Together in ‘23 We all emerged from the pandemic with various “ailments” ranging from brain fog to a lapse in our personal fitness regimen and the commensurate “few extra pounds,” to what Board of Supervisors Chair Phyllis Randall calls a kind of “collective trauma.”

Feelings of isolation may persist more than a year later, especially in teens who lost so much of their shared experience when their school schedules were disrupted. Making an appraisal of our own mental health is as important as reckoning the challenges of addressing what has been called a “mental health crisis” locally.

Particularly around the advent of a new year, our thoughts turn to our own mortality, and possibly to the prospect of losing loved ones. Not all of the latter is a BAD thing, as Dr. Martha Calihan teaches us in her extraordinary book, A Death Lived.

But now for the “scarier news.” In a piece in The Washington Post by economist Bryce Ward (washingtonpost.com/opinions/2022/11/23/americans-alone-thanksgiving-friends), we read that we’re not just feeling isolated – we ARE alone. He writes, “Our social lives were withering dramatically before covid-19. Between 2014 and 2019, time spent with friends went down (and time spent alone went up) by more than it did during the pandemic.” Just one example: Americans spent 38% less time with family and friends over Thanksgiving during the last two years than we did the decade prior.

If you read our December piece quoting Sandy Lerner decrying higher screen time among adolescents, you know that we agree with her that volunteering, serving others, giving back, getting involved – whatever you call it – is the true panacea for loneliness and its potentially more debilitating corollary – being alone, without longing for company.

What you’re likely missing in solitude is incalculable. It’s the comfort of a friend knowing what difficulties you may be going through and maybe praying for you. It’s doing the same for them. It’s the joy that comes from hearing someone exclaim happily upon seeing you, and their affirmation that you look great in that dress. It’s the shared sweat and struggle of working out before work takes hold of your schedule, or grabbing a cup of coffee during the day. It’s a shared free skate at Ion, taking in a dinner show at StageCoach Theatre, or going to hear a Loudoun Symphony string concert at a winery.

As Bryce puts it, whatever your shared activity, “You will feel better, create memories, boost your health, stumble across valuable information — and so will your companions. Put effort into building relationships that you can count on in good times and bad because, as the song goes, that’s what friends are for. Besides, you just might have a good time.”

More than 1200 patients suffering from mental health issues check into Inova Health System Emergency Rooms every month, says Dr. Linda Lang, a psychiatrist and Inova’s president of the Behavioral Health Service Line. “50 percent of the 1200 get discharged to the community and we have to find resources for them and another 50% get admitted because they’re in a higher level of crisis,” she explained. They might have depression, anxiety, or fear they want to harm themselves.

Dr. Lang added, “Our inpatient behavioral health units are at full capacity. We are facing similar constraints across our community providers as well. So, we're really all in this together and our community has to figure out how to address the mental health crisis that we're in. I can't think of a time over the last three years that I've been in practice where the crisis has been this great.”

Now at least some local patients who admit they need help can find assistance at Inova Cares’ new Clinic for Behavioral Health, where Dr. Lang shared her concerns. The clinic, formally opened in November at the Inova Loudoun Hospital Cornwall Campus (211 Gibson St., NW, Suite 220), will provide adult mental health counseling services to those struggling with symptoms of mild to moderate mental health conditions. Services include individual therapy services on a weekly or biweekly schedule for a limited time, defined as shortterm, goal-directed therapy for 10-12 sessions — at least time to determine what other services or medication might help patients heal. The clinic also provides telemedicine appointments for those who can’t attend in person, and provides referrals to community resources and primary care services.

“Opening our first Inova Cares Clinic for Behavioral Health is a direct response to the 2022 Loudoun Community Health Needs Assessment which identified mental health as a top community need,” said Karen Berube, Chief, Community Health & Health Equity with the Inova Health System.

At the clinic’s launch, the small training room, named for donor Debbie Piland with the O’Shaughnessy-Hurst Memorial Foundation which contributed to its funding, was packed with local political representatives and nonprofit care-givers. None was more passionate about the mental health needs of Loudoun County than Loudoun Board of Supervisors Chair Phyllis Randall, who was herself a mental health and substance abuse therapist in the prison system before entering public service.

Adding to the overall picture of the local mental health crisis, Chair Randall said law enforcement officials typically respond to 10 calls per week caused by someone with a mental health disorder. “We don’t say that enough because mental health issues are stigmatized and minimized and because of that, by the time you hit an emergency room or a clinic the acuteness of your disorder is much higher that it would have otherwise been.” The pandemic deepened the crisis because of its accompanying isolation and what she called the “collective trauma” everyone experienced, similar to after 9/11.

