am deeply saddened by the recent publicised spate of taxi rapes in Johannesburg and these are only a few of hundreds that occur daily. It is unacceptable that we live in a society with a rape culture, where the victim is often shunned for “bringing it upon herself” typically by means of what she chooses to wear. I want to make it clear that a woman should feel comfortable to show as much or as little skin as she wants without fear of sexual harassment or rape. It is not the length of her skirt that should be called into question, but the values and attitudes of the perpetrators of sexual violence. And rape is a violent act. There is no love or romance attached to the invasion of a woman’s body. Sexual violence and South Africa’s embarrassing rape statistics have become our enduring nightmare, but we have the power to change this legacy. Why are we teaching our daughters “anti rape 101” instead of schooling our sons in the sanctity of human rights and a woman’s right to say no? Children learn what they live. Show them how to treat each other with dignity and that no means no. Let’s nurture our sons and raise them to be protectors of women and children rather than perpetrators against them.

We want to hear from you!

Our parents may have casually mentioned “the birds and the bees” to us as we entered puberty, but our own children are far beyond these misguided metaphors. We need to put our personal feelings and discomfort aside and start the conversation as early as possible. They need frank discussions around their bodies, their rights and consent. Sex is not a patriarchal right and it is time we eliminated all sense of entitlement and misogyny around it. When the cellphone video of Sesethu did the rounds, the young teenager was quickly slut shamed by the public, friends and even her family. Yes, the video was shocking but let’s use this experience as a teachable moment. As the mother of a girl child, I am even more determined to love and nurture my daughter so that she feels secure at home. I will also teach her that she was born free and no one has the right to hurt or abuse her. And when she is faced with the inevitable curiosity, confusion and temptations of adolescence, I hope that she will not fear judgement from those closest to her but seek their guidance and comfort. She will know that real men don’t rape.

TREATING INFERTILITY WITH

Technology T

he advancements in science and technology have not only made our lives more convenient, but they have also increased our chances of giving life. The use of ultrasound equipment, incubators, microtools and microscopes has opened the doors for improved chances of conception.

1. TRANSVAGINAL ULTRASOUND An ultrasound test uses high-frequency sound waves to create images of your internal organs. Imaging tests can identify abnormalities and help doctors diagnose conditions. A transvaginal ultrasound is a type of pelvic ultrasound used by doctors to examine the female reproductive organs. These include the uterus, ovaries, cervix and vagina. “Transvaginal” means “through the vagina” and is an internal examination. Unlike a regular pelvic ultrasound, where the ultrasound wand rests on the outside of the pelvis, this procedure involves your doctor or a technician inserting an ultrasound probe into the vaginal canal. This is used extensively in fertility treatment to examine egg follicle development and the uterine wall thickness. It is also used for transvaginal egg retrieval (egg collection). Under ultrasound guidance, the operator inserts a needle through the vaginal wall and into an ovarian follicle, taking care not to injure the organs located between the vaginal wall and the ovary. The other end of the needle is attached to a suction device. Once the follicle is entered, suction is gently applied to aspirate follicular fluid and with it, hopefully, cellular material including the oocyte (egg). The follicular fluid is delivered to a technician in the IVF laboratory to identify and quantify the ova (eggs) using a microscope.

2. EMBRYO INCUBATOR This is used for embryo culture, a component of in-vitro fertilisation where resultant embryos are allowed to grow for some time in an artificial medium. The incubator takes the

place of the fallopian tube for five days and is calibrated to a specific temperature and pH.

3. MICROMANIPULATOR This device is used to physically interact with a sample under a microscope, where a level of precision of movement is necessary that cannot be achieved by the unaided human hand. It may typically consist of an input joystick, a mechanism for reducing the range of movement and an output section with the means of holding a microtool to hold, inject, cut or otherwise manipulate the object as required.

4. MICROSCOPE INTEGRATION Micromanipulators are usually used in conjunction with microscopes. Depending on the application, one or more micromanipulators may be fitted to a microscope stage or rigidly mounted to a bench next to a microscope. A typical application of micromanipulation is human intracytoplasmic sperm injection. Here, a sperm cell measuring just three to five micrometres across is injected into an oocyte of approximately 100 micrometres in diameter, under the direct manual control of an embryologist. A disposable glass micropipette is fitted to a tool holder mounted on the output of the manipulator. The tool holder can be adjusted for different sized tools, as well as the angle at which the tool is held.

5. LAZT – Laser-Assisted Zona Thinning Prior to the implantation, the embryo has to escape from the zona pellucida, the outside capsule of the embryo, in a process known as hatching of the blastocyst. Some embryo implantation problems in patients with recurrent implantation failure may be explained by the inability of the embryo to hatch out of its zona pellucida. In such cases, the zona pellucida can be thinned in one part using this laser technique to improve the pregnancy and implantation rate.
