aring for loved ones at the end of life is challenging, both professionally and emotionally. Stress levels have been documented to be higher than a woman newly widowed or someone who has just been diagnosed with breast cancer. Compassion fatigue is a form of burnout characterized by emotional, physical, and spiritual exhaustion. Caring for seriously ill family members over an extended amount of time can intensify it. We, as caregivers, are exposed to pain, suffering, or sadness—all while being compassionate and understanding The causes of compassion fatigue include: work over-load, emotional demands, feelings of helplessness, accumulated loss, and self-imposed pressure to live up to your own high standards. warning signs of compassion fatigue include: abusing alcohol or food, anger, blaming, chronic lateness, depression, high self-expectations, hopelessness, increased irritability, low self-esteem, and sleep disturbances. Sometimes a vacation or time of rejuvenation can help ease compassion fatigue. Here are a few tips: • The most important step in recovery from Compassion Fatigue is to get in touch with yourself. This doesn’t mean you have to “stop caring” or “toughen up.” It requires honest self-reflection. Talk out your feelings with a non-judgmental listener (a spouse, co-worker, friend, counselor). We all need to have a trusted, understanding resource to confide in.


•R ecognize that you may need to have your batteries re-charged. Get in the habit of doing something everyday to replenish your energy. Go for a walk, listen to a favorite CD, or watch a movie you enjoy. Take time for yourself! • Develop a self-care plan. Create your own personal mission statement, which includes your own values and comfort levels. Rediscover what gives your life joy and meaning. Begin to work within these principles and let them guide you. Use them as a future compass for your daily decisions. Compassion Fatigue can happen to anyone at any time in their life. It doesn’t mean you have failed or are a “bad caregiver” for your loved one. We all bring personal experiences with us that can impact our ability to cope effectively in the present. 1/4 cup extra virgin olive oil 2 cloves garlic, fresh minced 1/2 teaspoon red chili flakes, crushed 1 tablespoon oregano, fresh minced 1 tablespoon rosemary, fresh minced 1 teaspoon lemon zest 1 pound chicken breast, boneless and skinless 1 3/4 cups California green seedless grapes, picked from stem and rinsed 1/2 teaspoon salt 2 tablespoons extra virgin olive oil 1 tablespoon lemon juice, fresh In small bowl combine olive oil, garlic, chili flakes, oregano, rosemary and lemon zest. Whisk together marinade. Cut chicken into 3/4-inch cubes. Alter­nate chicken and grapes and thread onto 12 skewers. Place skewers into a baking dish or pan large enough to hold them. Pour marinade over skewers, coating each one. Marinate for 4 to 24 hours. Remove skewers from marinade and let excess oil drip off. Season with salt. Grill until chicken is cooked through, about 3 to 5 minutes on each side. Ar-

Frozen Grape and Banana Skewers with Chocolate Drizzle These delightful frozen treats are healthy, too. Makes 4 servings

2 firm ripe bananas 2 tablespoons orange juice 1/2 cup red grapes (about 16) 1/2 cup green grapes (about 16) 8 10-inch wooden skewers 1 1/2 ounces dark chocolate (60-70 percent cocoa solids), finely chopped

Line a baking sheet with wax paper. Cut the banana into 1/2-inch-thickrounds.Place in small bowl and toss gently with the orange juice. Skewer fruit, alternating two grapes for each piece of banana, and place fruit skewers onto lined tray. Place chocolate in small micro­wave safe bowl. Microwave for 1 minute, then stir well, and microwave another 10 seconds if necessary to melt chocolate. Using a spoon, drizzle melted chocolate onto fruit skewers. Place the tray in the freezer for at least 2 hours. Once frozen, skewers may be transferred to a sealable plastic bag where they will keep in the freezer for up to a week. Allow to soften at room temperature for 5 minutes before eating.