She first urged everyone to “contact your state officials and say that we need to reopen our state hospitals. That's what has to happen.” Beyond that, everyone needs to discuss the issue and “destigmatize this disease. Until we do that, it’s not going to get better and we’re going to lose people we don’t need to be losing every single day.” Public-private partnerships are key, she noted, as did Inova officials who were quick to thank donors Mike and Teresa Wheeler and Patti and John Sinnen. A spokesperson for Inova said “often donors don’t want the amount [of their gift] disclosed, and that is the case for this project,” but Debbi Piland, one of our loyal readers and a donor to the project, shared that the O’Shaughnessy-Hurst Memorial Foundation for its part made a grant commitment of $150,000.

Because not enough attention is paid to such donations — perhaps because of the participants’ reluctance to go public — we dug deeper and learned that the O’Shaughnessy-Hurst Memorial Foundation was started in 1986 with funds from the estate of Charlie and Mary O’Shaughnessy, from the Bailey’s Crossroads section of Fairfax County. “It’s a small foundation whose mission is to improve the quality of life, health, education and welfare in Northern Virginia,” Debbi, a Foundation principal, said. “At the same time, we make every attempt to be responsive to evolving community needs such as the recent COVID-19 outbreak.”

In the past, the Foundation also has made grants to expand Cloverleaf Therapeutic Riding (formerly Northern Virginia Therapeutic Riding), to build a birthing center for the Fauquier Hospital, to partially fund the Falls Church Children's Center, to Inova, to the Loudoun County Bar Association Law Camp, George Mason University, 89 Ways To Give, A Place To Be, Catholic Charities, and Faith and Family, among others. It also made a grant through the Community Foundation of Loudoun and lower Fauquier County to partially fund a state-of-the-art vocational school in Winchester.

Said Debbi, “The Foundation is very sensitive to the mental health crisis in our country today. It impacts people at all ages and stages of life. Even with good health insurance, a person is not assured of being able to obtain quality mental health care — much less people with fewer resources. This grant was a no-brainer for us, and we are very proud to be a small part of a huge benefit for our community.”

Adolescence is a critical time to explore and engage with the world, but for many the pandemic took away this opportunity and left teens in a mental health crisis. Despite efforts to break the stigma and bring awareness to suicide prevention, too many parents shy away from the discussion. The Ryan Bartel Foundation asserts that suicide has touched the lives of too many unsuspecting families to risk avoiding the issue.

Suicide remains the 2nd leading cause of death among teens. “So many programs focus on intervention at the time of crisis. That’s too late,” said Suzie Bartel, founder of the Loudoun County-based teen suicide prevention organization. “If we develop in them the skills to get through the hard times, we’re not going to need intervention.” Ryan Bartel Foundation’s upstream suicide prevention programs are designed to help all teens by providing them with skills and strategies to navigate life’s challenges with strength, connectedness, purpose, and hope.

After losing her son to suicide in 2014, Suzie Bartel founded the group in 2015. It’s programs now include Sources of Strength – a nationally acclaimed, evidence-based, upstream suicide prevention program used in the Loudoun County Public Schools; FORTitude Teen Workshops; the FORT – multiple, physical safe, inclusive, and accepting spaces for teens to connect with their peers, quiet their minds, and have fun; FORTitude for Parents; and FORTitude Support Groups for parents and teens.

Monthly in-person FORT programs occur at Trillium Farm in Purcellville, Fusion Academy in Leesburg, InMed Partnership for Children in Sterling, and Crossroads United Methodist Church in Ashburn. Here, teens participate in experiential workshops in expressive arts, mindfulness, animal-assisted groups, and other activities to learn and practice coping skills they can use on their own when times are tough.

One mother wrote of the FORT, “In the time we have been attending, my child has re-engaged in social activities and found strengths within himself. Dr. Lola Ogbonlowo M.D. called her Family Medicine & Wellness practice in Lansdowne “Lighthouse,” because she intends it to be a beacon to those who are struggling with health issues and in need of both healing and hope. “A lighthouse is a place of guidance, direction, navigation, calm, refuge, a safe place. A lighthouse structure can be seen from far away, like a city on a hill that cannot be hidden. I would like our practice to embody this in our community....

What that means in practice is that when a patient comes in complaining of headaches, she doesn’t immediately ship them off to neurology. She methodically develops a treatment plan to address the pain and possible underlying causes and lines up a specialist when necessary. Ditto with patients with arthritis; the extent of joint damage can be established through X-rays, but as a “Full Spectrum Family Medicine” practice, Dr. Ogbonlowo believes in providing personalized, medical care that’s within the full range of her expertise. “I can give them a steroid injection and send them to physical therapy and they feel much better.”

As a resident, she also trained in hospital medicine where continuity of care was key, she explains. She then spent several years in the Valley Health System in Winchester before starting her new practice in Loudoun County a little over a year ago. “There’s an important place for specialists, but if we can provide treatment, that can cut down on the health burden.” Her commitment to family medicine also inspires her to stay up to date on medical break-throughs and pursue certifications that will help both current and future patients. Recently, she spent the last year and a half becoming certified in obesity medicine, for example.

Digging deeper into her background, it becomes clear that her entire career has been about just making herself available to God’s calling as a healer. Even before medical

school, she worked as a board-certified lactation consultant. In college she previously volunteered with the D.C. free clinic Bread for the City and shadowed Dr. Randy Abramson. She remembers how the underserved were treated with dignity and respect and that later inspired her to pursue a licensed medical career. Because of the gap in her undergrad work and deciding to pursue med school, she first obtained a masters in biotechnology from Johns Hopkins University in Baltimore, then graduated from the University of Medicine and Health Sciences, St. Kitts in 2014. She completed a family medicine residency at Genesys Regional Medical Center, Grand Blanc, Michigan in 2018.

During medical school she founded an organization called Bridging the Gap to help connect the students with local service opportunities. She has participated in medical mission trips to Haiti and El Salvador and volunteers locally at the Loudoun free medical clinic in Leesburg. It could be, she allows, that practicing medicine in poorer countries with fewer resources made her both more resourceful and confident in her diagnostic and treatment skills, and that she can master any medical approaches or treatments necessary to help her patients thrive.

“I do believe that. I think for me, if I can learn it to help someone then I will.” Fascinated with a whole person approach and exploring root causes – which also includes using food as medicine and optimizing overall health factors – she now also provides bioidentical hormone therapy in addition to weight management therapies. “This is the heart of personalized medicine,” she explained; it’s not a one-size fits all. “It’s what puts the ‘family’ in family medicine.”

Arizette Carrinho, CEO of HomeWell Care Services, was a nurse for 12 years – everything from labor and delivery to the ICU – before starting her own in-home healthcare service. “I’ve always loved caring for people. The only thing is, I just fell out of love with the hospital bureaucracy – just not being able to give the care that I wanted to, to my patients.”

She’s been working with HomeWell Cares for the last year and couldn’t be happier. “My ‘why’ is being able to see my patients beyond the hospital. A lot of times we would discharge them to go home and we would give them the instructions and then that was it. Just being able to have that continuum of being part of their care is more of who I am as a person. Being part of just their everyday life and providing them with that peace of mind is key.”

On her website (homewellcares.com/va184) she writes, “Kindness is the most underestimated component of a business.” She goes so far as to say that, in her experience, you can teach anyone to be a care-giver but you can’t teach kindness.

“You can hire someone with 20 years of experience caring for patients or clients but if they don’t have the heart to do what we do, you just can’t work in this field.” It’s a quality she’s made her number one priority in hiring, and one she has modeled in her own life since learning to help care for a brother with autism.

When she discovered that one of her care-givers was homeless, she put her in a hotel until she was able to get back on her feet. “That’s what we have to do. It just goes back to kindness and being there. Maybe that one glimpse of hope helps someone and it becomes contagious when you pass it on to those you’re caring for.”

“When I meet my clients for the first time, I don’t talk to them about what’s medically going wrong with their family member in need of care. I tried to find what’s emotionally going on. When I connect with families in that way, I find that a lot of them are exhausted, they’re tired, or they feel guilty that they just can’t help their family member anymore. I really take the time to get down to what’s really going on emotionally and try to just give them peace of mind.”

For a closer look what that looks like, we’ll introduce you to Brian. Three years ago, Brian was diagnosed with ALS (a.k.a.

Lou Gehrig’s disease) at 38. When his wife left, and multiple assisted living establishments declined to admit him, his parents took over his care. Arizette could tell they were exhausted. When they talked, Arizette encouraged Brian’s mom and dad to share the memories they had of their son and the times they enjoyed together. In so doing, she learned that his parents had moved from their retirement home in South Carolina to Northern Virginia so Brian could be a part of his children’s lives.

