We’re pleased to welcome you to beautiful Bachelor Gulch and look forward to this great opportunity to celebrate our past successes and share new ideas. During our time together, you will learn about opportunities for future growth within the UNIFI Companies. You’ll have a chance to hear from our Executive Office and from Division and Regional Officers on issues key to our continued growth. Of course, you will also be sharing your successes with each other, strengthening existing relationships and building new ones! Nido Qubein came to the U.S. as a teenager with no knowledge of English, no contacts, and only $50 in his pocket, yet ended up an extraordinary business entrepreneur. He is president of High Point University, which enrolls 3,000 undergraduate and graduate students from 50 countries. He has authored more than two dozen books and audio programs distributed worldwide in over 20 languages and is a columnist for several publications. His foundation has granted more than 600 scholarships to college students nationwide.

Crisp mountain air and a stunning Colorado sunset await you on an authentic, 600-acre ranch. A cozy log cabin is the perfect setting for this evening. This authentic ranch includes homesteader cabins that date back to the 1880s, wide ranging uncluttered views and hay fields dotted with cattle and horses. Enjoy some of the finest cowhand cooking this side of the Continental Divide. After dinner, you can kick up your heels as a Western dance band sets the rafters to shakin.’ Stop by the Old West photo station complete with costumes, props and sepia prints to serve as a souvenir of your night on the Western frontier. Note: Be sure to wear jeans, cowboy boots (or tennis shoes), and bring a jacket. This is a fun, casual night, much of which will be spent outdoors!

Travel Information Required Travel Document

All Travelers: Please be aware that you may be denied boarding if the names on your passport/photo identification and airline tickets do not match. Photo ID must be current and government issued, such as a driver’s license or passport. You are responsible for securing the required travel documentation.

Airport Security

• Travelers may now carry through security check points travel-size toiletries (3 oz or less) that fit comfortably in one quart-size, clear-plastic, zip-top bag. • At the checkpoint, travelers will be asked to remove the zip-top bag of liquids and place it in a bin or on the conveyor belt. X-raying separately will allow TSA security officers to more easily examine the declared items. • Larger amounts of prescription liquid medications, baby formula and diabetic glucose treatments must be declared at the checkpoint for additional screening. • After clearing security, travelers can now bring beverages and other items purchased in the secureboarding area onboard the aircraft. • The following items cannot be included in carry-on: any sharp or flammable objects, such as knives (except round-bladed, butter and plastic cutlery), scissors with a cutting edge of four inches or more, razors, pocketknives, aerosol cans, cigarette lighters, etc. • Keep carry-on items to a bare minimum. • All shoes must be removed and placed on an X-ray belt for screening. • Passengers are asked to arrive at the airport at least 90 minutes before your departure. • All luggage must be clearly labeled.

• Test the batteries in your camcorder and/or laptop computer prior to your departure; you may be asked by security to turn on electronic equipment to prove that it is what it appears to be. Note: Prior to traveling, contact your airline to obtain updated security information. Ground Transportation

Round trip ground transportation has been arranged for you and your guest between the Denver airport and the Ritz-Carlton, Bachelor Gulch, if you provided your flight itinerary to Ameritas Travel Headquarters. Ameritas is providing airport transportation for you and your guest, every hour on the hour between Denver International Airport (DEN) and The RitzCarlton, Bachelor Gulch. For example, if your flight arrives in Denver at 1:10 p.m., you would be on the 2:00 p.m. transfer. If you choose not to take the transportation provided on the hour, your airport transportation will not be reimbursed by the company. Note: If you’re extending your stay at The Ritz-Carlton, up to three days before or after your program dates, Ameritas will reimburse you for the cost of airport shuttle van service, up to $100.00 per person, one way. One-way airport shuttle service is $100.00 per person, including tip. Payment can be by cash or credit card. You should request reimbursement in your expense report with proper receipts.

Bellmen gratuities have been prepaid for luggage handling on arrival and departure.