Back to School and Back to Sports: How to Help Your Youngster Avoid Sports Injuries By Stephanie Dykstra, Imaging Center

A

fter months of fun in the sun, kids go back to school and back to sports often unprepared for the toll it will take on their bodies to get back in shape and back in the game. Being young certainly has its advantages, but youth and young adults are still prone to sports injuries, making back to school time also one of the highest times of the year for ER visits among young athletes. According to the American Academy of Orthopedic Surgeons, the injury rate for teen athletes is roughly the same as among the pros. They warn that what might be a minor injury to an adult may be of greater concern in a growing athlete.

“Most parents like their children to be active but it is just as important that they are safe.” Many kids get out of shape during the summer, and then jump back into practice with weakened muscles, a recipe for muscle strains, sprained joints, and bone fractures. Not only can these injuries sideline an athlete for weeks, they can also have more permanent damage as the athlete grows into an adult. Most parents like their children to be active but it is just as important that they are safe. Whether on the hockey rink or the football field, there are ways to help prevent injuries. • Follow the rules: Encourage your children to know the rules and follow them. Most sports have rules in place to help keep kids safe. • Gear up: Make sure your child has the recommended play equipment and are wearing it properly.


• Make sure they stretch: One of the greatest ways to avoid sports injuries is to perform proper stretching techniques. Coaches can usually help your child develop a stretching regimen. • Take a time out: It’s important that your child take several breaks when playing on a hot field and if they get hurt, to hit the sidelines. Never let your child feel pressured to play through pain or excessive heat. Get Injuries Checked Out: If your child gets injured, see their doctor immediately. Whether you go to their pediatrician or the ER, know that you have a choice of where to get their x-ray, MRI, or ultrasound. Some imaging service providers stack up better than others in terms of staff and equipment. Also know that outpatient imaging centers are often less expensive than the hospital. • Tone it down: Some kids are just not cut out to be athletes, and that’s ok. If your child dislikes the sport, complains of frequent pain

or injuries, or has a negative attitude towards the coaches, maybe you should reevaluate their choice of activity. Bottom line, only you and your child know how to judge whether a sport is right for their body and if they are up for the required conditioning. Most youngsters compete with no problem and by following a few simple steps, they can have a safe and happy time, playing the sports they love, without injury. Common Foot Ailments Submitted by Next Step Foot and Ankle Centers

I

n today’s fast paced world, the average person can take 9000 steps in a typical day. Many people suffer from foot and ankle pain that can slow them down. Some examples could be minor problems such as fungal toenails or a major problem with a fractured (broken) ankle. Podiatrists, who are physicians trained to provide complete care foot and ankle care to people of all ages. When choosing a podiatry group, check to make sure the podiatrists are all board-certified or qualified by the American Board of Podiatric Surgery. One of the most common complaints seen by podiatrists is heel pain. Heel pain may be caused by several reasons ranging from trauma to simple over activity. One such cause is Plantar Fasciitis. Plantar Fasciitis is an irritation or inflammation of the ligament situated along the bottom of the foot and attaching to the heel. This constant “pull” or “inflammation” can then result in a heel spur. Symptoms are usually present upon walking first thing in the morning or after resting. There are several treatment options that can alleviate the symptoms and pain. Treatments may range from simple conservative methods to surgical intervention for the more prolonged severe cases. There is also a new procedure that may be available for plantar fasciitis, the Tenex FAST Procedure. Another problem many patients present with are bunions. Bunions are a common deformity that may result in pain along the inside of the foot beside the base of the great toe. Other symptoms include a red, swollen bump on the inside of the foot. by the misalignment or enlargement of the bone at the base of the big toe. This causes the bump that most people refer to as the bunion. Many bunions begin by inappropriately fitted footwear, excessive physical activity, or simply may be hereditary. A complication of bunions is joint arthritis if left untreated for long periods of time. Treatment may include a conservative approach that may relieve the painful symptoms; however, many cases require surgical treatment. This involves a shifting in the long bone behind the great toe to position it to a more “natural” alignment. Some people may describe the same pain and presentation on the outside of the foot, along the base of the fifth toe. This is called a “bunionette” and requires the same treatment plan as a bunion. Illinois Cares RX Program Eliminated:

ue to Illinois state budget reductions, funding for the Illinois Cares RX program will be eliminated as of June 30, 2012. This means that as of July 1, 2012, Illinois Cares RX will no longer help you pay for your prescription drugs and Medicare Part D plan premium. There are a few things you will want to keep in mind: • If you are enrolled in a Medicare Part D plan, you will continue to receive prescription drug coverage through that plan. You will not be dropped from your Part D plan; however, if your Part D plan has a monthly premium you will need to begin paying that. (Please note that if you do not pay your monthly Part D plan premium they will drop you from the plan). You will also need to pay any remaining deductible for your drug plan if there is one. In addition, the copay amount you pay for each prescription may change. Please call your Part D plan for information regarding your monthly premium, deductible, co-pay costs and how you will pay your premium. We recommend that you get a three-month fill of your prescriptions right now, if you can, before Illinois Cares RX ends. • If you would like to switch Part D plans upon losing Illinois Cares RX you will need to make that change between June 1 and August 31, 2012.

What is the Medicare Advantage Plan Submitted by Illinois Oncology, LTD.

I

ndividuals with Medicare have several options to consider regarding their own out-of-pocket expenses. If you choose to have Medicare only, with no supplemental (or secondary) insurance, you will owe 20% of the Medicare allowable on many of your charges. For example, if Medicare approves $100.00 of an office visit—you will owe $20 and Medicare will pay $80.00. You may elect to have Medicare and a supplement or Medigap policy. These policies will cover the 20% Medicare does not pay. However, with these policies you may still owe the Medicare deductible. You may, however, elect to enroll in a Medicare Advantage Plan. These plans combine Medicare Parts A & B into one plan and may or may not offer a prescription (Part D) plan. With Medicare Advantage, it is important to know what your out-of-pocket is and what will be covered. For instance, a Medicare Advantage plan with a $4500.00 outof-pocket means that some services (such as chemotherapy drugs) will be paid at 80% and the patient will owe 20% until the $4500.00 out-of-

pocket is met. This is calculated for a calendar year, i.e. this repeats every January 1. Therefore, it is advisable to compare your Medicare Advantage plan and their out-of-pocket to the cost of Medicare and a supplement. Individuals can switch from one Medicare Advantage plan to another during open enrollment (October 15 – December 7). Again, it is important to review each plan’s specific benefits package with special attention to the out-of-pocket or prescription coverage. In addition, you can switch from your Medicare Advantage plan to original Medicare from January 1 – February 14, 2013. he Emergency Medical Services for Children Region IV Coalition recently presented two $250 scholarships to college students seeking healthcare-related degrees. The applicants chosen were Molly Etling of Freeburg, Illinois and Amy McEvoy of Edwardsville Illinois. Etling is attending the University of Illinois majoring in Molecular and Cellular Biology concentrating on premedicine. Etling, whose ambition is to become a pediatrician, is a sophomore at U of I with high honors.

“Region IV EMSC Coalition supports the efforts of Illinois EMSC to improve the care of pediatric patients through the education of physician, nurse, and EMS personnel.” McEvoy is attending Southeast Missouri State University working towards a bachelor’s degree of nursing. Her ambition includes a doctorate degree in nursing and becoming a nurse practitioner. McEvoy, who will be a freshman at SEMO this fall, was an honors student at Edwardsville High School and was part of the Medical Occupations program for three years. Scholarship applicants had to complete an application and compose a short essay describing their healthcare journey. Additional items required included letters of recommendation, proof of enrollment/admission to a college or university or other medical education and proof of residency in Region IV. The goal of Illinois EMSC is to ensure that appropriate emergency medical care is available for ill and injured children at every point along the continuum of care—including primary prevention, acute care, and rehabilitative care. Some pastimes do seem like “riding a bike” in that one remembers how to do it after just a few moments of getting started. Also key to the activity is the group effort involved, points out Webb. When a person is used to something like gardening, es-

