Okay, now that you know who I am and why I’m doing this, let’s talk about the 7 Biggest Mistakes Sciatic and Back Pain Sufferers Make When Finding Their Sciatic and Back Pain Specialists And How To Avoid Them!

BIG MISTAKE #1 Assuming That A Primary Care Physician Understands Sciatic and Back Pain

I

am not here to fault any specialty and I have the upmost respect for Primary Care Physicians, but no one can know everything about everything and they have to deal with all types of patients and all types of ailments. Aside from Orthopedic Surgeons who only specialize on the back, there are not too many people like us who just work on back and sciatic pain. When you mention back pain to your family doctor they will never perform a physical exam, orthopedic, neurological tests, review your x-ray or MRI right in front of you. The reason why they don’t is because they do not have training in that area of the body. The Journal of Bone and Joint had a study that revealed that 82% of the medical resident examinees failed to demonstrate core competencies for assessing back pain. Another study revealed the average amount of time spent on muscular, joint and nerve pain was only 2.1 weeks and 33% of the residents spent zero time studying orthopedics.

While it is true that blindly trusting that your provider truly understands your problem and how to correct is a lot easier than researching it yourself, there is no assurance that you are going to get the results that you want and deserve.

Dirty little secret #1 hospital doctors don’t want you to know: Local hospitals, such as Eisenhower frown upon hospital doctors referring outside of their hospital network. Hospitals have large overhead and hence charge a facility fee which more than doubles your costs and if you have a large deductible or co-insurance than that could lead to thousands of dollars in additional costs without getting a referral to potentially who they feel is the best. Hospital doctors always refer to their hospital colleagues because their administrators frown upon outside referrals to private clinics that could solve your problem with a fraction of the cost.

Dirty little secret #2 doctor’s don’t want you to know:

Specialists always get hung up on a diagnostic test result and often forget to actually listen to you or perform a physical examination. Often, your description of your pain, how you injured it and what flares it up with a physical examination compared with your diagnostic results will give us the best picture. Specialists have been overwhelmed with patients and new government regulations on healthcare, which is why they will often spend most of the office visit on their computer and will rarely pull their head out of their computer to actually look you in the eye. Many of them are burned out on helping patients and view you as another number and another potential law suit.

Dirty little secret #3 doctors don’t want you to know: I have a story about a previous patient who was unsuccessful after seeing the pain management specialist for his sciatic pain, who had arthritis on his MRI of his lumbar spine and had received multiple prescriptions, Chiropractic, Physical Therapy and spinal injections without any long term improvement. The problem was in a joint in the pelvis, which was not shown on the MRI because the MRI was ordered of the spine and not the pelvis which lead to his treatment focused on an arthritic back rather than an unstable pelvic joint. I don’t know if you know this, but almost anyone over the age 50 will have some arthritis in their spine somewhere and very commonly in the back. To fix this patient’s sciatic pain we just applied our 4 pillars of pain relief to this patient. We decreased the pain and inflammation with medicine, which allowed our Chiropractors and Physiotherapists to work on this patient without muscle spasms. To stabilize their unstable pelvic joint we provided Physical Therapy to strengthen the muscles, and a growth factor injection into his pelvic joint restored the stability to his pelvis and eliminated his sciatic pain. This is just a another example of why working together with different types of health professionals is paramount to treating difficult painful conditions.

Hospitals, Doctors and Therapists »» Did they perform an examination? If so, were they able to reproduce your problem mechanically through movement? This is what I call a bingo sign, because if your doctor is able to reproduce your pain then they will know what is causing it most of the time. »» Do they charge a facility fee? If you are at a hospital or hospital affiliate office then the answer is yes, that literally means that you will pay more than double for anything you receive. In fact, your bill will be between $800 to $1500 minimum per visit just for walking through the door, plus whatever procedure you will receive from the doctor themselves. I literally suggest you ask them if they charge a facility fee if one of our local hospital names is listed in their business name. If you don’t know, the names here they are: Desert Regional, Eisenhower or JFK in front of the practice name. Bottom Line: Hospitals have an insane amount of overhead and must cover this with these additional facility fees.