Arizette told the family that of course they were aware that there’s no cure for ALS, and wanted to know how his mom and dad were dealing with that. Rather than feeling guilty about seeking help, Arizette said, “I helped them appreciate that they didn’t want their last memories of their son be only about their exhaustion in caring for him. They were shocked that I took the time to listen to them. It made me very sad because kindness is missing in a lot of businesses, especially in health care.”

Given the green light, Arizette found Brian a full-time care-giver, whom they all adore, and she visits Brian every day to talk and pray with him.

“I’ve become like family.... Eventually, Brian’s parents trusted us enough to move back to South Carolina, but they now drive or fly here so their son can enjoy spending time with his kids for five days every two weeks.”

Despite all the travel, they feel they can enjoy their grandchildren and son together, without feeling like his care falls entirely on their shoulders.

That’s not to say the burden isn’t still substantial. Arizette tells us that Brian’s care costs upwards of $150,000 a year and that insurance doesn’t pay for home health care. “It’s out of pocket and it’s 24/7,” she explained. She’s willing to match up to $10,000 for anyone willing to donate to Brian’s GoFundMe campaign (see URL above). Kindness wouldn’t have it any other way.

Most people think a plant-based diet is strictly for those vegan and vegetarian types, but there is no debating the overwhelming evidence showing the benefits of improved health for those who make that diet choice. Plant based eating is more of a lifestyle than a diet. You are simply eating more foods that are closer to their harvested state with minimal processing.

Meatless diets have been shown to improve health outcomes by lowering the risk of developing heart disease, diabetes, high blood pressure, inflammation, and some cancers. It also improves gut health because of added fiber to your diet.

A plant-based diet still allows you to consume the carbohydrates, protein, fats, minerals and vitamins that are necessary for good health, with the added benefits of being higher in fiber and phytonutrients.

You do not have to give up meat entirely, and there are labels to identify where you fall in your plant eating journey. Do what is comfortable for you so you will stick with it.You can choose to be a f lexitarian which means you occasionally eat meat but you choose plants as the primary foods you eat.

You have some f lexibility with your diet. If you choose to be a vegetarian, then you do not eat meat but you still eat eggs and dairy products as opposed to being a vegan who eats no animal products.

There is also another reason to add more fruits and vegetables to your daily food intake; It isbetter for the environment. The industries that produce our dairy and meats are said to account for a third of the world’s water usage in addition to producing greenhouse gas emissions.

1. Make small changes to test your comfort level when reducing meat intake. There is a risk of being overwhelmed if you suddenly stop eating meat altogether, and it will be harder to stick to your choice long term. Increase consumption of plant foods you already like.

5. If you do not eat any animal products, you will need to take a B12 and possibly an iron supplement. Vitamin B12 is an essential vitamin available only in animal products. Low b12 can cause anemia and damage to the nervous system. Some vegan foods are fortified with vitamin B12.

2. A plant based diet doesn’t have to be boring. You can create a variety of dishes including ethnic meals that are meat free and delicious. This is a great time to try new spices to add a punch to your vegetables and add green leafy vegetables to your smoothies.

6. A meat-free diet does not automatically mean healthy if you replace the meat with highly processed junk food and meat substitutes. You need to eat good, fresh quality fruits, vegetables and grains to get the health benefits that can add years to your life.

3. Use the whole plant because there are nutrients in every part of the plant including the stems, and leaves. Use the leaves from celery, radishes, beets and carrots, and use the stems from spinach. Don’t throw away vital nutrients like potassium, vitamin C and fiber. You are also reducing waste.

4. Eat the rainbow! Choose different colored fruits and vegetables to get a variety of nutrients. The color of plant-based foods is often an indicator of the vitamins and minerals they contain.

So, more colors mean more nutrients. Example: Orange foods contain beta carotene, the precursor for vitamin A.

7. If you haven’t already tried it, shop the EatLoco Farmer’s Market at One Loudoun for a variety of fresh, seasonable fruits and vegetables, whole-grain breads, fresh mushrooms, fresh juices and prepared, meatless meals.

“Death is the most common and least understood event in our lives, but we know that the dying process can be a powerful, beautiful and spiritually rich experience.”

Back in 2010, Dr. Martha Calihan, M.D.’s husband of 30 years began to die. Being combination wife, physician and founder of Five Stones Healing Arts and Wellness Center, she did the only thing she could do – she meticulously and compassionately described what that was like. “I wrote [the book A Death Lived] because it needed to be written,” she told us.

“I would not be naturally inclined to share something that personal, but my experience as both wife of a dying man and physician gave me a unique perspective on the experience that certainly was important to share. Our experience was that Charles by his actions so lived his death, which is why that is what the book is named. It just screamed to be shared,” she added.