pecially this time of year, it doesn’t take much to link the person up with a favored routine and all the fantastic memories that come with it. Godfrey also has a terrific team focused on activities in the community’s memory care wing, with Activities Assistant, Regina Cook, heading up that group’s effort to bring lively and engaging ideas to the monthly calendar. Cook faces a different sort of challenge, with the added goal of simply achieving a momentary change of pace being so important for residents with dementia and Alzheimer’s. Admittedly the potential for frustrating situations is a bit higher with the ever present need for care, and events such as gardening lessen the intensity of the caring environment to a degree, as residents take on an activity that often quickly comes back as a normal, healthy exercise. Moreover, with ever-present reminders that exercise is crucial for fighting memory loss, it’s helpful to have the spring season as a landscape for outdoor activities. The pleasant weather of 2012, thus far, has been a blessing and has inspired an early interest in these pastimes. Indoor activities such as a weekly happy hour, which runs from around 3:00 to 4:00 or 4:30 and features live entertainment several times per month, is a popular activity at Godfrey. In addition, popular activities include card games, wii bowling, crafts, Bible Study, and of course, Bingo! The community is also beginning to offer chair massages, a relaxing perk after getting one’s hands dirty planting flowers or sweating to the oldies in exercise class. Knee Replacement Surgery Submitted by VNA-TIP HealthCare

K

nee replacement surgery is the most common joint replacement procedure. Patients suffering from chronic knee pain and decreased mobility, most likely from arthritis or injury, can be candidates for knee replacement. A physician may recommend surgery when the painful symptoms can no longer be treated with non-operative measures like joint supplements, weight loss, anti-inflammatory medications, or injections. According to the Agency for Healthcare Research and Quality, more than 600,000 knee replacements are performed yearly in the United States. While the surgery is common, the knee is the largest joint in the body and replacing it is still a major surgery in which the diseased or damaged parts of the knee joint are replaced with artificial substitutes, usually a metal and plastic implant. Patients often have many concerns regarding knee replacement surgery. Knowing what to expect and then preparing before surgery can alleviate some of that stress.

Before Knee Replacement Surgery

• Some physicians may recommend lifestyle changes to better prepare the body for surgery. These could include smoking cessation, proper diet, starting an iron supplement, or weight loss. Specific exercises to strengthen the surrounding area of the knee may also be recommended, some of which may be suggested at least six weeks prior to surgery. • Home Safety evaluations can help prepare the home for recovery. By making simple changes in daily routines, patients can function easier and potentially avoid injury. Some modifications could include attempting to live on the main level to avoid repeated stair climbing, creating clear paths for walker or other assisted walking device, scheduling someone to care for or walk pets, moving kitchen items and food to waist or chest level, and creating a place with everyday items within reach for recovery. • Hospital stay may last from three to seven days, depending on the patient’s healing process. For several, help will be needed in the home and if there is not sufficient help at home, the patient may be transferred to a rehabilitation center. If the patient is not able to go home due to lack of assistance, the discharge planner or physician can recommend a list of facilities for in-patient recovery. To prepare, obtain a list of approved rehab facilities from the patient’s insurance company prior to surgery. Be sure to find out their requirements for pre-certification and how many days insurance will cover.

