If you love the results you can get with dermal fillers then why not keep them for the long term? This new-generation volumiser integrates with your own skin tissue to provide subtle, stable results that last up to 10 years. Ask your cosmetic physician about long-term dermal filler for lasting treatments.

Reconstructive filler for lasting volume

If you have scars or dints and depressions anywhere on your body that you’re unhappy with, or facial gauntness due to weight loss or gastric banding, or simply want to improve the contours of your nose or chin, ask your cosmetic physician about this extremely effective new long-lasting dermal filler especially developed for reconstructive purposes.

Smoothing to perfection For soft, plump, kissably enhanced lips or correction to fine lines in delicate areas such as the under-eyes or upper lip, this ultra-smooth fourth generation dermal filler delivers beautifully subtle results. Ask your cosmetic physician about the HA-based biocompatible filler that can be injected without any telltale beading or streaking.

Filling to perfection Turn the clock back 10 years in just 30 minutes. This exceptionally volumising filler restores the firm curves of youthful facial contours and is completely natural feeling to the touch. Ask your cosmetic physician about the volumiser with the ‘gummy bear’ consistency that can be used to correct deeper wrinkles and folds or to enhance your facial outlines.

Hydrating to perfection For those hard-to-treat areas that can sometimes look older than you are: the hands, neck and décolletage— and for your face, a rehydrating treatment that restores suppleness and smooth texture. Ask your cosmetic physician about the injectable treatment that harnesses the moisture-retaining synergy between hyaluronic acid and glycerol.

A NOSE FOR BEAUTY .................42

THE SEVEN DEADLY SINS OF WINTER SKIN ..............................16

When it comes to cosmetic procedures for the nose, the approach taken today is far more holistic than it used to be. Advanced surgical and injectable options are now delivering the right size, shape and width of nose to suit not just the individual’s face, but their lifestyle, cultural background and personality, too.

Winter is hard on your skin. Dry air and harsh wind can sap moisture, causing cracking, chapping and irritation, and you’re still vulnerable to sun damage, even in the cold. But you can take action.

FACE THE FUTURE ...................... 30 If the image staring back at you in the mirror just doesn’t look as vibrant as the person you feel inside, you can choose from any number of surgical and non-surgical options that will help slow the tick of the biological clock – on the outside, at least!

SUN UNDONE ....................................52 There are some benefits to winter! Here’s how to maximise your indoor time during the cooler months by addressing your sun damage and skin pigmentation.

SUN

CHEEKS LOSE VOLUME

There’s no better time than winter to tackle the pigmentation, sunspots, UV damage and general lacklustre skin that come about from overexposure to the elements. For one thing, the UV onslaught is less than in summer because we spend less time outdoors (although daily use of a high-SPF sunscreen is still essential). Also, no-one expects you to be as social in winter, so a few days of downtime after more invasive age-reversing procedures is likely to go unnoticed by friends. What’s more, the healing process from resurfacing treatments will be far more comfortable when the mercury isn’t soaring, because we sweat less in winter. “Perspiration contains salt, which can cause further irritation to skin,” says Lisa Sullivan-Smith from The Clinic in Bondi Junction, Sydney. “You don’t want to expose deeper layers of skin in hotter months for this reason alone. “There’s also a chance that if you have treatments in the summer, your pigmentation will just bounce straight back. You can’t escape the summer sun in Australia, so winter is by far the best time to start undoing your sun damage.”

Sun exposure and the resulting sun damage manifest in many ways, which often dictate the type of treatment or procedure required to address the issue.

pigmentation Solar pigmentation is a direct result of sun exposure. “In youth, we call it freckles; as we mature, we refer to it as sun spots,” says Lisa. “The skin is under attack from UV rays and it produces pigment to defend itself. With age, this reaction becomes worse.” And how do we deal with it? “Fraxel Re:Store Dual is the gold-standard treatment for removing pigmentation,” adds Dr Jeremy Cumpston, from Ageless Clinics in Randwick, Sydney.

surface veins and broken capillaries Heat generated from the sun’s UV rays causes blood to rush to the skin’s surface in an effort to cool it down. As a result, the veins or capillaries swell, their walls burst and a cluster of broken capillaries or veins may appear on the surface of your skin – most commonly around the nose and across the cheeks, but sometimes on the chin. Light treatments such as LimeLight (an advanced form of IPL) can be very effective in minimising the appearance of veins on the skin.

Pregnancy, dramatic weight loss, weight gain or even just the effects of gravity over the years can leave you looking and feeling a little less than lithe and lovely. Unfortunately, sometimes even the healthiest diet and rigorous exercise routine can’t shift the stubborn fatty tissue that creates those trouble spots or niggling wobbly bits. Thankfully, there is a plethora of cosmetic options – both surgical and non-surgical – that can enhance and uplift breasts, whittle away a waistline that won’t shift, or restore the shapely thighs and buttocks of your youth, safely and effectively.

A beautiful body isn’t just about maintaining a healthy weight and shape. It’s about being flexible and strong, too. Be sure you back up any procedure with a regular exercise program.

lines and wrinkles The sun damages collagen fibres and causes the accumulation of abnormal elastin, which leads to lines and wrinkles, as well as sagging skin. Laser treatments can soften the appearance of fine lines and wrinkles and restore some of the skin’s firmness. Teamed with injectable fillers, they can help make skin look more youthful.

external factors Changes in the skin’s tone and texture, caused by the degradation of collagen and elastin and other ageing factors, can leave it looking lacklustre and imperfect.

Sun exposure can quickly sap skin of its moisture, leaving it dull, uneven and drab, and this can be more ageing than lines and wrinkles. Drink plenty of water, skip tea, coffee and alcohol, and use good skincare products. A peel can quickly bring skin back to life and restore its moisture balance. MORE ›

IT’S CLASSIC, SEXY, DRAMATIC AND WILL NEVER GO OUT OF STYLE. BUT RED LIPSTICK WORN THE WRONG WAY CAN ALSO AGE YOU BEYOND YOUR YEARS. YOU CAN AVOID THE PITFALLS WITH OUR LITTLE RED GUIDEBOOK. BY ELISABETH KING

Bold, bright and glamorous – red lipstick can turn even shy and retiring types into instant divas. And you’d better get used to this season’s outbreak of scarlet fever. Pure red lipstick is as classic as a Chanel suit and it’s making an even bigger splash this winter in versions ranging from value-for-money lippies priced at less than $15 to designer tubes costing up to $50. You have to be careful, though, when you’re playing with passion’s war paint. Even on young faces, red lipstick can turn a small mouth into a mean line and drain any other colour from the complexion. The key to va-va-voom success, especially for women in their 30s, 40s and 50s, is to find the right shade – one that will truly flatter your skin tone and make you feel and look fabulous.

The high-drama look of red lipstick shifts the emphasis away from the eyes to the lips, so they should be in top condition. Gently exfoliate the lips with a toothbrush or your facial exfoliant to remove any flaky skin. Apply a lip balm and blot away any excess. Then apply a light layer of foundation and allow to dry. Because of the strong pigments in red lipstick, it’s best to outline the lips with a lip pencil (to prevent the colour bleeding) and fill in using a lip brush, says Natasha Severino, make-up artist for YSL Beauté. “After lining the lips, apply one layer of lipstick and then dust with translucent powder to make the lipstick stay put,” she advises. “Finish by applying a second coat of lipstick with the lip brush.”

the right shade

Nearly all make-up artists agree on the basic rules of matching a red lipstick to your skin tone. Michael Brown, Lancôme’s national make-up expert, says reds falls into three major categories: orange, pink and blue-based. “If a red lipstick looks out of place on your face, you’ve usually picked the wrong shade,” he explains. Women in their 30s look best in pink-based reds as they’re softer and more feminine, so the mouth doesn’t get all the attention. “These work well with most skin tones as they complement pink-toned skin and brighten yellow-toned skin,” says Michael. Orange-based reds are brighter and warmer. “These look really good on blondes, whether they’re pale-skinned or tanned,” Michael explains. “They’re also a great colour for women in their 50s who prefer to wear less eye make-up and are also finding their complexion has lost colour or has a tendency to flush.” Blue-based reds recall the perfectly painted lips of Hollywood’s golden era – elegant and sexy at the same time. “They particularly suit darker and brunette women with olive or yellow-based skin,” says Michael. “But women of any age can wear them if they have the defined features to carry it off. Chic women in their 40s wear blue-reds very well, and they’re a great choice for winter.” Even with all the best advice in the world, many women are still intimidated by red lipstick. According to Michael, though, there is another option: brown-based reds, which he calls the “I’m not sure” reds. “They’re more of a brick-style red and are a great compromise for women who don’t want to attract too much attention or feel too ‘done up’,” he says. “They’re a very flattering choice for women of all ages and skin tones. Brown-reds brighten skin tone, balance a strong eye look or offer a focal point for those who prefer softer eye make-up. A top choice for winter, too.” MORE ›

THE QUIET

REVOLUTION The average age of the Australian woman is getting older (38.1 years at last count) and her life expectancy longer (83.67 years). Yet she is looking younger than ever before. A quiet revolution has occurred over the past five years — a boom that has defied world economic trends. Aesthetic anti-ageing was once the province of the rich, famous and infamous and involved dramatic procedures followed by lengthy recuperations at exclusive hideaways. Today someone from any walk of life can walk into a doctor’s surgery or cosmedical clinic and emerge a veritable lunchtime later, leaving unwanted years behind. Faces can be revolumised and wrinkles and furrows all but erased with dermal fillers; frown lines and crow’s feet reversed with muscle-relaxant injections; sagging faces firmed with skin and muscle-tightening systems like Thermage and Ulthera; and sun-damaged or flawed skin revitalised with new-generation lasers such as Fraxel and Cutera. Yet the revolution has been quiet because although rejuvenation procedures are now considered mainstream, largely affordable and socially acceptable, many people still prefer to keep them a secret, even from family and friends. “The use of fillers has probably increased 300 to 400 per cent over the past five years and now rivals the frequency of

JENNI GILBERT

EDITOR, YOUNGER YOU

muscle-relaxing injections,” says associate professor Greg Goodman, of the Dermatology Institute of Victoria. “Lasers have probably increased about 100 per cent over the same time period. The main shift in these is the substitution of safer and faster-recovering equipment than that used in the more severe treatments.” CONTINUED OVER ›

THE QUIET

REVOLUTION CONTINUED The Cosmetic Physicians Society of Australasia estimates that Australians spent nearly $450 million on non-invasive and minimally invasive treatments in year to March 2010, representing an increase of 30 per cent on the previous year. “We believe this growth can be attributed in part to the increase in choice for dermal fillers and an evolutionary leap in laser technology,” says CPSA president Dr Gabrielle Caswell. “The most popular non-surgical treatments are proving to be medical-grade skin care, dermal fillers (for full facial sculpturing), anti-wrinkle treatments with Botox and laser resurfacing. “Dermal fillers have grown enormously in popularity. For a long time there was only one brand of hyaluronic-acid filler. Now there are competing brands that are gel-like and pain-free as well as longer-lasting, with very low risk of adverse events.” These new-generation fillers — that come with names like Juvéderm, Restalyne and Esthélis — allow doctors to choose different thicknesses according to what part of the face is being treated, giving more natural results. “Australians have a large amount of sun damage. The new laser technology delivers good results with little downtime and now gives the doctor great tools to tackle deeper wrinkles and stubborn problems such as previously permanent pigmentation (melasma),” says Dr Caswell. “I find that my patients are outcome-driven and therefore unlikely to spend money on ‘fluff and buff’ but prefer proven medical technology,” she adds. “They’re strategic in their spending and want outcomes, not promises.” Leading plastic and cosmetic surgeon Dr Warwick Nettle,

from Sydney’s Silkwood Medical, says Australians’ uptake of cosmetic medicine has been “slowly increasing over the past 10 years, with a major increase over the past five years. Our business has more than doubled and that’s against a background of an explosion in providers of these services.” Dr Nettle attributes this boom to several factors. “Surgical and non-surgical rejuvenation is becoming more acceptable as baby boomers are ageing and have more disposable income,” he says. “Baby boomer women are increasingly in the workforce, especially in senior professional and management roles. They’re often working with people many years junior to them, and so feel the need to look fit and well. “The market for well-paid jobs is very competitive — people need to look good. And more women are divorcing and feel they need to freshen up to be attractive for the next phase of their life — competition is tough. “On top of that, procedures are getting better and better. Compare today’s injectables (biocompatible hyaluronic acids lasting nine or more months) with yesterday’s offering (potentially allergy-causing collagen lasting three months, with luck). “There is an explosion in what can be done, both surgically and non-surgically, from lasers and injectables to minor and major surgery. And the internet has made this explosion immediately available to people.” The phenomenon is not restricted to the capital cities, either. “When I first opened my practice (Eyra Medical in regional NSW), my colleagues thought I was quite mad and that country women ‘liked things natural’,” says Dr Caswell. “As one of my patients once put it, ‘It’s about trust (in your doctor) and access. We may be in the country, but we use the internet and read the world’s media. We know what’s available, but we don’t want to risk a bad job.’ That just about says it all.”

“This growth can be attributed to the increase in choice”

FACE & SKIN

01 dehydration, dryness and wrinkles DEADLY SIN

No matter what our skin type, we’re all prone to dehydration, especially during the winter months. “About 20 per cent of the skin’s moisture sits above the epidermis,” says Dee Davies from the Clear Skin Clinic in Double Bay, Sydney. “This is where most moisture-loss occurs, but in winter moisture loss can occur below the epidermis, too. The result is severe dehydration, which can make lines and wrinkles not only look more pronounced, it can actually cause them to deepen at a faster rate.”

7DEADLY SINS

OF WINTER SKIN CARE WHAT TO DO ABOUT IT Clearly, using a richer formula to hydrate the skin is paramount, but you also need to ensure the skin is well prepared so it absorbs the product well. Exfoliate once or twice a week using a product that employs both mechanical and chemical exfoliation. 02 eczema and dermatitis DEADLY SIN

“Eczema and dermatitis are interchangeable terms for itchy, scaly and inflamed skin,” says Dee. “There are many forms of the condition, most of which worsen in winter due to lack of humidity, a reduction in skin hydration and an upset barrier function. Anything that changes the lipid layer will aggravate the problem, such as overuse of soaps or over-cleansing the skin.”

WHAT TO DO ABOUT IT “Cut back on those long, hot showers and baths,” recommends Dee, “and increase humidity at home with a bowl of water in each room and by lowering heat levels. Avoid harsh soaps anywhere on the body, using an unscented, dermatologically tested cleanser instead, and switch to a gentle, fragrancefree, anti-allergy moisturiser that will soothe and calm the skin.” Try Cetaphil Restoraderm Skin Restoring Body Moisturiser, $25.99 (all prices RRP); Aveeno Stress Relief Body Wash, $9.99; Dr Lewinn’s Medic Bio-Care Oil, $24.95; QV Cream, $30.75.

03 broken capillaries and redness DEADLY SIN

During the cooler months, skin can become prone to a red, ruddy appearance. This is likely to be due to sensitive conditions, like rosacea, or small, superficial broken capillaries on the surface of the skin. They tend to flare up because of extreme temperatures and other triggers, such as central heating, hot showers, alcohol, spicy foods and hot drinks.

