Franklin Health is the largest provider of health care services in the region. Primary Care Services All Franklin Health primary care medical practices (Farmington Family Practice, Internal Medicine, Livermore Falls Family Practice, and Pediatrics) have made the transition to a PatientCentered Medical Home (PCMH) model of care. The PCMH model designates a primary nurse-provider team that coordinates patient care across a continuum of settings ranging from the primary care office to specialty clinics to inpatient areas. The PCMH model demonstrates the benchmarks of patientcentered care, including: • Same day appointments as needed, • Expanded hours, • 24/7 access to a provider, and • Appropriate use of proven health information systems. Research shows that medical homes can lead to higher quality and lower costs, and improve patients’ and providers’ reported experiences of care.

Cardiovascular Health Services Adults with newly diagnosed heart conditions, along with those with chronic heart and vascular conditions need the care of heart specialists. Cardiologists provide comprehensive cardiovascular assessment and evaluations for patients at Franklin Memorial Hospital. Consultations range from medication adjustments, diagnostic testing, and pacemaker evaluations, to lifestyle management and risk assessments. Cardiologists affiliated with MaineHealth Cardiology provide the services including: Dr. Lesley West, Dr. Heinrich Grube, and Dr. Paul Frey. Cardiology and vascular services are also provided at the Androscoggin Valley Medical Arts Center at 21 Main Street in Livermore Falls. Those who have endured a major heart event, such as heart attack or heart surgery, will benefit from our medically supervised cardiac rehabilitation program. Participants safely and gradually increase physical activity, while learning how to reduce heart disease risk factors such as high blood pressure, high cholesterol or lipid levels, diabetes, smoking, and being overweight.

Dr. Paul Frey specializes in the treatment of peripheral vascular disease, a slow and progressive circulation disorder in which organs may not receive adequate blood flow for ordinary function. Dr. Frey offers monthly vascular clinics at the Androscoggin Valley Medical Arts Center, a new service that was added in 2014. Diagnostic vascular imaging is also provided by a certified vascular technologist.

Pulmonary Rehabilitation

Pulmonary rehabilitation programs are designed to help people with pulmonary problems such as COPD, emphysema, chronic bronchitis, and other respiratory diseases, recover faster and improve their quality of life. It includes exercise, education, counseling and support for patients and their families. Pulmonary rehabilitation can not cure lung disease, but it can greatly improve one’s quality of life. In October 2013, the Pulmonary Rehabilitation Program at Franklin Memorial Hospital received its first-ever certification by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR). AACVPR-certified programs are recognized as leaders in the field of cardiovascular and pulmonary rehabilitation because they offer the most advanced practices available.

This past year, more than $620,000 was received from the Estate of S. Paul Washburn. On November 21, 2013 Paul died peacefully at his home at the age of 92. He had been a resident of Livermore Falls for most of his adult life. On August 28, 2014 the Androscoggin Valley Medical Arts Center was dedicated in memory of Paul, whose generous bequest to Franklin Memorial Hospital came with his enduring wish to have a health center built in a prominent Livermore Falls location. This wish came to fruition in January 2012. NorthStar EMS

NorthStar is the regional ambulance service of Greater Franklin County. Its professional staff of Emergency Medical Technicians (EMTs) respond to more than 5,000 calls a year spread over 2,800 square miles. NorthStar is dispatched out of five base locations strategically positioned throughout the region. Paramedics are the most advanced EMTs and can administer medications, start intravenous lines, provide advanced airway management for patients, and resuscitate and support patients with significant problems such as heart attacks and traumas. There is always a paramedic available to respond in these kinds of emergencies. NorthStar crews are additionally involved in thousands of hours of community service each year. CPR trainings, bicycle safety rodeos, presentations at area schools, and mock accident scenes are just a few of the ways that NorthStar contributes back to the community. In April, NorthStar unveiled an ambulance painted in a purple-andwhite color scheme and adorning symbolic oversized ribbons to bring awareness to the issue of domestic violence. The ambulance is another representation of our organization’s ongoing commitment to domestic violence awareness, community education, and support services.