Luggage Tags

Enclosed are Ameritas Group Sales Congress luggage tags (2 per person) for you to attach to your luggage, including carry-on bag. These tags are essential in iden-


tifying you to airport and hotel personnel as a guest of Ameritas. As an extra precaution, you may wish to tape a business card to the inside of each piece of luggage, in the event luggage tags are lost or destroyed.

Arrival

Upon arrival at Denver Airport, please proceed to the baggage claim area to personally claim your luggage. Travel staff with Ameritas signs will greet you and assist with your luggage and transportation to the Ritz-Carlton. Note: Should any of your luggage be missing or damaged, please do not leave the airport before notifying the airline. Travel staff will assist you in filing a claim.

Departure

You will receive a departure notice in your room, the day before your scheduled flight departure. It will indicate the time your bags need to be available for bellmen to pick up, and your scheduled hotel departure time. Please be on time, and identify your luggage before boarding transportation.

Luggage Restrictions

Expect a high level of security and enforcement of policies for both checked luggage and carry-on luggage. You should check with your originating airport and/ or airline carrier for details regarding the most current airline safety issues. Basic luggage restrictions include: • Checked Luggage: The number of allowable, free checked bags varies according to each airline. Some carriers charge for all checked luggage, while others allow one free piece, but charge a fee for a second bag. Ameritas will reimburse the charge (if any) for the first two pieces of checked luggage per person. In addition, if your luggage exceeds the carrier’s specified weight allowance, you’ll be charged for the extra weight or asked to eliminate items from your luggage. Contact your specific airline carrier for detailed

information regarding their luggage policy. • Carry-On Luggage: Most airlines limit carry-on luggage to one piece plus a personal item (purse, briefcase or laptop). Airports are strictly observing policies regarding items that cannot be included in carry-on bags. For detailed information and the latest listing of all prohibited and permitted carry-on items (and how to pack them), refer to the Transportation Security Administration website: ww.tsa.gov/travelers.

Personal Travel Arrangements

If you would like to make travel plans for either before or after the Ameritas Group Sales Congress program, Maritz Travel can assist you with your extended travel arrangements. All expenses associated with extended travel will not be reimbursed. Please send an e-mail to ameritasgsc@ travelhq.com indicating the destination(s) you wish to visit, dates of stay, name of hotel(s) should you have a preference, preferred price range (such as moderate, deluxe), preferred car rental agency, type of car, rental dates, etc. Please be specific. There is a $75 concierge fee per extension for this service. Personal Expenses

Incidental expenses include, but are not limited to, phone calls, valet and laundry service, meals and activities not included in your itinerary, and personal bar bills, including mini-bar charges. The resort cashier will keep a separate record of these expenditures for you. Please settle your personal account with the resort cashier prior to or upon checkout.

The 21,000-square-foot Bachelor Gulch Spa includes a heated outdoor pool and hot tubs, a fabulous fitness center with locker rooms, and grottos and relaxation areas that are complimentary to all resort guests. Meals at Leisure

While staying at the resort, please charge your meals at leisure to your room. Ameritas will reimburse you, after you submit your expense report. The maximum reimbursable amount is $35 per person, for you and one guest, per breakfast or lunch. You will not be reimbursed if you choose not to dine with the group at scheduled meals.

Hospitality Desk

Throughout your stay, travel staff will be available at the Ameritas hospitality desk to assist you. Upon arrival on June 14, we will be conveniently located on the Mountainside Terrace. All other days, we will be located in the Ballroom Foyer area. All bulletins will be posted here. Stop by often for answers to travel questions, activity information, reservations and to reconfirm your daily itinerary.

Your Guest

The invitation to attend Group Sales Congress has been extended to you as a qualifying Ameritas rep., and your spouse or adult family member or friend. Anyone under the age of 21 will not be accommodated at activities, meals, business meetings or in ground transportation. Note: Guests are highly encouraged to attend all three morning business sessions.