WHAT TO DO ABOUT IT Clearly, it’s important to avoid the triggers listed above, but using calming, strengthening and healing skin care will work, too. “Avoid anything with active ingredients such as acids or vitamin A,” advises Associate Professor Greg Goodman, who’s a practising dermatologist. “Ingredients including Azelaic acid, green tea and other antioxidants are ideal. Always use non-irritating cleansers and moisturisers, and a water-based or light liquid sunscreen applied daily is essential.” He adds that any product used on the skin should be able to be easily removed. “You don’t want to apply anything that is too claggy or thick and requires a lot of rubbing when cleansing,” he says. For your make-up, look for a green-tinted primer to help counteract the redness in the skin. “Apply it with caution and choose a good-quality product,” says L’Oréal Paris make-up artist Rae Morris. “You don’t want a film of green over your skin – you simply want the primer to counteract and negate any redness.” Try Pelactiv Precious Oils, $41 (all prices RRP); Dermalogica Sheer Tint Redness Relief SPF 15, $77; Clinique Redness Solutions Makeup SPF 15 with Probiotic Technology, $49; L’Oréal Paris Studio Secrets Professional Anti-Redness Primer, $37.95.

THOU SHALT EXFOLIATE REGURLY Your skin sheds dead cells regularly, but the process slows in winter because cold skin doesn’t function as efficiently. 04 stinging, sensitive skin DEADLY SIN

This can occur when you apply a product with active ingredients that you may not be used to, or quite simply a reaction to weather extremities. Over-exfoliating could be another cause. When trying to soothe irritated, dry skin, you need to be an educated consumer. Always check the ingredients, and try to avoid those that contain the two biggest causes of irritation: alcohol and fragrances.

WHAT TO DO ABOUT IT “Anything that stimulates the skin is to blame,” says Dee, “so you need to keep it cool and calm. Steer clear of alcohol, spicy foods and caffeine and wear moisturiser with sunscreen. Avoid harsh cleansing and over-exfoliating. Gentle soothing masks will calm angry complexions.” Look for products containing ingredients such as allantoin, anti-oxidant vitamin E, lavender and chamomile. THOU SHALT PRACTISE DEFENSIVE MOISTURISATION

In winter, dehydrated skin happens faster than you can say “I’m thirsty”, which can lead to serious stress. 05 chapped hands, hangnails, splitting nails DEADLY SIN

The colder, drier air of winter draws so much moisture from the skin that the hands may become cracked, chapped, sore and red, and the nails more brittle and prone to splitting. This can be aggravated by bathing in hotter water than usual. “Nail infections and hangnails can result from as little as one neglected cuticle,” warns beauty therapist Liza CurwenBegg, founder of Skinlab in South Yarra, Victoria. Avoid further irritation by minimising exposure to hot water.

WHAT TO DO ABOUT IT

LIP SERVICE

“It’s imperative to hydrate and nourish, and never, ever cut your cuticles. Simply push the cuticle down gently with your fingertip, and take a few minutes before bed to massage cuticle oil into the nail bed. Reapply a rich moisturising hand cream frequently throughout the day.” Never tear, cut or pull on broken skin around the nail bed. Instead, nurture it nightly and give it time to heal. Try Lanolips Rose Balm Intense for Very Dry Hands & Nails, $15.95 (all prices RRP); Burt’s Bees Lemon Butter Cuticle Creme, $14.95; Elizabeth Arden Eight Hour Cream Intensive Moisturizing Hand Treatment, $35.

“Lips have no oil glands to keep them naturally hydrated,” explains Dee. “This makes them particularly vulnerable to dehydration in winter. Add constant licking and sipping of hot drinks, and lips can become chapped in no time. “ Repair takes longer,” adds Liza. “If you chap, chafe or cut the lips, it can create an ulceration that might be problematic all winter.” Invest in a super-hydrating lip balm and apply frequently. “The higher the SPF, the better,” stresses Liza. Try Maybelline Baby Lips in Smoothing Cherry, $3.95; Carmex Lip Balm Click Sticks with SPF 15, $5.99.

06 dry eyes DEADLY SIN

The fragile skin around the eyes comes under greater fire from winter’s icy winds and chilly temperatures and can quickly become dry. If left untreated, this will lead to thinning and flaking and take on a crepey appearance. Crow’s feet may also worsen if not treated properly. Other causes for dry skin around the eyes include excessive washing in hot water or an allergic reaction to cosmetics and make-up ingredients.

WHAT TO DO ABOUT IT Don’t simply swap to a richer eye cream or moisturiser thinking it will ward off crow’s feet. “The skin here is finer than on the rest of the face and it can’t absorb heavy, oil-based formulas, and this can lead to millia, the little white bumps that sit under the skin,” says Liza. Use a small amount of a light cream that absorbs quickly, and don’t apply it close to lashes. Try Elizabeth Arden Prevage Eye Ultra Protection Anti-Aging Moisturizer SPF 15, $150 (all prices RRP); Estée Lauder Re-Nutriv Ultimate Lift Age Correcting Eye Creme, $200; Elizabeth Arden Prevage Eye Advanced Anti-Aging Serum, $65; DermaQuest Revive Eye Firm, $80; Skin Physics Cellugen Eye Revive, $49; Olay Pro-X Eye Restoration Complex, $74.99. THOU SHALT BLOCK THE SUN

The UV rays in winter sun are every bit as damaging as in summer, and in fact can often cause more damage, because lower temperatures make us less conscientious with sun block. 07 dry, scaly skin on body DEADLY SIN

Legs, elbows, knees and heels can suffer in winter when humidity is low, as can shins and upper arms, which may experience dry, small bumps. Being wrapped up in woollen and synthetic fabrics might also lead to irritation. Dry skin often produces itching, and repeated scratching can make the area rough and thickened. This can then crack or cause dermatitis when the skin becomes red and inflamed.

WHAT TO DO ABOUT IT Keep skin smooth with daily dry body brushing, which will not only slough away dead cells, it will also boost lymphatic drainage, helping to keep mottled, dimpled skin at bay. Or exfoliate with a body scrub two or three times a week, being careful not to overdo it, or you may ultimately make dryness worse by stripping the skin. Similarly, too much hot, soapy water – a “treat” we can indulge in too much during the cold weather – depletes our natural oils. Once you’re out of the shower, take time to massage a beautifully hydrating body cream all over. Try Vaseline Deep Conditioning Body Lotion with Cocoa Butter & Vitamin E, $10.99; Palmer’s Cocoa Body Butter, $10.99; A’kin Lavender & Geranium Body Moist, $22.95.

FACE THE FUTURE CONTINUED Full Facelift |

S U R G I C A L

Also known as the traditional lift, this is major surgery that separates the skin, pulling it back and making it look tighter. On the day of surgery, incision lines are marked, as is the “vector” – or direction – in which the deeper tissue needs to be lifted. The patient is anaesthetised (general or twilight) and the face is infiltrated with a large volume of local anaesthetic and adrenalin to reduce bleeding. The skin is lifted and the deeper (or SMAS) fibro-fatty muscular layer is shifted to the right vector and suspended. The excess skin is then trimmed and the wound is closed meticulously to ensure minimal tension or distortion to the ear. Some structural fat graft may also be added to the cheeks to further enhance results.

WHO IT’S FOR People who have severe laxity and skin excess in the face and neck (the latter known as the “turkey gobbler”). The aim is a natural, refreshed look, with a fuller cheek, defined jawline and neck without the over-pulled “wind tunnel” look. Scars will be well hidden when fully healed, so you can wear your hair however you like.

FULL FACELIFT RECUPERATION Downtime is usually two weeks, with sutures out after a week and no strenuous exercise for four. Pain can normally be managed with Panadol or Panadeine. Optimal results will be seen in three-plus months. HOW LONG IT LASTS This varies among patients, but generally from five to 10 years. COST The ballpark is from $18,000 up to $30,000, depending on the surgeon and the extent of treatment. Claimable only if it can be proven the facial droop is due to disease or trauma.

S-Lift or Mini-Facelift

Mid-Facelift

Lower Facelift

The S-lift gets its name from the smaller (than the traditional facelift’s) S-shaped incisions made in front of the ear. Through this incision, the surgeon can lift skin and underlying tissues. Using either general anaesthetic or twilight anaesthesia, an incision is made in the creases in front of the ear and curled behind the ear lobe. The skin of the cheek is elevated for whatever distance is necessary to move the fibrous/muscular layer beneath that gives the strength to the lift. The skin is trimmed and closed. Other elements may be incorporated at the same time, such as liposuction to the neck or tightening neck bands.

As we age, the tissues of the cheek and middle part of the face, which give a youthful healthy appearance when we are young, descend “southward”. The mid-facelift procedure repositions the mid-face fat-pad to a more youthful plane. Usually performed under general or twilight anaesthetic, it lifts the cheek fat back up onto the cheekbones, raises the outer eyebrows and lifts the corners of the mouth. Incisions are hidden in the temple hair. It may often be performed in conjunction with a necklift or browlift.

Rejuvenates the bottom third of the face by tightening the face’s underlying structures that slacken with age, forming jowls and a fleshy neck. It’s performed under general anaesthetic, with the surgeon working through small or hidden incisions and carefully separating the skin from the tissue below. Excess fat is removed and facial muscles are tightened and restored to a more youthful position. Skin is smoothed before the excess is trimmed away. An elasticised face garment needs to be worn for at least two weeks after surgery.

SURGICAL

WHO IT’S FOR It’s especially good for people aged 45 to 55 with early to moderate gravitational change who want to lift the lower face and fix jowling, but who don’t need the full facelift.

S-LIFT OR MINI-FACELIFT WHAT TO EXPECT You might look 10 years younger, but the results are individual. RECUPERATION Said not to be a particularly painful operation (overnight stay in hospital optional) but there is discomfort, bruising and the face feels tight. Expect to take 10 to 14 days off work, although some people are back to their normal routine after one week. COST Around $6000 to $10,000. Not claimable.

SURGICAL

WHO IT’S FOR People who have descent of the midface fat pad, rather than atrophy or wasting of the whole face. There is no age limit, but the signs of ageing usually start occurring in the 30s and progress from there. It works well in conjunction with non-surgical treatments such as Botox, Dysport or fillers, and also (as with other facelifts) hyperbaric oxygen therapy. This raises the levels of oxygen in the blood plasma to assist healing of bruising and scarring.

MID-FACELIFT WHAT TO EXPECT Cheeks may look higher and plumper, especially if implants have been inserted. There may be bruising and swelling, but this subsides after one or two weeks. HOW LONG IT LASTS Optimal results are expected within three months and last five to 10 years. COST Around $7000 to $10,000. Not claimable.

SURGICAL

WHO IT’S FOR Men and women whose face and/ or neck has begun to sag, but whose skin still has some elasticity, and whose bone structure is strong and well defined. Most patients are in their 40s to 60s, but these lifts can also be successfully performed on people in their 70s.

LOWER FACELIFT WHAT TO EXPECT A more defined jaw-line, reduction of jowls, more firmness and youthfulness in lower half of the face. Overnight hospital stay is optional. Downtime is about two weeks, with minimal activity for five to seven days. HOW LONG IT LASTS Expect the effects to last seven to 12 years, with an average of seven years. COST Around $6000 to $7000. Not claimable. MORE ›

As we get older, our faces lose fat and hence youthful volume, especially in the mid-face and under-eye areas. Using more viscous, new-generation hyaluronic acid-based injectable dermal fillers like Fortélis and Juvéderm Voluma, higher and fuller cheekbone structure can be restored for 12 to 18 months. Aquamid, which lasts about seven years, could be used when both the doctor and patient have seen results with shorter-lasting fillers and are confident of the outcome. These products are generally used for deeper placement in the skin to create a contouring effect, rather than filling superficial lines, wrinkles and depressions. They restore structure to sagging skin and so give a rejuvenating boost to the whole face. As hyaluronic acid (which also naturally occurs in the body) is a water “magnet”, the fillers also help promote the skin’s overall hydration. However, if a practitioner LIQUID LIFT isn’t experienced, SIDE EFFECTS product could be placed Redness and swelling may last too superficially in the a day or two; bruising (if it face, causing lumpiness. occurs), a week or more.

WHAT’S INVOLVED Local or topical anaesthesia is applied and dermal filler is injected into the appropriate zones and moulded into position. This typically takes around 30 minutes, with no downtime.

HOW OFTEN Typically one to three sessions around a month apart are required for optimal results. HOW MUCH 1ml of filler is around $600, but 2ml to 4ml of product are used overall, so up to $2000.

Promoitalia Revitalising Threads |

O N- SURGICA L

This radiofrequency (RF) device provides both “unipolar” and “bipolar” energy to non-invasively treat wrinkles and tighten skin on the face and neck. Unipolar RF energy penetrates up to 20mm into the skin, while bipolar is more superficial, penetrating 2mm to 4mm, gradually heating through the dermis, including the deep dermis. As this is electromagnetic radiation, not current, it doesn’t require a grounding pad like other RF systems and so is not painful. Temperature of the tissue increases with depth of energy penetration due to superficial cooling of the epidermis through the handpiece. The connective tissue structure is stimulated through collagen contracture and formation of new collagen.

WHAT’S INVOLVED While there is some immediate contraction, results are dependent on the “trauma response” of tissue and so are typically seen after the third treatment. The skin will continue to improve and optimal results may not be evident for three months. There may be some temporary redness but there is very low risk (none documented) of blisters or burns.

NON-SURGICAL

Promoitalia Revitalising Threads are suited to patients who want to preserve the natural contours of the face while refreshing a tired look. They have small, directional soft barbs that are able to “fix” onto the soft tissues of the skin to refresh sagging cheeks, tired jawlines, necks and brows. The threads eventually dissolve after nine to 12 months, with new collagen having formed around them, improving skin tone, colour and texture. They’re suited to people with mild to moderate facial droopiness or excess skin on the mid-face, jowls and neck and for brow elevation. Angelina Jolie and Madonna are thought to have had thread-lift procedures. They can also be used to revitalise lip borders for a more youthful contour, rejuvenate the backs of hands and now even firm saggy upper arms. Check that your practitioner is properly trained.

ACCENT HOW LONG On average treatments take about 30 minutes each. There is no downtime and normal activities can be resumed immediately. HOW OFTEN Six to eight fortnightly treatments are recommended. HOW MUCH From $200 to $350 per session, depending on area treated.

WHAT’S INVOLVED Performed in little more than an hour under local anaesthetic in a doctor’s surgery, the cost of Promoitalia Revitalising Threads varies from $3000 to $6000, depending on the areas being treated and how many threads are used. The procedure lasts one or two hours, again depending on the areas treated. Normal activities can be resumed soon after the treatment and there is no scarring.

Patients receiving an Ultherapy treatment for a browlift, for example, report less hooding and laxity of the eyelid skin and a more “open” look around the eyes, thus a more refreshed look overall. HOW OFTEN Skin improves markedly within 60 to 90 days, but clients should notice a slight tightening and firming effect immediately after being treated. Results can last 12 to 18 months. HOW MUCH From $1000 to $3500, depending on how much of the face or neck is treated.

