Smart hospital is culmination of fantastic building, best technology, efficient doctors and nurses. In addition, processes have to be simple, turnaround time should be minimal, and information has to be available at the point of care.” SMART HOSPITALS SHOULD BE SMART ON EVERY FRONT PERTAINING TO PATIENT CARE The critical component for smart hospitals revolves around the ability to provide a valuable service of insight, which was simply not possible or available earlier. This is what makes a hospital a step further from being just digital to becoming smart in true sense. Simply digitising, or making the hospital paperless (although a great achievement) is not sufficient. From exploratory implementations to an intermediate stage, to finally becoming smart where hospitals have complete alignment of clinical processes and management systems, are hallmarks of the smart hospitals. cheaper, and better delivery of services. Just having processes and skilled staff won’t help. A holistic approach to healthcare i.e architecture of the hospital, flow of patients, processes, and cleanliness, is the need of the hour.” There are three areas that any smart hospital addresses – operations, clinical tasks and patient centricity. Operational efficiency can be achieved by employing building automation systems and smart asset maintenance and management solutions, along with improving internal logistics of mobile assets, pharmaceutical, medical device, supplies and consumables inventory as well as control over people flow (staff, patients and visitors). Clinical excellence encompasses improving patient outcomes by ensuring patient engagement and monitoring. “The concept of these hospitals starts right from building. Facilities are the first component of these hospitals followed by processes, clinical, nursing part and overall patient engagement,” Dhomne further adds.

SMART HOSPITALS CONCEPT IS NOT LIMITED TO DIGITALISATION ONLY Changing healthcare trends coupled with huge demands of quality and affordable healthcare from people of all strata of society unleash a barrage of conflicting challenges at hospitals. They need to optimise and automate processes as per the patients’ requirements, maintaining a decent revenue margin. A digitally organised hospital integrates various departments, services, and clinical to do away the manual work. The objective is to improve patient care through digital processes and new technologies through various communication platforms, a cross-

It becomes easier for a digitized hospital to build on its database to design efficient health care delivery services. Digitalisation holds a huge significance to facilitate things but it’s only a single component of whole mechanism of smart delivery of care. “Smart hospital is more about journey. Technology, ease, and convenience are building blocks of smart hospitals. How you apply the technology and smartness to care is what makes a huge difference. Technology is easy to develop but bigger challenge is its adoption by doctors and patients,” says Chaitanya Shravanth, Chief Digital Officer, Cloudnine Group of Hospitals. EHR, MHEALTH NEEDED FOR PERSONALISED HEALTHCARE EXPERIENCE The first step and the foundation of a smart hospital is the electronic health record (EHR). If all patient data is available in a digital format, it can be accessed by all departments and hospital stakeholders in no time at all. In

an emergency, the physician is able to check for any pre-existing conditions or allergies the patient might have. Needless to say, this speeds up all subsequent treatment steps. Despite its many advantages, EHRs are not being used prominently in many of public and private hospitals in India and the Government needs to take cognisance of it. Then mhealth is another area which proves crucial. A physician can use their smartphone or tablet to access a patients Electronic Health records (EHRs), review medical histories, send followup emails, and even complete prescriptions. There are many mobile apps which provide more high-quality services, and at the same time are cheaper for patients to access. mhealth apps are very useful in chronic care management, medication management, fitness and weight-loss, and managing personal health records. Both EHR, mHealth are basic ingredients of a smart hospital which contribute to give personalised healthcare experience to patients. “Technologies can be harvested
to ensure basic patient care delivery with better outcomes. The basic essence of any smart hospital is that it has to be patient-centric. The most important thing we need to look at is the outcome. When you are talking about smart hospitals, you need to run it in a smarter way which could increase profitability,” says Dr Govindaiah Yatheesh, Unit Head, Apollo Speciality Hospital, Karnataka. MODERN TECHNOLOGIES INCLUDING AI PLAY A KEY ROLE Cutting-edge technology is proving to be a biggest game changer. Artificial intelligence, IoT, robotics, and 3D printing, have been playing a transformative role in enhancing different deliverables in healthcare sector. In cardiology, 3D mapping to facilitate customised cardiac mapping of a patient’s heart is gaining popularity. This technology lets physicians gain new insights into the heart of the individual patient. Artificial intelligence and machine learning used for radiographic assessments show great potential. Robotics are proving to be boon in reducing waiting time, and other operational chaos, thereby increases

PATIENT SATISFACTION IS AT CENTRE OF SMART HOSPITAL CONCEPT Patient satisfaction is an important aspect of smart hospital which mainly revolves around quality and affordable care. “Quality and affordability are two factors which decide the patient base of a particular hospital. Patients don’t get influenced by swanky buildings and fancy staffs, but the kind of care they get from the provider,” believes Dr vijayabaskaran Sundararaju, Executive Director, Kauvery Hospital. Patients want best value of their money which makes things difficult at times for the providers who try to find out ways how to live up to their expectation. One of the solutions is to maintain a transparent approach on every front. For an example billing should be done against particular service offered to patient and so on. Santosh Marathe, COO, Apollo Hospitals, believes, “Affordable care and patients’ expectation both are subjective in nature which depends

upon what value is being offered in lieu of money charged by providers. Standardising cost and quality are need of the hour.” Human touch is another aspect which influences patient satisfaction level. People find it more appealing if a hospital has good staff including doctors and nurses. “Quality of human touch point is very important. Empathy plays a pivotal role in patients’ satisfaction which in turn influences patient care. Empathy and kindness of the lowest level of staff including nurses and housekeeping employees matter a lot,” says Dr Aloke Mullick, Group CEO, Omni Hospitals.

the overall productivity. Internet of Things has immense potential to give a new high to healthcare sector. It has numerous applications in healthcare, from remote monitoring to smart sensors and medical device integration. “Infusion of technology is not easy. The willingness to adapt technology by all the stakeholders is very low. Even doctors of more than 20 years experience find it tough to adapt to new technology. Therefore it is very imperative to train the staff first. We enable them to adapt to new technology. We have to handhold them so that both patients and hospitals would be benefitted,” says Col Hemraj Parmar, Group CEO, B R Life. Though a section of experts vouch about the potential of these tech-enabled solutions in laying a strong foundation of healthcare delivery system, they believe that technology must be leveraged intelligently to improve total outcomes. Developing technology is one aspect but challenges lie in integration and adoption by hospital staff to leverage it. “Getting the right kind of people who fit your organisation is first requirement for smart hospital. Then using smart technology or using technology smartly holds huge importance,” says Dr Dilip Rangarajan, Medical Director,
ENSURING BEST CARE WITH DECENT REVENUE FOR HOSPITAL IS CHALLENGE In the wake of costly equipment and other expenses on various fronts, providers face tough task to make a balance between quality and costefficiency. “In the wake of huge running cost of healthcare services, providers need to recalibrate their cost-strategy. Providers need to focus on innovative practices, envisioning designing and operationalising smart hospitals,” says Parmar. Guruprasad B R, Vice President, Sparsh Group of Hospitals opines, “Quality is proportional to cost. Every hospital should pronounce that they have some specific standard of quality associated with particular amount of cost.” In nutshell, a smart hospital looks for holistic way of delivery of care. Though approaches may vary from hospital to hospital, the objective remains same— providing best clinical outcome through integrating all aspects including technology, innovative approaches and human resources with minimum hassles at operational level.

Technology Paving Path For Digital Pathology As the healthcare standards keep rising, the need and demand to revamp pathology is even more evident than in recent times, writes Reetika Bose of Elets News Network (ENN).

athology is one of the most significant mainstays of the healthcare ecosystem. Thanks to the paradigm shift in the healthcare segment that led revolution of digital pathology. For the past 150 years, pathology has been the essence of the health sector with pathology being at the forefront in understanding and diagnosing diseases for decades. While the medical industry has been blessed with incredible technology and unbelievable innovation on other fronts, the core of the diagnosis process continues to incline heavily on pathologists in labs. Talking about tackling the digital pathology space, Dr Jayaram N Iyengar, Managing Director, Neuberg Diagnostics said, “The scenario is complex with high establishment cost of digital scanners on one hand

and relative paucity of experts in histopathology on the other. Trained and competent pathologists would not prefer to work in tier 2 cities and district health centres. Establishing a laboratory equipped with trained technicians and a digitisation equipment would facilitate study of the digitised slides by any expert pathologist anywhere in the country.” As the healthcare standards keep rising, the need and demand to revamp pathology is even more foreseen than in recent times. Pathology needs to be ahead of time which will directly imply the capacity to work faster and with more productivity. In that lies the ascent of rise of digital pathology. “Introduction of low cost scanners albeit with acceptable shortcomings compared with the high through put equipment, would be the way
forward”, Dr Iyengar added. DIGITAL PATHOLOGY OVER CONVENTIONAL PATHOLOGY In spite of witnessing the quick advancement of innovative medical technology such as blood-drawing robots, augmented reality, synthetic tissues, and machine-run organs, pathology continues to depend on the similar old technique of looking at tissues with a microscope within laboratory walls. However, experts are of the opinion that with innovations in pathology, the pathologist can have a complete control of the entire process. Live sharing of images becomes easy and high resolution snapshots are made just by a click on the use. “It takes an average pathologist almost 20 minutes to scan and analyse

a slide that digital pathology systems would be able to do within 3 minutes leveraging artificial intelligence. Moreover, conventional pathology is also not very amenable to proper indexing (incorporating all relevant searchable fields) and tougher to retrieve,” said Pallavi Jain, Managing Director, KRSNAA Diagnostics. “Digital pathology has been used widely for teaching purposes. Recent years have witnessed an increased use in expert consultations and as an archival tool for storage of digitised slides. Use of digital pathology for routine daily diagnosis, and for conducting proficiency testing is now gaining increased acceptance,” Dr Iyenger added. Conventional pathology workflows have innate delays built into the processes. Transferring unique glass slides runs the risk of loss, and when collaborating with others it requires time, which delays diagnostics. A BOOMING OPPORTUNITY The utilisation of advanced pathology has given enormous opportunities to pathologists working in labs, colleges, or even research centres. But the question lies on what makes this sector a booming opportunity? Earlier in a pathology lab, a great deal of time was squandered in investigating the slides and examining the tissue tests directly from the slide. At times transportation of these slides turned into a sensitive issue because of its delicate nature. Things have taken a U-turn, thanks to digital advancements made in the healthcare space. With the new digital scanners and computer software, sharing of digital images of tissues electronically with a doctor has

become very easy and possible. When asked about market opportunities for the diagnostics space, Zoya Brar, Founder, Core Diagnostic, said, “While diagnostics makes up only 3% of total healthcare expenditure in India, it controls 70% of the expense downstream and 100% of the outcome. This problem is even more amplified when you consider digital pathology. The market continues to be constrained by the lack of affordable technology and skilled manpower. However, given the huge delta between what we are spending and the impact of digital pathology, I am confident that this will change.”

Diagnostic said, “Adoption of digital pathology facilitates faster reporting that has made it easy to obtain second opinions across geographies without transporting images. It leverages increased efficiency with a reduced turnaround time for reporting clinical cases by reducing error rates.” Replacing the old technique of laboratory examination with the new digital technique means pathology has now become completely image-based information, which is retrieved with the help of computer technology.
CHALLENGING THE CHALLENGES In India where the ratio of pathologists to patients is very ordinary, digital microscope improved the efficiency and productivity of pathologist and technicians and is able to connect with their peers for second opinion. Through this process, patients of far flung places will also able to access accurate diagnosis in a short span of time. Moreover, implementation of digital pathology system will help in reducing costs especially in larger institutions as it replaces multi-head microscope with live digital one which is easy to store and image production

