It is my pleasure to share the good news that our chairman has approved the introduction of two new super-specialties of Radiotherapy and Nuclear medicine. We will be launching a new RN block project soon. Our postgraduation seats have also increased from 64 to 77. Our faculty members and superspecialists have done some of the very difficult and unusual cases for the first time in GCS hospital's journey towards excellence in clinical care. Such as, Awake craniotomy, successful treatment of encephalitis case, Balloon mitral valvuloplasty. Our journey to improve services to PMJAY scheme patients and strategy of provision of free service to the community through this route is working out. GCS Medical College UG wing underwent surprise inspection successfully. As a welfare measure, we were able to increase salaries of nursing manpower, Assistant Professor, and senior residents of all specialties thereby improving their retention. I wish all my doctors, nurses, paramedical staff, and support staff a very Happy and Healthy New year.

Welcome to the 37th edition of the GCSMCH&RC Newsletter. As the year is ending, let me sum this last quarter's journey and milestones of GCSMCH&RC. We started the last quarter with the “Coffee with Lipi” as a par t of the International Coffee Day celebration. These three months were full of festive and celebrations along with this the Omnicron also raised its head, but we are all set for fighting the third wave with adequate equipment and competent staff. I also want to warmly welcome our Medical Superintendent Brig. Dr. Bhandaru Shridhar, with your experience and dedication, I know that we'll accomplish amazing things for the GCSMCH&RC. GCS Womaniya had its first official meeting to discuss further goals and objectives. After the success of GCS Academy courses, we also launched Homecare Assistant Course for further expansion. This upcoming new year will bring many challenges and surprises to our journey and our first challenge would be to face NABH's final accreditation assessment but I'm sure our team will ace this as a piece of cake. I wish all our GCSians a very happy and successful new year. You can also read about various other achievements, activities & updates from around the organization in this 37th issue of the GCSMCH Newsletter. As always, please send us your suggestions on how to improve our newsletter at communications@gcsmc.org. Happy reading.

I hope all of you are vaccinated by now and if not kindly take your shot of immunity. As COVID cases are surfacing, everyone should be extra precautions about their lives. We have inaugurated State-of-the-art Auditorium at GCS Medical College with the capacity of 350 people, I would like to h thank Mrs. Kanchanben Bhikhalal Shah and her family for their generous donation for building this auditorium. Now, we have full-time availability of all major Super-Speciality such as Neurology, Neurosurgery, Gastroenterology, Oncosurgery, Interventional Cardiology, Nephrology, and Urology at GCS Hospital which will be beneficial to patients. Let us take all the necessary precautions to be safe. Cheers to health, happiness, and prosperity in 2022!

I wish all of you Healthy, Joyous and Flourishing New Year of 2022 on behalf of GCS Medical College, Hospital & Research Centre. In 2021, we witnessed enormous challenges to fight against COVID-19 crisis of the second wave. Yet we continued our academic excellence along with pandemics to nurture our goal of producing globally & locally competitive medical graduates. This new year of 2022 brings a new challenge of Omicron Variant. However, the upcoming precaution dose of vaccination may boost the immunity of people and it will help us to reduce the burden of hospitalization as well as morbidity & mortality. I also like to wish the students of Under-graduate and Post-graduate students at our institute for their excellent and bright academic performance in the upcoming Gujarat University Examination. GCS Medical College is eagerly waiting to welcome our upcoming undergraduate & post-graduate students of the new academic batch. Lastly, I wish at GCSMCH&RC we will achieve the training of competent, compassionate, and caring physicians through excellence in patient care and medical research.

CLINICAL CASES Anaesthesia Department

Surgery Department

First time in GCS hospital awake craniotomy was performed. A 52-year female patient had left side superior frontal lobe Glioma. Tumour was near Motor cortex and Brocas area, so awake craniotomy was planned to minimize damage to eloquent cortex and resulting postoperative neurological dysfunction. We managed the case with a Bilateral scalp block along with IV dexmedetomidine. Challenges for Anesthesiologist were: - to keep the patient calm and seek her cooperation during the assessment of the Motor cortex. - to prevent Respiratory depression/ Respiratory arrest. - manage brain edema and intraoperative convulsion. It was really fascinating to talk with a patient with an open brain. The surgery lasted for 4 hours without any complications. The patient started moving with all four limbs with intact sensory and speech.

Department of Surgery organized a Live operative workshop of Hernia Surgeries at GCS Medical College, on 22 Dec'21. During the workshop, a total of 8 surgeries was performed. Four laparoscopic Hernia repair surgery were done for inguinal and ventral Hernia which included TEP, TAPP, eTEP, IPOM. Four open Hernia repair p ro c e d u re s w e re p e r f o r m e d w h i c h i n c l u d e d Licheinstein Tension-free Inguinal hernia Repair, Preperitoneal Mesh Repair for Ventral Hernia, and Component Separation Repair for Incisional Hernia. The workshop was aimed at demonstrating the classical steps of Hernia repair surgery, both open and laparoscopy, for residents and Faculties. It was first of a kind workshop for the training of in-house doctors to learn and master the techniques of Hernia surgery at GCS Medical College.

Orthopaedics Department A case of 55 years old female who had complaints of right hip pain with difficulty in walking and was operated outside for right side Primary Total Hip Arthroplasty in March 2021. Patient came to orthopedic OPD at GCS Hospital with complaints of inability to bear weight and persistent complaint of pain since primary surgery. Patient was diagnosed with infected case of Total Hip Arthroplasty. Patient was operated for removal of implant + cement spacer. Patient was given Inj Meropenem post-operatively. Patient came for regular follow up during which patient was admitted for IV Tigecycline as culture showed growth of Klebsiella sp. Later, when patient was diagnosed to be non-infective at the end of 10 weeks, definitive surgery was done in form of Right side Revision Total Hip Arthroplasty using multi-hole acetabular cup, constrained liner and long femoral stem by Dr Jyotish G. Patel under Orthopedic unit 1. After which pain subsided and patient can sit without any difficulty.

CLINICAL CASES Physiotherapy Department

Gynaecology Department

A 26-year-old male patient had a history of low back pain for 6 months. He came to GCS hospital and was referred for physiotherapy. Co-relating physiotherapy assessment and X-ray investigation he was diagnosed with POSTURAL LEFT LUMBAR SCOLIOSIS. After 14 sessions of physiotherapy treatment which included Interferential therapy (IFT) initially followed by Intermittent lumbar traction for 20 minutes Ultrasound 3mghz continuous mode. Back stretchingstrengthening exercises, the deviation was corrected, and the patient has 70-80% relief from pain

Vaginal Natural orifice transluminal endoscopic surgery (vNOTES) is an emerging technique and has attracted attention as a less invasive procedure with better cosmetic results, decreased postoperative pain, reduced hospital stays and recovery as well as decreased perioperative morbidity than conventional laparoscopy. Dr. Divyesh Panchal performed the first vNOTES hysterectomy of our hospital and Gujarat. Surgery was successfully completed in less than 90 minutes and patient is recovered fully. Thanks to my residents, anaesthesia dept, nursing and OT team for their support.