NON-SURGICAL

Platelet Rich Plasma is blood plasma with concentrated platelets packed with growth factor proteins. These initiate and accelerate tissue repair and regeneration. For skin rejuvenation, a small amount of blood is taken from the patient and placed in a spinning centrifuge to separate the main components of the blood and produce PRP. Multiple injections of PRP into the skin causes a mild inflammation, which triggers healing. New cells develop with restored blood flow and tissue regeneration occurs. Regen PRP (as it is known in Australia) can be used on many areas of the face and body, including around the eyes, the cheeks and mid-face, the neck, along the jawline, even the backs of the hands and the décolletage, and other areas of the body requiring rejuvenation, including post-pregnancy tummies and arms, are being trialled.

WHAT’S INVOLVED Topical anaesthetic cream is generously applied and PRP is injected into multiple sites. It can be used on many areas of the face and body, and improvement accumulates over a period of weeks. Regen PRP is charged by the vial. It costs from $500 for one vial (usually used to treat a small area such as the eyes and lips) to around $1400 for four (for the entire face and neck). MORE ›

If the lifting and line-smoothing effects of muscle-relaxing injectables such as Botox and Dysport are no longer enough, you might consider a surgical browlift. This removes excess fat and skin, as well as tightening muscles in the brow area. It can correct sagging brows and eyelids or deep furrows between the eyes. Horizontal lines are much improved and even vertical lines may be eliminated, but skin texture will remain the same and may benefit from other, non-surgical rejuvenation procedures such as lasers or peels. A browlift restores a more youthful, refreshed look to the face and can often make a person appear to be a decade younger than their actual age.

THE DETAILS Only a qualified surgeon can say if you’re suited to and would benefit from a browlift, but try looking into a mirror and placing the palms of your hands at the outer edges of your eyes, above your eyebrows. It will only take up to 6 weeks before visible signs of regeneration are seen & the process will continue over the following months, providing you with a gradual & long lasting enhancement. Redefine naturally over time.

Dermaroller™ - Scar Reduction Therapy procedure works by releasing old fibrotic scar tissue deep within the skin. Scar tissue is what creates visible scars on the skin’s surface. The treatment will repair the skin naturally, working below the surface in the dermis. This takes time as new collagen & elastin fibres are formed, softening scars, generating new skin cells & enhancing blood supply. Results can be seen within 6 weeks, and a total of 70-80% improvement of scars.

Droopy or baggy eyes can make you look old and tired and even interfere with vision. Blepharoplasty removes excess skin, muscle or fat from the upper and/or lower lids. You’ll look younger and more rested, but it won’t remove crow’s feet, eliminate dark circles or lift sagging eyebrows.

Lines and wrinkles around, between and under the eyes can be smoothed and ageing hollows plumped with hyaluronic acid dermal fillers of varying consistencies. Products like Esthélis and Juvéderm are used to reduce wrinkling, while the more viscous Juvéderm Voluma and Fortélis would be used at a deeper level for contouring.

Advances mean the delicate skin on the upper and lower eyelids can now be targeted during full-face treatments with fractional laser systems such as Fraxel Re:Store Dual. Crepiness, lines and wrinkles can be addressed and the skin given a much more even, clearer, brighter appearance.

SURGICAL

THE DETAILS The upper lid incision/scar lies in the natural crease. Fat pads are removed through incisions inside the lower lid; muscle and loose skin under the eyes via incisions along the lower lashline.

NON-SURGICAL

THE DETAILS The depth at which the filler is injected depends on whether superficial lines are being addressed or the tear trough and under-eye area need “revolumising”.

NON-SURGICAL

THE DETAILS The treatment helps tighten the more superficial layers of the skin. It causes immediate visible improvements and boosts formation of new collagen and elastin over the ensuing months.

HOW LONG Surgery takes around two hours, in hospital or day surgery, under general anaesthetic or twilight sedation.

HOW LONG Treatment takes from 15 to 60 minutes. Wrinkle fillers last six to nine months, while contouring lasts 12 to 18 months.

RECOVERY Swelling and bruising resolve over one or two weeks. Scars fade over six months to a barely visible white line.

RECOVERY There will likely be some temporary redness and swelling; bruising may occur and last for three to 10 days.

HOW LONG A full-face treatment lasts 30 to 45 minutes. A numbing cream is applied and metal shields may be used to protect the eyeballs.

HOW MUCH Upper lids cost $4000 to $5000, lower lids $4000 to $6000.

HOW MUCH Prices vary, so go to www.youngeryou. com.au/fillercost for a guide.

Botox |

NON-SURGICAL

The brand name of a toxin produced by the bacterium Clostridium botulinum, Botox blocks signals from the nerves to the muscles. This means the injected muscle can no longer contract, allowing wrinkles to relax and soften. It can treat crow’s feet and creases under the eyes. Placed strategically by an experienced injector in vertical “glabellar” lines between the eyebrows and at points around the brow, Botox can also make the eye area look more “open” and youthful. It’s injected with a fine needle into specific muscles with only minor discomfort, with the procedure taking only a few minutes. There may be temporary redness and swelling and sometimes bruising for a few days. Effects are seen at three to seven days and last on average three to six months, depending on the patient and the area treated.

RECOVERY Expect swelling for 24 to 48 hours, with redness persisting for several days. HOW MUCH As a ballpark figure, full-face Fraxel Re:Store Dual costs $1200 to $1500.

THE DETAILS The cost of Botox treatment depends on the practitioner, the area being treated and the amount of product needed for each patient. As a rough guide only, a woman having her crow’s feet treated can expect to pay anywhere from $350 to $400. The price per unit of Botox generally decreases as the number of units required become greater.

“Droopy and baggy eyes can make you look old and tired” Dysport |

NON-SURGICAL

The brand name of Botulinum toxin type A, Dysport is injected into a muscle and breaks down a protein called SNAP-25 that’s necessary for contraction, relaxing the wrinkles that normally appear when the muscle moves. This means that undesirable lines between the brows, at the corners of and under the eyes can be smoothed to restore a more youthful, less stressed appearance. Results begin to appear within a day after Dysport is injected, day three being the median. Effective treatment requires three to five very small injections that are practically painless. The initial effects generally last from two to six months, and many patients find that after a few treatments the effects last longer, meaning follow-up treatments are likely to be required less frequently.

THE DETAILS The cost of Dysport treatment depends on the practitioner, the area being treated and the amount of product needed for each patient. Necklift

Neck Tightening

Lips

You may opt for surgery if you have weakened or loose neck muscles, sagging skin as the result of the ageing process or weight loss, or localised deposits of fat in the front part of the neck (the “double chin”). Weak muscles contribute to the “turkey gobbler” or appearance of neck bands. Necklifts are typically combined with other surgery such as S-lifts or browlifts, but can be performed alone as some people notice a hereditary chubby neck even from their late teens.

If surgery isn’t needed (or desired), non-invasive skin-tightening procedures such as Ultherapy can assist with firming moderately saggy skin. The full face and neck are generally treated but the neck can be done separately. A handpiece delivers “passes” of acoustic sound energy (sound waves) section by section across the treatment area. An immediate tightening effect is noticeable but optimal results, including a fresher, younger complexion overall, are achieved over three to six months, lasting about 12 to 18 months.

More youthful lip contours are easily achieved with injectable dermal fillers like Juvéderm and Esthélis. They can add definition to a softened lip border and enhance the cupid’s bow. Subtle volume is also often added to the lip pad. Lip augmentation helps to stop lipstick bleeding that occurs with the onset of smoker’s lines. Using fillers to soften nasolabial folds (the lines from the nose to mouth) and marionette lines (the grooves at corners of the mouth down towards the chin) will rejuvenate the whole mouth area.

SURGICAL

THE DETAILS A necklift is actually a set of procedures: cervicoplasty to remove excess skin and platysmaplasty to remove or alter neck muscles. A necklift is done with a small incision behind the ears and another under the chin, both places where the scars will usually be inconspicuous.

NON-SURGICAL

THE DETAILS This energy penetrates deep into the skin where cells are formed. It triggers the body’s natural healing process, resulting in strengthened collagen fibres, which work to safely firm and lift skin tissue.

NON-SURGICAL

THE DETAILS The top lip usually needs more enhancing than the bottom, but experienced injectors are conscious nowadays of maintaining the most aesthetically pleasing upper/lower lip ratio. The days of the notorious “trout pout” are over – at least, there’s no excuse for them now!

NECK TIGHTENING

HOW IT FEELS There isn’t a great deal of discomfort associated with this procedure, but there can be a sensation of tightness for a while where the muscles have been pulled together.

HOW IT FEELS Numbing cream is used but there may still be some discomfort deep in the skin, especially in bony areas like the jawline and temples. Laughing gas may also be offered. Your skin is likely to look a bit pink for a few hours.

HOW IT FEELS Numbing cream and often local anaesthetic are used, dulling most pain. There may be some redness and swelling, which subsides within 24 hours; any bruising fades within a week.

HOW LONG Surgery may last one to three hours or longer if you’re combining it with other procedures. Swelling and bruising may last seven to 10 days. HOW MUCH As a ballpark guide only, expect to pay from $10,000 to 15,000.

HOW LONG After cream is worn for half an hour, the treatment takes 30 to 40 minutes. HOW MUCH Treatment on the full face costs about $3500, or you can have just the neck treated for around $1500.

HOW LONG Each procedure takes up to 30 minutes. Effects last around six to nine months. HOW MUCH Procedures can be performed alone or together and cost upwards of $600 or $800 per procedure, depending on the practitioner and amount of filler used.

Muscle-relaxant injectables Botox and Dysport can be used in a variety of ways on the face to recontour the lower face. Applications include: Creating a beautifully upturned mouth by relaxing the depressor angularis oris muscle (which keeps the mouth downturned). “Smile” muscles are then better able to do their job. Even when static, the corners of the mouth are slightly upturned, creating a happier, softer expression at all times. THE FUTURE OF THE INJECTABLE NOSE JOB “The evolution of the instant nose fix is due to the availability of longer-lasting fillers that are safe to use,” explains Dr Gabrielle Caswell, president of the Cosmetic Physicians Society of Australasia. “It allows the patient to decide if their cosmetic blemish is worth having a rhinoplasty or if they’d like to continue with an artful injection of filler. “The beauty of this treatment is that it can be done in your lunch hour. It may have a little bruising, but there is none of the pain, discomfort or extended recovery associated with surgery. “Rhinoplasty, on the other hand, is a permanent option, and for some deformities, particularly those that interfere with breathing, this surgical procedure may be the only option.”

Which injectables are used in a non-surgical nose job? The injectable fillers used to build up the nose can be temporary hyaluronic acid fillers, such as Juvéderm, Restylane, Fortélis or Esthélis, or Radiesse, which is a filler comprised of calcium-based microspheres suspended in a water-based gel. These last between 12 months to two years. Because the nose is relatively static in comparison to other areas of the face where fillers are used, they tend to last longer in this area. A longer-lasting alternative is the filler Aquamid, which is a sterile hydrogel consisting of 97.5 per cent water and 2.5 per cent cross-linked polyacrylamide. It will last up to 10 years or more. A surgeon will decide on the filler to use on an individual basis, taking into account skin type and the irregularity that needs to be addressed. During the first consultation, a saline solution is injected into the nose to allow for a preview of how the nose will look. Pictures are taken and the patient can then use the time until the next appointment (usually one week) to decide if that’s the look they want. Then, on the second appointment, the appropriate filler will be injected. This takes approximately one hour, and the patient can return to work immediately afterwards. There is no downtime, although some patients may experience slight swelling and tenderness.

OVER-PROMINENT TIP SOLUTIONS How an injectable fixes it ”Filler can be injected directly into the area to build it up, reducing the curve angle. This would lessen the visual impact of the tip,” says Dr Caswell. How surgery fixes it “Surgery can reduce the size of the nasal tip and bring it into balance with the rest of the nose,” explains Dr Paul Gerarchi, from Sydney’s The Face Institute.

What it looks like “This irregularity typically causes the nose to look out of proportion with the rest of the features on the face, and it tends to make the nose look pointed,” says Dr Caswell. “If you were to hold a pencil from the tip of the nose and down to the chin, the angle of the pencil would be quite recessed.” Amy Adams, star of The Fighter, has a nose with just such a feature, which is especially obvious in side-profile shots.

Injectables are ideal for minor contour irregularities, whether occurring naturally or caused by trauma or prior surgical procedures.

KATIE HOLMES THE SURGEON SAYS “Katie’s nose seems to come a long way down from her forehead before it has a sudden low take-off,” says Dr Marcells. “She’s an attractive girl, but I think her nose could be better. The bridge could be augmented to improve the flow. I’d do this by building it up just a millimetre or two with filler, an implant or by using the patient’s own tissue.”

How an injectable fixes it “About 2ml of filler would be placed centrally and built up to provide a visual correction of the bridge and reduce the gap between the eyes, giving a more balanced look,” says Dr Caswell. How surgery fixes it “I’d build it up by using the patient’s own tissue, such as cartilage,” says Dr Marcells. “Or I’d use an implant like Gore-Tex, which allows tissue to form around it.”

What I dislike is that it looks a little pinched and narrow to the middle third of the nose. She may have had a bump removed, at which time too much cartilage was removed. I would remedy this surgically by using special cartilage grafts to build up the middle.”

NEW-GENERATION FILLERS CAN CORRECT NASAL IRREGULARITIES DURING YOUR LUNCH HOUR > THE PROCEDURE The use of injectable fillers is becoming increasingly common in cases of deficits in the nose, saddle-nose deformity after trauma (which looks like a boxer’s nose), for divots or deviations in the nose or for a bent or crooked nose. Fillers are also useful for correcting post-rhinoplasty defects and after skin cancer excisions. They’re a welcome alternative for many patients, because they’re generally lunch-hour procedures performed under local anaesthetic, and patients can resume normal activities immediately. Saline is first injected into the area to allow the patient a sneak preview of how the implant will look. This remains in the nose for about 45 minutes, during which time photographs are taken and the patient can discuss the desired outcome with their surgeon. The patient then returns the following week to have the filler injected.

> INJECTABLE FACTS COST Expect to pay $600 to $2000, depending on the type of filler and the amount used. LENGTH OF PROCEDURE In most cases, the procedure takes less than one hour. IN/OUTPATIENT Outpatient. SARAH JESSICA PARKER

BUMPY NOSE SOLUTIONS How an injectable fixes it “This is normally done as a ‘spot’ treatment, with the aim to visually even up the nose so that the side and front profile are harmonised,” explains Dr Caswell. “I would use a softer filler, with the aim of smoothing the contours and ensuring that the apparent bumps blend with the overall contours of the nose.” How surgery fixes it “In a surgical rhinoplasty procedure, we would remove some of the patient’s bone and cartilage to create a better flow to the bridge,” says Dr Marcells. “The procedure often involves work on the tip of the nose to modify it at the same time.”

