Pathogenesis of Pain in Osteoarthritis TONIA VINCENT, PhD University of Oxford, United Kingdom Audience: Cross disciplinary | Topic: Musculoskeletal pain and rheumatology There has been considerable speculation over the years about where pain arises from in the osteoarthritic joint. Epidemiological studies provide weak correlation between structural change in the synovium and bone with pain onset and severity. Mechanistic studies in rodents are beginning to paint a different picture which is supported by molecular studies of human tissues. A new paradigm for origins of peripheral pain in OA is proposed.

This session will provide an overview of historical and current pathogenic mechanisms of diabetic peripheral neuropathy and of the associated neuropathic pain. The clinical presentation of diabetic neuropathy and neuropathic pain will be briefly reviewed, followed by an appraisal of the animal models of diabetic neuropathy and of the pertinence of common techniques for measuring neuropathy and neuropathic pain in these models. The relative importance of metabolic signatures of diabetes such as hyperglycemia, impaired insulin signaling and dyslipidemia to initiating pathogenic cascades that lead to neuropathy and neuropathic pain will be considered. Pathogenic mechanisms derived from cellular, biochemical and molecular consequences of diabetes in the nervous system that may contribute to diabetic neuropathy will be evaluated along with potential mechanisms of peripheral and centrally mediated neuropathic pain. Convergence points such as mitochondrial dysfunction will be highlighted and the potential for novel therapeutic interventions based on pathogenic mechanisms derived from animal models of diabetes will be illustrated by recent preclinical and clinical studies.
The spinal cord dorsal horn is an important region within the central nervous system for the processing of sensory information. Although we know there is specific circuitry for pain, itch, temperature and innocuous touch, we still do not understand how this somatosensory information is integrated in the dorsal horn. The development of cell type-specific genetic tools that allow us to modulate neuronal function has provided some new insight, but also highlighted the challenge of defining cell types. Here we discuss recent efforts to classify dorsal horn neuron populations and investigate their function.