Shedding light on tackling the challenges, Dr Iyenger said, “The usual practice in our country is for a pathologist to be a “generalist” and not a specialist with expertise and experience in only certain aspects of pathology. This undoubtedly works with the existing healthcare system but we need to have a healthy blend of specialists who would make our health care system in tune with latest advances.” As we are approaching towards 2020, the number of pathologists in the field is reducing. The ratio of pathologists to patients is extremely skewed towards being unfavourable for the former. Coupled with an

increasing pressure to move faster and more efficiently, pathology labs need to find ways to not only better assist pathologists but also in a way that’s future-ready, efficient, and scalable. Digital pathology holds the answer to this need, and much more. “India has only about 1,000 histopathologists that are trained in organ specific pathology. Compare this with the US where there are close to 18,000, and the problem is clear. Also, we don’t focus on organ specialisation as a country so often our skill-set makes us excellent generalists but when it comes to extremely rare or difficult patient diagnosis, we are at a loss. Collaborating with countries like the US is certainly
one way to overcome this challenge. This of course will require labs to be enabled with digital pathology and to have a collaborative and transparent mindset”, Zoya Brar opines. WHAT’S NEXT IN PATHOLOGY FUTURE? Medical science is rapidly evolving and so as advances made in the field of laboratory diagnostics. A pathologist has transformed from being a support, to an active partner in healthcare delivery. There is an ever-increasing dependence of the physician on the laboratory and this synergy works wonders in the era of precision and personalised healthcare. Broadly speaking, the government must help create a regulatory environment that ensures that organisations get incentivised to focus on providing the right quality. “India has tremendous potential of becoming the digital pathology hub of the future given the existing IT capabilities. Proactive policies by the government will take us a long way forward,” Dr Iyenger said.

What Healthcare Experts Decoded from Union Budget 2019-20

nion Budget 2019-20 aimed at creating new India with major focus on infrastructure, development and technology. Though, there was no major announcement pertaining to healthcare sector, except mentioning of creating healthy society via Ayushman Bharat in the budget present by Union Finance Minister Nirmala Sitharaman in the Parliament, it took into account holistic approach to build empowered and prosperous India. The Government allocated Rs 62,659.12 crore outlay for the health sector including Rs 6,400 crore earmarked for the Ayushman Bharat, the highest in the last two financial years. A section of experts believes that there is no clear cut mention of the plan in the budget on how the funds would be utilised to facilitate accessible, affordable care under the gamechanger scheme of Ayushman Bharat. Dr H Sudarshan Ballal, President, NATHEALTH, termed the budget as comprehensive, wide-ranging and balanced with many positives for the economy. “The government needs to revisit unfinished agenda such as ‘Priority Sector’ status to healthcare for smooth credit flow, mandatory universal health insurance, tax incentives for capacity building, creation of a dedicated infrastructure and