ANAESTHESIA Topical or local. RECOVERY TIME Little or none. There may be slight swelling and tenderness. RISKS No known risks are associated with this procedure. DURATION OF RESULTS Because the nose is static in comparison to the rest of the face, fillers last longer in this area. Temporary fillers last between 12 and 24 months, while longer-lasting fillers will last 10 years or more, depending on the metabolic rate of the individual. NOTE Plastic and cosmetic surgeons are seeing an increase in patients who are referred for problems after having nasal filler injections, so it is imperative to seek out a dermatologist or surgeon who has extensive experience of using injectables in the nose.

JENNIFER ANISTON THE SURGEON SAYS “Jennifer Aniston has a well-balanced, pretty nose that suits her particular facial shape,” says Dr Warwick Nettle, from Silkwood Medical in Sydney. “A pert, upturned, fine nose would not suit her face.”

CROOKED NOSE SOLUTION How surgery fixes it “Fixing crooked noses 100 per cent is actually quite difficult, as the bone can be repositioned relatively readily, but the cartilage (like ear cartilage) wants to rebound to the same position as before,” explains Dr Nettle. “This requires a series of small sutures and manoeuvres under the skin to straighten the nose, so it’s more common to get it 90 to 95 per cent straight.”

During rhinoplasty your surgeon may make an internal incision in the bones and move them with a “controlled break” in order to alter the appearance of your nose. This woman had the bump on the bridge of her nose reduced and her breathing obstruction removed. Performed by Dr Warwick Nettle.

How to gain balanced features “A balanced nose on a balanced face is the epitome of facial beauty,” says Dr Nettle. “The bridge and the tip need to be related in the golden ratio of tip projection to nose length of 1:1.618. This is a recurring pattern in nature. “Harmony is enhanced by not making the nose too big or too small for the face, by not being too broad or too narrow. For instance, in thin-faced, thin-skinned patients it’s good to have a fine rather than broad nose and vice versa for the broader-faced, thicker-skinned patient.” Dr Marcells adds, “The angle of the nose tip can vary – shorter people look better with a slightly upturned nose, while taller people look better with a less upturned nose. This has to do with the angle we look at them from – on shorter people, we look at them from above, and if their nose is downturned, we won’t see their nostrils. Similarly, we look up at taller people, so an upturned nose means we’re looking straight into their nostrils.”

What it looks like “A crooked or deviated nose may occur naturally or it may result from trauma to the nose,” says Dr Nettle. “It could have stemmed from a basketball hit at high school, falling off your bike or the accidental swipe from a younger brother. When is a nose job medically necessary? In some cases of nasal irregularity, there will be no option but to have surgery. “If you want to improve the function of your nose, surgery is required,” says Dr Gerarchi. “And if you wish to improve the shape and definition of the nose, particularly the tip of the nose, you need a rhinoplasty procedure. “Likewise, fillers can’t reduce the size of the nose or make it narrower, and surgery is also your best option for things like making significant changes to the angle of the tip, or correcting a droopy tip.”

full tip

What it looks like “A full tip is one that has poorly defined features,” explains Dr Gerarchi. “It becomes difficult to identify the nasal tip curves and light reflexes that we associate with a nose being attractive. The ‘full’ nasal tip blends into the nose above and below, lacking its own ‘space’, giving it a rounded or bulbous appearance. Sometimes the end of such a nose is described as a bit ‘blobby’.”

SCARLETT JOHANSSON

FULL TIP SOLUTION How surgery fixes it “This involves making the tip much more defined and ensuring there are distinct points that highlight where the nasal tip is,” says Dr Gerarchi. “This is achieved by refining and reshaping the tip cartilages and also possibly thinning the skin overlying the tip if there’s a lot of fatty tissue there.”

BASICS

RHINOPLASTY OFFERS PERMANENT IMPROVEMENT FOR NASAL FLAWS > THE PROCEDURE Surgery takes place after careful assessment to determine the correct size and shape of nose for the individual and a full functional assessment. A general anaesthetic is administered and the patient must spend the night in hospital. Most rhinoplasties today are performed using an open-structure procedure, which affords better access to the tip of the nose. This begins with a small incision across the skin between the nostrils and then involves reshaping the bone and cartilage skeleton of the nose. A dorsal hump can be removed, a nose straightened and a tip refined. Internal work then addresses any functional issues. There may be a tiny scar, but this fades as the nose heals.

> THE RECOVERY Today’s rhinoplasties are accompanied by much less pain than in the past, as the anaesthetic used blocks the pain for 24 hours or more. By the time this wears off, the greater intensity of pain will have subsided, and any further pain can be managed with over-the-counter medication. Initially, there will be swelling and bruising, with most patients sporting black eyes. A cast is worn over the nose and there may be a small stitch under the nose. The nose will feel blocked, but this usually subsides after a week. Patients can expect to return to work within two weeks, and the best results can be seen six to 12 months after surgery.

> SURGICAL FACTS COST Between $10,000 and $12,000 including surgical, hospital theatre and anaesthetic costs. If functional work is involved there may be eligibility for a Medicare rebate. LENGTH OF SURGERY Two to three hours. IN/OUTPATIENT Usually outpatient. ANAESTHESIA General or local with sedation. RECOVERY TIME Usually two weeks. TREATMENT FREQUENCY Usually once.

THERE ARE SOME BENEFITS TO WINTER! MAXIMISE YOUR INDOOR TIME DURING THE COOLER MONTHS BY ADDRESSING YOUR SUN DAMAGE AND PIGMENTATION. BY JESSICA MACDONALD

SUN UNDONE There’s no better time than winter to tackle the pigmentation, sunspots, UV damage and general lacklustre skin that come about from overexposure to the elements.

THE SIGNS OF SUN DAMAGE Sun exposure and the resulting sun damage manifest in many ways, which often dictate the type of treatment or procedure required to address the issue.

pigmentation Solar pigmentation is a direct result of sun exposure.

Light treatments such as LimeLight (an advanced form of IPL) can be very effective in minimising the appearance of veins on the skin.

lines and wrinkles The sun damages collagen fibres and causes the accumulation of abnormal elastin, which leads to lines and wrinkles, as well as sagging skin. Laser treatments can soften the appearance of fine lines and wrinkles and restore some of the skin’s firmness. Teamed with injectable fillers, they can help make skin look more youthful.

dry and lacklustre skin Sun exposure can quickly sap skin of its moisture, leaving it dull, uneven and drab, and this can be more ageing than lines and wrinkles. Drink plenty of water, skip tea, coffee and alcohol, and use good skincare products. A peel can quickly bring skin back to life and restore its moisture balance. MORE ›

THE

ISSUES

LASER TREATMENTS TYPICALLY DELIVER THE MOST DRAMATIC AND EFFECTIVE RESULTS IN REVERSING SUN DAMAGE. MOST COME WITH SOME DOWNTIME, BUT THE RESULTS ARE WELL WORTH IT

Cutera Pearl Fusion This treatment combines both Pearl and Pearl Fractional lasers, for full facial epidermal skin renewal. The Pearl laser treats the skin’s top layer with pulses of light to remove sun damage and brown spots, while the Pearl Fractional laser uses heat to create tiny holes in the skin to remove sundamaged tissue and stimulate new skin and fresh collagen. The treatment is ideal for people with light to medium skin tones with mild to moderate sun damage, and can address pigmentation on the face, neck, décolletage and hands. It also helps to firm the skin and minimise the appearance of lines and wrinkles. Immediately after treatment, a natural dressing forms on the skin to protect the treated area and prevent a wound or infection. Your skin will appear swollen and “sunburnt” for five to seven days. Ointment must be applied to the skin for four to five days. On day three or four the natural dressing peels off, revealing softer, betterhydrated and firmer skin with reduced pigmentation. Many patients see an improvement after one week, but best results are noticed after three to six months. Typically, only one treatment is required for the face, neck and décolletage, but the hands usually require two treatments as sun damage tends to be greater in this area due to them being very exposed. Expect to pay $2500 to $4500 for the whole face and around $1000 for both hands. BEFORE

ADDRESSING

Pixel RF This is a radio-frequency technology that helps reverse the appearance of ageing, sun damage and acne scarring. It works by affecting only a fraction of the skin at a time with thousands of microscopic RF pixel spots – a bit like Photoshop editing software for the face! It can be used to improve pigmented lesions such as age spots, sun spots and melasma (skin discolouration). Pixel RF is “colour blind”, so sun exposure or skin type are not such important issues as with other technologies – although of course it’s best not to tempt fate by overdoing UV exposure or neglecting sun protection! After treatment, skin generally feels softer, looks smoother and tighter, and pores are usually smaller. Skin may look pink for a couple of days before results begin to show, with further improvement over the following weeks. Two to four treatments are recommended, at fourweekly intervals. Prices start at $500 per treatment, depending on the area treated.

Fraxel Re:Store Dual This new-generation fractional laser is designed specifically for pigmentation and sun damage; it’s good for the face, neck, décolletage and hands. It boasts two unique heads – a thulium laser and an erbium laser. Practitioners can swap between the two, depending on individual skin issues. The erbium laser is deeper and causes formation of collagen and elastin, addressing wrinkles and sagging skin. Age spots, pigmentation and surface veins are then blasted away with a thulium treatment. The procedure requires multiple laser “passes” over the treatment area. A prickling sensation is felt initially, then slight heat. There will be one or two days of redness and mild swelling, followed by five to seven days of light peeling, after which skin will appear softer and brighter. Two or more treatments may be required for the best results.

SOLUTIONS

LESS INVASIVE PROCEDURES WITH MINIMAL OR NO DOWNTIME CAN ALSO HELP ADDRESS SUN DAMAGE AND REJUVENATE AND FRESHEN UP YOUR TIRED-LOOKING SKIN

Omnilux Plus Omnilux Plus is a gentle but powerful light therapy treatment that directs infrared lightemitting diodes (LEDs) to the skin to rejuvenate, firm and tighten, smooth the texture and restore radiance. It’s perfect for skin showing signs of ageing due to sun exposure. Typically, the light is included as part of a facial treatment, but it can be administered as a stand-alone treatment. While skin appears refreshed after just one treatment, a series of up to eight treatments over six to eight weeks gives best results. Expect to pay between $90 and $120 per treatment.

Microhydrabrasion This is microdermabrasion with a soothing twist. “A dry diamond tip with vacuum suctions blasts away dead skin cells, giving an intensive exfoliation,” explains Douglas Pereira, clinical skin therapist at Pure Aesthetics and Macquarie Cosmetic & Plastic Surgery, both in Sydney. “A second diamond tip uses pressurised water to clean the pores and remove built-up impurities.” Aloe is flushed onto the skin simultaneously, to give a soothing, cooling sensation and reduce any irritation. Expect to pay $150 to $165 for one treatment, with treatment packages available.

LimeLight Photofacial LimeLight is a programmable IPL treatment that can be dialled up or down according to the issues being addressed. “Different programs are used, labelled A to C, “says Dr Gabrielle Caswell, president of the Cosmetic Physicians Society of Australasia. “Program A works on redness and blushing, B is for vascular issues and brown spots, dyschromia or skin yellowing due to sun damage, and C is for really freckly and pigmented faces that have little redness, and is also great for the backs for hands.” A beneficial side-effect is that collagen production is triggered. Patients can have one, more powerful treatment, which will involve some post-procedure swelling and downtime, or a series of gentler treatments. Expect to pay about $500 per treatment, depending on your practitioner. BEFORE

AFTER

The Herbal Aktiv Peel This relies on a solution of natural stimulants, exfoliants and regenerating and soothing herbs, which is massaged into the skin to trigger a peeling, resurfacing effect. The downtime is five days, with reddening and tightening of the skin, similar to sunburn, experienced until day three, when shedding occurs. On day five, a follow-up facial removes any flaking skin, revealing a fresher complexion. “We get phenomenal results,” says Dee Davies from the Clear Skin Clinic in Double Bay, Sydney. “Radiance is restored, hydration improved and that dull yellowing of sun-damaged skin is erased.” Expect to pay $699, which includes three at-home products and a post-treatment.

Dermaroller Professional Treatment Looking like a mini paint roller, the Dermaroller uses ultra-fine needles to stimulate collagen production and create micro-channels in the skin through which anti-ageing ingredients can be better absorbed. After application of numbing cream, the roller is passed over the face, with each portion needled about 15 to 20 times. Afterwards you may be given a mask, or some clinics offer a soothing 20-minute Omnilux treatment. The Dermaroller suits all skin types, even dark skins that can’t have laser work. It promises firmer, plumper skin, reduced lines and wrinkles, a smoother and more refined complexion, and minimised pore size and pigmentation. There is no downtime, but you may look slightly sunburnt for 24 hours. Three treatments are recommended, six to eight weeks apart, and you can expect to pay $600 to $800 per treatment. MORE › MORE›

Thankfully, there is a plethora of cosmetic options – both surgical and non-surgical – that can enhance and uplift breasts, whittle away a waistline that won’t shift, or restore the shapely thighs and buttocks of your youth, safely and effectively.

hV er ERS internal factors Factors that can dramatically change your body’s appearance include weight gain or loss and pregnancy, which can cause breasts to swell and sag, the stomach muscles to separate internally and the buttocks and thighs to retain fat.

implant placement > BELOW THE MUSCLE

Also known as sub-glandular breast augmentation, the implant is placed just below the breast tissue or under the pectoralis muscle in the chest. This method is often used for women who are quite thin and lacking in breast tissue, because it seems to soften the edge of the implant and make it look more natural and less obvious. > ABOVE THE MUSCLE

This method of placement is better for fleshier or curvier patients. As the name suggests, the surgeon places the implant on top of the muscle, giving a pleasing shape and result.

incision placement > AROUND THE NIPPLE

Also known as a periareolar incision, this is when an incision is made halfway around the areola. It leaves a virtually undetectable scar but may sometimes interfere with breastfeeding, so it’s best for women who’ve finished having children. > ARMPIT

This is suited to women who are prone to keloid scarring (a condition in which scars are thick, firm and rubbery) as the scar is hidden away under the armpit. Surgeons use an endoscope, which allows them to see through the whole cavity, to guide the implant into place. > IN THE BREAST CREASE

Most commonly, the incision is made through the crease or fold underneath the breast. A small incision of around 3cm to 5cm is made and the implant is inserted. This is the quickest and easiest method for surgeons, and the scar remains hidden under the breast.

62

YY02_060-073_MOTHER.indd 62

11/03/11 4:36 PM

HOW IT WORKS

Anyone can choose to undergo breast augmentation, although the most common patients are young women whose breasts have never grown, or women who have breastfed their children and have consequently experienced sagging or “empty” breasts. Remember, the size and shape of the implant you have must be in proportion to your body for the best and most natural-looking results. A very thin woman with a natural size A cup would not realistically be able to go up to a D or E cup. The skin is very elastic and will stretch, so if the implants are too big, the skin will not be able to hold them up and they will appear to sag.

Breast augmentation is typically performed under general anaesthetic and is approximately a two-hour operation. Some surgeons will perform it using twilight sedation and local anaesthetic, which can reduce costs and sometimes recovery time, as you’re not affected by the general anaesthetic. The procedure, which is also referred to as ”augmentation mammoplasty”, involves making an incision and placing an implant, usually made from a cohesive silicone gel, into a pocket either behind the chest muscle or between the muscle and the breast itself, although variations and combinations of these can be performed to suit an individual’s anatomy (see opposite).

