The above mentioned child has been accepted for Herbie Fund assistance at The Hospital for Sick Children (SickKids), in Toronto, Canada. We are submitting information to assist with Silvia Alejandra Alcalde’s application for a Visa to travel to Canada in order to receive her proposed treatment for his treatment namely, Catherization Lab Closure. Silvia Alejandra Alcalde will be accompanied by her mother Mrs. Silvia Gomez Chang. Treatment at SickKids: Dr. Andrew N. Redington, Head, Division of Cardiology has accepted Silvia Alejandra Alcalde for treatment at SickKids. The Herbie Fund, a charitable fund helping children from around the world to receive medical treatment not available in their home country, will provide the funds for Silvia Alejandra Alcalde’s Cath Lab procedure, and is inclusive of the following: • • • • •

Cath Lab hourly rates, use of operating room and equipment therein Nursing care provided at the Hospital for Sick Children Supplies, customary and usual medical prescribed by the attending physician relevant to Cath Lab procedure being performed as an inpatient Diagnostic tests Pre-op and Post-op service specific clinics

Appointment Date: Silvia Alejandra Alcalde’s Cath Lab procedure is scheduled for Wednesday, December 8, 2010. However, she will be required to attend pre-op appointments approximately one week prior to this date. Length of Stay in Toronto: Silvia Alejandra Alcalde’s will need to remain in Toronto for approximately 4-6 weeks to complete the procedure and follow up care. She will return home to Peru upon her last clinic appointment at SickKids. Accommodations: While in Toronto, Silvia Alejandra Alcalde and her mother have made appropriate arrangements regarding accommodations. They will be residing with Mrs. Carmen Alcalde at 451 Blackthorn Avenue, Toronto, Ontario, M6M 3C5. Mrs. Carmen Alcalde can be contacted at 416-656-9472. The family has been informed that they must purchase health insurance for the duration of their stay in Canada. I hope that the above stated information is sufficient.