Community Health Outreach Healthy Community Coalition of Greater Franklin County, in existence now for 25 years, is one of the oldest health coalitions in the country. Its mission is to measurably improve the well-being of all people in Greater Franklin County using a coordinated public health approach of education, health promotion, and outreach. With its qualified staff of public health professionals, including health educators, a nurse practitioner, registered nurses, a registered and licensed dietician, a dietetic technician, and a certified nursing aide, Healthy Community Coalition offers health screenings, health information, and programs and events to support healthy lifestyles that prevent disease and improve quality of life. Community outreach efforts appear in every town and corner of the region. Each year more than 8,000 individuals benefit from the outreach activities of the coalition with many receiving health risk appraisals (HRAs) on the mobile health unit. HRAs include a blood pressure and cholesterol check and individualized health education. Additional services offered include: bone density screenings, flu immunizations, body composition analysis, free clinical breast exams, education on tobacco, nutrition, physical activity, lead, and substance abuse, and education on the prevention of breast, cervical, colon, lung, prostate, and skin cancer. In addition, Healthy Community Coalition also offers a fee-for-service custom worksite wellness program for employers.

The Emergency Department Our emergency department provides 24-hour care for individuals with urgent medical, surgical, and traumatic conditions ranging from heart attacks to broken bones. Patients with serious conditions are stabilized here and may be admitted to the hospital—or in the most critical cases—transported by NorthStar ambulance or LifeFlight helicopter to a major trauma or specialty center. Patients seeking health care in the emergency department should enter the designated entrance where a staff member will greet them and explain the next steps. All patients are first seen in triage for a health assessment. We treat patients according to the urgency of their condition, so those with more serious conditions are treated first. The sickest patients are placed in a room quickly, with registration taking place at the bedside. Those with non-urgent conditions like generalized aches and pains or a sore throat, should always call their regular doctor first, before going to the emergency department. Even when the office practice is closed, there is always a provider on call. The Franklin Memorial Hospital emergency department has six treatment rooms; a special eye, ear, nose, and throat room; a designated trauma room; a designated mental health room; and a special care treatment room. Since 2012, the department is a secure unit that provides for safer care and increased security.

According to the American College of Emergency Physicians, all the following symptoms are warning signs of a health emergency: unconsciousness; chest pain lasting two minutes or more; uncontrolled bleeding; sudden or severe pain; coughing up or vomiting blood; trouble breathing; severe or persistent vomiting or diarrhea; confusion or other changes in mental state; trouble speaking; changes in vision; and suicidal or homicidal feelings. The Inpatient Experience

Patients at Franklin Memorial Hospital receive care from our highly qualified medical/surgical team that includes doctors, a physician assistant, nurses, nursing and medical students, pharmacists, physical therapists, respiratory therapists, medical specialists, a case manager, dietician, and others who help make our patients’ experience positive, comfortable, and healing. Hospitalists take care of patients from admission through discharge. They are specially trained physicians who care for patients during a hospital stay. The hospitalist staff remains in contact with each patient’s primary care clinician, sending regular updates and asking for additional information as needed. Nearly 40 hospital-based registered nurses hold advanced certifications in at least one of the following areas: emergency critical care, medical/surgical care, oncology, chronic disease management, wound care, and diabetes education, among others. Each of these certifications requires years of clinical practice in the specialty, the completion of continuing education credits, and passing rigorous exams. Doctors and nurses, together with a physical therapist, respiratory therapist, dietician, and other care team members meet every morning to discuss discharge plans to ensure that patients have the services they need when they leave our hospital. A care manager arranges for these services for patients including home care visits, rehabilitation in nursing care facilities, and continued physical therapy.

