things to look for in an assisted living facility or skilled nursing home

Knowing the difference between the two can help you make the correct long-term decision for you or a loved one B Y MARIE PETERSON Long Term Care Ombudsman

A

re you sick and tired of being sick and tired? Are you bored and want to have fun with a group of active seniors and not worry about cooking, cleaning or yard work? Then an assisted living center might be for you. Have you just had surgery or a stroke and have been in the hospital? Do you need wound care or physical therapy to make you Peterson stronger? Then a skilled nursing home might be for you. And, Medicare might pay for it if you have been in the hospital three nights, the doctor writes an order for skilled nursing based on your hospital diagnosis and you go to the skilled nursing within 30 days of your hospital discharge. Do you or your loved one need minimal or moderate care, medication management or supervision because of memory loss? Is it unsafe for you or your loved one to remain at home? Then either an assisted living center or a skilled nursing home might be for you. There is a difference between assisted living centers and skilled nursing homes. Be sure your choice meets your needs. Ask questions. This set of questions or checklist will help you know what to look for when trying to decide whether an assisted living center or a skilled nursing and rehabilitation facility (nursing home) might be the best choice for you or your loved one. These are general questions to ask about both types of

facilities. If you or your loved one has special needs, be sure to ask if the facility can accommodate them. Before making your decision: Always visit, always test a meal or two. Always check activities. Always feel comfortable. Here are 10 things to check for when researching a facility: Geography: Is the facility located conveniently for visitors and preferred hospitals? Financial: What is the daily/monthly cost? What does this fee include? What are the additional fees? Is the facility receptive to accepting Medicare, Medicaid, VA

benefits, insurance, private pay? Diagnosis: Is the facility able to provide the appropriate level of care? Is the staff trained in the diagnosis of your loved one? How do they monitor wandering residents? Personal care and staff attitude: Is the staff polite and courteous to the residents and to you? Is there enough staff? Are call lights answered promptly? Are the residents clean, neat and tidy? Is personal hygiene addressed adequately? Are name tags visible? Resident attitude: Do the majority of the residents appear comfortable and content? Are

they smiling? Activities: Are there group activities? Are there individual activities? Are there weekend activities? Are there outings? Is exercise equipment available? Activities make life worth living. Choice: What choices are available in activities, room selection, menu, etc.? Are you able to keep a pet? Is there a hair dresser? Is there a suggestion box? Housekeeping: Is the facility neat and clean? Are spills cleaned up quickly? Dietar y: Does the monthly menu show adequate variety? Will they accommodate your particular diet and food preferences? Did you try a meal? Was it tasty and presented well? General atmosphere and appearance: Is the facility inviting, comfortable and home-like? Is the outside area easily accessible? Is there adequate protection from sun or rain? Ask to see the latest survey report from the Bureau of Facility Standards. The survey will point out problems noted during their last visit. The latest surveys are located in the facility or at www.facilitystandards.idaho.gov. Another resource, www.medicare.gov, compares nursing homes throughout the country. If you have any questions, contact your Long Term Care Ombudsman who investigates questions and concerns about long term care and makes routine visits to facilities.

Using the care conference at a nursing home or assisted living center B Y MARIE PETERSON Long Term Care Ombudsman

Whether you or someone you love needs care at either a nursing home or an assisted living center, it should be the best it can be. How do you get good quality care? Communication is important to let everyone know what is needed. How would you like to start your day? When would you like to get up? Do you like to sleep in? Do you prefer a shower or a bath? Would you prefer your shower/bath in the morning, afternoon or evening? What are your favorite foods? What foods don’t you like? Do you have any allergies? What do you like to do? Read? Listen to music? Pet your dog? Garden? Watch a movie? Just leave me alone. What help do you need? Do you have favorite clothes that you like to wear? What time do you like to go to bed at night? You have the right to make choices about your care. You are the consumer. Quality care includes all

aspects of living, not just physical needs. Have a care plan meeting. Doing the things you like can make your day better. How can you get your needs known by the staff? At a care plan meeting, also known as a care conference, all your needs can be discussed and decisions can be made to include your choices. Your decisions are written down in categories so that all staff will understand what care you need, what you like and dislike and what you like to do. This information is shared with the staff so that they know who you are and what you need to make your stay worthwhile. Know who to go to if you have a problem. No matter how well a plan is made, sometimes a problem may develop. Knowing who to go to when this happens is critical. Get a list of the staff that is responsible for day to day care (administrator, director of nursing, charge nurse or house man-

ager). Each facility has its own staff organization. If the facility is not addressing your concerns to your satisfaction, you have the right to call your long term care ombudsman, whose job it is to investigate complaints and work with the resident and staff toward resolution. Another avenue to follow is the state regulatory agency, Health and Welfare’s Bureau of Facility Standards and Licensing and Certification. You can reach the bureau at (208) 334-6626. They are also responsible to answer questions about rules and regulations and investigate concerns or problems. Persevering the challenges of raising my grandchild

one of these grandparents. My introduction to the Grandparents Raising Grandchildren Program and Emily came a few months after my granddaughter was born. The