After Knee Replacement Surgery

• Recovery from knee replacement surgery differs for each patient and circumstance. In most cases, the rest and rehabilitation period ranges from three to six months after surgery. Healthcare providers usually break down the total rehabilitation time into two parts that they refer to as short-term and long-term recovery. Short-term recovery is when a patient can walk with little or no aid and uses over the counter pain relievers instead of prescription pain killers. Short-term recovery averages usually 6-12 weeks after surgery. Long-term recovery involves the complete healing of the wound and soft tissues and when the patient returns to normal daily life, including work. On average, long-term recovery for most patients is between three and six months. • P atients can be overwhelmed by the amount of time and energy it takes to perform everyday living activities, but there are people who can help. Physical therapists will help

patients increase agility by performing strength exercises. Therapists, sometimes Occupational, will teach patients proper techniques for performing these activities to decrease the chance of injury during recovery. Home modifications can help aid in preventing injury as previously mentioned. Patients may also choose a home health agency to aid in recovery at home since mobility and travel may be difficult. Home health staff can help patients follow the surgeon/s exercise recommendations and also address safety issues in the home, including assessing the need for adaptive equipment and the patient’s ability to care for their self following surgery. They can manage wound and incision care to facilitate healing and prevent complications while improving patient comfort at home during the healing process. The staff can help manage pain medications and monitor their effects, as well as help patients to recognize signs and symptoms that should prompt them to seek emergency care. nowing your body can prevent health issues and minimize problems that may already exist. Early detection of issues often increases cure rates with easier, less invasive treatments. It is equally important to get to know your skin and be aware of any new or changing issues on the skin. The skin is the largest organ of the body but it is often neglected. It does not get the attention the heart and other organs receive. Following are some ways to get to know and understand our skin.

Moles are spots or patches of pigmented skin that range in color from tan, brown and black, to red, pink, blue or purple. They can be flat or raised. Moles are very common and almost everyone has at least one. The average number ranges from 10 to 40 on an individual. A person can be born with moles (congenital) or moles can appear after birth (acquired). People who have more than 50 acquired moles have a greater risk for the moles to change and become cancerous. Moles that are larger than average (>6mm) or irregular in shape and color are called atypical moles. Those with atypical moles have a greater chance of developing skin cancer. Regular self skin exams are recommended and should include all areas of the body including scalp, feet, between the toes and under finger and toenails. Dermatologist exams are advisable if an individual has numerous moles, has a personal or family history of skin cancer (especially melanoma), blistering sunburns, high sun exposure, tanning bed use, or if a mole develops suddenly, changes in shape or size or begins to itch, bleed or become painful.

So how do you know if your mole is cause for attention? Detection and prevention are vital. The American Academy of Dermatology encourages people to look for and to seek professional help if the ABCDE’s of a pigmented lesion are noted. How can skin be protected and skin cancer prevented? Although damage to the skin is cumulative, prevention of further damage today is crucial. Skin cancer prevention is possible by applying a broad-spectrum sunscreen (UVA and UVB) with titanium dioxide or zinc oxide, water-resistant sunscreen with a sun protection factor (SPF) of 30. Apply one ounce (two tablespoons) of sunscreen to your entire body 30 minutes before going outside. Re-apply approximately every two hours or more frequently if sweating or swimming and apply even if the sun is not out. Use UV-blocking sunglasses; wear a broad-brim hat, long sleeves, and pants. Try to avoid the sun between the hours of 10am and 4pm. Water, sand and snow have reflective qualities and can increase your potential for sunburn and sun damage. Protect children’s sensitive skin. Tanning beds and the sun can not only cause skin cancer but age the skin, so self-tanning products are a great alternative. Why do some people burn and others tan and what are their risks? Melanin, the dark pigment in the skin, can be a natural protectant from the harmful Ultraviolet B (UVB) rays of the sun that are associated with sunburn. People with a naturally darker complexion or skin tone may take longer to display damage. Thus fairer complexions are at greater risk. DNA changes from UVB radiation causes the suntan reaction and can also lead to the formation of skin cancers including melanomas. Skin types I, II and III are at greater risk than IV, V and VI. Rate your own risk: I – Pale white skin – always burns, never tans II – White skin – burns easily, tans little III – Average white skin – burns moderately, tans to light brown IV – Beige or light tan skin – burns little, always tans to medium brown V – Moderate brown or tanned skin – rarely burns, tans to dark brown VI – Dark brown or black skin – deep pigmented, never burns What about the Vitamin D from the sun? Vitamin D can be manufactured by the body from exposure to the sun, but only a small amount of sunlight is needed. The number of skin cancers has been increasing in the years since a suntan has been associated with health and good looks and popularizing “laying out”, beach vacations, tanning beds and suntan accelerators. Why not get the Vitamin D from diet or a supplement that does not increase the risk of skin cancer?! Additionally, tanning causes the skin to age at faster rate. Who wants more wrinkles, spots, splotches and a leathery appearance? Are tanning beds and sunlamps safer than natural sunlight? NO. Sunlamps and tanning beds expose the body to Ultraviolet A (UVA) radiation. UVA is associated with more damaging quali-


ties, due to its deeper, more penetrating radiation. An increase in melanoma cases for those in their teens and twenties correlates to tanning bed use. Our advice, STAY OUT OF THE TANNING BED! What is skin cancer? Skin cancer is the most common type of cancer and the most curable, when diagnosed and treated in early stages. All ethnicities can develop skin cancer but those who have light skin that burns easily have the greatest risk. People who have a family history of skin cancer, history of high sun exposure, live at high altitudes or near the equator, use tanning beds, have a lowered immune system, exposure to X-rays and carcinogenic agents are all at greater risk of skin cancer. Diagnosis is by visual examination and is confirmed by skin biopsy. Actinic keratosis (AK’s) are dry, pink, scaly spots that are considered pre-cancerous. Sun exposure and tanning beds increase the risk of developing AK’s. Treatment can include cryotherapy (liquid nitrogen) and photodynamic therapy (PDT) light therapy. Skin cancer develops when abnormal cells form and multiply in the various layers of the skin. • B asal Cell Carcinoma: The most common type of skin cancer that forms in base of the outer layer (epidermis) of the skin. It grows slowly and rarely spreads to other parts of the body but can enter nearby nerves and bones. It should be treated without delay. Treatment can include surgical excision, curettage & desiccation, micrographic surgery, prescription creams or radiation therapy. • S quamous Cell Carcinoma: The second most common type of skin cancer that forms in the middle of the outer layer (epidermis) of the skin. Squamous cell carcinoma can grow deep and spread to other areas of the body. Treatment can include surgical excision, curettage & desiccation, micrographic surgery, prescription creams or radiation therapy. • Melanoma: Represents about only 5% of all skin cancers, but is the most dangerous because of its tendency to spread to internal organs. It is the most common cause of cancer death in young adults. Melanoma forms in the melanocytes (the cells that make pigment) which is why it often appears as a new dark spot on the skin or in a mole. Treatment should be swift and requires surgical removal. A = Asymmetry – If you draw a line down the middle of a mole, they should be mirror images, when one half of the mole/spot is not like the other half, it is asymmetrical.

B = Border – The edge is irregular, jagged, not smooth and does not have definition.

C = Color – A mole has multiple colors or changes in color. A variety of color can be an indication for concern (variation of shades of tan, brown, black, white, red or blue). An exception is that solid black can indicate need for dermatological examination.

D = Diameter – The average mole is less than 6mm (1/4 inch, the size of a pencil eraser). Melanomas are usually larger than 6mm but they can be smaller, especially if detected early.

E = Evolving – Changing in size, shape, color or elevation or that start to itch, bleed or become painful should be seen by a dermatologist. A mole that is “lazy” or content to simply be there and do nothing is likely normal. Other warning signs may be firmness or growth, especially rapidly, such as a few weeks or months. Spread of pigment from the spot to surrounding skin, redness or swelling beyond the border or something that does not heal are also cause for concern.

