Mother of Mercy receives Excellence and Innovation Award and Recognition On Wednesday, February 8th, 2012, staff members of Mother of Mercy Campus of Care, Albany, were recognized at the Aging Services of Minnesota Institute at the Hyatt Regency Hotel in Minneapolis. Mother of Mercy was recognized as part of Care Ventures Cooperative, a group of 18 rural long term care service providers. Mother of Mercy received the award for their efforts and implementation of the innovative Fit 4 Life Ventures Program. Fit 4 Life Ventures has been running at Mother of Mercy for about 3 years and takes a unique approach towards exercise that includes Balance, Strength, Flexibility, and Endurance to improve physical and mental health, muscle strength and overall functioning in older adults. Staff members of Mother of Mercy evaluate participants to determine baseline fitness levels. All seniors may benefit from this program which is tailored to their level and ability of participation. This program is also geared toward those individuals who may be at risk for falls. Mother of Mercy strives for a culture of independence with a primary focus on

balancing abilities versus disabilities so that all individuals who are at risk of falling may benefit from the program. Staff of Mother of Mercy and its falls prevention committee consistently monitor residents who may be at risk for falls by assessing their risk for falls, fall patterns, and then implementing safety measures to prevent reoccurrences through the Fit 4 Life Program. Mother of Mercy has also implemented several community outreach programs that allow seniors to remain in their communities longer. While the Fit 4 Life program was initially tailored toward nursing home residents it has already been extended to tenants that reside elsewhere on campus, a campus consisting of a 76 bed long term care facility and apartment complexes totaling 112 units comprised of independent and assisted living as well as memory care, and to members of the community.
