Having trouble sleeping? Try these tips

A

re you one of the millions of people who are suffering from chronic sleep disorders? Take a few seconds and answer the following questions: 1. Do you have trouble staying awake during the day or early evening hours? 2. Do you snore during sleep, often with pauses in your breathing? 3. Are you easily irritable or suffer from poor judgment or concentration? 4. Do you suffer from high blood pressure, morning headaches or sexual dysfunction? 5. Are you obese or do you have a large neck size? If you answered yes to one or more of these questions, you may be suffering from a sleep problem. Your doctor can help make that diagnosis with a thorough examination and testing. An estimated 30 million Americans suffer from sleep disorders, with the most common problem being Obstructive Sleep Apnea (OSA). Studies have shown that 5 to 10 percent of middle-aged males have clinically significant OSA. OSA is characterized by an irregular sleeping pattern that is accompanied with snoring and pauses in breathing. It occurs when the soft tissue in the airway collapses, preventing a free flow of air. Unfortunately, most of the people with OSA are not diagnosed and thus are not treated. Left untreated, OSA can lead to serious health problems such as congestive

heart failure, heart attacks, hypertension and stroke. If the physician suspects a sleep disorder, a series of tests may be ordered. The gold standard for diagnosing a sleep disorder is called a polysomnography, also called a sleep study. During this overnight study, a sleep technician monitors your sleep using a machine that records your breathing pattern, your heart and respiratory rate, your oxygen level, your stages of sleep and much more. A doctor trained in sleep medicine will look at the data gathered by the technician and arrive at a diagnosis and recommend a treatment regime. If your first sleep study is positive, a follow up study may also be ordered. During the second study, a device known as CPAP (Continuous Positive Airway Pressure) will be utilized. Treatments for Obstructive Sleep Apnea range from improved sleep hygiene to surgery. Procedures include nasal surgery, injection snoreplasty, laser assisted uvulapalatopharyngoplasty (the removal of some of the tissue in the mouth, nose and upper throat), and dental/oral appliances. Along with sleep hygiene, CPAP is the most common therapy prescribed. Sleep hygiene consists of several practices that help you get better sleep. The following simple ideas can help people sleep better: • The single most important item is to lose weight. • Decrease your caffeine, especially after dinner. Also decrease liquid intake after dinner. • Eliminate alcohol and tobacco products, especially prior to sleeping (after dinner). These products affect the stages of sleep that your body normally cycles through during the night. • Try going to sleep and waking up at the same time each day. Also, eliminate naps, especially in the late afternoons. A set routine helps the body’s internal clock run properly. • Try sleeping on your sides. You may also try to sleep with your head elevated. • Avoid over-the-counter sleep medications unless directed by your doctor. There are many sleep disorders other than OSA. Sleep terrors, sleep walking and excessive leg movements (PLMS) are examples of such disorders. Again, a thorough examination and sleep study will assist your physician in diagnosing these sleep problems. Maria Parham Medical Center has a comprehensive sleep center located within the hospital. Falls are dangerous for elderly F

alls in elderly people are a major health problem. Approximately 20 percent of all elderly people have at least one fall each year. These falls result in many bruises and lacerations. At other times, it is worse; broken bones are common from even relatively minor falls. Often a fracture means the end of independent living, at least for the time it takes the injury to heal. When the fractures are in the hips or legs, the recovery period may take a couple months and make it quite difficult for a person to get back to his or her normal activities and level of independence. Many things can be done to decrease the risk of falls at home. Failing to see potential hazards causes many falls so it is important to have good lighting in the home and to have one’s vision checked each year. Avoid the possibility of tripping on the edge of a rug by eliminating, or at least minimizing, the use of area rugs. And, of course, keep all walking areas free of clutter. Hand rails should be installed where there are steps or stairs and particularly in slippery areas, such as bathrooms or porches. Remember that good shoes provide foot protection as well as better traction. A misstep is most likely to occur when one is in a hurry, so slow down. Also, rising too quickly may cause lightheadedness for a moment — just long enough to cause a fall. Medicines can contribute to falls by causing confusion, dizziness, unsteadiness or a brief episode of low blood pressure. Review your medicines with your doctor. Since telephones are one’s source for help in case of injuries, they should be conveniently placed and accessible from the floor if possible. An excellent precaution to take is to enroll those susceptible to falls in an emergency response system that allows the person to simply press a button when they need help after a fall or other physical ailment. Maria Parham Medical Center offers an emergency response system called Lifeline. Lifeline allows users to signal for 24-hour emergency assistance from their homes by simply pressing a button. Lifeline is an immediate response system for emergencies in the home. A speaker telephone unit is

installed through the normal household telephone jack, and a wireless remote personal help button is worn on a necklace or wrist band. Should an emergency occur, the personal help button is pressed and the Lifeline staff is alerted that help may be needed in the home. The Lifeline staff will then determine the type of help needed, and assistance will be dispatched to the home. There are currently over 130 active Lifeline units serviced by Maria Parham Medical Center, with the bulk of these being in Granville, Vance and Warren counties. Healthy resolutions for 2015

s we begin to look ahead to the year 2015, it’s time to reflect on the past year, recognizing our accomplishments, identifying the things we could have done better, and setting new goals. If you haven’t thought about your own resolutions yet or just can’t think of something different from years past, read on: The ideas below may spark an interest in you. • Pick a reasonable weight goal, realistic timeline and a good friend to help you reach that goal: Shoot for a weight goal and date three to four months out and tell your friend, who may have similar aspirations, that is your goal and to help remind you of your targets. Studies show that the buddy system is more effective than going it alone or with a large group. As the target dates come and go, adjust future goals

accordingly and do it again. • Think healthy eating: Take some time to research healthy meals and make a commitment to serve two to three of those meals each week. Think portion control as you plan your meals and especially when you are snacking. Never sit down with the entire bag of chips; by placing a limited amount into a small bowl, you will greatly reduce the impulse to binge. • Resolve to walk, not stroll: Walking is one of the best exercises you can do. But if you really want your efforts to pay off, you need to pick up the pace. A heart healthy walking speed is 15 to 20 minute miles. You may have to work up to that pace, but if you are going to put the time into walking, why not make your time really count? • Treat yourself when you hit your goals: Whether it’s weight goals, exercise

goals or healthy eating goals, do something special for yourself when you achieve that milestone. With each milestone, get yourself something you want. Associate that item with your new resolution, and starting working for towards the next milestone. • Buy properly fitting clothes and put the bigger far away: You would be surprised how properly fitted clothes accentuates the improved you. Many people are reluctant buy new clothes just in case the weight loss isn’t permanent. Others may buy new clothes, but keep their older, bigger clothes in the closet to remind them of how far they have come. A better idea would be to put them in a closet far, far away. As the popular legend of Cortes goes, “When he landed in the New World, he burned his ships so his men couldn’t retreat.â€? Don’t

make it easy to retreat! • Don’t forget to exercise your brain: Make a mental goal for yourself this year as well. Good mental goals include learning the basics to a new language, reading a book each month or doing the daily crossword puzzle. Perhaps when you reach one of your goals, your gift to yourself could be one of the electronic reader tablets. These tablets can even help you with your brain exercises. • Most importantly, don’t get discouraged: As an inventor, Thomas Edison made 1,000 unsuccessful attempts at inventing the light bulb. When asked, “How did it feel to fail 1,000 times?â€? Edison replied, “I didn’t fail 1,000 times. The light bulb was an invention with 1,001 steps.â€? May 2015 be heart healthy, prosperous and joyful.

What is physiatry? O ne of the busier specialties at Maria Parham Medical Center is physiatry. However, very few people actually know what a physiatrist does. What they do know is that a physiatrist can definitely make you feel better. Physiatrists, or rehabilitation physicians, are specialists who treat injuries or illnesses of the nerves, muscles and bones. Physiatrists take into account the body as a whole and develop care plans that can enhance movement and overall quality of life, often without surgery. These treatment plans can include specialized exercise regiments, pain control and alternative forms of therapy. Allergies to cats and dogsÂ

P

ets are great. They are good companions, they help you relax, and they even help lower your blood pressure. But some people are allergic to their pets. Cats and dogs can cause sneezing, watery eyes and occasionally asthma.

These symptoms can be stopped by removing the pet from the household, but it is so hard to part with the adorable little family member. It is mainly the dander, or skin cells, that contain the allergens, but the allergic proteins are also found in the hair, saliva and even the urine. A very allergic person may get hives when a cat or dog licks or scratches his skin. Cats cause much worse allergies than dogs. A person with a cat allergy is apt to break out with symptoms immediately when a cat enters

the room. Asthma is brought on so easily in sensitized people that it has been called “cat asthma.� Dogs are much less likely to cause serious problems, but they do cause mild symptoms in many people. Many people think that getting a dog that does not shed or has short hair will not cause allergies. These dogs are less likely to cause troublesome allergies, but they still cause some — skin cells are constantly lost whether or not the dog sheds hair. Since dog allergies are a much smaller problem, when a person appears to have allergies to a dog, complete testing is worthwhile to look for other causes, such as house dust or molds, which may actually be easier to treat than getting rid of the pet.

When a person has a sensitivity to dogs or cats, one should keep the pets outdoors or at least limit the amount of time the animals spend in the house. It is also helpful to give the pet a bath once a week.

efore a person has a stroke, he frequently has some warning signs. If these signs and symptoms are heeded, a stroke can be averted and years of good health can be added to a person’s life. With a warning (or transient) stroke, the circulation to an area of the brain is temporarily decreased or obstructed. This results in weakness, numbness and difficulty with speech. These transient strokes completely resolve within 24 hours and a person feels fine again. If a person takes preventative measures at this time, he is likely to prevent a much more serious stroke. Common causes of transient strokes are small clots that travel from the arteries of the neck to plug the arteries of the brain, a drop in blood pressure from dehydration or shock, and temporary clotting off of a small

artery in the brain. If a person has a transient stroke, the cause should be looked for by exam, lab tests, doppler studies of the arteries of the neck, a CT scan, and even evaluation of the heart to make sure small clots are not caused by an irregular heartbeat. Stroke-like symptoms can also be caused by tumors, migraines, seizures, hypoglycemia or aneurisms, all of which deserve attention. Treatment is fairly effective and may be as simple as taking one aspirin a day. This cuts the chances of stroke by up to 50%. If the aspirin alone is ineffective, stronger blood thinners can be used, but they also increase the chance of more severe bleeding problems. Surgery on obstructed arteries in the neck and treatment of medical problems such as an irregular heart beat may also be necessary.

pen enrollment for health coverage on the Health Insurance Marketplace closes Feb. 15. If you haven’t yet signed up for individual or family coverage, there’s still time. Health coverage helps cover the cost of services, tests and treatments to help you get and stay healthy. Items covered include preventive care, health screenings, well woman and prenatal care, immunizations for adults and children, treatment for pre-existing conditions and the cost of some prescription medications. All plans must offer this basic level of coverage. Health insurance is important. It ensures that you and your family have access to the care that you need to be healthy and that you can weather any unforeseen health issues, such as an unexpected illnesses or injuries that may arise. Who’s eligible? If you haven’t yet enrolled or re-enrolled

for health insurance coverage for 2015-2016, you may be wondering if a Marketplace plan is right for you. The Marketplace is designed to make health insurance accessible and affordable. The nagging truth about pain C

ontrary to popular belief, pain is not something you have to put up with. The old adages, such as “grin and bear it,� “bite the bullet� and “Learn to live with it� are not words of wisdom, but simply bad advice when it comes to pain. Millions of people suffer needlessly from chronic pain. There are things that your doctor can do to help relieve your discomfort. It has been shown that proper pain management can help you get better, faster. Patients often skip or resist certain activities and therapies simply due to pain. With that pain under control, you will be more willing to perform these procedures as ordered. Quite often, a nice benefit of proper pain management and necessary rehabilitation is being discharged from the hospital more quickly. Both drug and non-drug treatments can help in preventing and controlling pain. You and your doctors will decide which regime is right for you. Sometimes both methods are combined for greater relief. While these treatments may not relieve all your pain, the goal is to help you be as comfortable as possible. Pain medication is the most common treatment, but there are other things that can be done to help you manage your pain. Changing positions, using ice packs, massage and distraction are examples of non-drug treatments.

 The patient’s role It is important for you to: • Always tell your health care provider when and where it hurts as soon as the pain begins. • Inform your health care provider about any personal, spiritual or other beliefs about pain. • Relay any “side effectsâ€? from your treatment (i.e., itching, nausea, vomiting, excessive sleepiness, constipation, confusion) to your health care provider • Tell your health care provider what works and what doesn’t work. Â

Your care givers should: • Ask you if you are

having pain as part of the care you receive. • Ask you to rate your pain on a scale. This is to help monitor your level of pain during treatment and activities. • Inform you of any potential side effects from the treatments.