BIG MISTAKE #2 STICKING WITH JUST PHYSICAL THERAPY OR CHIROPRACTIC FOR CHRONIC PAIN

C

hiropractic and Physical Therapy have been a considered the standard beginning point of treatment for musculoskeletal pain in most clinics. There are plenty of cases that improve eventually with these therapies, but treatment visits could be cut dramatically if you incorporated pillar number 1 which is the reduction of inflammation through medication, joint injection or trigger point injection. Very rarely do you see Chiropractors and Physical Therapists working together either and Chiropractors are traditionally superior at joint alignment, which is part of pillar number 2 and Physical Therapy is great at performing exercise for muscle stabilization which is part of pillar number 3. All of these are great, but in most offices and hospitals you will notice all three of these professions do NOT work together with each under one roof. Reduction of pain and inflammation

Inflammation is necessary for survival when you have an injury, but it also causes pain when you have an injury. An easy way to visualize inflammation is by imagining a metal wire brush scraping your arm. This will make your arm red and inflamed, causing pain. This is what occurs in your joints, tendons, ligaments and muscles which is why most people will feel better when they take a non-steroidal anti-inflammatory like ibuprofen, Advil, Motrin or Aleve. However, this is one of the leading causes of kidney failure and can cause internal bleeding if taken long term.

We are not against taking these medications short term, but finding long term solutions for inflammation is the best solution. To reduce inflammation we offer a wide variety of methods from traditional medical treatments to Acupuncture or ultrasound guided joint injections which will allow us to view the joint in real time and gives us the ability to view important structures to ensure the medication is delivered the appropriate area to get optimal results. Other 21st century methods include deep tissue laser and pulsed electromagnetic therapy.

PILLAR # 2 = Alignment of Structure Just like a car out of alignment will create additional wear and stress on your car, stiff, unaligned joints will do the same thing. Your body was created to move and flexibility is a key factor to good health. Just think about how flexible young health babies are, and eventually everything stiffens up at the end stage of life. Maintaining muscle, ligament and joint flexibility with proper alignment does help with your overall joint function and is a key factor in long term pain relief. Proper joint motion allows for a process called imbibition which is a natural pumping motion created by full joint movement that pumps synovial fluid into the cartilage of your joints; this provides nourishment to your joints and this is why movement and flexibility is key to long term health.

PILLAR #3 = Reinforcement Through Muscle Stabilization This is what traditional physical therapy accomplishes. I tell patients that physical therapy is great, but if you try to reinforce a misaligned joint, poor posture or fixated joint, then you will not achieve the same results as we do. You will also notice that you will be able to exercise better and improve at a faster rate since we provided significant relief through the first 2 pillars of pain relief. Strengthening and activating weak and inactive muscles will create a natural splint for your joint and help maintain this correction long term. This will prevent future flare ups with your condition. This functions much like the tooth brush for the dentist. We may have our teeth checked and cleaned at the dentist, but without maintenance in-between office visits, we would develop tooth decay. These simple exercises shouldn’t take you more than a few minutes on a daily basis once you condition has stabilized, but it will save you thousands of dollars in time and money by adding it to your daily regimen. BIG MISTAKE #3 Using Opiates/Narcotics Over Technology

here has been an increased push from the government and certain regulatory agencies to reduce the use of narcotics because of the harm they has caused many chronic pain patients. That is why insurance companies and Medicare have started covering procedures like this one to reduce the use of narcotic prescription to patients. One of the devices we have had a great response from is something called a neurostimulator. These original devices have been surgically implanted into your body with wire leads feeding into nerve bundles, but a new external device has hit the market recently. This device was originally brought to the United States by Dr. Bonakdar at Scripps Clinic to treat patients who

