning Guitar & Ukulele Lessons

Candle Light Snowshoe & Ski

Grasp the tools needed to learn how to play the guitar or ukulele in these one on one music lessons. Lessons will focus on basic guitar or ukulele knowledge, chords, strumming techniques, note reading and other skills needed to successfully play the guitar or ukulele. Lessons will be taught by Mr. Shanafelt who has over 15 years of experience playing the guitar. He is certified in General Music and teaches music at Lake Mills Elementary School. Walk the trails of Korth Park by candle light! This year you can enjoy two lit trails as you explore Korth Park at night! Then relax next to the fire pit and enjoy some popcorn and hot cocoa or cider. Concessions help support the LMES School Garden & Greenhouse Project. Day trips include travel, ticket purchases, and scheduling of events. Participants may be responsible for tipping or additional purchases, while on the trip.

Lake Mills Pickleball Group In order to meet the growing pickleball demand in our area, Lake Mills Recreation Department joined with the Rock Lake Activity Center to share spaces, equipment, and time slots. The following procedures were implemented starting Monday, November 5th, 2018. Dates Mondays:
