Medical devices play a crucial role in in prevention, diagnosis, treatment and rehabilitation of illnesses. Globally, medical device manufacture is governed by rigorous safety standards in order to ensure the safety of patients. Given their importance in treatment, billions of healthcare givers and seekers the world over are eagerly looking forward to the latest developments in the medical devices industry.

The huge machines seen in hospitals are now replaced by smaller ones. Even the results of the diagnosis are now available in a tablet or a smartphone. Now there is no need for the patients to wait for hours in hospitals for examination and to get the test results as ‘point of care’ concept is developed where the machines will reach out to the patients. Even though the machines are smaller, these do not compromise on accuracy. Now the trend is to bring in “Point of Care’ (POC) devices so that there is no need for the patients to visit hospitals or laboratories for diagnosing diseases. It is used in doctors’ room, hospitals, and in patients' homes which gives quick feedback on many sorts of medical tests. POC devices are commonly used for testing glucose and cholesterol levels, do electrolyte and enzyme analysis, test for drugs of abuse and for infectious diseases and for preg-

nancy testing. Blood gases, cardiac markers, and fecal occult blood tests can also be done with POC diagnostic devices. But, the technology had developed with more medical devices companies using most modern technology for manufacturing POC devices. New techniques have helped to reduce the radiation level of machines for treatments like chemotherapy and also for X-rays.

According to Nithin Besent, Bio Medical Engineer at Medical College Ernakulam, Kerala, the new trend in the medical device industry is to replace the conventional machines to hand- held machines. Earlier, there were huge machines seen in Intensive Care Units and now it is slowly disappearing with doctors getting all the data of the patients in their tablets or smart phones. “ Technology has developed to such an extent that the use of huge Ultra

There is no need to go to hospitals for detecting cancer as hand- held devices have been developed for mammography. Most of the hospitals have this facility so that screening has become very easy. During colonoscopy, the doctor uses a colonoscope, a long, flexible, tubular instrument about 1/2-inch in diameter that transmits an image of the lining of the colon so the doctor can examine it for any abnormalities. But now swallowing a tablet will enable the doctor to get images inside the body. The tablet will transmit the images to a machine, tablet or the mobile phone of the doctor. The tablet will be dissolved when it reaches the intestine.

He also pointed out that the smaller machines also do not compromise on technology. The technology development helps doctors to diagnose disease quickly and take decisions for treatment. Now X-ray machines with low radiation to patients and staff are available. Now the machines are designed in such a manner that the machines could position itself to whichever area the X-ray should be taken without the movement of the patient. There is no need for fear of hair loss during Chemotherapy, if this device is used. “This machine will eject a liquid which will cool the scalp and there will not be any hair loss. The machine has liquid gas inside it and it also prevent from medicine entering into the scalp” he said.

Dental Surgeon Dr. Deepak Babu B confirms that the treatment has become more efficient with the introduction of new devices. Laser technology is used for surgeries in mouth without the presence of blood and even there is no need for anaesthesia.

echnological innovations has created a revolution in the health care sector and changed the way how diseases are viewed and managed across the world. Cardiovascular science has advanced so far that the diagnosis and treatment has not only become simple but health information is available at the finger tips of both the health care provider and health care receiver. Starting from the age old tradition when teaching was limited to the patient bedside, we are living in the age of electronic gadgets and internet where diagnosis and treatment is possible anywhere in

the world even from a pocket mobile. The global health care has been miniaturized as a result of these innovations. Cardiovascular medicine is undergoing radical changes day by day as a result of the vast amount of information and resources being channeled into the field across the globe. Important advances are being made in all areas starting from acute ischaemic syndromes to heart failure and end of life care.

THE EVOLUTION OF CARDIOVASCULAR SCIENCE Before the advent of the stethoscope, heart sounds were heard by putting

the ear to the chest wall of the subject. Then came the stethoscope, the main tool for the diagnosis of diseases related to the heart. It was invented in France in 1816 by RenĂŠ Laennec at the Necker-Enfants Malades Hospital in Paris. However its importance is coming down day by day all because of technological advancement. The first diagnostic coronary angiogram was performed by Dr. Mason Sones in 1958. This was followed by the first balloon angioplasty procedure on a coronary artery in Zurich, Switzerland, by Andreas Gruentzig, a German-born physician in 1977. Now only 15% of the cases are managed by CABG for the same indication. Catheter based technologies have improved so much that even valve replacements are performed via catheter based routes ( TAVR) . Now we are living in an era where people walk with wearable gadgets and integrated systems that monitor, control and optimize the functions of the heart.

A PEEP INTO THE WORLD OF TECHNOLOGY Advances in cardiovascular science can be stratified into 3 main areas: 1. Diagnostic technologies 2. Drug treatments 3. Interventions The scenario has changed drastically from what was 50 years ago to the present day and we shall discuss how the management of heart diseases has evolved over the decades.

DIAGNOSTIC TECHNOLOGIES Since the late 1990s, imaging in cardiovascular field has evolved

from routine coronary angiography to computerized tomography [CT], cardiac magnetic resonance, electron-beam computerized tomography [EBCT], and advanced echocardiography. Intravascular coronary imaging techniques like IVUS (intra vascular ultrasound) & OCT (optical coherence tomography) has created a revolution in the field of coronary angiography and angioplasty. Functional imaging techniques like Fractional flow reserve (FFR) has changed the way we manage atherosclerotic diseases. 3D electrophysiologic mapping techniques have in fact created a revolution the way rhythm disorders of the heart are managed and treated. Now hand-held biosensors capable of detecting a wide range of diseases within minutes by analyzing a drop of blood, urine, saliva, or breath have been introduced. This is in fact a “lab on a chip� (a miniature chemical laboratory as small as 3 mm square also called micro-chip) that detects the presence of particular proteins or gene expressions as well as compares patterns of genes or proteins characteristic of particular disease states. Micro- chips also are capable of detecting single-nucleotide

The work of the Human Genome Project, in the early 2000s was a turning point in the history of cardiovascular medicine and health care. The Genomic revolution has been able to provide individualized and personalized treatment and changed the health scenario drastically. The end result was this: genomics (the study of the genome), proteomics (the study of the full complement of human proteins), bioinformatics (the methods of gathering and processing the above information) and systems biology (the study of how all of these processes work as a complex adaptive system). As a result, in cardiovascular medicine two vast fields were created: predictive diagnostics and pharmaceuticals. Pharmacogenomics encompasses all genes in the genome that may determine drug response.

INTERVENTIONS The device therapies have continued their rapid improvements during the course of the last 30 years. Stents have evolved from bare metal stents to drug eluting stents to absorbable ones of late. The platforms have changed and can be placed as small as 2 mm coronary arteries. New materials have greatly reduced the incidence of re-stenosis. Catheters have become more maneuverable and smaller, affording interventionists a wide range of capabilities, from the traditional angioplasty. Laser

techniques encouraging the growth of new blood vessels, and the direct placement of new angiogenesis drugs are in the evolving phases. Apart from this, smart phones, smart watches and a wide variety of gadgets have surfaced which accurately capture various biochemical and vital information without the need to draw blood as was the case before. We have pacemakers, AICDs and CRT devices which are MR compatible to those which automatically switch modes when a magnetic field is detected. Leadless pacemakers have been launched. Telemetric monitoring is increasingly been used to detect events and administer therapies as is routine in our institution. Beating heart surgeries have become the talk of the day. Minimally invasive surgery and intervention techniques are now used for all routine heart surgeries. Sternotomy is now becoming rare and employed only in difficult cases or for full organ replacement procedures. Aortic aneurysms and dissections which were previously unmanageable and associated with high mortalities and morbidity has been simplified with the advent of various endovascular repair techniques. Robotics has emerged as a tool for managing a wide variety of cardiovascular diseases. Tele robotics has made its advent in India of late and is leading the world stage. Replacement hearts (electro-mechanical and swine hearts) are going to be so common in the near future and we are expecting replacement coronaries constructed from bovine and swine collagen as well to be the talk of the day in the days to come. Neo-organs grown in the laboratory from the patient’s own cells are starting to capture headlines. Researchers have been successful in growing thin sheets of heart muscle capable of sustaining a heartbeat to growing entire hearts, complete with vessels for carrying the blood supply. Several other high-technology devices can enhance the diagnostic capabilities of the practitioner which include DetecTar, a light-emiting diode to visualize and detect subgingival calculus, the Perioscope, a highly illuminated microcamera lens that can peer into a periodontal pocket and magnify the area almost 50 times.