Income Tax

It is important to keep in mind that your attendance at Top Performers and/or Sales Congress will impact your taxes. As with past incentive conferences, the Internal Revenue Code requires that the additional costs incurred because of your guest’s presence (typically their transportation, meals and entertainment) be attributed to you as wages subject to withholding and employment taxes. The tax rate you can assume is generally around 41%. Top Performers will be taxed on the full value of their two-day program, because no business meetings will be held June 18-19. You will be notified of the additional wages after all conference charges have been reviewed and appropriate amounts have been allocated. Average cost per person for both Top Performers and Sales Congress is estimated at $5250.00, but your taxable amount will depend upon many variables.


Climate

The weather in Beaver Creek is practically perfect during the summer. Daytime average high temperatures are pleasantly in the high 60s, up to 70˚F, while evening temperatures cool down considerably to the 30s.

Clothing

Our morning business sessions call for casual business attire – casual dresses, pantsuits and skirts or slacks with blouses for women, dress slacks with collared shirts for men; jackets are optional. Your selected activities will determine what clothing you should bring. The sun’s intensity can be quite strong in the mountains, so remember a hat or visor, sunglasses, and sunscreen. Appropriate attire per activity is noted in Agenda section of this program. Formal attire is called for on the Awards evening; tuxedos are required for men and cocktail dresses or gowns for women. Tuxedo fittings will take place between 3:00 and 5:00 p.m. on Sunday, June 14 in 7 Deli. Some evenings call for Evening Resort Wear. Women will be suitably dressed in casual dresses, skirts and pants with coordinating tops. Men will find casual slacks such as Khakis with collared sport shirts to be appropriate. Remember a sweater or jacket for cooler evenings.

Tuxedo Rental

If you registered to rent a tux for the Awards evening, you should have completed the online form with your measurements. Tuxedos will be delivered for fittings between 3:00 and 5:00 p.m. Sunday, June 14 in 7 Deli. Cost for tuxedo rental is $200.00, including tax, fitting and delivery. Don’t forget your black socks! Tuxedo rental is a non-reimbursable, personal expense.

Gratuities

Arrangements have been made to cover all gratuities in connection with the Ameritas Top Performers and Group Sales Congress programs. Guides, drivers, bellmen and housekeeping staff have all been tipped on your behalf. We recommend that you tip only for services not included in the program, or for outstanding service. Please check with travel staff should you have any questions regarding tipping.

Name Badges

You will receive your name badge in your welcome packet at the Ameritas hospitality desk. Please wear your name badge to all scheduled events as they are an important form of identification to travel staff, transportation and hotel personnel and all others providing services for you. Perhaps more importantly, name badges are greatly appreciated by fellow attendees until all faces become familiar.

Time Zone

During your stay, Beaver Creek will be operating on Mountain Daylight Time. Therefore, when it is noon in Beaver Creek, it is 11 a.m. in Los Angeles, noon in Denver, 1 p.m. in Chicago and 2 p.m. in New York.

Effects of High Altitude

At the high elevation in the Rocky Mountains, everyone is affected by altitude to some degree. As you go to higher altitudes, the barometric pressure decreases, the air is thinner, and less oxygen is available. The air is also dryer and ultraviolet rays from the sun are stronger. Elevations of 6,000 feet and higher cause your body to respond by breathing faster and more deeply, resulting in shortness of breath, especially during exertion. Some people develop mild symptoms of headache, nausea, trouble sleeping and unusual tiredness, also known as Acute Mountain Sickness or A.M.S. These symptoms usually go away in a day or two. If symptoms are severe, persist or worsen, you should consult a doctor. A short visit to the physician may enable you to enjoy the rest of your stay. You can decrease the effects of high altitude by following these recommendations: • Increase fluid intake, beginning a couple of days before your trip. Drink two to three times more fluid than usual. Water and juices are best. Adequate hydration is the key to preventing altitude sickness. • Avoid alcohol and minimize caffeine on your arrival day and one to two days thereafter. Be very careful consuming alcohol and remember, at high altitudes you may be much more sensitive to the effects of alcohol and sedatives. • Decrease salt intake. Salt causes your body to retain fluid (edema), increasing the severity of altitude illness. • Eat frequent small meals high in carbohydrates that are low in fat and protein. • Maintain moderate physical activity and get plenty of rest.
