The United States has long been the world leader in medical research and patient care. Across the nation, patients have access to top-notch healthcare providers, advanced diagnostic systems and leading-edge pharmaceuticals which help them to lead longer and healthier lives.

Unfortunately, despite the high standard of care offered, the United States healthcare system is broken.

Patients and providers are often mired in a maze of regulations and restrictions imposed by a patchwork billing and payment system controlled by large insurance companies. These difficulties can make getting access to needed healthcare more difficult, and they increase the stress felt by ailing patients.

Over the years, there have been attempts to reshape the healthcare system, including the implementation of Medicare and the Affordable Care Act. However, these efforts have not been entirely successful. Many patients still fall through the cracks of the healthcare system, failing to receive appropriate care and ending up saddled with onerous medical debt.

In recent years, many physicians across the country have decided to “opt-out” of the current healthcare model. Instead of having their patient treatments and billing arrangements determined by 3rd party insurers, these providers have developed direct primary care medical membership systems for their patients.

Under these systems, patients receive all their primary care from their doctors by paying a monthly subscription fee.

In Hendersonville, a few primary care practices have adopted the membership model. One of them is Blue Ocean Family Medicine. Led by Dr. Kent Lofley, Blue Ocean Family Medicine offers a complete range of traditional family medical services.

“In our practice, we provide full-spectrum outpatient family medicine,” says Dr. Lofley. “Our services include preventive care, sick care, chronic disease management, physical examinations and treatment procedures. We also provide many medications and lab tests to our patients at cost, resulting in

significant savings for them. One additional advantage for our patients is that we also offer Osteopathic Manipulative Treatments (OMTs), which provide a supplemental hands-on approach to diagnosing and treating illnesses and injuries.”

Recently, Hendersonville Lifestyle sat down with Dr. Lofley to find out more about his practice and how direct primary care works.

WHAT PROMPTED YOU TO BEGIN A DIRECT PATIENT CARE MEMBERSHIP-BASED PRACTICE? “I have always felt that the insurance model for primary care is bloated and unnecessary. After medical school, I decided that I wanted to find a way to continue to practice medicine, take excellent care of my patients and still be able to eat dinner with my family. A membership-based practice allows me to provide personalized healthcare and achieve worklife balance.”

HOW DOES YOUR PRACTICE DIFFER FROM TRADITIONAL PRIMARY CARE PROVIDERS? “We believe that going to the doctor should be simple and affordable. So, we do things differently at our office. Instead of accepting health insurance, our patients pay a monthly fee, and we see them as often as they need for whatever health concerns that may arise. The monthly membership allows for a spreadout predictable cost.”

DO YOU WORK WITH INSURANCE COMPANIES? “No, we do not bill insurance providers, and we do not bill Medicare. We believe that our patients who have insurance should save it for emergencies. They can use it to pay for things that we don’t provide in our office, including expensive prescriptions from pharmacies, specialized labs and imaging diagnostics.”

HOW DO YOU HANDLE SPECIALIST REFERRALS, E.G. SURGEONS? “Like any primary care practice, we refer to specialists when needed. For our patients who have CONTINUED >

insurance, nothing is different. They simply use their insurance for their specialist visits and treatments. For those without insurance, we work hard to find specialists who offer fair prices and are willing to work with cash-paying patients.”

HOW LONG DOES IT TYPICALLY TAKE FOR ONE OF YOUR PATIENTS TO GET AN APPOINTMENT WITH YOU? “At Blue Ocean, our number of patients is limited to 500-700, which facilitates our patient scheduling. Sick patients can usually be seen the same or next day. More routine visits are usually scheduled within one week. Our limited patient count also allows us to spend more time with our patients.

Initial patient visits usually last 60 minutes, with follow-up visits lasting between 30 – 60 minutes. This extra time allows us to provide

more in-depth diagnostics and care than traditional primary care practices.”

WHAT FACTORS SHOULD PROSPECTIVE PATIENTS CONSIDER WHEN CHOOSING A FAMILY CARE PROVIDER? “When you choose a family medicine physician, you are choosing a coach, educator and coordinator for your health.

The provider should be responsive to your concerns, open-minded about treatment options, and willing to consider your personal preferences and needs.”

To minimize the effects of SAD, it is vital to get at least one hour of natural or artificial sunlight every day.

For many people, especially outdoors enthusiasts, winter is the best time of the year. They embrace the cold, crisp weather, choosing to spend their time skiing, sledding and relaxing by roaring fireplaces.

However, for others, winter is a bleak and depressing season.

Arriving after the excitement of the holidays has ended, the cold temperatures and short periods of daylight in January and February can adversely impact mental health, particularly in those who suffer from Seasonal Affective Disorder, or SAD. SAD can manifest itself in different forms, ranging from mild cases of the blues to full-blown wintertime depression, making life miserable for those affected.

Fortunately, there are effective techniques for combating SAD and other wintertime ailments.