Surgeons will benefit the most from such systems by extending the safe limit for more efficient operations. The VR and Augmented Reality systems seem promising, but there are still technological challenges that researchers and developers must face in order to fulfil these promises.

Now an array of new products and equipment are used for enhanced treatment. Computer-aided design/ computer-aided manufacturing, or CAD/CAM, has evolved within the past few years to create dental restorations. As technology developed, more powerful processors are available at lower costs and it is economically feasible to install such a device in a dental practice. The non-clinical benefit of installing such device is the time savings for patients and dentists, because the process can be completed in one visit. Wax-ups, copings and restorations can be created in a dental laboratory with great accuracy using laboratory CAD hardware and software and also

umanity has mastered the art and craft of prolonging life of the people with treatment utilising the help of various medical equipment. We have come a long way from the days of the apothecare who was considered the First in the fraternity, Onnaaman in Malayalam, In my childhood, I had seen an apothecare who used to divine diseases with the help of feeling the pulse, of course, he had a stethoscope of yore. Also he had what was called a steel pen and ink-pot, with the ink ball dissolved in water, for writing out prescriptions of medicines. Even during those times various experiments were going on in different

parts of the world to tame the deadly diseases that wiped out millions of people globally. Of course, these experiments were going on with the help of tools, medical gadgets and instruments. In the case of medicines also, there were fast- changing developments for the betterment of the people. At one time, millions of people had died of typhoid before the invention of penicillin. Later developments had seen to it that deadly diseases like pneumonia, small pox, various types of plague and subsequently, HIV etc could be contained. Leptospirosis, Nipah virus, Dengue fever etc have taken the toll of many lives, no doubt about it. Developments have overtaken in many cases to tame abominable diseases, for example, using anti-retroviral medicines HIV patients can survive for a very long time. Modern medicine is a fast- developing stream of dealing with every conceivable ailment, to the betterment of human kind. Now even pre-term babies can be supported with the help of incubators, nebulizer for taking care of breathlessness and asthma and the like. I can vouchsafe it for the fact that my son had arrived in the world at slightly less than seven months of his mother’s pregnancy. Our lives were churned by hospitalization of the kid for taking care of tenuous situations. The baby had also suffered for a very long time on account of wheezy bronchitis. He could stand on his own feet and take “frightful� (for the parents) steps only at the age of three years. But he would not sit idle anywhere, one should hold both of his stands over his head and he would run around the house. I had noticed one thing, that was a special gift, he could talk cogently

from the very early stage, which means he could express his feelings and fleece from his parents whatever he wanted. His life has been ensured using what all equipment were available there at the hospital. The pediatrician taking care of him all through his life (At the age of 14, the doctor had told us he is no more a child, he should be examined by a doctor for adults, but since the pediatrician was his stay-secure we used to consult him even later) had taken special care for him always. When we visit hospitals with various problems, necessitating consultation with very many specialists we come across use of innumerable number of gadgets, without which the patients would not have been all right to such an extent as is possible. ECG, Ultra Sound system, MRI, etc are now commonly used in addition to the preliminary X-ray examination. I know a person in the neighbourhood of the ancestral house who used to live 20 years extra using a pace-maker for the heart, that means he was 80-plus when he collapsed and died. Of course, he had recharged the battery once earlier. He had gone for recharging the battery a second time, but he was unwilling as he and his family members were witness to a patient dying during the process of recharging. An old friend of mine here had developed heart problems which necessitated implanting of a pace-maker. He was reluctant at first saying he is now an old man and so let fate decide about his life. But his family members insisted on his prolonging life with the help of the pace-maker. He has since been all right after implantation of the gadget. of a Parkinson’s Disease patient using such a facility for his brain. The process is called Deep Brain stimulation. The patient himself is a medical doctor, and after undergoing the expensive method, he could strut around with the help of a walking stick. Tremor, propensity for falling down have all been reduced considerably. I have read in the literature that expenses for the same could come up to Rs 90 lakh. Not all ordinary mortals can choose this method, for sure, but those who can afford should definitely take the chance. NATIONAL MEDICAL DEVICES PROMOTION COUNCIL TO COME UP IN INDIA