Glaucoma: Silent Thief of Sight By Patricia K. Novelly, Director of Practice Relations

G

laucoma is the second leading cause of blindness in the U.S. and is often referred to as the “silent thief of sight.” There are many types of glaucoma and many theories about the causes of glaucoma. Although the disease is usually associated with an increase in the fluid pressure inside the eye, other theories include lack of adequate blood supply to the nerve. Doctors at the Illinois Eye Surgeons are national leaders in diagnosing and treating all types of glaucoma, including involvement in FDA clinical trials for new glaucoma medicine and surgical devices. “There are several different types of glaucoma, and it’s important to know exactly what type you have so you can get the correct treatment,” says Eric Wigton, MD, Director of Illinois Eye Surgeons Glaucoma Services. • Primary open-angle glaucoma is the most common form in the U.S. “This type develops slowly and usually without symptoms until noticeable vision loss occurs and it’s in an advanced stage, so early detection is key,” states Dr. Wigton. This type of glaucoma initially affects peripheral or side vision, but can advance to central vision loss. If left untreated, glaucoma can lead to significant loss of vision in both eyes, and may even lead to blindness. • A cute angle closure glaucoma, a less common type, usually occurs abruptly due to a rapid increase of pressure in the eye. Symptoms may include severe eye pain, nausea, redness in the eye, seeing colored rings around lights, and blurred vision. “This Page 28 — Healthy Cells Magazine — Metro East Area ­— August 2012

condition is an ocular emergency, and medical attention should be sought immediately, as severe vision loss can occur quickly. Laser treatment can be given right away to relieve pressure,” notes Dr. Wigton. Illinois Eye Surgeons have lasers available 24 hours at multiple locations around the state to deal with these emergencies. • S econdary glaucoma occurs as a result of an injury or other eye disease. It may be caused by a variety of medical conditions, medications, physical injuries, and eye abnormalities. Infrequently, eye surgery can be associated with secondary glaucoma. • N ormal-tension glaucoma is when eye pressure remains within what is considered to be the “normal” range, but the optic nerve is damaged nevertheless. Why this happens is unknown. It is possible that people with low-tension glaucoma may have an abnormally sensitive optic nerve or a reduced blood supply to the optic nerve caused by a condition such as atherosclerosis, a hardening of the arteries. Under these circumstances even normal pressure on the optic nerve may be enough to cause damage. Dr. Wigton says, “This type of glaucoma can be very difficult to detect until it’s too late. Early detection of optic nerve damage can be done with special imaging equipment, known as OCT.” OCT imaging is available at all IES locations.


Risk Factors • Age: People over age 60 are at increased risk for the disease. For African Americans, however, the increase in risk begins after age 40. The risk of developing glaucoma increases slightly with each year of age. • Race: African Americans are significantly more likely to get glaucoma than are Caucasians, and they are much more likely to suffer permanent vision loss as a result. People of Asian descent are at higher risk of angle-closure glaucoma and those of Japanese descent are more prone to low-tension glaucoma. • Family history of glaucoma: Having a family history of glaucoma increases the risk of developing glaucoma. • Medical conditions: Some studies indicate that diabetes may increase the risk of developing glaucoma, as do high blood pressure and heart disease. • P hysical injuries to the eye: Severe trauma, such as being hit in the eye, can result in immediate increased eye pressure and future increases in pressure due to internal damage. Injury can also dislocate the lens, closing the drainage angle, and increasing pressure. • Other eye-related risk factors: Eye anatomy, namely corneal thickness and optic nerve appearance indicate risk for development of glaucoma. Conditions such as retinal detachment, eye tumors, and eye inflammations may also induce glaucoma. Some studies suggest that high amounts of nearsightedness may also be a risk factor for the development of glaucoma. •C orticosteroid use: Using corticosteroids for prolonged periods of time appear to put some people at risk of getting secondary glaucoma. Diagnosing Glaucoma Glaucoma is diagnosed through a comprehensive eye examination, although specialized testing is needed to confirm suspect cases. Specialized testing includes computerized peripheral vision measurement, ultrasonic or laser measurements of corneal thickness, and laser measurement of optic nerve damage (the aforementioned OCT). “If you’ve been previously diagnosed with glaucoma, make sure all of the above tests have been done to confirm that diagnosis,” states Dr. Wigton. “The reverse is also true: if you’ve been told you don’t have glaucoma but you have a strong family history, make sure all of the proper testing has been done to rule it out.” Treatment of Glaucoma The treatment of glaucoma is aimed at reducing intraocular pressure. The most common first line treatment of glaucoma is usually prescription eye drops that must be taken regularly. “There is a promising future for prescription drops to further lower pressure in glaucoma patients,” states Michael Jones, MD, Director of Clinical Research at Illinois Eye Surgeons. “The latest FDA trial we participated in involved a new combination medicine that showed drastic reductions in eye pressure in long term glaucoma patients.” That new drug could be on the market as soon as this fall adds Dr. Jones. If prescriptions drops don’t work to lower pressure the next line therapy is a new laser called SLT that increases drainage away from the eye. “The new SLT laser has greatly changed how we treat glaucoma patients,” notes Wigton. “With a 90 second in-office laser treatment we are able to reduce pressures in even the most resistant cases of glaucoma.” For the most difficult cases of glaucoma where drops or laser do not lower the pressure enough, surgery is required. The newest surgery