BREAST

AUGMENTATION

SURGERY IS THE ONLY WAY TO ENHANCE YOUR BREAST SIZE IN THE LONG TERM. THERE ARE MORE OPTIONS AVAILABLE THAN EVER BEFORE, AND NEW IMPLANT MATERIALS MEAN THAT A SAFE, NATURAL-LOOKING RESULT IS EASILY ACHIEVABLE. THE KEY IS NOT TO OVERDO IT. YOU MAY ENJOY THE LARGER, MORE OPERATED LOOK AT FIRST, BUT IT MIGHT NOT BE SO DESIRABLE AS THE YEARS GO BY

AFTER THE PROCEDURE Some bruising and tightness in the breast area is normal following augmentation, but this usually subsides within a few days. Any discomfort can be managed by taking oral painkillers. There will be some swelling, but this will go away within about six weeks, when you’ll begin to see the best results from your procedure. A supportive sports bra should be worn during this time. Patients will need to take two days or more off work and should avoid any exercise or activities that involve using the arms, such as swimming or lifting weights, for at least two months.

breast lift

BEFORE

THE IDEAL CANDIDATE

Typically women who are unconcerned about their breast size, but instead have issues with their placement and loss of firmness, are ideal candidates for a breast-lift procedure (which is also known as mastopexy). Their breasts are beginning to sag, or indeed have already sagged and are pointed downwards. Women whose breasts have enlarged and consequently sagged due to gravity, through weight gain or loss, or through pregnancy or hormonal changes associated with menopause, may be good candidates for the procedure, too. The ability to breastfeed is very unlikely to be affected by having had a breast lift.

HOW IT WORKS

A few incision techniques can be used, which your surgeon will determine. Surplus skin, tissue and fat are removed and the surgeon sometimes performs liposuction to remove excess fatty tissue. The nipple and areola are then lifted to a higher position and the skin and tissue are stitched into place.

AFTER THE PROCEDURE

Expect to take at least two weeks off work after a breast lift. There’ll be pain and discomfort, so it’s important to take things easy – even coughing can hurt. Lifting anything – even your kids – is off limits, as are bending down and driving. Avoid exercise for a month, and then ease into activity gently, avoiding strenuous activity for at least three months. Expect to see best results several months later, when swelling subsides and your breasts have settled into their new shape and position.

THE IDEAL CANDIDATE

Any woman with extremely large breasts that cause embarrassment, poor posture, neck and back pain or shoulder problems is an ideal candidate for breast-reduction surgery. Many women find that having the operation reduces pain in the shoulders, neck and back as well as discomfort during physical activity. In addition, it can greatly improve posture, appearance and self-esteem.

Procedure performed by Dr Benjamin Norris.

HOW IT WORKS Your surgeon will discuss your goals and desires, how having large breasts impacts on your lifestyle, and

how you would like them to look. A number of techniques can be used, each of which involves removing some skin and breast tissue, reshaping the remaining breast tissue and moving the nipple and areola. The surgery is performed under general anaesthetic and takes two to three hours. The results are long lasting and the breasts are unlikely to ever return to their original size.

A LIFT OR REDUCTION CAN HELP BRING BREASTS INTO PROPORTION WITH THE REST OF THE BODY, FOR A MORE BALANCED SHAPE THAT HAS BOTH PHYSICAL AND MENTAL BENEFITS

AFTER THE PROCEDURE After surgery, you should be up and about in a day or two, but plan on taking it easy for the first week. Swelling and bruising usually subside within one or two weeks. Many patients report tightness of the breasts after surgery, but little pain. Most stitches will be removed after five to nine days, and tape is applied to the incisions for six weeks. It’s important to wear a support bra 24 hours a day (unless showering) for the first five to six weeks to maintain shape, improve healing and ensure long-lasting results.

liposuction THE IDEAL CANDIDATE People who have tried to lose weight and have stubborn fatty deposits that won’t shift are suited to this procedure, as is anyone who has disproportionate collections of fat on the tummy. It has to be noted, however, that liposuction is not a weight-loss alternative, but rather a technique used to shape the body and create a sleeker contour. The patient should also have a good quality of skin, as it needs to be able to spring back into shape, without looking lax or sagging.

HOW IT WORKS

A liposuction procedure is performed under general anaesthetic, although some surgeons are using twilight sedation in a sterile environment, which allows the patient to move around for better access and results. It generally takes about two hours. Small incisions are made in the groin and around the belly button, and a metal cannula is inserted to remove fat.

AFTER THE PROCEDURE Patients should be prepared to take a few days off work and will need to wear compression garments for up to six weeks. There will be swelling, bruising and some discomfort that can be managed with oral pain relief. Strenuous exercise should be avoided for six weeks or more, although a gentle power walk is fine after seven to 10 days. Best results will become apparent about three months after treatment, although you’re likely to see a noticeable difference within days or weeks, as swelling subsides. It’s important to maintain a good diet and exercise program after your recovery.

tummy tuck

BEFORE

AFTER

BODY

THE IDEAL CANDIDATE

A tummy tuck, or abdominoplasty, is performed to remove excess skin and fat from the middle and lower abdomen. The best candidates are people concerned by loose skin and fat deposits that don’t improve after diet or exercise. The operation is particularly helpful for those with excess abdominal skin after childbirth or weight loss. Procedure performed by Dr Benjamin Norris.

HOW IT WORKS

The surgery involves an incision being made across the lower abdomen, extending out to the hips. Excess skin and fat are removed and upper abdominal skin is stretched down to the lower abdomen. A new opening is made for the navel, which is resited in its normal position. Abdominoplasty is performed under general anaesthetic and takes around three hours. Patients are usually required to spend two nights in hospital.

AFTER THE PROCEDURE

A compression garment is worn for six weeks after surgery. You’ll need to take two or three weeks off work, although some people require longer. Gentle exercise can be resumed after three to four weeks, but avoid vigorous exercise for six to eight weeks. It may take two or three months before you can return fully to normal activities.

accent Accent body contouring uses a radiofrequency (RF) device to give the abdomen a flatter and smoother profile, slim the hips and thighs and firm the upper arms without incisions, pain or downtime. A handheld device is used to apply deep dermal heating to the fat bundles to bring about new collagen formation and shrink fat cells. It also stimulates circulation, which helps drain trapped fatty deposits and toxins. Depending on the area, the treatment may take 15 to 45 minutes. Patients will see an immediate visible improvement, but results are cumulative over the following months. Generally, no more than five to seven treatments at two-week intervals are required. Prices range from $250 to $350 per treatment. MORE ›

coolsculpt THE IDEAL CANDIDATE

Ideal candidates for the CoolSculpt procedure are those who are within their goal weight, or perhaps are 5kg to 15kg overweight. This is not a weight-loss procedure, but rather it’s designed to help shift the stubborn fatty deposits that diet and exercise can’t quite seem to eliminate. Anyone seeking spot reduction for specific areas of fat but not considering surgical procedures could consider this procedure.

HOW IT WORKS

Dr Jeremy Cumpston Cosmetic physician and anti-ageing medicine specialist

CoolSculpt targets a particular area of fat such as muffin tops or love handles. The device is positioned on the area to be treated. The body fat is suctioned between two cooling plates and cooled to 7ºC for 60 minutes. This process (called Cryolipolysis) is a precisely controlled cooling method that eliminates fat cells without damaging neighbouring cells or tissue.

AFTER THE PROCEDURE You may feel some tenderness just after treatment, but this subsides in an hour. You may also experience tingling, redness and tenderness for up to a week. A small percentage of patients experience a small bruise in the area treated. In a treatment that lasts just a few hours, CoolSculpt can target and kill fat cells without harming surrounding tissue. Fat cells are frozen, they then crystalise and are eventually eliminated through the body’s natural processes over six to 12 weeks. The fat cells destroyed are gone for good, and the process is non-invasive and pain-free, with very mild discomfort throughout the procedure. liposuction

ionithermie AFTER

THE IDEAL CANDIDATE Anyone experiencing disproportionate deposits of fatty tissue in the buttocks and thighs is suited to liposuction. It may also be useful in minimising the appearance of cellulite and dimpled skin. A major area of concern is at the top of the buttocks, where excess tissue produces a squarish shape, rather than a feminine curve.

HOW IT WORKS THE IDEAL CANDIDATE

Ionithermie treatments are suited to anyone wishing to reduce the appearance of cellulite and improve skin and muscle tone, with the added benefits of slimming and detoxification. These treatments are useful for spot reduction in the thighs, abdomen, love handles, hips and upper arms. They can also be great for kick-starting a weight-loss program, or for people who’ve lost weight but seem to have stubborn fatty areas clinging on.

HOW IT WORKS

The spa-only treatment, which originated in France, works by using two kinds of electrical stimuli, accompanied with algae and thermal clay containing active ingredients derived from plant extracts. Without using any injections, Ionithermie penetrates deep into the cellular level, providing an effective total-body cellulite treatment.

AFTER THE PROCEDURE The Ionithermie treatment incurs no downtime, and results of up to a 20cm reduction in measurements have been reported, with a marked reduction in the appearance of cellulite. The benefits are easily maintained with daily body brushing and application of at-home products.

The 90-minute procedure is performed either under general anaesthetic or twilight sedation in a sterile environment in a doctor’s rooms. The latter allows the patient to move around and gives the surgeon greater access to the areas being treated. A fluid is injected to dissolve the fat, which is then sucked out through tubing connected to a cannula.

AFTER THE PROCEDURE Patients will need to take up to one week off work and will be required to wear compression garments day and night for a few weeks to assist with swelling and bruising. Any pain and discomfort can be managed with prescription or over-thecounter oral pain relief. Within a few weeks, the area will take on a more sculpted and shapely silhouette, which will last as long as a healthy lifestyle incorporating sensible diet and an exercise program is maintained.

Few people have cash on tap to pay their doctor upfront, or even on the day of the surgery. Many people are turning to specific health loans to have a procedure done as soon as possible, and according to Jodie Laing, CEO of Mediplan Healthcare Finance, people no longer need to wait for their cosmetic procedures. Mediplan lends money for most healthcare procedures, including cosmetic or plastic surgery, which is an advantage when applying for a loan. “Many people prefer borrowing from us rather than from a bank because we make no judgements about what people want done,” says Jodie. “Often people find it embarrassing giving details to their banks, but we’re accustomed to what’s involved.”

“Many people prefer borrowing from us because we make no judgements” Mediplan lends from $1000 to $25,000 over four years, so most procedures are covered. You can also opt to pay the debt off more quickly or more slowly. “We don’t specify any particular criteria for approval, except that the client be over 18 years old, working 20 hours or more a week and have a good credit rating,” Jodie says. You could be approved for finance within 24 hours and the set amount can be advanced directly to your doctor or clinic. You may even want to use the funds as a drawdown facility so you can have several treatments over a period of time. And Jodie points out that Mediplan interest rates can be more favourable than using credit cards. “Our current interest rate is 19.45 per cent, whereas most credit cards are more than 20 per cent,” she says. Dr Gabrielle Caswell, president of the Cosmetic Physicians Society of Australasia, agrees that services such as Mediplan can be cheaper than credit cards, but adds that patients should do their sums carefully. “The patient will still need to budget and stick to their repayment plan,” she says.

THE IDEAL CANDIDATE

BODY

Pregnancy and childbirth, ageing and congenital malformations can contribute to a range of distressing genital and pelvic symptoms. These include urinary and bowel incontinence, pelvic organ prolapse of the vagina, cervix or uterus, vaginal relaxation (stretching of pelvic-floor supports, connective tissue, ligaments and muscles), difficulties with or reduced sensation during intercourse, overly large, uneven or unsightly labia. Demand for genital procedures is growing rapidly. Among the most sought-after are surgical correction of prolapses and vaginal wall relaxation using Laser Vaginal Rejuvenation, and reduction of the labia minora (labiaplasty).

SURGICAL OPTIONS

Many women suffer from their symptoms for many years before seeking help because they feel embarrassed, and lack of information can engender fears about what’s involved in treatment. Yet most procedures are minimally invasive. Laser Vaginal Rejuvenation, for instance, is the modification of a gynaeocological procedure first used to treat stress urinary incontinence. The laser technique used allows for controlled accuracy and relatively bloodless surgery, leading to faster healing and resumption of activities in a shorter period of time.

GENITAL

REJUVENATION

THERE’S NO NEED TO PUT UP WITH DISTRESSING SYMPTOMS. TREATMENTS TODAY ARE MINIMALLY INVASIVE AND OFFER FANTASTIC RESULTS

WHAT TO EXPECT AFTER A GENITAL PROCEDURE “Minimally invasive” means performing surgery in many cases without large, open incisions and sometimes under sedation and local anaesthesia. For patients, this can translate into much less post-operative pain and discomfort, lower risk of infection, faster recovery, increased mobility and function and little scarring. However, although the techniques involved in Laser Vaginal Rejuvenation are minimally invasive, patients are strongly counselled that the first three weeks are the most important in terms of recovery and outcome. Therefore, a woman undergoing genital rejuvenation or correction surgery is encouraged to rest appropriately. “Women can feel fearful or embarrassed to seek treatment for gynaecological issues that are causing great distress,” says Dr Oseka Onuma, of the Laser Vaginal Rejuvenation Institute of Adelaide. “Many of my patients say after surgery that they wish they’d done something many years before.”

LIFT MEET DR STEVE MERTEN AND HELEN THOMAS – STARS OF NEW YOU TV’S FIRST MAKEOVER “WEBISODES” > HELEN’S CONCERNS

BEFORE THE

treatments Not only did Helen want her breasts lifted, but also the dehydrated, uneven look of her skin corrected. Botox and a regimen including medi-spa treatments Microhydrabrasion and Dermaroller were “prescribed” by therapist Douglas Pereira.

Helen, 32, was concerned how her large breasts – one much bigger than the other – had sagged and made her look bigger overall. “I’d buy DD cup bras to fit the smaller breast and squeeze in the other E-F cup as best I could,” Helen says. “Not a good look!”

Dr Merten injects Helen’s brow area with Botox.

After a consultation at his Pure Aesthetics practice in Sydney, plastic surgeon Dr Steve Merten recommended that Helen undergo a breast lift and reduction. For a kick-start on her self-enhancement journey, he injected Helen’s brow area with Botox, which over the ensuing three to seven days would subtly relax, lift and brighten her appearance. Helen had surgery three weeks later at Macquarie University Hospital. During the procedure Dr Merten removed excess tissue and skin to create perter breasts that were more in proportion with Helen’s size and repositioned them higher on her chest wall.

Clinical therapist Douglas Pereira performs Microhydrabrasion.

treatments > FACE “Botox has ‘opened’ my eyes and given my whole face a more relaxed, lifted look,” says Helen. Before surgery, she gave up smoking and has been taking vitamin C and zinc supplements to aid her recovery, and she feels they’ve significantly improved her skin texture. Treatments with Dermaroller (to stimulate collagen production) and Microhydrabrasion (to gently resurface), along with a home-care routine, have transformed her complexion’s clarity and moisture levels. The uneven pigmentation and dry areas so noticeable at her first consultation with Dr Merten are no longer in evidence. “When I catch sight of myself in the mirror, I get a real buzz,” Helen says. “When I look in the mirror, I seem smaller overall – and my friends say the same thing. Yes!”