T

o give a boost to the medical devices industry, which is a sunrise sector, the Union Govt will set up of a National Medical Devices Promotion Council. It will be an agency under the Department of Industrial Policy and Promotion (DIPP) in the Ministry of Commerce and Industry. The proposed national council, headed by the Secretary, DIPP, would achieve convergence as medical devices were covered under various Ministries and departments. Apart from the departments concerned, it will also have representatives from healthcare industry and quality control institutions. 24 Medical Tourism Jan. - Mar. 2019

FUNCTIONS OF THE PROPOSED NMDPC The NMDPC will undertake several activities, including facilitation, promotion and development of the sector, besides holding seminars and workshops to garner views of industry and understand best global practices. It would also identify redundant processes and render technical assistance to the agencies and departments concerned to simplify the approval processes involved in the sector. It will enable entry of emerging interventions and support certifications for manufacturers to reach levels of global trade norms and lead India to an export- driven market in the sector.

It supports dissemination and documentation of international norms and standards for medical devices, by capturing the best global practices and facilitate domestic manufacturers to rise to international level. It will also drive a preferential market access policy, by identifying the strengths of the Indian manufacturers and discouraging unfair trade practices in imports. Besides, it would make recommendations to government based on industry feedback and global practices on policy and process interventions to strengthen the medical technology sector. THE MEDICAL DEVICES INDUSTRY IN INDIA The Medical Devices industry in India has steadily grown and is valued at more than $5 billion. As per industry estimates, the Indian medical devices market will grow to USD 50 billion by 2025.

Currently, India is counted among the top 20 global medical devices market and is the 4th largest medical devices market in Asia after Japan, China, and South Korea. Up to 100% Foreign Direct Investment is permitted in Medical devices through the automatic route.

CHALLENGES TO MEDICAL DEVICE INDUSTRY IN INDIA Low Penetration - The per capita medical device spending of USD 3, compared to USD 7 in China and USD 42 in Russia is significantly low. Lack of Accessibility - The Indian healthcare system is inadequate, inefficient, and unevenly distributed. 69% of the Indian population lives in rural areas, while 73% of qualified consulting doctors reside in urban areas. Eight percent of qualified doctors are in rural areas, and the remaining 19% are in semi-rural areas. Inadequate regulatory systemsNon-alignment with global standards and the lack of quality product testing infrastructure are issues that hinder sectoral progress. Real estate prices and high capital costs limit the growth of delivery infrastructure. Insufficient attention by policymakers and a complex tax regime are also responsible for the sector’s under-development. The lack of a comprehensive policy and focus to develop the healthcare eco-system. The limited attractiveness of India as a destination for medical devices due to uncertain regulations and pricing environment, unavailability of skilled resources, and ease of doing business compared to other comparable destinations in Asia. The best solution would be to get back home and into their own healthcare system. For Orthopaedic and spinal surgeries, since the x-rays clearly indicate the surgical pathway, the doctors can offer reviews via e-mails.

However, this would not be the appropriate way to address follow-up in a case of Lap Band surgery where the band needs to be adjusted periodically. INDIAN CITIES AND STATES IN THE PICTURE Chennai – has emerged as the healthcare capital of India - gets more