knowledge and insight Emily has plus the support I’ve received from other grandparents in the program has helped pull me through this difficult year. My daughter suffers

cross America more than six million children are living in grandparent (or other relative caregiver) households. The main caregiver of many of these children is a grandparent, and I am

from schizophrenia and struggles daily with her illness as well as side effects of her medicaBobby tion. While she was pregnant I imagined that she would need a lot of help with her new addition. I hadn’t realized what an overwhelming task I had before me. After the birth my daughter could not wake up to care for or feed her new baby. As her health started to improve she spent more time with her baby. The burden of care still fell on me for nighttime feedings and most of the daily care. I was the one to see that my granddaughter’s needs were consistently met. Numerous hospitalizations complicated my daughter’s ability to parent her daughter. This continues today, more than a year later. It was after the second hospitalization that I sought out the Grandparents Raising Grandchildren Program. Emily listened to my story and shared some of her vast health of information with me. I could work with legal aid for the guardianship. The Department of Health and Welfare could help me keep my granddaughter on Medicaid. The annual back to school picnic was coming up, so I could meet and socialize with other grandparents. I found out that she was

looking for a new employee in her office. I applied for the job. A couple months later I was hired. Working with the Grandparents Raising Grandchildren Program has been wonderful for me. I have met many wonderful people, and feel the support of other grandparents, the advisory council as well as the support of the community. I have to admit, that although I adore my grandbaby, I was ill prepared in many ways. The financial burden has been hardest to bear. Most grandparents qualify for a “child only” TAFI grant through the department of Health and Welfare for $309. But because I care for my daughter as well as my granddaughter I do not qualify for this grant. I was able to qualify for ICCP, but the co-pay portion of my bill, which includes my granddaughter, my 5 year old and two school-age children was more than my net pay per month. I felt trapped! I have overcome the problem of childcare costs, but I’m sure this is only the first of many difficult situations I will face in the future. Knowing that I am not alone and that there are avenues to turn to for support has made me feel more prepared for what lies ahead. I will survive, in fact I will persevere and as a family we will accomplish great things!

Fraud Alert: This scam has been traveling across the United States Individuals in the states of New York, Maryland and California are reporting receiving calls from a person claiming to be from a medical office wanting to send the individual a new Medicare card. They already had the person’s name, address, social security number, and they just needed to confirm the bank account number so they could take the $48 fee for the card out of their account. Unfortunately, most of the recipients of the call gave out their bank information to the caller. Later they found out the call was bogus and notified their bank and the police. he Silver Sneakers program for senior citizens has a simple goal: “Get fit, have fun, make friends.” Apple Athletic is one of about 10,000 sites nationwide that offers classes through Silver Sneakers to senior citizens, 65 and older. The program is offered through a Humana Medicare supplement under Medicare Advantage and supplies a free membership to clubs such as Apple Athletic Club. “We have lots of classes with music that’s age appropriate for 50-yearolds to people in their 90s,” Laurie Wright, an instructor at Apple Athletic said. “The purpose is to strengthen them and give them skills for active, daily living.” Incorporated into the Silver Sneakers classes is improving increased strength, balance, coordination, flexibility, range of

Humana,” Cushman said. “I was paying $600 a year before this.” Cushman enjoys the yoga and water aerobics classes and the fact that the exercise has improved her health. “Every senior citizen should do it,” Cushman said. “It helps prevent falls.” Sandi Gordon, a trainer at Apple Athletic, said

Silver Sneakers club members have unrestricted access to the club, just as any other member would. Wright said the options available for seniors also include social and educational benefits. “It really is a social network for a lot of them,” Wright said. “Especially those that have lost a spouse or they’re single. It’s a very social thing that gets them up and out of the house to fight depression. It keeps them active, keeps them moving.” The activities hosted by Apple Athletic help form friendships. “We also have socials, educational workshops that are age appropriate, and they become friends,” Wright said. “It’s a social thing for their better wellbeing. As a senior, it helps with depression.” Recent educational workshops the club hosted included cooking for one or two people. The club also recently had a workshop on osteoporosis.

