SUBMIT INFORMATION Please mail completed application along with non–refundable $50 application fee to Kroc Keiki Learning Center, 91-3257 Kualaka’i Parkway, Ewa Beach, HI 96706. Make checks payable to The Salvation Army Kroc Center Hawaii. We also accept Visa, Mastercard or American Express. Please call (808) 693–8336. Enrollment is on a first come basis, based on the date your application and deposit are received. You will be notified as openings become available.

PREPARE FOR SCHOOL Upon enrollment, you will receive an enrollment packet to complete before beginning school. These will include various release forms and Student’s Health Record, form 14 (to be completed by your child’s pediatrician). The Department of Human Services requires us to have information on your child’s last physical exam (within one year of starting school), immunization records and TB clearance.