numbers than any other city in India. The other cities, which are statistically noteworthy, are Delhi, Mumbai, Hyderabad and Bangalore and, as states, Kerala and Goa are very much in the race. Kolkata and Ahmedabad are the new entrants. Chennai - The gateway to South India is renowned for its tertiary care facilities and comparative low cost as compared to Mumbai and Delhi. Delhi - As the national capital, well connected with all the major cities of the world, home to institutions like AIIMS, Apollo, Escorts, etc. Delhi comes a close second in attracting medical value travelers. However, non-medical expenses are higher here. Mumbai – India’s commercial capital, its well -connected airports make it a hub for medical tourism. As in the case of Delhi, Mumbai’s high priced non-medical charges makes its affordability a matter of need-based option. Kerala – Kerala has pioneered India’s wellness tourism with a host of renowned Ayurveda centers. However, it has failed to maintain this leverage especially with countries like Sri Lanka going heavy on wellness tourism. The cities of Trivandrum and Cochin have placed themselves well in attracting medical tourists. Trivandrum caters to 70% of Maldivians coming to India while Cochin with leading brands caters to Middle East traffic. The current trend is positive for Kerala. Other major cities like Hyderabad, Ahmedabad and Kolkata are all getting into the global foray with stateof-the-art facilities and international accreditations.

A notable effort was the establishing of the Indian Healthcare Federation with about 300 members organized by the CII.  Increasing competition and market offers. An apt example would be of the Bariatric surgery (a weight reduction procedure) offer in Mexico with 3 days for recuperation, including up and down flight fares from the USA for USD 8000. This procedure in India for someone from the US would cost about USD 5500 for the procedure and USD 2000 for flight fares. South Africa is another country that has set its focus on tapping medical tourists. We may not feel the threat yet considering the value of the Rand over the INR, but, in the long run, this can affect the current high traffic for treatment from African countries and the Indian Ocean island countries like the Seychelles and Mauritius. This again points to the urgent need for a coordinated approach from the Indian Healthcare industry to focus on those specialties in which the Indian medical fraternity has a distinct

advantage.  Finally, how well will the Indian private healthcare industry address the growing requirement of healthcare delivery in the domestic market? The rapidly advancing middle class, aided by an increasing health insurance spread, seeks a lot more from the private healthcare sector that must account for about 80 per cent of healthcare delivery in the country. The domestic disease pattern is yet another feature, and lifestyle diseases - especially cancer and heart disease - will increase the requirement of in-patient facilities that remain mainly with private sector tertiary care providers. Government spending in healthcare is more focused on primary and secondary care and although life expectancy in India has almost doubled from 32 years in the 1940s to 66 years in 2004, it still has a long way to go. Here, too, the huge opportunity for the private sector in Indian Healthcare is so abundantly visible.  Lack of standardization in the holistic options - including Ayurveda, Yoga and a gamut of such services - and lack of effort in combining

alternative medicine systems with allopathic treatment must be explored. This is a large area of untapped opportunity which has long been neglected. The Apollo Hospitals has already begun to focus on ‘Wellness Centers’ where alternative therapies are given their due. This is one area where India has a definite and indigenous edge and the opportunity to unleash its potential is phenomenal. Kerala has already shown us the way. F

ew months back I attended a public meeting in Alleppey organized as part of block level cancer screening project. A cancer awareness class was delivered. I was accompanied to the place by Venugopal who was one of the co-ordinator of the function. Venugopal also narrated to me his personal experience which spurred him towards this project. He said: “by the time we, the children, attained good positions, our mother had passed away without giving us an opportunity to take care of her”. His voice quivered and he stumbled over his words. A moment later he continuedmy mother was losing weight daily, became easily tired and her stomach was distended with water. We consulted numerous doctors in various hospitals. After conducting various tests they came to the conclusion that my mother had liver cirrhosis. Five to six valuable months were taken to arrive at this conclusion. Subsequently, we consulted the leading gastroenterologist in Ernakulam who confirmed that my mother had final stage Ovarian Cancer. It was already too late by then. We had already lost a valuable period of nine to ten months. There was therefore no opportunity for any effective treatments to be administered. She passed away soon after. I had been searching since then for an opportunity to do something for the early detection and treatment of Cancer. We instituted a charitable society in the name of our mother. But it was very hard to mobilize money and government grants to administer it. Therefore I believe that this block level project is an opportunity given by the Almighty to fulfil my ambition. I perceived the sincerity in his voice. Venugopal suddenly queried me: ‘Madam ... Hope we get good treatments for Ovarian Cancer now? Is it possible to cure it completely?