innovation fund,” he said. Gautam Khanna, CEO, P D Hinduja Hospital & MRC, said, “Although there have been no direct mentions for the healthcare industry, the proposed Rs 100 lakh crore investment in infrastructure in the next 5 years could pave way for a boost in the healthcare segment.” “The government’s objective to move towards building a healthy India with the continuity of the Ayushman Bharat scheme and the proposal to expand the Swachh Bharat mission by means of sustainable waste management and building of more toilets thus aiming India to be open defecation free by this year, is a worthy objective,” he further stated. “National Research Foundation for higher research will help indigenous research in pharmaceutical industry, bio-medical industry, innovation in indigenous treatments modalities,” said Dr Rajeev Boudhankar-CEO, Bhatia Hospital. here has been an increased use of information technology across fields in enhancing productivity, cost efficency and streamlining the processes. In healthcare, IT has proved to be an important tool to improve patient care, contain cost, minimise errors, and standardise the protocols.
IT ENABLES TO MAINTAIN ROBUST DIGITAL RECORDS IT has been very useful in the maintenance of patient records. Digitalising records has helped in saving the storage space and thereby retrieval has been made easy. These digital records are safe unlike the conventional paper records which are susceptible to physical damages. Digital records can be accessed and shared remotely. The lab and
radiology reports can also be scanned and stored. There are newer technology to store the radiology reports in required resolution. Follow ups have been made more systematic by setting up of timely notifications to the patient and the doctor or hospital. This has been very useful in cases where patients are asked for short interval follow up visits such as in vaccinations, chemotherapy, postsurgery follow ups, physiotherapy, study of slowly progressing chronic ailments etc. PACS--Picture archiving and communications system is a network of computers used by radiology departments which replaces film with electronically stored and displayed digital images. This uses a server to store and allow facile access to highquality radiologic images, including conventional films, CT, MRI, PET scans and other medical images over a network.
Patient care and management through information technology: • IT has revolutionised the whole paradigm of patient care. Today, small and large healthcare setups are largely dependent on appointment booking platforms which have replaced the conventional calling systems. It saves time and cost both to hospitals and the patients. Moreover, IT proves its worth in minimising human efforts, reducing potential errors and delay of conventional calling systems. • Lead management through software has helped track the inflow of patients and thereby provides a wide opportunity for data analysis. • The innovation approach of such technology has largely been patient centric. Patients are provided all the required attention and constantly motivated as well to seek support. • Technology like patient queue management

systematically manages the patient inflow to a doctor. • Quick exchange of information between doctor / hospital and the patient, saves time and cost for both the sides. • Patients who are staying miles away can now have their first consultation with the doctor without even meeting him personally. Rajasthan Budget: ‘Janta clinics’ among series of measures to bolster patient care ‘Janta clinics’ was among the many schemes announced by Ashok Gahlot-led Congress Government of Rajasthan while presenting budget in the State Assembly recently. The 2019-20 budget also announced a Rs 1,000-crore farmer welfare fund, a Rs 5,200-crore dedicated feeder to ensure regular supply to farmers and other schemes for green energy, irrigation and health sectors In the healthcare sector, the government announced a series of measures in a bid to ensure accessible, affordable and quality care to people at large. On the lines of a successful model of Delhi government’s Mohalla Clinics, the Chief Minister announced the opening of ‘Janta clinics’ near the residential areas across the State. It will enable people to avail better patient care near doorsteps.
The Union Budget allocated Rs 62,659.12 crore outlay for the health sector in the 2019-2020 fiscal including Rs 6,400 crore earmarked for the Ayushman Bharat – Pradhan Mantri Jan Arogya Yojna (AB-PMJAY), touted as gamechanger healthcare insurance scheme of the Central Government. The outlaw for the healthcare sector is the highest in the last two financial years. The allocation for healthcare this fiscal is around 19 per cent more over the 2018-2019 fiscal when it was Rs 52,800 crore. Rs 249.96 crore has been allocated for setting up Ayushman Bharat Health and Wellness Centres under the National Urban Health Mission to provide comprehensive primary care close to the community, while Rs 1,349.97 crore earmarked for setting up health and wellness centres under the National Rural Health Mission.
ISIC becomes the first in India to perform spine robotics surgery The Indian Spinal Injuries Centre (ISIC), New Delhi has claimed to be the first hospital outside the US to have recently performed surgeries with the help of an advanced robotic system. Considered to be a first of its kind in India, the Indian Spinal Injuries Centre (ISIC) at Vasant Kunj has introduced robotics as an advancement in spine surgery, a specialised approach to a complex procedure that allows planning a surgery and facilitates accurate and predictable execution of the plan. It helps in inserting implants in the spine. On achieving this milestone, Dr H S Chhabra, chief of spine services, ISIC, said, “Surgeries conducted through advanced robotics reduce implant inaccuracies, revision surgeries, radiation exposure, length of stay and infection. Such high improvements in so many parameters simultaneously can drive significant clinical efficiency and reduce the burden on healthcare system in the medium to long run. This will help the doctors of ISIC serve the patients better and in a cost-effective manner.”