> BREASTS Two weeks after surgery, when Helen showed up do this photo shoot and interview, she was glowing – both physically and in praise of Dr Merten’s work. Her breast incisions were still “taped”, but it was evident that the scars (which fade to fine white lines after 12 months) would be neat and discreet. The largest incisions were made under the natural curve of each breast, so will remain somewhat hidden anyway. A short vertical incision was made from the base of each breast to the nipple and around it, because of course the nipple had to be repositioned along with the breast itself. “I feel fantastic,” Helen says. “I was a bit groggy for a few days, but there’s been no pain, just some minor discomfort. When I look in the mirror, I seem smaller overall – and my friends say the same thing. Yes!”

what the expert says > DR STEVE MERTEN Helen came to see me to discuss the concerns she had with her breast size and shape. She disliked their droopiness, which she’d experienced for many years, and this problem had worsened recently as a result of moderate weight loss through an exercise program she’d been undertaking. Helen felt that her breasts were too big, with all her breast tissue falling down the bottom and out the sides. This was making her embarrassed, with a loss of confidence in her appearance, as well as causing her physical discomfort, particularly when exercising. She’s in good health and had recently stopped smoking. I saw her for two pre-operative consultations to assess and advise her of her best options. I recommended she have a “short scar” breast reduction and lift, and explained the surgery to her extensively, including the potential risks and the recovery process. A lesser concern for Helen were early signs of ageing of her face, with frown lines around her eyes, and for this we performed musclerelaxing injections in my office before her surgery, as well as starting a skin-care program. Helen’s surgery took place under a general anaesthetic and took about two hours. It went exactly as planned, and I was very pleased with the result. She stayed one night in hospital and then went home. She’s making excellent progress, has had very little pain and her breasts are healing very well. The early cosmetic result is excellent, as will be the long-term result, I expect. I’m sure Helen will benefit significantly from her surgery, both physically and psychologically, and it has been a great privilege to be involved in her care.

EYES BRIGHTER SICK OF BEING TOLD SHE LOOKED TIRED, TRACY HARRIS TRIED TEAR-TROUGH REJUVENATION AND SHED YEARS FROM HER FACE > THE PROBLEM “No matter how much I spent on hair, clothes or make-up, every time I went out I’d hear repeatedly how worn out I looked,” says Tracy, a publishing professional in her 40s. “As a busy person in daily contact with others, that irked me – especially if I’d slept for more than seven hours the night before. No matter how much corrective make-up I applied, the under-eye darkness and hollowness made me look sleep-deprived.”

the treatment regimen After a 20-minute consultation, in which Dr Calfas clearly outlined the tear-trough rejuvenation procedure to Tracy, the method and risks involved, he started injecting the area where her cheekbone area merged with the softer tissue beneath the eyes. She opted not to use Emla, which is a numbing cream normally offered for these procedures and is worn for up to an hour before injecting begins. Tracy recalls that the level of pain she experienced was a “six out of 10 on my personal Richter scale” but that, even so, the sensation was manageable. Her entire eye area was completed within 40 minutes and the results were visible immediately. A softening of the area was apparent as well as a reduction in depth under the eyes, with the darkened “cavities” restored to a plumper and more rested look. Tracy says the area felt sensitive for the remainder of the day and she was advised not to rub or touch it if possible, so that the filler could “assemble” itself naturally, but she had no problem with following that suggestion.

Tracy is delighted with the results of her treatment.

Tracy’s olive skin (inherited from her mum’s side) made her more prone to dark circles. Hollows had started to form under her eyes as part of the natural ageing process.

> PROBLEM SOLVED Tracy opted to have a tear-trough procedure performed by Dr George Calfas, of GMC Cosmedical in Sydney. The under-eye area was injected with Esthélis, a soft, malleable, hyaluronic dermal filler. This plumped up the area, restoring it to a healthier, more energised look and giving the whole face a rejuvenating boost.

I’m absolutely thrilled with the results and not only do I look better but I feel it too. No-one ever says I look tired now – unless, of course, I really am!”

DEDICATED MUM REBECCA TELLS HOW SHE HAD BREAST AND BELLY SURGERY, WITH THIGH AND HIP LIPOSUCTION, TO GET HER PRE-BABY BODY BACK

> WHAT I DID AND WHY I wanted surgery to regain my pre-baby body and give my confidence a boost. In late 2010, Sydney plastic surgeon Dr Robert Drielsma performed an abdominoplasty (tummy tuck), mastopexy (breast lift) with implants and some liposuction around my hips and thighs. At first it’s a little daunting when you wake up with all these major changes and you’re in discomfort at the same time. I remember thinking my boobs looked and felt so round and, well, “out there”, which was a very funny feeling!

> HOW IT FELT The common advice is that you may feel regret or depression after the surgery, but I knew it was important for me to give my body time to heal and settle before I could appreciate the outcome. You really have to rest, rest and rest, especially for the first four to six weeks. It’s the only way to recover properly. My body was sore and tender for a few months and I had to take things very easy.

BEFORE THE

treatmentS I’m a single mother of three beautiful kids and I work really hard with them. I wanted to feel and look great as well as teaching them that it’s good to have pride in your body and to like yourself. I felt that was especially important to pass on to my 11-year-old daughter.

AFTER THE

treatmentS I love my new body – or rather my rejuvenated old one! I’m four months out of surgery now and am looking forward to getting back to the gym and keeping my body toned and fit. People who don’t know about the surgery tell me how great I’m looking and think I’ve lost weight or have been working out. In fact, I haven’t lost weight at all, it’s just that my body shape is nicely balanced now. Oh, and I’ve added some cute bikinis to my wardrobe! I have absolutely no regrets about going through with these procedures. My advice to any other women who might be thinking of having surgery to improve their bodies is to just go for it!

what the expert says > DR ROBERT DRIELSMA Rebecca was an ideal candidate for this kind of surgery. Her weight had been stable for more than 12 months and she exercised regularly and maintained a sensible diet. She also had realistic expectations of what the surgery could achieve for her. We combined abdominoplasty to tighten abdominal skin and remove stretch marks with liposuction to the hips and thighs. Despite a few nerves on the morning of her surgery, Rebecca’s seven-hour procedure went smoothly and she recuperated in hospital for four days. Almost four months down the track, I’m pleased she’s back in her bikini and enjoying her rejuvenated body shape.

IS MORE HAVING ALWAYS BATTLED OBESITY, GAEL HAD GASTRIC BANDING A YEAR AGO. SHE’S NOW LOST 32KG AND IS ONLY 11KG FROM HER GOAL WEIGHT > WHAT I DID AND WHY

BEFORE THE

treatment Gael Donaldson, 42, is director of marketing and PR at a leading Sydney hair salon, RAW Anthony Nader. Having been overweight since she was a child, Gael had become morbidly obese by her 30s, plagued by emotional and physical issues. “I’d tried every single diet known to man,” she says. “Nothing could beat my lack of willpower.” In March 2010, Gael decided to have gastric (or lap) banding surgery. Her journey started at 116kg. She now weighs 84kg. Her goal weight is 75kg, and she’s well on her way to achieving it. Around 11,000 Australians a year undergo gastric banding.

I’d been overweight since childhood and it’s affected my whole life. It’s made me have to think about things that other people take for granted: How will I keep up with the others? Is everyone laughing at me? Are my friends ashamed of me? If I order that food will people think “look at the fat chick gorging herself”? I had a loving family but I was ridiculed at school and would hide the hurt with humour. I ate secretly, using food to compensate for my feelings of low self-esteem and loneliness. You could say I “ate” my feelings. I pretty much did whatever I could to try to feel accepted as a teenager, and I succumbed to peer pressure. I’m not proud of sacrificing my values along the way. I can see that I’ve craved reassurance in present and past relationships and friendships. I have to learn that I do deserve good things and that I am a good person. Having surgery to improve my appearance and my life has been part of that learning process.

> TREATMENT PLAN Five years ago, at the age of 37, I started investigating the idea of having a gastric-banding procedure. A friend had had one and was dropping a lot of weight. Mine was performed by Dr Craig Taylor, of Sydney’s OClinic. It cost a lot of money for me (around $10,000) but my lovely mother helped me out. I’m so happy with the results. I should have done it years ago! With a great family and circle of friends and wonderful employers, I’ve realised that anything is possible. I couldn’t have done it without them.

treatment The gastric band creates a smaller stomach pouch, so that less food is required to feel satisfied. Because the band has reduced the size of the passage through which food passes, it also takes longer to digest. “My procedure was performed laparoscopically and there was hardly any pain, and barely any scarring,” says Gael. “I took a week off work but I really didn’t need all that time. Not once have I felt any regret. After the surgery I was on a fluid diet for about two weeks while I healed. In weeks three and four you go through a soft, mushy food phase and then at week five you graduate to eating a wide range of everyday solid foods such as chicken, fish, soft meats, pasta, rice, fruit and vegetables. Now I never feel hungry. I’ve learnt only to eat and drink when my body tells me to and not at dedicated meal times. Life is easier in that I can walk upstairs without huffing and puffing, I walk further and more often, and I now feel more confident – I don’t feel people are sniggering at me.”

DR WARWICK NETTLE

The lower half of my face seems to be sagging and jowling, while my eyes and brow still look relatively youthful, other than the normal expression lines. Is it possible to have work done on just the lower half of my face? Facial rejuvenation surgery can be customised to those areas of the face that can be enhanced. If the lower half needs help and the upper half doesn’t, there’s no problem at all with doing more on the lower half only. And while many people are content with the gentle improvements possible with non-surgical procedures such as skin collagen replacement, dermal fillers and stimulants, they’re not always enough. Neckline improvement, jowl improvement and general sagging of the face require surgery in the form of a lower facelift or a necklift.

I think I’m ready for a facelift, but I’m scared that I’ll look like I’ve had work done. Are there new techniques that deliver a natural, non-‘lifted’ result? There are indeed new techniques that can ensure a natural look – many people have had facelifts these days and you’d never know. Modern techniques reshape the underlying layer of connective tissue, unlike the facelift procedures of the 1970s and 1980s, which stretched only the wrinkled surface of the skin. The main determinants of a natural look rely on tightening the underlying muscles of the face in the appropriate vertical oblique direction, and not pulling the skin too tight, as well as paying close attention to a myriad of details such as avoiding hairline alteration and pixie ear.

03

I really need a nose job to correct the large hook in my nose, but my friends tell me it will affect my breathing. Is this true? Breathing is evaluated in all cases prior to nose surgery. One of the big changes in rhinoplasty procedures today is that almost all patients have internal nasal surgery performed to improve breathing, as well as external appearance surgery. So, in fact, the opposite of what your friends tell you is usually true. Cosmetic rhinoplasty tends to improve rather than compromise breathing.

Q&A 01

Having children has left me with a lax tummy and stubborn areas of fat on my core and around my bottom. Is it safe to team a tummy tuck with liposculpture, or is it best to have these done separately? Safety has nothing to do with the operation itself or combination of procedures, but rather with patient selection and the surgeon’s skill. It’s perfectly safe to team an abdominoplasty with liposculptural procedures and I generally perform them both at the same time, as I believe liposculpture further perfects the result of the tummy tuck. My only rule is that patients should not be on an operating table for this type of elective surgery for longer than eight hours, if possible.

I have large, heavy breasts that cause me a lot of back pain. How simple is a breast-reduction procedure? Will it leave scarring? And do they regain their size? These are complex operations that should only be performed by wellqualified plastic surgeons. However, most high-profile surgeons like myself now perform a short-scar technique instead of the old T-scar technique. All operations involve scarring; the trick is to ensure the scar is as discreet as possible. Pregnancy and large weight gain may result in a subsequent increase in breast size, but in my two-year follow-ups I find most patients do not have a subsequent increase.

DR GEORGE MARCELLS

I broke my nose in a sporting accident some years ago. Aside from it now being misshapen, I also experience breathing problems. Can rhinoplasty fix the way my nose works, as well as the way it looks? As an ear, nose and throat specialist as well as a facial plastic surgeon, I’m able to deal with both functional breathing problems and the aesthetic appearance of your nose. I use techniques based on openstructure rhinoplasty. These are more demanding on the surgeon and take longer than other methods to perform. However, I believe they help to ensure the basic foundation of the nasal structure is kept strong. In turn, this ensures a predictable shape to the nose after surgery that will resist collapse, maintain a natural shape and also improve breathing. These techniques were used to reconstruct the nose of the man below after trauma. The post-op view shows excellent improvement in appearance, with strong natural profile and supported tip. A marked improvement in breathing was also achieved. BEFORE

People tell me I look tired or unhappy a lot. I think it’s because my brow is lined and my eyelids are heavy. How can this be improved? Rejuvenation of the upper face can restore a younger and fresher appearance to tired-looking eyes and the ageing face. Drooping eyebrows, excess skin and fat in the upper eyelids, and bags under the eyes are signs of upper-face ageing. Browlift surgery is performed to re-suspend the eyebrows to their natural and more youthful position. The female eyebrow usually sits just above the bony upper rim of the eye, whereas the male eyebrow usually sits on the rim. This surgery can help remove “hooding” over the upper eyelids. I most commonly use the endoscopic (small incision) browlift technique, although I may use other techniques if they’re more appropriate. Surgery is performed to remove excess skin and fat in the upper lids and also correct bags and sagging under the eyes. Small incisions around the eyelids or behind the lower eyelids heal very well. Eyelid surgery may be performed alone but is often performed with browlift surgery, and also with other procedures BEFORE such as facelift and necklift surgery. The lady on the right had left-sided browlift and upper lid blepharoplasty to restore symmetry to the eyebrows and remove excess hooding of the lids. The post-op view shows a natural, refreshed look with improved symmetry. Before and after browlift and eyelid surgery, the skin of the upper face can be kept youthful by non-surgical procedures such as laser and light treatments, microdermabrasion, micropeels, soft-tissue fillers and muscle relaxants.

DR GABRIELLE CASWELL

How can I look younger and fresher without resorting to plastic surgery? I’d suggest using a combination of laser or IPL (light) treatment and dermal fillers, which come in a range of consistencies and can be used in different parts of the face for different effects. Thicker fillers such as Juvéderm Voluma can correct “sag and bag”, while lip augmentation doesn’t need such a thick substance, so I might choose Juvéderm Ultra XC – there’s no pain and it’s a very flexible gel for soft, poutable lips. To remove redness from the face, we can use a vascular device such as Dual Yellow (for larger vessels) and then IPL LimeLight. Fractional laser resurfacing is great for reducing deeper lines and wrinkles and dealing with sun-damaged skin. The results last five to ten years and the treatment can be repeated. Social downtime is only around seven days. BEFORE

I’ve heard a lot about fractional laser treatments and how they can give a smooth, even skintone and firmer skin. How do they work? The patient above has had dermal fillers, anti-wrinkle and Cutera Pearl Fusion (a combination of Pearl and Pearl Fractional) laser resurfacing treatment. Fractional laser devices work by causing ablation of small spots on the skin. For example, when using the Pearl Fractional device with a single pass, about 76 per cent of the skin remains intact. Small plugs of skin then work their way out and our innate repair process begins, leaving the skin smoother, firmer and with thicker collagen. Patients will see noticeable results in a short period, and the skin will continue to remodel and refine its collagen if a home support program is continued for at least six months. The process can also clean up the background parchment effect of sun-damaged skin, reduce larger facial veins and be an effective treatment for acne scarring.