Her first thought was that, “if I can help any other person or family going through the experience or who will eventually go through the experience realize the benefits and value of having the conversations which lead to the making and sharing of the decisions that need to be made when someone is dying, then that’s a win.”

What made the journey special and important on that front was the honesty with which Charles faced his death and dealt with the kinds of decisions that medical people use to govern their own actions. Dr. Calihan had been included in many such decisions in her family medical practice, but what came as a surprise to her in experiencing the loss of her own soul mate was, “that it would be a deeply sacred and profound experience, and ultimately, a gift far beyond anything I could have imagined,” she writes

in the book’s prologue. “I deeply believe that one of Charles’ purposes in this life was to teach as many of us as would listen, how to die.”

The detail she provides, including the things that she felt and what she and her family discussed, are a critically important tool in helping any family cope with a similar process. Moreover, it’s an invaluable read for any physicians who face death in their practices. Rather than treating their patients’ dying as a failure of medicine, they should accept the fact that it is an inevitable part of life and become channels of learning how to make this process more positive, she asserts. “Death is the most common and least understood event in our lives, but we know that the dying process can be a powerful, beautiful and spiritually rich experience,” she writes.

And finally, A Death Lived is important as both a love story and testimony that death isn’t the end.

We won’t bury the lede here – we can use the word “positive” about the process of dying not only because Dr. Calihan does, but because she says that when Charles expired, “I saw Charles’ spirit leave his body.... I felt an instant, deep sense of peace and absolute certainty about what I had seen.”

She knew then not only that the body and the spirit are separate, but, “that the ultimate force is love. It’s the only thing that matters. When you die, you can’t take your possessions with you, you can’t take your body with you. After your spirit leaves, all that lasts in the love that you shared.”

Dr. Susmita (Misty) Paul owner of Basics First Chiropractic, is an expert in helping her patients practice “self-care.” She says, “as a chiropractor I treat neck pain, back pain, carpal tunnel, knee pain, jaw pain – I treat a lot of different things.

But one of the things that I’ve seen a lot of especially over the pandemic, is pain from sitting.”

Sitting? Yes sitting. “it’s something that we’re doing all day long. Before the pandemic, most of us had a long commute. And then we’d go sit at work. And then we’d get into our car and drive home and lay down on our couch and say, ‘I’m so tired. I can’t do anything.’ How can that be if you’re sitting all day?”

To Misty, “sitting is the new smoking. Sitting is just as bad for you as smoking.” Why? It’s compressing the discs in our backs It limits circulation in our muscles, and so oxygenation of those muscles. Without movement, what happens is your muscles start to stagnate; they become weaker.

You get flat butt syndrome, and text neck syndrome – not to mention headaches.”

Sitting also puts pressure on your lungs because you’re not getting enough oxygen throughout your body, so you’re more prone to diabetes and cancer, not to mention the shoulder stiffness, back pain and a slowing of the cognitive processes of your brain.

“You are slowly killing yourself and you’ve got to make some changes!” she insists. “If we’re all worried about aging to the point of taking pills and potions and putting lotions all over, it’s worth remarking on how old we’ll look if we’re all hunched over, she adds. “You’re aging rapidly from a foggy brain, poor posture and flabby abs.”

Standing desks are no better for you if you don’t take the time to stretch your legs and move around, she adds. But what does help are these three things: 1. For every 30 minutes sitting you need to move for three minutes. “You don’t have to be distracted from your task at hand. Simply get up. You can still be on the Zoom call. You can just walk in place. You can still be talking on a meeting and you can keep some light hand weights nearby so that, while you’re in that meeting, you can just be doing some curls, some tricep extensions.”

2. Keep your curves. “As you sit, start from the top of your head and concentrate on where you need to preserve your curves. Your ears are supposed to be over your shoulders.” Keep a mirror close by so every once in a while you can glance at yourself and say, “stop slouching!”

3. Motion is lotion and hydrate to regulate. “We forget to drink during the day, so keep a water bottle close by. Use a physio ball. Switch it up, anything to keep yourself moving.” Just getting your hips moving, having a box with bicycle pedals on it to move your legs, or using stretch bands for resistance are all things you can do to get your blood moving and stop yourself from aging in place.

Those in leadership positions at work need to make “dates” with themselves to exercise and to establish good sleep patterns, she added. An essential oils whisperer, she also recommends using peppermint oil as a natural muscle relaxer and wild orange for mental stimulation.