Odisha government to form new health policy to stop doctors’ brain drain Health Minister Naba Kishore Das recently said that the Odisha Government will form a new health policy to stop the brain drain of doctors and address the shortage of doctors in the State. He also said there would be no shortage of doctors after two years as the State would produce about 1,500 doctors every year from various medical colleges. “Under the new health policy, steps will be taken to retain the doctors who are moving outside the State after completing their education on State government’s expense,” said Das. The health minister also said that incentives will be provided to the doctors to retain them in the State.
Govt forms panel to consider central law on doctors’ security
The Union Ministry of Health and Family Welfare has formed a committee to look into the feasibility factor of a central law to stop violence against doctors across the country. The move comes to the fore in the backdrop of last month all India strike by doctors following attacks on medicos by the family of a patient in Kolkata. Apparently, the panel has been directed to check the “pros and cons” of bringing central legislation and whether it will be effective to check violence on medical professionals in hospitals. The 10-member panel which held its first meeting recently comprises members from the Home Ministry and the Legal Affairs Department. It also has representatives from the Medical Council of India and doctors’ associations, and medical superintendents of AIIMS and RML Hospital.
Women who wake up early have lower breast cancer risk: study If you are prone to cardiovascular diseases and finding it tough to come to term with the same, there is good news in the offing. Scientists have found that use of pasteurised form of Akkermansia muciniphila bacteria may cut such risks. According to a study published in the journal Nature Medicine, the intestinal bacteria provides greater protection against various risk factors. According to the findings, the research team from the University of Louvain, Belgium developed a clinical study in order to administer the bacteria to humans. For the study, 40 participants were enrolled and 32 completed the trial. The researchers administered Akkermansia to overweight or obese participants, all displaying insulin resistance (pre-diabetes type 2) and metabolic syndrome, in other words, having several elevated risk factors for cardiovascular diseases.
Jharkhand: Slums to get free medical care under Mohalla Clinics The Raghubar Das Government is mulling to replicate the success of “Mohalla Clinics” in Jharkhand. Reports say that the State Government has decided to provide free medical care to people living in slums under “Mohalla Clinics”, a successful model of Arvind Kejriwal-led Delhi Government. “These clinics will be like primary health centres and will offer free diagnostics facilities, treatment and medicine for minor ailments. These are being started for the benefit of daily wagers, who lose a day’s salary if they visit a regular hospital for treatment. The clinics will open every day in the morning and evening. Serious patients would be referred to bigger hospitals for treatment,” State’s Health Minister Ramchandra Chandravanshi was quoted as saying.

The series A funding round was led by Blume Ventures, an early stage venture capital fund. The healthtech startup was founded in in 2011 by Varun Gera. It is an aggregator of primary care services for corporates and individuals and helps bring day-to-day care closer to individuals. The company will use the fund to augment its health-tech platform, create sharper products that serve various primary care needs of the Indian market more effectively including insurance OPD products, build better data capabilities leveraging AI & ML and strengthen distribution. Healthtech startup 1MG raises $70 million in series D funding 1MG, a Gurugram based healthtech startup, has raised $70 million funding led by Switzerland’s Corisol Holdings with participation from the World Bank’s investment arm IFC. The funding will be used to maximise its investments in AI, mobile engineering and hardware integration. Under the Insolvency and Bankruptcy Code (IBC), The National Company Law Tribunal (NCLT) approved Gurgaonbased Dhanuka Laboratories’ resolution plan of Rs 1,116 crore to take over the financially troubled pharmaceutical company, Orchid Pharma. The Chennai bench of NCLT held the view that a majority of the financial creditors had approved the bid by Dhanuka overruling an objection filed by Accord Life Spec. As per the terms set, the lenders will get about Rs 1,116 crore including Rs 570 crore quoted by Dhanuka Lab. A consortium of 24 banks has lent a total of over Rs 3,200 crore to the drug maker and the lenders will be taking a haircut of around 65 percent.