03

I want to have some fillers done, but what if I’m unhappy with the results? Is there anything I can do to correct them? New-generation fillers are based on hyaluronic acid, which occurs naturally in the body so there’s no need for allergy testing (unlike with collagen-based products). They swell less once injected and some are packaged with pain relief, making the procedure truly walk-in walk-out. They can be dissolved with hylase, an enzyme that’s injected to dissolve the filler, providing the patient with ultimate flexibility with their total look.

DR PAUL GERARCHI

Is it possible to have a necklift as a single procedure? My face is relatively line-free and not really sagging, so I don’t require a surgical procedure, but my neck is quite crepey and is showing some sag. A necklift can be performed as stand-alone surgery or in combination with other facial cosmetic procedures. It’s very common to feel your neck has aged out of proportion with the rest of your face; people are often bothered by a pouch or sagginess that develops under the chin while the rest of their face appears relatively beautiful. A necklift is an excellent procedure for removing this fatty deposit and loose skin and creating a more defined cervicomental (neck to chin) angle. However, a necklift has only a limited benefit in improving the jowls that form along the jawline. If these are an issue for the patient, a facelift as well as a necklift is likely to be more beneficial.

My teenage daughter has a bump in her nose. What age is best for her to consider rhinoplasty? I’m worried about her doing it too soon in case her face is still changing as she matures. Traditionally, nasal surgery is delayed until most facial growth has occurred, commonly agreed to be at about 16 years old. This is because of concerns about interfering with facial growth, although evidence for this is limited. In reality, if there are reconstructive or significant functional concerns, we’d generally proceed with surgery. So, if your daughter is 16 years old, has gone through most of her pubertal growth and you and she both agree that removing the large bump from her nose would be beneficial, then rhinoplasty would be a good option to consider.

Falling of the eyebrows begins in the third decade, along with excess skin developing on the eyelids. Procedures to correct them are common in people in their 40s or older, but are relevant to anyone with significantly ageing features around their eyes.

Because we have so many modalities at our disposal, we can tailor treatment plans to suit the client’s aspirations, budget and time frame.

Our approach is to understand the client’s main concerns and recommend treatments that will have the most successful, effective results to treat those concerns first. We ensure that each patient has all the information they need, and manage their expectations of outcomes, with a focus on rejuvenation.

Dr Jeremy Cumpston AGELESS CLINICS Q&A WITH DR CUMPSTON Why did you decide to specialise in anti-ageing aesthetic medicine? After developing an interest in diet and hormone-related weight loss and anti-ageing medicine about 15 years ago, it seemed like the logical step to undertake extra cosmetic physician training.

What is unique about the way you deal with your patients? I have a strong aesthetic, I’m confident in the procedures that I perform and, most importantly, I genuinely like people. These three factors result in my patients always leaving my practice looking and feeling better.

How do you deal with patients’ expectations about how much younger they can look? I’m just honest. In some cases this means patients need to be referred on to cosmetic surgeons to achieve the results they’re looking for. I’m lucky in that I know excellent plastic surgeons, both here and overseas, whom I trust to deliver brilliant results.

ABOUT DR CUMPSTON Dr Jeremy Cumpston studied at the University of Adelaide’s School of Medicine. A progressive GP with a thriving medical practice in Sydney’s eastern suburbs, Dr Cumpston has accumulated more than 15 years’ experience both in Australia and overseas, working in preventative and anti-ageing medicine, including training in 3D anti-ageing techniques. In 2009 he established Ageless Clinics in Randwick, Sydney, aimed at meeting the need in Australia for doctor-owned and operated medical centres dedicated to aesthetic and anti-ageing treatments. Ageless Clinics has rapidly grown in number, now serving five key towns throughout regional NSW: Bathurst, Dubbo, Goulburn, Orange and Tamworth. There are plans for more locations to follow. Ageless Clinics was chosen by leading US laser manufacturer Solta Medical Inc to be the first in Australia to offer the revolutionary Fraxel Re:store Dual System, thus enabling Ageless to deliver the ultimate technology in laser skin rejuvenation. Ageless is also the first clinic in Sydney’s’ eastern suburbs to offer the latest non-surgical fat-reduction procedure, CoolSculpting by Zeltiq. Studies out of Harvard have established a minimum 22 to 25 per cent fat reduction per area treated, regardless of diet or exercise.

Dr Robert Drielsma Sydney Cosmetic Plastic Surgery Q&A WITH DR DRIELSMA What attracted you to plastic surgery? I initially trained as a general surgeon and, while it was very satisfying, I found fine and delicate surgery more appealing. With a flare for art and music, I became drawn to plastic and reconstructive surgery, both highly technical and creative surgical disciplines.

What procedures have your patients expressed excitement over? Basically any procedure, no matter how big or small, offers immense satisfaction and happiness. The most “feel good procedures tend to be breast enlargement, breast lifting and breast reduction. Any facial rejuvenation also causes much joy and excitement, as do body-contouring procedures such as tummy tucks and liposuction.

The Institute of Facial Plastic, Nasal & Cosmetic Surgery Q&A WITH DR MARCELLS WHAT DO YOU DO TO PUT PATIENTS AT EASE? FACIAL HARMONY This patient presented to our clinic with the following concerns: breathing and sinus congestion, a bump on the nose, plus excessive view of the nostrils. After extensive consultation and examination we looked to computer imaging to design a nose in harmony with the face. She underwent surgery to improve her breathing and sinus condition, decrease the bump, refine the tip and address the nostril view. Post-operatively we see an elegant, natural nose in harmony with the face.

“Natural, individually tailored results” DR MARCELLS FINDS THAT, FOR MANY, THE GREATEST MOTIVATING FACTOR FOR COSMETIC SURGERY IS TO REDRESS THE IMBALANCE THEY MAY FEEL BETWEEN THEIR AGEING VISAGE AND A STILL YOUTHFUL WELLBEING. “FACIAL PLASTIC SURGERY TODAY IS ABOUT DELIVERING A NATURAL RESULT, TAILORED TO THE INDIVIDUAL,” HE SAYS.

Dr Steve Merten PURE AESTHETICS Q&A WITH DR MERTEN What gives you the most satisfaction in your work? Seeing my patients achieve the results they have wished for, whether it’s treating a skin cancer, right through to a major physical transformation, and becoming a small but important part of their lives during this time.

I commonly combine breast augmentation, lifts or reductions with tummy tucks (abdominoplasty), nose reshaping (rhinoplasty) or other procedures.

What is the most common feedback you receive? That the surgery was easier and less painful than they expected, and that the care we provided, from the first inquiry right through to the anniversary of our care, has been exemplary.

ABOUT DR NETTLE Few cosmetic plastic surgeons in Australia share the same level of training as Dr Warwick Nettle. He always knew he wanted to be a doctor and, when he entered medical school, he realised it was plastic surgery he was most passionate about. His work began with reconstructing congenitally deformed faces, or faces injured in trauma. Yet he was increasingly drawn to facial aesthetic surgery, which is technically demanding and requires great attention to detail. He found this suited his personality and his lifelong interest in artistic endeavours. Dr Nettle’s ultimate aim is to achieve natural-looking and harmonious results for the face, eyes, nose, jowls, neck and jawline. Dr Nettle has gained extensive experience by serving at two of the most respected institutions in the United States: the MD Anderson Cancer Centre in Houston and the Manhattan Eye, Ear and Throat Hospital in New York. In addition, he has developed innovations in facelift and endoscopic facial surgery. He is continuing his research and has published many articles in the areas of eliminating the “facelifted” look, using dermal stimulants as an adjunct to facial rejuvenation surgery, and refining techniques in eyelid surgery.

Dr Benjamin Norris SILKWOOD MEDICAL Q&A WITH DR NORRIS How do you manage patient expectations? By taking time during the initial consultation, being a good communicator and therefore allowing the patient to freely discuss all of their likes, dislikes and desired goals, I can very quickly gain a grasp of whether the patient is realistic about what they expect the outcome of any surgery to be.

What does a consultation with you entail? I treat the initial consultation like a chat, to make the patient feel comfortable in a nonjudgemental environment. But it’s designed so that the basic information – general medical background, family medical history, previous surgery and the like – is gleaned.

What single concept best summarises your practice? Without doubt an individualised approach to every single patient. This again comes down to communication and my ability to listen to what is being said and interpreting what is not being said.

EFFECTIVE SOLUTIONS FOR INTIMATE PROBLEMS Pelvic and vaginal surgical rejuvenation or correction can transform a woman’s life. Procedures performed by Dr Onuma can enhance sexual gratification, improve the look and function of labia, cure urinary incontinence, restore pelvic support and resolve menstrual dysfunction. “Women can feel fearful or embarrassed to seek treatment for gynaecological issues that cause great distress,” says Dr Onuma. “After surgery, many of my patients say they wish they had done something many years before. Ultimately, every individual has to get to a point at which they feel their problems are not getting any better and are often getting worse. Being afraid, being embarrassed is part of our humanity. 01 blueberries These little berries are one of the best sources of antioxidants, which protect the body against free radicals and ageing. Try Yes To Blueberries Age Refresh Daily Repairing Moisturizer, $34.95, available from David Jones, to improve skin firmness, diminish the appearance of fine lines and wrinkles, and nourish and protect ageing skin by providing a defensive barrier on its surface.

DID YOU KNOW? You can eat your way to beautiful skin with foods rich in omega 3, 6 and 9. Oily fish, avocados, almonds and pumpkin seeds are great sources.

02 oats Beta glucan, which is a type of molecule found in oats, has been found to accelerate the skin’s healing process and improve its appearance by stimulating collagen and providing a firming and moisturising effect. Oats are also great for sensitive skin. Try Aveeno Daily Moisturising Lotion, $9.99, which soothes and protects sensitive skin and provides moisturise for 24 hours.

03 pomegranate Pomegranates are a great source of antioxidants, vitamin E and flavonoids. When applied to the complexion, coldpressed pomegranate oil firms and smoothes skin. Weleda Pomegranate Firming Face Serum, $45.90, provides hydration, increases the skin’s elasticity and diminishes the appearance of wrinkles and fine lines over time. It invigorates cell renewal and improves the skin’s surface. Also try Burt’s Bees Naturally Ageless Intensive Repairing Serum, $44.95, and Korres Pomegranate Cleansing Wipes, $18.95.

04 okuru A plant found in western Africa, okuru has been farmed for centuries and has been recognised for its high nutritional food value and skin benefits. La Pureté has found a combination of peptides in okuru that minimise expression lines in a similar way to Botox. La Pureté Okuru Triple Effects Eye Crème, $69.90, targets expression lines such as crow’s feet while also reducing the appearance of dark circles and under-eye circles.

Caffeine is a common ingredient in anti-cellulite creams. It reduces fluid retention, temporarily smoothing the dimpled appearance on the top layers of the skin. Phyt’s Minceur Bio-Active Body Slimming Gel, $99, uses caffeine to stimulate and accelerate the clearing of fat over the entire body. Extracts of fig tree bud and organic red vine also help reduce cellulite.

06 royal jelly As we age, our hair follicles lose protein, amino acids and lipids, leaving our locks dry and lifeless. Royal jelly replenishes the hair with missing nutrients and vitality so that you’re left with smooth and shiny strands. L’Oréal Paris Elvive Re-Nutrition Night Elixir, $11.95, is a hair treatment enriched with royal jelly and is absorbed into the hair to intensely nourish it overnight and leave it repaired in the morning.

07 honey This sweet ingredient is not only a natural healing agent, but also a great source of antioxidants and hydration for skin. Comvita Medihoney Gentle Body Lotion. $21.60, contains antibacterial Manuka honey, a medical grade of honey found only in New Zealand that helps the skin retain moisture and relieves dryness and itchiness. It also contains chamomile, aloe vera and evening primrose oil.

As well as being packed full of antioxidants and vitamins, raspberries help stimulate micro-circulation. This concealer uses the power of these berries to help awaken your under-eye area and conceal uneven skin tone. Bourjois Healthy Mix Correcting Concealer, $24.

09 kakadu plum This fruit, found right here in Australia’s outback, is one of the highest plant sources of age-fighting vitamin C in the world. Used in skincare products such as Skin Physics Cellugen Ultra Night Repair, $89, kakadu plum carries regenerating collagen deep into the skin to help fight the signs of ageing. It can also be found in Endota Spa Organic Kakadu Plum Face Mask, $55, and U Little Beauty Kakadu Intox-CCation Skin Firming Serum, $39.95.

10 green tea Most people know about green tea’s benefits when sipped. It boosts your metabolism and contains little caffeine compared to black tea and coffee. But its topical benefits should also be noted. Green tea is the least processed and most nutrient-rich version of tea, and can help reduce sun damage by quenching free radicals and reducing inflammation. It’s a great addition in anti-ageing products such as Natio Sensitive Skin Green Tea & Chamomile Night Cream, $19.95.

01winter frizz and flyaways HAIR S.O.S

> QUICK FIX To instantly combat unruly frizz and flyaways, try L’Oréal Paris Elvive Smooth Intense Anti-Frizz Shampoo and Conditioner, $6.95 each. And deep-condition weekly with a hydrating hair mask like L’Oréal Paris Elvive Smooth-Intense Smoothing Masque, $11.95.

No-one is immune to frizz, and it can get out of control in the Australian winter, due to high levels of rain and humidity. Dry and dehydrated hair and a compromised hair cuticle – often the result of bleaching, chemical processing and over-styling – intensify the problem, drawing in moisture from the humid environment, causing hair to frizz. “The classic trick that we hairdressers adore is to rinse hair in cold water. It closes the cuticle and imparts shine and manageability,” explains Anthony Nader, from Raw Anthony Nader in Darlinghurst, Sydney. “Of course, you should also always use a creamy shampoo and conditioner with a frizz-defence formula to rehydrate the fragile hair shaft.” In terms of your haircut, look towards stylish, long layers, says Anthony. “They’ll give the hair weight and fullness and make any bouts of frizz less obvious. Keep the ends of your hair blunt, rather than flyaway, and avoid razor cuts and thinning scissors, which will simply make frizz seem even worse.” Cut back on heated styling, too. If you must use straightening irons, iron the outer layers of the hair only, and if you can’t go a day without blow-drying, use one that has ionic technology, such as the Parlux 3500 Ceramic & Ionic Dryer, $184.95. “Ionic technology uses negative ions to strengthen and smooth the hair strand from within, leaving it shiny and glossy,” explains Anthony. “The negative ions work cleverly to reduce the size of water molecules while styling.” Always angle the nozzle of your hairdryer down, so that the heat is directed along the hair shaft as opposed to directly at it, or upwards. Use a low-speed, low-heat setting and dry hair slowly in small sections.

