Director’s Statement Jane Dasouza Managing Director Open Door

“My ethos has always been to empower people with deciding their needs to meet their life time goals.”

There are currently a lot of changes in Policy and Legislation in Health and Social Care Sector which are meant to improve outcomes for service users. As a social worker for over 10 years I have worked in different Health and Social Care Sectors, I am aware of the changes in line with the new government agenda. My ethos has always been to empower people with deciding their needs to meet their life time goals. Personalisation agenda incorporates these goals, the current environment provides the platform for all social and health care providers to support service users to have greater choice and control over the services they receive. My experience as a social worker together with my team will enable me to support clients in assessing and encouraging them to identify their own needs, discuss what their needs are and decide how they want their needs met. We will ensure they receive good quality service and achieve positive outcomes at all times. In Manton Lodge, we will deliver a personalised model that increases choice and control while enabling staff to deliver a safe and effective rehabilitation and recovery service. We will recognise self determination of individuals, support them to achieve their goals and aspiration. Acknowledging that the goals of people with mental Health problems have in their lives are the same as everyone else would have. We will support service users to have fulfilled lives, find meaningful activities, build friendships, socialise, enjoy close relationship and security with friends, family and their community. Often these may be difficult to obtain but in Manton Lodge we will endeavour to support them through the stages. We will develop strategies to nurture “champions” among service users in Manton Lodge, who will achieve, develop skills and abilities that they can spread the word and share positive stories and approaches.

Taking responsibility for one’s life The re-establishment of a positive identity

Referrals All referrals, will meet the following criteria to be accepted • The person is between 18yrs to 65yrs of Age • Has a mental health diagnosis • Assessed to be vulnerable • Assessed to be unable to maintain independent living • In need of short-term support and assessment of skills where this has not been possible • Funding has been approved or will be approved by commissioners of care and support needs • Willing to comply with terms and conditions of the tenancy

Referral Process

• New referrals are normally via Social Workers, CPNs, and health workers and occasionally through other agencies/services. All new referrals must be approved and funded by Local or Health authorities; therefore it is essential for a Social Worker/Care coordinator/Health representative to be involved at some stage for securing appropriate care funding. • The first stage of the process is to complete an initial referral form and Risk Assessment and email/post or fax to 01992 787447 • If the referral is accepted, the first visit will be arranged as an opportunity for us to meet the client, and for them to meet with us and get an idea of the setup and range of support/care services we offer. • If both parties wish to proceed, a formal offer of placement will be made, and a final planning meeting will be organised. • Obtain funding • In normal circumstances the contract must be drawn up prior to a move in date being agreed, however this can be negotiated in emergency situations on the condition that funding is agreed in writing by the team manager. • Agree a move in date