If ovarian cancer is discovered early, that is when it is at the localized stage, it can be treated and cured. But the unfortunate reality is that it is quite onerous to discover at that stage. Venugopal’s mother passed away 25 years ago. I felt sombre when I thought of the fact that even today the majority of patients are diagnosed at the same final stage as that of Venugopal’s mother. However, today better surgical procedures and superior chemotherapy drugs are available. We are thus able to prevent the advancement of ovarian cancer and control it, similar to controlling ailments like diabetes and high blood pressure. However, a complete cure is unfortunately still in the distant horizon unless detected in the early stages, more so in relation to epithelial ovarian cancers, which is the most common form of ovarian cancer in women. The Ca 125 blood test and ultrasound scanning are the best methods for early diagnosis of this cancer. However, both these tests have got certain limitations. Scientific studies propound that it is impractical for everyone to undergo these tests since the accuracy and cost effectiveness of these screening tests are not optimal. However, all women who have a family history of breast or ovarian cancer are recommended to undergo these tests biannually or annually. It should also be noted that the Ca 125 level need not be elevated in all incidents of ovarian cancer. The normal value of Ca 125 is below 35u/ml. However, the normal value may differ from individual to individual. For example, in some individuals it might be below 4 u/ml, for others their normal level might be 30 u/ml or 20 u/ml. It can be elevated to very high levels in non-cancerous conditions, for example, endometriosis. If the normal value increases suddenly or serial increase from what is generally an

individual’s normal level, caution will have to be exercised in evaluating this increase and close monitoring or further scans will be required. Despite the controversies, undergoing a Ca 125 blood test biannually or annually is considered to be useful in identifying early ovarian cancer. Having witnessed numerous piteous and hapless ovarian cancer patients, I think that it is only proper that a Ca 125 test be done by every woman at regular intervals, the regularity depending on their risk of getting ovarian cancer. An ultrasound scan also can be performed at regular intervals. Furthermore, it is also advocated that women undergoing hysterectomy (removal of uterus) for non-cancerous conditions subsequent to the age of forty-five, give serious consideration to removing their ovaries as well. Removal of both fellopian tubes is also considered as an ovarian cancer risk reducing strategy. Prevention measures of ovarian cancer based on genetic testing known as “Angelina Jolie Effect” sends immense hope to people across the world.

Studies say that the possibility of an Indian suffering from a lifestyle disease is 4 per cent more than one of any other nationality. According to a survey conducted by the Associated Chamber of Commerce and Industry (ASSOCHAM), 68% of working women in the age bracket of 2152 years were found to be afflicted with lifestyle ailments such as obesity, depression, chronic backache, diabetes and hypertension. In India, 10% of adults suffer from hypertension while the country

is home to 25-30 million diabetics. Some of the common diseases encountered because of occupational lifestyle are Alzheimer's disease, arteriosclerosis, cancer, chronic liver disease/cirrhosis, chronic obstructive pulmonary disease (COPD), diabetes, hypertension, heart disease, nephritis/CRF, and stroke. A healthy lifestyle must be adopted to combat these diseases with a proper balanced diet, physical activity and by giving due respect to biological clock. The most prevailing lifestyle disorders are induced by stress. Stress causes many diseases like obesity, mental disability, Alzheimer’s, arteriosclerosis and gastrointestinal disorders. People now-a-days are ageing very fast due to their work stress. Anxiety due to change in sleep pattern or improper sleep is leading to mental illness. To decrease the ailments caused by occupational

postures, one should avoid long sitting hours and should take frequent breaks for stretching or for other works involving physical movements. A diet of only junk food, overeating and lack of physical activity are not only responsible for diseases related to nutrition, but also anorexia nervosa, which involves many people starving themselves for maintaining their figure. This type of disease is more prevalent in the fashion and showbiz industry. The situation demands a general growth of awareness among today’s youth and shaking them from their complacency towards the self-destructive lifestyle they have adopted, and requires to make them realize the dangers posed by it. It calls for implementation of workplace health programmes, as the workplace environment largely influences the youth to promote better health behaviour.

uffering from hypertension? Take heart, Italian researchers have identified Spirulina, a superfood made from algae, that could help to reduce blood pressure. The superfood, described as the “most complete food source in the world”, contains a specific protein which was found to relax arteries in the laboratory and in animals, the Daily Mail reported.

