George Junior Republic Preventative Aftercare (PAC) provides communitybased behavioral health services to children (birth through 21 years), parents/caregivers, and adults in contracted counties in Pennsylvania and Ohio. Highly trained staff provide services in homes, schools, and the community.

The Family Based program supports the systemic changes needed for families involved with multiple county safety nets to structure their stability. Family Based services utilize Ecosystemic Structural Family Therapy (ESFT) as the evidence-based model, emphasizing the broader context of family distress and the need for community support.

PROGRAM GOALS

Create a collaborative team that attends to the intersection between the family’s social location, unique cultural norms, and values/beliefs

Provide team delivered treatment in the family home and community

Deliver intensive treatment involving highly-focused weekly sessions with the client, caregivers, and family for approximately 32 weeks

Empower caregivers as primary change agents

Build on strengths, promote resilience, and contain blame

Emphasize safety and stability first, including 24-7 on-call crisis availability

EXPECTED OUTCOMES

Establishes and maintain relationship rules and household routines. Establishes and maintain an emotionally safe, nurturing, and accepting relationship with the client. Develops skills to de-escalate conflict and problem solve when tension is high. Reduce documented symptoms and levels of distress.

Client participates more fully and adaptively in home, school, and community.

Client feels safe, secure, and protected in their relationship with caregivers. Client reduces risk for psychiatric hospitalization, out-of-home placement, and the need for other restrictive social services.

Working collaboratively, the family and the team develop the next steps in transitioning from Family-Based services to a lower level of care, solidifying the leadership of caregivers in the process.

REFERRALS & INTAKE

Children and youth under age 21 diagnosed with a serious emotional disturbance (SED) who are at risk of psychiatric hospitalization or out-of-home placement. The referred client typically lives within a highly stressed family environment. In addition, the families have grave chronic personal, social, and economic challenges that impede caregiving and disrupt the family process.

Referrals are received from hospitals, prescribed by physicians, residential programs, schools, OCY, JP and other community mental health organizations. All identified clients must have or be open to obtaining Medicaid (PAC staff will provide enrollment assistance as needed).

A psychological evaluation recommending Family Based Services must be in place within 60 days of the referral. The Program Director will support families in understanding the program, ensuring goodness of fit, and obtaining the evaluation as needed. The Program Director will connect the family with a team within the program.
