The City of Hamilton partners with Kettering Health to promote the health and wellness of employees and their families.
Employees enrolled in the City of Hamilton’s health benefit plan are eligible to receive a contribution to their Health Savings Account (or HRA, when applicable) by participating in the Living Well Program.
2025 Health Savings Account/Health Reimbursement Arrangement contributions are tied entirely to the Living Well Program. All employees who are enrolled in the City’s 2025 health benefits plan, and employee’s spouse, if also covered by the City’s 2025 health benefits, MUST complete the Living Well Program Steps, outline on page 3, and submit the appropriate forms.
The City provides employees who COMPLETE all Living Well Program Requirements with HSA/HRA funding according to the outlined schedule below. Contributions are distributed to qualifying employees in a single payment. HSA/HRA contributions are received when ALL program requirements have been completed.
HSA funds care be used for qualified healthcare, dental, and vision expenses for yourself, spouse, and eligible dependents. Please check out the HSA Store for additional items you can use your HSA funds.
2025 Exam Reporting Form
1. Biometric Measures - complete the form by participating in an on-site biometric screening or a lab test blood draw from your Primary Care Provider.
2. Wellness Exam - complete the form by scheduling an annual Wellness Exam (physical) with your Primary Care Provider.

• Based on the health outcome values on the Exam reporting Form, your Primary Care Provider will determine if you have Metabolic Syndrome. If so, you are required to complete a reasonable alternative.

Complete all requirements and return all completed forms to Kettering Health by August 31, 2025. Submit all completed paperwork together to Kettering Health via one of the following:
