Once a family has been served, they will be given or mailed a Family Contact packet containing an letter of explanation, evaluation form, and return envelope for them to fill out at their own convenience. The Family Contact packet can also be included in the clientfamily’s aftercare information and materials. OGR will handle the rest. Once client-families return the completed survey forms, OGR compiles the responses. Actual survey forms are returned to funeral homes each month. Any negative comments are faxed immediately to prevent any additional lag time to evaluate and follow up on the comments. Surveys can be easily tracked by writing a coded case number on the survey of any particular family. At the end of the month, OGR provides a Monthly Report of activity along with all the surveys received on the members behalf during that month. Also, each January participants will receive an Annual Summary with ratings of all participating OGR Members to benchmark your firm.

OGR also uses client-family comments to select Exemplary Service recipients each quarter for OGR’s publication, The Independent, and for eligibility for the Annual Exemplary Service Awards given at OGR’s Annual Conference & Supplier Showcase. OGR values our members and client-families. Information gathered from Family Contact is never shared with third parties. Client-families can also choose to fill out the survey form anonymously.