The spirulina triggered relaxation of the artery, a process called vasodilatory action, which can help lower blood pressure by increasing the amount of blood pumped in one motion.

Spirulina, scientifically called arthrospiraplatensis, is technically a type of bacteria. Researchers at the Mediterranean Neurological Institute in Pozzilli, Italy, isolated certain molecules from the spirulina to test its health benefits.They simulated the effects of digesting spirulina on isolated arteries and animals in the lab.

Spirulina contains protein, iron, potassium, magnesium, and many other minerals and is widely used in supplements and smoothies for a host of conditions, including weight loss, hay fever, PMS and diabetes.

“We reproduced what happens in the human gut after ingesting the substance,” lead author Albino Carrizzo was quoted as saying.“This way we have been able to isolate the peptides that would be absorbed by our body,” he added. The study, detailed in the journal Hypertension, also discovered for the first time molecules in spirulina, called SP6, which lowered the blood pressure of animals. When put into the body SP6 is thought to stimulate a process in the body which releases nitric oxide.

This is an effect caused by nitric oxide, a chemical known for maintaining healthy blood pressure, the researchers reported.

Though more research is needed into the potential of spirulina, but the findings are promising for the future of “natural” treatments for high blood pressure, the researchers noted.

Spirulina is an organism that grows in both fresh and salt water.It is a type of cyanobacteria, which is a family of single-celled microbes that are often referred to as blue-green algae. It is often claimed that spirulina contains vitamin B12, but this is false. It has pseudovitamin B12, which has not been shown to be effective in humans. Spirulina is a fantastic source of antioxidants, which can protect against oxidative damage. Its main active component is called phycocyanin. This antioxidant substance also gives spirulina its unique blue-green color. Phycocyanin can fight free radicals and inhibit production of inflammatory signaling molecules, providing impressive antioxidant and anti-inflammatory effects .Phycocyanin is the main active compound in spirulina. It has powerful antioxidant and anti-inflammatory properties. Studies indicate that spirulina can lower triglycerides and “bad” LDL cholesterol and may simultaneously raise “good” HDL cholesterol. The antioxidants in spirulina appear to be particularly effective at reducing lipid peroxidation in both humans and animals. Some evidence suggests that spirulina has anti-cancer properties. Research in animals indicates that it can reduce cancer occurrence and tumor size. Spirulina may have anti-cancer properties and appears especially effective against a type of precancerous lesion of the mouth called OSMF. A higher dose of spirulina may lead to lower blood pressure levels, a major risk factor for many diseases. arayana Super-speciality Hospital, Guwahati, conducted surgeries for two extremely rare and complicated diseases under the supervision of Cardiothoracic Surgeon, Dr Rajnish Duara. The first patient, Utpal Barman, 34yrs(M), was suffering from autoimmune disorder involving the Thymus Gland (Myasthenia Gravis) which causes extreme weakness of the muscles and Thyroid Gland (Grave’s Disease) for which a hyperactive thyroid gland present with toxic features, had to be taken care of.

The DVT was managed with blood thinning agents but due to extensive progress of the thrombosis the patient was implanted with IVC Filter by Cardiovascular Surgeon, Dr Rajnish Duara. This is the first time that such a procedure has been done at Narayana Superspeciality Hospital, Guwahati.

This type of operation involving thyroid and chest bone splitting procedure for Thymectomy is extremely rare in India. In another case, a critical patient from, Assam presented with a near fatal vascular disease with acute renal failure at Narayana hospital, Guwahati. Doctors said that like any other parent, Meena and Vinod were delighted after the birth of their son, but their joy didn’t last for long as they got to know that he was suffering from “Transposition of Great Arteries,” which is a congenital heart defect where the large blood vessels that carry blood from the heart to the lungs and to the body are connected exactly opposite of a normal heart’s structure. Adding to the criticality, the 20- day- old child also had two big holes in his heart, along with patent ductusarteriosus (PDA) causing breathlessness. The critical surgery was performed by senior consultant pediatric cardiothoracic surgeon, Indraprastha Apollo Hospitals, Dr Muthu Jothi and his team. Explaining about the case, Dr Jothi said, “Within a week after the child’s birth, the parents witnessed discoloration in his skin tone. His body was turning blue so the parents took him to a local doctor in their area where his echo was done. The reports revealed that he has transposition of great arteries.”