02 limp, oily locks with split ends HAIR S.O.S

Limp, lightweight hair is mostly genetic, but with winter’s damp conditions limpness can affect us all, even those with the thickest of hair. You can easily pump up the volume with a few simple tricks. “A shorter haircut looks fuller and provides manageability without weighing hair down,” says Anthony. If you must wear hair long, “be careful not to over-layer it, which causes wispiness,” advises Hamish Glianos, from People Hairdressing in Sydney. “A structured shape with longer layering works well.” Watch the oil balance in your hair, too. It can go into overdrive in winter to compensate for harsh temperatures that sap moisture from the hair. An oily scalp can often exacerbate limpness and goes hand in hand with dry, fragile ends. Always condition the ends of your hair well to prevent further splitting (apply product from mid-lengths down). Try De Lorenzo Instant Accentu8 Conditioner, $21.95. When styling, Hamish recommends using a high-hair-inducing mousse. “It will help build volume from within. Apply it close to the roots and along the shaft prior to blow-drying, then style hair around a large round, natural-bristled brush, pulling away from the scalp while aiming the nozzle of the dryer directly at the roots.” Try Wella High Hair Styling Mousse, $22. “Avoid waxes or serums,” adds Anthony. “They may weigh your hair down more. Blast hair with cool air once you’ve finished styling, because the heat from your dryer can stimulate sebaceous glands and activate oil flow. The cold air can remedy that.” Also, keep a dry-hair shampoo handy. “I swear by Klorane Gentle Shampoo with Oat Milk, $16.95,“ says Anthony. “It amps up thickness, adds texture and mops up oil.”

03 out-of-control curly hair HAIR S.O.S

Curls are by nature unruly. They’re tricky to manage and suffer dryness faster than any other hair type. “A great cut can help,” says Anthony, who has developed a unique cutting technique to deal with them. “I call it ‘Seamless Texture’. It re-proportions the pyramid effect of hair sitting flat at the top and wider at the bottom. If want to dry hair straight, you won’t get a layered feel, and if you want to wear it curly you get a beautiful shape all over.” Opt for a semi-permanent colour to help control the dryness and tame the curl, use curl-specific hair care and never rub the hair when towelling it. “Just blot it dry,” says Anthony. “Use a

super-absorbent towel designed specifically for hair.” Jayne Wild, from Wild Life Hair in Sydney, suggests using a cloth kitchen towel. “Don’t be afraid to overload product,” adds Hamish. “Curly hair needs the moisture that products can provide. Any kind of wet or shine wax will help, as will curl-smoothing lotions. Apply to hair then leave. Allowing hair to ‘air-dry’ can often be the best solution. You can then form curls into shape using your fingers once hair is touch-dry.” Try Aveda Be Curly Curl Enhancer, $44.95; Inecto Pure Citrus Shine Boost Hair Serum, $14.95; Kérastase Nutritive Oleo-Curl Crème d’Huile, $39.50.

> STAR MAKEOVER Nicole Richie is a natural dark brunette, so all the bleaching she goes through can make her hair stand on end. A good product feed calms it right down, giving a far smoother look.

Dry hair equals damaged hair and the culprits are varied – stepping outside with wet or damp hair (this can cause breakage in cold temperatures), over-styling, over-processing or simply infrequent trims. “The six-weekly trim is essential to prevent split ends and weak strands,” says Anthony. “This is especially so in winter, when cold winds and cool temperatures can cause hair stress.” Of course, if hair is damaged beyond repair, the only solution is to opt for a chic, short crop and begin growing your locks long again. However, if it’s not super-stressed there are ways to nurture it back to health. Try to wait as long as possible between chemical treatments like bleaching or colouring, minimise use of heated appliances, use ionic appliances to help protect the hair shaft and always apply a heat-shield product before styling. Try Andrew Collinge Heat Tamer Conditioning Spray, $12.99. Chemically treated hair that’s porous, fragile and prone to breaking requires strengthening once a week with a protein-based treatment. Try Redken Extreme CAT Treatment, $28. And use a moisturising treatment after every wash (with a moisture-infusing shampoo, of course), not just as a once-a-week treat. Try Alterna Caviar Anti-Aging Overnight Hair Rescue, $48.60. Anthony adds that using a leave-in conditioning treatment daily will also do wonders for weary locks. “A leave-in treatment is designed to protect, hydrate and strengthen hair throughout the day,” he says. Try Garnier Fructis Nutri-Repair Leave In Crème, $10.85.

05 colour fade HAIR S.O.S

> QUICK FIX Try the Goldwell Color Glow range of shampoo, conditioner and treatment products, each priced at $25.95. There’s a care-and-protect solution for all hair hues.

While summer is the worst time of year for leaching the pigment from your luminous locks, winter can be every bit as harsh for colour fade. In some cases, it may even be worse, as the sun doesn’t seem to be as strong, hidden behind clouds. However, UV rays are every bit as powerful even through cloud cover. Ensure you use a colour-locking – or boosting – shampoo and conditioner and always ask for a colour glaze with your salon colour. “Red hair fades faster than any other hue,” says Anthony, “because the red pigment molecule is actually smaller than other colours. There are some at-home tricks you can use, though, depending on the depth of colour you want. Rinsing hair with carrot juice can add vibrancy to copper locks, as can a beetroot juice rinse.” Brunettes should avoid too-light highlights, which can quickly turn brassy. “Instead, look towards caramel hues that will make the face seem candle-lit,” advises Anthony. “Blondes have brassiness and yellowing to contend with, too, so it pays to use a clarifying shampoo once a week for deep cleansing. Brunettes and redheads could do the same to avoid dullness from product overload.” MORE ›

HAIR S.O.S

Hair ages for two primary reasons – firstly, because you’re getting older (doh!) and also because of damage from chemical styling (such as colouring, straightening or perming) and physical styling (with heated appliances). Ironically, a woman of 20 who bleaches her hair and uses straightening irons regularly but neglects its condition can have “older” tresses than someone of 65 who lovingly tends them. The years do play a significant role, of course. Menopause can occur at any time during adulthood, but most commonly during the late 40s to mid-50s. During this time, as the protective effects of oestrogen decline, your hair can become thinner, finer, limper and drier. And of course the passing years make it greyer, which can also create changes in hair texture. Proper hair care is just as important as good skin care because unhealthy, lacklustre locks do more to dent a youthful image than wrinkles. Luckily, the anti-ageing beauty boom has also moved into hair care. New ranges are being launched at a rapid rate, targeting 40-plus women, including cleansing, conditioning and styling formulas to repair, rehydrate and revolumise, restoring more youthful shine and bounce. Try Alterna Caviar Anti-Aging Moisture Shampoo, $37.95; Kérastase Age Premium Bain Substantif Rejuvenating Shampoo, $35. As you get older, using a conditioner after shampooing may no longer be enough, so weekly deep treatments such as hair masks are a good solution. The best time to use them is on the weekend, when you can relax and let your hair “marinate” for longer.

CAN I USE SERUMS IF I AM PREGNANT OR BREASTFEEDING? Always check with your doctor before using anything you might be unsure of. Generally, though, unless your serum is a vitamin A-based formula, it should be safe to use and may even be helpful in controlling hormonal pigmentation during pregnancy.

HOW DO I APPLY MY SERUM?

MOISTUREBOOSTING SERUMS

SERUMS FOR THE EYE AREA

RADIANCEENHANCING SERUMS

In these formulas targeted skin-drenching ingredients such as hyaluronic acid (a hydrating agent that’s present naturally in the skin), algae extracts and antioxidants combine to quench thirsty skin, address moisture balance and help to soothe and protect against irritation. They directly address dryness and dehydration, helping to re-plump and rejuvenate and prevent damage that arises from a compromised protective lipid barrier.

Eye serums aim to brighten, tighten and de-puff the orbital area. It’s important that their formulations can be readily absorbed because the fine skin here can’t cope with overload. Expect to find ingredients such as light-reflecting pearls to counteract dark circles, antioxidants to ward off lines and wrinkles, and cooling botanical extracts to soothe and de-puff.

These pack a punch with hydrating ingredients that plump fatigued skin and improve cellular function. They address lacklustre, fatigued complexions, greyness and dullness of skin tone. The secret lies in illuminating ingredients in their formulas, such as tiny mother-of-pearl particles or other light-reflecting treasures that brighten upon application to instantly boost dreary skin, and which will work with continued use to give skin serious long-term sparkle.

Always follow the instructions, but most will indicate that you apply the serum liberally to cleansed skin before moisturising. Don’t massage the product in – it could emulsify the ingredients on the surface of the skin before they have time to penetrate and act within the dermis. Wait until it is fully absorbed and skin is dry to the touch before applying moisturiser or treatment cream.

SKIN-FIRMING SERUMS

BALANCING SERUMS

LIGHTENING SERUMS

Expression lines, wrinkles, fine lines and furrows are all addressed with these potent serums, which include ingredients such as collagen and elastin (to plump wrinkles from within the skin), retinol and AHAs (to rejuvenate and smooth the skin’s surface), and antioxidant-rich fruit extracts like pomegranate (to protect against free radical damage and help prevent more furrows).

Targeting loss of elasticity and firmness in skin of any age, firming serums seek to relieve temporary sagging due to loss of moisture as well as the longer-term effects of collagen degradation and gravity. They directly target the loss of collagen and elastin that leads to jowling or sagging skin on the face and neck by using specific tightening ingredients such as egg white extract, to deliver an immediate and cumulative firming effect on the skin.

Blemished, unbalanced, oily and changing skin will benefit here. These serums are tailored to troubled skin with irregular oil flow or open pores. They can also be useful in treating skin disturbed by postpregnancy hormonal fluctuations or menopause. Ingredients often include non-irritating substances, such as glucosamine or rice extracts, to help smooth and refine pores. They may also include calming essential oils, such as lemon and bergamot.

These potent formulas target pigmentation, age spots, freckles and generally uneven complexions, improving clarity and consistency of skin tone. They employ ingredients including vitamin C, licorice extracts and other such brightening or whitening ingredients.

cell renewal S E R U M S POWERHOUSES FOR YOUR SKIN These powerhouse formulations assist your body’s own natural process of skin cell regeneration to attain the highest possible level of youthfulness and health. Try: Origins Plantscription AntiAging Serum, $97. Treats skin nasties like photosensitivity, shedding, stinging, redness, dryness, flaking and itching.

Try: Priori Cellular Recovery Serum, $220. Contains a complex that mimics our own DNA-repair enzymes to help reverse the damage caused by free radicals.

It’s hard to look after your feet during the day because you’re constantly running around. So bedtime is the perfect opportunity to revive them. Clarins Foot Beauty Treatment Cream, $40 (all prices RRP), is formulated with nourishing shea butter and cashew nut oil, arnica to relieve tiredness and swelling, and mattifying Laponite powder which absorbs and traps water on the skin’s surface, creating an antiperspirant action. Massage this treatment into your feet before bed and pop on a pair of socks to trap in the nutrients.

LOVEABLE LIPS Lips tend to dry up the most at night, so prepare them well with lip balm before bed. Dr Spiller Biocosmetic Aloe Vera Lip Balm, $15.70, contains vitamins B and E, which leave lips plump and soft.

BEDTIME TRESSES HAPPY HANDS Overnight is a great time to rehydrate hands. Just like with your feet, apply a nourishing cream and lock in the moisture (and keep your sheets clean) with a pair of gloves, or try this two-inone product: Bliss Glamour Gloves, $80 a pair, available from Myer. The gloves last for up to 50 uses and feature a self-activating gel treatment of olive oil, jojoba oil, grapeseed oil and vitamin E.

Treat your hair to a deeply nourishing hair mask while you sleep. It’s a great chance for key nutrients to penetrate any damaged strands and replenish protein. Dove Damage Therapy Intensive Repair Overnight Treatment, $10.99, contains fibre actives and a micro moisture serum to nourish and seal each hair strand from the root to the tip, reducing future breakage and restoring the strength of your hair. Also try Pantene Pro-V Night Miracle Leave On Creme Serum, $9.99, and Alterna Caviar Anti-Aging Overnight Hair Rescue, $48.60. MORE›

SLEEPY SKINCARE

SPOT TREATMENT

Overnight is the best time to give your complexion a high dose of anti-ageing ingredients. While we sleep our body releases growth hormones which restore skin elasticity and smooth wrinkles, and at the same time lying under the covers raises body heat and increases absorption. Apply an antiageing night cream every night to take advantage of these benefits. Bobbi Brown Hydrating Intense Night Cream, $88 (all prices RRP), boosts the skin’s ability to recharge and rehydrate during its repair process, plumping fine lines and improving tone and texture.

Acne is still a problem for some adults. To keep it under control, apply a nightly treatment like The Body Shop Tea Tree Blemish Fade Night Lotion, $23.95, a lightweight, soothing gel-creme that helps minimise the appearance of scarring and discolouration on blemished skin. The tea tree controls oiliness and replenishes moisture for a smooth and repaired look.

NAIL STRENGTH For brittle nails, apply a nail strengthener overnight. OPI Original Nail Envy Nail Strengthener, $34.45, contains strengthening properties of hydrolysed wheat protein and calcium, which penetrate into the cuticle throughout the night. It’s an award-winning product that’s been created to help nails grow harder, longer and stronger.

POWERFUL POTIONS For a more intensive delivery of moisture and antioxidants to the skin, try a mask like Origins Drink Up Intensive Overnight Mask, $45. The potent ingredient list replenishes moisture levels and gives skin a powerful barrier repair complex. It contains avocado and apricot kernel oils, as well as Japanese seaweed to repair the skin’s barriers and prevent future dehydration.

YOUTHFUL EYES As we mature, the thin area of skin around the eyes shows signs of ageing, including fine lines, wrinkles, hyperpigmentation and loss of contour. Treat it to a high dose of antioxidants and vitamins overnight for a bright-eyed look the next morning. Applied at night, Dermalogica Age Reversal Eye Complex, $95, delivers a highly active blend of retinol, peptides, vitamins and botanicals that fortify underlying tissues and increase blood microcirculation.

Star sleep secrets Some stars make it a priority to get enough sleep. Actressturned-singer (and recent triathlete!) Jennifer Lopez, 41, swears by eight hours a night as her top beauty secret. “Sleep is my weapon,” she says. “I try to get eight hours a night. I think what works best is sleep, water – and a good cleanser.”

The key to va-va-voom success, especially for women in their 30s, 40s and 50s, is to find the right shade – one that will truly flatter your skin tone and make you feel and look fabulous.

lip prep and practice

> With any red lipstick, go easy on the blusher and heavy eye make-up to avoid the “dragon lady” look. In Europe, many women frequently wear no make-up except for a sweep of red lipstick. > Red lipstick looks best with clothes in solid colours such as black or white. If you want to wear a red dress or coat, stay away from clashing shades. For example, never wear an orangebased lipstick with a wine-coloured jacket. > Michael suggests applying the lipstick like

Fractional RF Technology Combining the benefits of deep tissue contouring with RF Fractional Resurfacing for superior results

“The Alma Accent XL has been Clearskincare’s preferred skin tightening and skin resurfacing technology for the past 12 months. The Accent’s are easy to use, the results are consistent and our staff and clients love it. The treatments are comfortable with a combination of local anaesthetic and skin cooling and downtime is generally 1-2 days, a much more doable option for most clients than 7-10 days downtime with fractional CO2 Laser.” Dr Philippa McCaffery Clearskincare
