Welcome to Your 2023 Benefits

We recommend that you thoroughly review this entire guide before making your benefit decisions.

In addition, we encourage you to:

• Compare this coverage to your spouse’s coverage to optimize all benefits available to you.

• Estimate the types of services you might need in the upcoming year to ensure you aren’t under or over-insured.

• Consider the needs of your child(ren) and any other dependents.

Benefit Eligibility

Full time employees working at least 30 hours per week and their eligible dependents are permitted to participate in the Temp-Con/TP Mechanical benefits program. Benefits effective date: 1st of the month following a 30-day waiting period.

IRS Qualifying Life Event

After your initial enrollment, you are not permitted to make a change to your benefits unless you experience an IRS qualifying event or until open enrollment (January 2024). If you experience a qualifying event, you must notify the Human Resources Department within 30 days of the event to make a corresponding change to your benefit elections.

1. Log in at tp.4mybenefits.net.

2. Click on “Enroll”.

3. Login with your username and password.

Temp-Con Employees

Your username consists of:

• “TC”

• Your current Employee ID Number, with leading zeros to make it a 4-digit number

• The year you were born

Your password is your birthdate in this format: YYYYMMDD

EXAMPLE: John Smith, whose Employee ID number is 4 and was born in 1975, would have a login of “TC00041975”.

A birthdate of February 7, 1975 would look like this: “19750207”.

TP Mechanical Employees

Your username consists of:

• “TP”

• Your Employee ID (can be found on your check stub)

• The year you were born

Your password is your birthdate in this format: YYYYMMDD

EXAMPLE: John Smith, whose Employee ID is SMITJ and was born in 1975, would have a login of “TPSMITJ1975”.

A birthdate of February 7, 1975 would look like this: “19750207”.

4. Click “Login”. You will then be taken to the Welcome Screen to begin your enrollment, make changes to your benefits, see your benefit summaries & review benefit plan information, among other resources.

5. Click “Get Started” to begin enrolling.

6. Once you are enrolled, you will be given a Confirmation Statement to download, email or print for your own records.

7. Review your enrollment elections for accuracy.

8. If you need additional assistance or prefer to enroll over the phone, please call 1-866-575-5089.

How To Enroll
All employees are required to complete the benefit election process in 4myBenefits. If you do not complete the election process, you will not have coverage for the 2023 plan year.

What’s the Benefit?

Employer-sponsored health insurance pays for part or all of the medical services you (and your covered family members) receive.

How Does it Work?

You receive an ID card to share with your health care service providers, such as your primary care doctor, that will give them information about whom to bill for the services you receive. You will get an explanation of benefits from your medical carrier after your visit. This document will tell you how much of the total cost was covered by the medical plan and the portion remaining for you to pay your medical provider(s).

Considerations:

Temp-Con/TP Mechanical offers three medical plans options. Here are a few tips to consider when determining which plan is right for you and your family:

$10 per week will be added to your rate if you are a tobacco user. An additional $10 per week will be added to your rate if your spouse is a tobacco user.

Employees who are benefit eligible on or before 05/01/2023: The discount will apply to you if you (and your enrolled spouse) are tobacco free as of 05/01/2023.

Covering Your Spouse

If you decide to enroll a spouse who has access to medical coverage through his/her employer, $30 per week will be added to your rate. Employees enrolling a spouse on the medical plan must complete a Spousal Affidavit on the enrollment site.

It is recommended that you use in-network providers and facilities for medical care. You do have access to out-of-network benefits, but there is an additional cost associated. You may be balanced billed if you go to an out-of-network provider.

To determine if a doctor or facility is considered in-network:

1. Go to www.umr.com and click on “Find a provider”.

All Temp-Con/TP Mechanical medical plans are part of the “UnitedHealthcare Choice Plus” network.

2. When prompted, enter the network name in the text box and begin searching.

Health Reimbursement Arrangement (HRA)

What’s the Benefit?