have been prescribed morphrine, oxycodone and other harsh narcotics. Dr. Bonakdar was tasked with treating the most difficult pain patient and some of them were war veterans who suffered from severe and chronic pain from permanent battle wounds suffered from an explosion. Opiates have been proven to be effective for pain reduction and are great if you are recovering from a major surgery, but if your pain is from incurable causes, this is the wrong approach. That was one of the issues Dr. Bonakdar had a dilemma with and was always searching for a non-opiate treatment for pain. That is when he tried Acupuncture on his patients and although it was unsuccessful for long term relief, his patients did note that they had relief temporarily during the treatment time. That is when he theorized that if he could find the right combination to stimulate the correct nerves for an appropriate amount of time he could get long lasting relief. He was invigorated with the results, but needed to find a solution to his problem of limited time relief from Acupuncture treatment. So he did what any good doctor would do, which was searching publications for anything that might solve his dilemma. That was until he discovered two studies that were performed with continuous electrical stimulation for chronic pain. The study noted that patients received pain relief after 4 weeks of treatments, which prompted him to contact the inventor, Dr. Josef Szeles, a vascular surgeon in Vienna, who traveled to San Diego and showed Dr. Bonakdar how to use the device.

This device is called pulse stimulation treatment (PST) which is based on cranial electrotherapy stimulation (CES), which has been used for treating depression, but is now being researched for the treatment of chronic pain. Dr. Bonakdar found it particular useful for nerve pain, arthritis pain, spinal pain, complex regional pain syndrome, postsurgical pain and many other types of pain. Regarding neuropathic nerve pain conditions, many patients will experience a central sensitization phenomenon where the brain takes over and amplifies what would otherwise be a normal healing process and make the pain worse. This device works to counterbalance central sensitization phenomenon and will literally block your body from feeling this amplified pain. Some doctors will treat this with anti-seizure medications and anti-depressants, but this is a benign way to signal the brain to correct those pain signals. This device has now been approved by the FDA and only weighs about 7 grams and is fastened behind the ear by an adhesive backing.

How Does This Technology Work? Specific nerve bundles for your type of pain are found with a locator that will measure the nerve activity of back pain nerves, nerve pain bundles, neck pain, etc. There is a map of nerve bundles on your ear that will correlate with any part of your body. Once the appropriate nerve bundle is located, the Physician will now attached tiny titanium acupuncture needles into the outer part of the ear, which is attached to the

neurostimulator itself. This will deliver 1Hz of energy 3 hours at a time over the course of 4 days. Over a period of 4 to 6 treatments the device will stimulate your body to produce more pain fighting endorphins for a period of about one year so your pain will be reduced.

What Are Beta-Endorphins And Why Do They Matter? Endorphins are neurotransmitters, chemicals that pass along signals from one neuron to the next. Neurotransmitters play a key role in the function of the central nervous system and can either prompt or suppress the further signaling of nearby neurons. Endorphins are produced as a response to certain stimuli, especially stress, fear or pain. They originate in various parts of your body -- the pituitary gland, your spinal cord and throughout other parts of your brain and nervous system -- and interact mainly with receptors in cells found in regions of the brain responsible for blocking pain and controlling emotion. Until recently, much of what we’ve learned about endorphins has been gained from monitoring endorphins in the human bloodstream and in rats’ brains. It wasn’t possible to measure endorphin levels in the human brain without harming the subject, so the role of endorphins in the “runner’s high” and other periods of euphoria or mood change were still hotly debated. However, new imaging methods allow

researchers to study the ebb and flow of endorphins as they interact with human brain cells, verifying their role in the rush that exercise -- and other triggers -sometimes prompt. There are at least 20 different kinds of endorphins, and one kind, beta-endorphins, are stronger than morphine and have been shown to play a part in everything from alcoholism to diabetes to aging of the brain. These beta-endorphins will bind to opiate pain receptors under the same mechanism as opiate based narcotic medication like: morphine, OxyContin, Oxycodone, Hydrocodone, Vicodin and Norco work. Those medications are great for reducing pain, but have extreme effects on your liver and are extremely addictive, which is why they have the highest level of regulation. Even with these high levels of regulation, they are still abused by Physicians and patients, and are targeted by burglars when they break into a medical office.
