BENSPlus Group Insurance Plan Feature Benefits The following benefit enhancements are included as standard features in the BENSPlus Group Insurance Plan at no additional cost. Critical Disease Benefit: This benefit is available to employees under age 65 who become disabled from a covered critical disease. It will provide a lump sum payment equal to 10% of their Accident & Serious Illness Benefit amount up to a maximum of $50,000. Covered diseases include Polio, Parkinson’s, MS, ALS, Alzheimer’s, Huntington’s Chorea, Type 1 Diabetes, Peripheral Vascular Disease and Necrotizing Fasciitis. Serious Illness Benefit: This benefit is available to employees under age 65 that are diagnosed with one of the covered serious illnesses and provides a lump sum payment of 10% of their Accident & Serious Illness Benefit up to a maximum of $10,000. Illnesses include Cancer, Heart Attack, Stroke and Kidney Failure. Hospital Indemnity Benefit: This benefit provides a cash payment of $40 per day after an eligible claimant has been hospitalized for 5 consecutive days up to a maximum of 180 days. This benefit is in addition to hospital coverage and is not dependent on including hospital coverage. Trip Cancellation Benefit: The included travel benefit provides payment up to $5,000 per claim for trip cancellation when cancelled due to specified reasons such as illness or accident. MRI Benefit This benefit gives plan members $1,000 per calendar year towards obtaining an MRI (providing it is not prohibited by provincial legislation).

STANDARD Insurance Plan Feature Benefits Diagnosis +: This service allows plan members to obtain a valuable medical second opinion when diagnosed with a critical illness. It provides a case specific second opinion and suggested treatment plan from highly qualified specialists. The service is accessed remotely, with no travel required. The specialist provides your physician with unique resources and expertise. This is a fully Canadian service offered in partnership with McGill University Health Centre. Lifeworks: This benefit is available to all employees and their dependents, and is a comprehensive resource to help employees address emotional problems and to seek practical solutions to both personal and professional issues. The service is available 24 hours, 7 days per week and is 100% confidential. There are no fixed limits on service utilization. DMI: Disability Management Institute DMI is a feature included as a supplement to Long Term Disability benefits. DMI services address all aspects of an employee absence due to non-work related injury or illness. DMI will provide administrative claims management services; manage communication between all parties; provide confidential, free support for employees and employers; and develop customized return-to-work plans.

Living Benefits Living benefits gives plan members who have been diagnosed with a terminal illness access to 50% of their group life insurance benefit up to a maximum of $50,000. Guaranteed Issue Optional life and Critical Illness BENSPlus provides access to both employees and their spouses to guaranteed optional life and critical illness benefits. Amounts up to $50,000 do not require medical evidence.