A Health Reimbursement Arrangement (HRA) is an employer-funded account that helps to offset some of the major medical expenses employees receive.

How Does it Work?

Only employees who select the “HRA” medical plan option, will have access to the HRA benefits. When you incur medical expenses that are subject to the deductible, the HRA will help pay for a portion of those expenses. However, the HRA will only pay 50% of deductible expenses up to the maximum amounts listed in the table below.

The HRA is fully integrated with the UMR HRA medical plan – this allows employees to automatically obtain the HRA benefits without having to take any additional steps.

You will be able to see how the HRA works by viewing the Explanations of Benefits (EOBs) sent to you after receiving medical care.

Health Savings Account (HSA)

What’s the Benefit? A health savings account (HSA) is a savings account that can be used to pay for qualified health expenses (medical, dental, vision, etc.)

How Does it Work? Only employees who select the “HDHP with HSA” medical plan option, will be able to contribute to an HSA. When you enroll in an HSA, you elect an amount to be taken from your paycheck (pre-tax) and placed into a savings account. You can use your HSA at the time of service, when you get a bill or to reimburse yourself on the back end.

Eligibility:

There are certain reasons why you may not be eligible for an HSA, these include:

• You or your spouse are enrolled in a NON-QUALIFIED health plan

• You or your spouse are contributing to a healthcare flexible spending account (FSA)

• You are enrolled in VA, CHIP, Medicare or Medicaid benefits

• You can be claimed as a dependent on another person’s tax return

Considerations:

• You must be enrolled in a qualified high deductible health plan to contribute to an HSA.

• Your contributions into the account and distributions made from the account are tax exempt.

• Make sure you use the account only for qualified expenses or else penalties will apply. A list of qualified expenses is set by the IRS and can be found at www.irs.gov.

• HSA funds rollover from year to year and you may have the opportunity to invest the funds.

• You can use your HSA on your spouse or tax-dependent children (even if they aren’t covered by your medical plan).

Healthcare FSA Limited FSA*

A healthcare flexible spending account (HCFSA) is an account that can be used to pay for qualified medical, dental and vision services.

The full amount you elect to contribute is available to you upfront. You can use your FSA at the time of service, when you get a bill or to reimburse yourself on the back end.

You cannot contribute towards an HSA if enrolled in a healthcare FSA.

A limited flexible spending account (LFSA) is an account that can be used to pay for dental and vision services ONLY.

The full amount you elect to contribute is available to you upfront. You can use your LFSA at the time of service, when you get a bill or to reimburse yourself on the back end.

*The LFSA is only an option for those who are also contributing towards an HSA.

Dependent Care FSA

A dependent care FSA (DCFSA) is an account that can help pay for daycare or other eligible expenses.

The funds that you elect to contribute will be available as you contribute them. You can use your DCFSA when you get a bill or to reimburse yourself on the back end.

You CAN have an DCFSA and be contributing towards an HSA. Make sure you are using an eligible daycare provider.

Funds do NOT roll over to the next calendar year - any unused funds will be forfeited at the end of 2023.

Disability Coverage

Employer-paid Short-term Disability (STD) Coverage

What’s the Benefit?

All employees are provided with short-term disability coverage at no cost! Short-term disability coverage protects a portion of your income should you be unable to work for a short time due to illness or injury.

How Does it Work?

A weekly benefit, paid by a mailed check, is provided to help pay for your living or medical expenses once you have met the elimination period (time before the benefit will begin to pay).

Option to Enhance Your Short-term Disability

Voluntary Long-term Disability (LTD) Coverage*

What’s the Benefit?

Long-term disability coverage protects a portion of your income should you still be unable to work due after 90 days. All employees have the option to purchase long-term disability coverage, but will be responsible for the full cost of the coverage.

How Does it Work?

A monthly benefit, paid by a mailed check, is provided once you meet the elimination period. However, if you go out on disability due to a prior condition, coverage may be excluded for a period of time. LIMITED