involves implanting a titanium pressure release valve into the eye to keep the pressure low. Dr. Michael Jones of Illinois Eye Surgeons was the first to implant that valve in Downstate Illinois in 2008. Lifelong Treatment There is no cure for glaucoma. Patients with glaucoma need to continue treatment for the rest of their lives. Because the disease can progress or change silently, compliance with eye medications and eye examinations are essential, as treatment may need to be adjusted periodically. By keeping eye pressure under control, continued damage to the optic nerve and continued loss of your visual field may slow or stop. Your eye care provider may focus on lowering the intraocular pressure to a level that is least likely to cause further optic nerve damage. This level is often referred to as the target pressure and will probably be a range rather than a single number. Target pressure differs for each person, depending on the extent of the damage and other factors. Target pressure may change over the course of a lifetime. Newer medications are always being developed to help in the fight against glaucoma. Early detection, prompt treatment, and regular monitoring can help to control glaucoma and therefore reduce the chances of progression vision loss. An Association Between Oral Health and Heart Health? Submitted by Troy Family Dental

he American Association for Dental Research (AADR) acknowledged the very comprehensive review of the literature undertaken by the American Heart Association (AHA) on the relationship between periodontal or gum disease and heart disease, The review titled “Periodontal Disease and Atherosclerotic Vascular Disease: Does the Evidence Support an Independent Association?: A Scientific Statement From the American Heart Association” was published online in Circulation. The review assessed: 1) whether available data supported an association between heart disease and periodontal disease; and 2) whether available data supported that periodontal treatment might change heart risks or outcomes. The extensive review of current literature—including more than 40 observational studies—did indeed support an association between

periodontal disease and heart disease. “The validation of an association between periodontal disease and heart disease from the American Heart Association is an important statement from a public policy perspective,” said AADR President Rena D’Souza. While the association has been demonstrated, the literature review failed to support either a cause-effect relationship, or that periodontal treatment might prevent heart disease or change its outcome. AADR has many members who are researching the association between oral health and cardiovascular health, but long-term studies on the association between periodontal infection and cardiovascular disease or long-term periodontal treatment studies on cardiovascular health are certainly understudied. Hence, it was not surprising that the authors failed to find evidence in the literature demonstrating either a cause or a treatment effect. Importantly, the AHA review highlighted significant gaps in our scientific understanding of the interaction of oral health and heart disease and stated that well-designed, controlled studies would be required. “Further research on the association of oral health and cardiovascular disease is critical,” stated D’Souza. She states that with the continued evaluation of all the research tools that are available, advancement will be possible toward the understanding of this important relationship between oral health and heart health.