Numerous mental health agencies and professionals are available across Middle Tennessee to help sufferers deal with mental health issues, including SAD. One of the best in the area is the Davidson County Chapter of the National Alliance on Mental Illness (NAMI). NAMI is the largest organization in the country dedicated to the well-being of individuals and families who are directly impacted by mental illness issues.

Founded in 1979, NAMI has grown from a small grassroots concept to a nationwide organization of over 600 chapters that help thousands of people deal with mental health issues each year. Locally, NAMI Davidson is led by Executive Director Robin Nobling, who has many years of experience in dealing with mental health issues, including SAD. “SAD can be debilitating for many people during the winter months, but it doesn’t have to be,” says Robin. “There are many small, yet significant changes that people can make that can dramatically improve their moods and attitudes during this time of year.”

To help those impacted by SAD, Robin offers five tips for thriving during the wintertime.

1. “Nature tends to quiet down during winter. The newfound quiet provides a good cue to slow down, savor your surroundings and embrace nature. One good idea is to set up a winter bird feeder and watch the birds, either by sitting outside or through a window from inside.”

Exercise, such as group water aerobics, can significantly improve mental health.

2. “Regardless of the temperature, bundle up and go outside to get at least one hour of sunlight on sunny days. For gloomy, overcast days, use a readily available therapy lamp to get a healthy dose of artificial sunlight. These lamps are very effective in combatting the effects of SAD. In fact, they are standard equipment in Scandinavian countries where winter daylight is severely limited.”

3. “Exercise! Pick a regimen that you enjoy and try to get in a workout every day. Whether walking, running, riding a recumbent bike, or taking water aerobics, exercise offers tremendous benefits. Many people with mental health conditions report being symptom-free for two hours after only 30 minutes of exercise.”

4. “Keep a daily diary/journal or download a mood journaling app. Track your mood and note how it varies with sleep, food intake, weather, socialization and prescribed medications. All of us have our own unique daily cycles/rhythms. Tracking these things consistently for 8 to 12 weeks can help you determine your individual mood swings, and what impacts them.”

5. “Take time every day for at least ten minutes that is dedicated to meditation, allowing your mind to relax to a quiet state. This quiet period of reflection can take many forms, including walking, listening to soft music, cooking, and sewing – there are no hard and firm rules. Just find and adopt an activity that takes your mind away from your worries.”

While these tips are aimed at combatting wintertime blues, they represent good routines to practice year-round. In today’s stressful world, mental health issues can manifest themselves at any time. Maintaining a strong state of mental wellness demands attention and self-care throughout the year, not just during the winter months.

“While most of us are quick to take care of loved ones and friends, we often ignore our own needs,” says Robin. “We need to learn to use the same compassion on ourselves that we extend to others. We are worth it.”

One good way of relieving winter stress is a quiet walk.

From boutique gyms to workout boot camps, there are plenty of wellness choices available.

One of the fastest-growing group workout activities is pickleball. According to the Sports & Fitness Industry Association, the sport grew by over 39% from 2019 through 2021.

Pickleball originated in 1965 on Bainbridge Island located near Seattle, Washington. It was created by a trio of fathers who wanted to keep their kids entertained during the summer doldrums. Since its inception, pickleball has evolved into a popular sport for players of all ages and skill levels, with leagues available worldwide.

At its core, pickleball is a fusion of tennis, badminton, paddle ball and ping-pong. It is played on badminton-sized courts (either CONTINUED >

indoors or outdoors) using paddles and perforated plastic balls. Like tennis, the game can be played by either two or four players, with pickleball equipment readily available at area sporting goods retailers.

“Pickleball is both fun and easy to play,” says Sumner County Pickleball Ambassador Chris Taylor. “Many people find that they are addicted to the game after playing the first time.”

Pickleball is played at public and private locations across Sumner County, including Gallatin, Hendersonville and Goodlettsville. Some players choose to play in small individual groups, but most join a league.

“Our local Gallatin Mixed League has over 190 members, and the Gallatin Pickleball Facebook page has more than 500 followers,” says Taylor. “As more people hear about the sport, it will only continue to grow.”

Pickleball’s rapid rise has led to an increasing demand for additional courts to handle the growing number of active players. Currently, league and individual play take place in Gallatin

on six new courts that opened in Municipal Park last July. In addition, indoor play is also available at the Gallatin Civic Center.

In Hendersonville, pickleball is played on Memorial Park’s tennis courts 9 – 12, but only during warm weather. Officially scheduled play is currently suspended in January and February but will resume in March. In addition, the city is currently planning to construct several new pickleball courts in Memorial Park in 2023.

“We are so excited to get to work on bringing dedicated pickleball courts to Hendersonville,” says Hendersonville Parks & Recreation Director Andy Gilley. “It’s a great phenomenon across the country, and just like any other sport we’ve added, we want to do it the right way. We are currently in the planning phase, but we hope to get started building our new courts very soon.”