The patient Nageshwar Rao, who was admitted with severe chest pain and mild heart stroke, was suffering from Chronic Total Occlusion, which is complete blockage of coronary arteries. “Heart patients with complete artery blockages can now avoid bypass surgery thanks to this technique called Retro Grade Angioplasty. The procedure helps patients recover very fast and get back to their normal life quickly,” said senior interventional cardiologist, MaxCure Hospitals, Dr.Sharath Reddy. According to cardiologists, the Retro Grade Angioplasty technique, which was invented by the Japanese, must be performed by trained doctors and a good team of healthcare workers from multiple health departments.

SURGERY ON A PATIENT WHOSE BRAIN TISSUE WAS OOZING OUT THROUGH A HOLE IN THE EAR Loganathan, 54-year-old from Erode, was suffering from severe periodical headaches and right ear pain/block for the past one year. Though the patient visited several hospitals in and around Tamil Nadu, the reason for his head or ear ache could not be diagnosed. One of the hospitals even performed a tympanomastoidectomy for Right Chronic Otitis media considering this as an ear infection. A few months later, the patient started developing further complications including a clear watery discharge from his right ear and persistent headache. Patient later consulted a couple of other hospitals before presenting himself to Apollo Hospitals OMR, Chennai. On investigating the patient Dr.Krishnakumar, HoD, ENT, Apollo Speciality Hospitals OMR, identified a pink, pulsatile, soft, insensitive mass, which did not bleed on touch filling the external auditory canal. A CT scan and a Brain MRI revealed a huge defect(hole) in the upper wall of the inner ear (right side) through which a small portion of the brain's temporal lobe was prolapsing out. On observing this condition Dr.Krishnakumar K, HoD, ENT, Apollo Speciality Hospitals, OMR and Dr. Joy Varghese, Senior Consultant, Neuro Surgeon & Interventionist, Apollo Speciality Hospitals, OMR performed a procedure to close the defect (hole) in the ear's acoustic canal containing the brain tissue that was exposed. Dr. Joy Varghese, Neurosurgeon opened/removed a small piece of the skull and

peeled out the inner softer portion of the skull (the hard part was replaced from where it was removed from) and used it to seal the hole (10mmx6mm) in the ear, after removing the prolapsed part of the brain. ENT surgeon Dr. Krishna Kumar meticulously removed the prolapsed brain tissue and sealed the defect from below. Efforts of both the expert surgeons saved the 54-year-old man from a life-threatening problem. Dr.Krishnakumar, HoD, ENT, Apollo Speciality Hospitals, OMR explained, "It was an eight-hours’ long complicated surgery that saved the patient. Such occurrence is incredibly rare in adults, but the tough membrane around that part of the brain can occasionally rupture under pressure. The cause of this hole in his ear might have been a result of an accident that the patient met with 10 years ago or even a prolonged ear infection. If left untreated the patient might have had sequence of seizures and would have eventually collapsed." Speaking about the procedure, Dr Joy Varghese and team at Apollo Speciality Hospitals, OMR, said. "In this case, the protrusion was a part of the temporal lobe, that is responsible for memory and emotions, removal of this would not affect the patient because it was such a small area and it was any way scarred. Post-operative period was uneventful, with a very smooth recovery from anesthesia and complication free postoperative stay. Patient has recovered well and was discharged, a week after surgery."

The procedure for both children was done under the Aarogyasri scheme of Andhra Pradesh government.Muni Dimple, a 12-year-old girl from Madanapalle of Chittoor district, and Lokesh Babu (6) from Prakasam district admitted to the KIMS pediatric cardiac surgical team with an uncommon

condition where the blood comes to and goes out of the heart in a reverse fashion. After being followed up closely for two years by Dr D. Anil Kumar, senior consultant, pediatric cardiac surgeon, and his team, both the kids underwent a 12-hour challenging operation. Dr. Kumar said the condition is not very common and to do complete surgical correction, both pathways coming into the heart and going out have to be switched.