“You fulfill the social outlet which makes them just feel better about themselves,” Wright said. “They feel more alive, more vibrant, but the benefit of the exercise is just great. It’s a great support system and I think that’s worth as much as the exercise benefit.” Gordon said each course could be adjusted to suit whatever a senior’s needs are. She has had people come in who recently had heart and kidney transplants, several who have had cancer, multiple sclerosis, knee replacement and hip replacements. “I used to have several that came to class with oxygen and their canes,” Wright said. “Some eventually don’t need to use their cane anymore and they’re so excited because it’s a big deal, it really is. B Y NICK BURROWS, MBA, CCAP arlier this month, I had the opportunity to present at the monthly Geriatric Forum held at EIRMC. In all honesty, until then, the magnitude of what is offered for seniors through the Area Agency on Aging had not hit me in my two years as the Director of the Area Agency on Aging. During my day, my focus shifts from one program to another as my staff and I work diligently for the seniors of eastern Idaho. Before I go on, I want to thank the staff of the Area Agency on Aging, because they are the lifeblood of what we do! There are a few staples of what an Burrows Area Agency on Aging does, including: Information and assistance, long-term care ombudsman, case management, adult protection, caregiver services, and congregate and home delivered meals. These services exist in every Area Agency on Aging in Idaho and are covered regularly in this annual insert. At our Area Agency on Aging, we strive to be the leaders in Idaho, with regard to service delivery. Two areas which we innovated in the past are our Grandparents Raising Grandchildren Program and the Volunteer Ombudsmen Improving Care for our Elders program. The former provides a wide array of assistance specifically to grandparents and other relative caregivers of minor children. The VOICE program trains volunteers to make regular, unannounced visits to long-term care facilities to observe the quality of care, advocate for resident rights, and investigate and resolve problems as they arise. We are continuing to be leaders of innovation in our aging network through two new programs. First, we have partnered with the Community Family Clinic to provide dental vouchers to seniors who do not have dental insurance. The Community Family Clinic provides services to eligible participants on a sliding fee scale. Our vouchers will pay at least a portion of that fee. For low-income seniors, our vouchers will pay the fee in its entirety. Another area of innovation is the inclusion of an online resource database. At the Area Agency on Aging’s website — www.eastidahoaging.com — we have created a link to our resource database, which is searchable. It is our hope that if you have a need, our database can connect you to a local resource to address that need. With everything that we do, we have great plans for the future. We are actively working as part of a coalition with area hospitals to put systems in place to improve the well being of patients as they are discharged into their own homes. In doing so, we hope to see a decrease in hospital readmissions and an overall improvement in the well being of eastern Idaho’s seniors. The last year has flown by. We still encourage people to “know your neighbor.” We are cautiously watching the political landscape as the battle to protect Medicare, Medicaid and Social Security continues to rage. Primarily, though, all of our efforts are focused around the importance of creating systems to allow seniors to live independently and safely, in their own home, for as long as possible.

As shown by recent indictments, Medicare is an attractive target for criminals. Attorney General Eric Holder and Health and Human Services (HHS) Secretary Kathleen Sebelius announced that a nationwide takedown by Medicare Fraud Strike force operations in seven cities has resulted in charges against 107 individuals, including doctors, nurses and other licensed medical professionals. According to court documents, the defendants allegedly participated in schemes to submit claims to Medicare for treatments that were medically unnecessary and oftentimes never provided. These operations mark the fourth in a series of historic Medicare fraud takedowns over the past two years. “Health care fraud is not a victimless crime,” says FBI Deputy Director Sean M. Joyce. “Every person who pays for health care benefits, every business that pays higher insurance costs to cover their employees, every taxpayer who funds Medicare — all are victims. The FBI will continue to work closely with our federal, state and local law enforcement partners to address health care fraud and abuse. Ombudsmen lend their voices to those who cannot speak for themselves

Volunteer Ombudsmen Improving Care for our Elders (VOICE) is the Assistant Ombudsman Program which recruits and trains volunteers to visit nursing and assisted living facilities in Bonneville County on a weekly basis. The mission of the Long Term Care Ombudsman Program is to provide ombudsman services to all residents of all long-term care facilities through advocacy and education. The goals of the program are: To advocate for resident rights, To investigate complaints on behalf of residents and work toward resolution, To provide community education on long-term care issues, To provide ombudsman services to all residents of all long-term care facilities in a nine county area, (one volunteer to each of our 40 facilities). Each volunteer receives 24 hours of curriculum and 32 hours of one-on-one mentoring. Background checks are mandatory. A technical assistance meeting is held monthly with updates and training to the volunteers. With our volunteers, facilities are visited weekly and the residents know that they have advocates who care and will solve problems for them. In our nine-county area, the 2010 census showed that out of the 202,463 residents, 20,460 were over the age of 65. Of those, 1,172 are living in long-term care facilities. The Ombudsmen Program provides a forum for citizens who need advocacy for long-term care issues. Why are seniors targeted for fraud? If you are a senior individual, you might be asking yourself this question: Why are seniors more vulnerable and more often

targeted than a younger individual? Here are some reasons: They are more likely to be at home to answer the door or the telephone. They tend to be from an era when people were more trusting and are less likely to spot a con artist. Often lonely, they are more susceptible to listen to a friendly pitch. They may be physically incapable of doing their own home repairs, thus making them the perfect recipient for this type of scam. They are targeted for financial schemes because they are more likely to own their homes free and clear. How can you protect yourself from scams and fraud? First, register your

telephone number with the National Do Not Call Registry at (888) 382-1222 or their website at www.donotcall.gov. Never do business with strangers who just show up at your door. If you don’t know them, don’t open your door. Never give out your personal or financial information to an unknown caller. A rule of thumb is — if you didn’t call them, you don’t give out your information. Always write down the date, name, telephone and address of anyone you talk to, in person or on the telephone. Never sign anything unless you fully understand what you are signing. If you are not sure, don’t sign.