Medical Imaging Services

Medical imaging is the specialty that involves the noninvasive production of images of the inside of the human body to diagnose and treat diseases. Radiology, which makes up a large part of medical imaging, involves the use of radiation to obtain visual information, so patients’ safety is of upmost importance. At Franklin Memorial Hospital, patients’ exposure to radiation is minimized by way of equipment upgrades, appropriate imaging decisions, and low dose technologies. Franklin Memorial Hospital offers the following medical imaging modalities: x-ray, mammography, CT (Computerized Tomography); MRI (Magnetic Resonance Imaging); DEXA (bone density); nuclear medicine; and ultrasound. MRI and ultrasound do not use radiation. Physicians now use the American College of Radiology appropriateness guideline tool, which uses evidence-based guidelines to assist physicians and other providers in making the most appropriate imaging decisions for their patients so that imaging isn’t over or underutilized. The criteria have been shown to improve quality, reduce unnecessary exams, and lower costs.

Franklin Memorial Hospital Chief Radiologist Dr. Byung Kim is a board-certified diagnostic radiologist who specializes in diagnosing diseases and injuries using many modalities of medical imaging services. Radiology tests are ordered by your physician and the radiologist is the one who provides the interpretation of those imaging tests. Although you may never see the radiologist that provides the service for you, the role of the radiologist is crucial to accurate diagnosis and proper care.

Martha B. Webber Breast Care Center The Martha B. Webber Breast Care Center offers timely access to state-of-the-art screening and diagnostic, clinical, and care support services by a multi-disciplinary team of providers. Services are provided in a comforting environment where patients get the results they need quickly and compassionately. Digital mammograms are performed with advanced medical equipment and a board-certified radiologist interprets each exam. A care support nurse is available to personally guide each patient through the screening, diagnosis, and if necessary, the treatment process. In October 2014, the center achieved the status of a Certified Quality Breast Center in the National Quality Measures for Breast Centers Program™. This quality program measures the performance of National Quality Indicators™ of data, such as timeliness of care from screening mammograms to diagnostic mammograms, surgical timeliness of care from time between needle biopsy and initial breast surgery, and pathology report completeness, among others. This distinguished honor represents the center’s commitment to provide the highest level of quality breast health care to its patients.

Chronic or difficult-to-heal wounds caused by diabetes, poor circulation, and other conditions keep many from doing the things they love. In these cases, healing becomes a complicated process requiring unique treatment and care. Franklin Memorial Hospital’s outpatient wound care specialty services are designed to complement the primary care physician by providing highly specialized care for chronic wounds. Wound care interventions offered include: wound debridement, vascular testing, nutrition counseling, pressure relief, specialty wound dressing, bioengineered skin substitutes, compression therapy, antibiotic therapy, vacuum-assisted closure, and total contact casting. Physicians, along with registered nurses trained in wound care, develop individualized treatment programs for patients. Dr. Gerald Tinguely, a board-certified family medicine provider certified in wound care, oversees wound care services and provides wound care, along with Dr. Daniel Buck, a surgical podiatrist, who treats patients with wounds involving the feet.

Franklin Health Orthopaedics, in a partnership with St. Mary’s Center for Orthopaedics, offers a full range of orthopaedic and sports medicine services, including surgery, by a highly qualified team of health care professionals. Same day appointments are available for urgent conditions and x-rays are performed on site. When indicated, surgery can be quickly scheduled. Our providers offer comprehensive treatment for a full range of orthopaedic services, including: • Bone, joint, and muscle evaluations • Arthroscopy • Fracture care • Joint reconstruction • Sports medicine, including concussion management • Podiatry • Worker compensation evaluations The medical practice offers a weekly sports medicine clinic on Tuesday mornings for individuals of all ages who suffer from a sports-related injury. Clinicians include a doctor, a licensed physical therapist, and certified athletic trainer. For convenience Franklin Health Orthopaedics also offers services at the Androscoggin Valley Medical Arts Center at 21 Main Street in Livermore Falls, where Dr. Pulling sees patients on Thursdays.
