There’s no better time than winter to tackle the pigmentation, sunspots, UV damage and general lacklustre skin that come about from overexposure to the elements. For one thing, the UV onslaught is less than in summer because we spend less time outdoors (although daily use of a high-SPF sunscreen is still essential). Also, no-one expects you to be as social in winter, so a few days of downtime after more invasive age-reversing procedures is likely to go unnoticed by friends. What’s more, the healing process from resurfacing treatments will be far more comfortable when the mercury isn’t soaring, because we sweat less in winter. “Perspiration contains salt, which can cause further irritation to skin,” says Lisa Sullivan-Smith from The Clinic in Bondi Junction, Sydney. “You don’t want to expose deeper layers of skin in hotter months for this reason alone. “There’s also a chance that if you have treatments in the summer, your pigmentation will just bounce straight back. You can’t escape the summer sun in Australia, so winter is by far the best time to start undoing your sun damage.”

Sun exposure and the resulting sun damage manifest in many ways, which often dictate the type of treatment or procedure required to address the issue.

Bold, bright and glamorous – red lipstick can turn even shy and retiring types into instant divas. And you’d better get used to this season’s outbreak of scarlet fever. Pure red lipstick is as classic as a Chanel suit and it’s making an even bigger splash this winter in versions ranging from value-for-money lippies priced at less than $15 to designer tubes costing up to $50. You have to be careful, though, when you’re playing with passion’s war paint. Even on young faces, red lipstick can turn a small mouth into a mean line and drain any other colour from the complexion. The key to va-va-voom success, especially for women in their 30s, 40s and 50s, is to find the right shade – one that will truly flatter your skin tone and make you feel and look fabulous.

The high-drama look of red lipstick shifts the emphasis away from the eyes to the lips, so they should be in top condition. Gently exfoliate the lips with a toothbrush or your facial exfoliant to remove any flaky skin. Apply a lip balm and blot away any excess. Then apply a light layer of foundation and allow to dry. Because of the strong pigments in red lipstick, it’s best to outline the lips with a lip pencil (to prevent the colour bleeding) and fill in using a lip brush, says Natasha Severino, make-up artist for YSL Beauté. “After lining the lips, apply one layer of lipstick and then dust with translucent powder to make the lipstick stay put,” she advises. “Finish by applying a second coat of lipstick with the lip brush.”

Nearly all make-up artists agree on the basic rules of matching a red lipstick to your skin tone. Michael Brown, Lancôme’s national make-up expert, says reds falls into three major categories: orange, pink and blue-based. “If a red lipstick looks out of place on your face, you’ve usually picked the wrong shade,” he explains. Women in their 30s look best in pink-based reds as they’re softer and more feminine, so the mouth doesn’t get all the attention. “These work well with most skin tones as they complement pink-toned skin and brighten yellow-toned skin,” says Michael. Orange-based reds are brighter and warmer. “These look really good on blondes, whether they’re pale-skinned or tanned,” Michael explains. “They’re also a great colour for women in their 50s who prefer to wear less eye make-up and are also finding their complexion has lost colour or has a tendency to flush.” Blue-based reds recall the perfectly painted lips of Hollywood’s golden era – elegant and sexy at the same time. “They particularly suit darker and brunette women with olive or yellow-based skin,” says Michael. “But women of any age can wear them if they have the defined features to carry it off. Chic women in their 40s wear blue-reds very well, and they’re a great choice for winter.” Even with all the best advice in the world, many women are still intimidated by red lipstick. According to Michael, though, there is another option: brown-based reds, which he calls the “I’m not sure” reds. “They’re more of a brick-style red and are a great compromise for women who don’t want to attract too much attention or feel too ‘done up’,” he says. “They’re a very flattering choice for women of all ages and skin tones. Brown-reds brighten skin tone, balance a strong eye look or offer a focal point for those who prefer softer eye make-up. A top choice for winter, too.” MORE ›

REVOLUTION The average age of the Australian woman is getting older (38.1 years at last count) and her life expectancy longer (83.67 years). Yet she is looking younger than ever before. A quiet revolution has occurred over the past five years — a boom that has defied world economic trends. Aesthetic anti-ageing was once the province of the rich, famous and infamous and involved dramatic procedures followed by lengthy recuperations at exclusive hideaways. Today someone from any walk of life can walk into a doctor’s surgery or cosmedical clinic and emerge a veritable lunchtime later, leaving unwanted years behind. Yet the revolution has been quiet because although rejuvenation procedures are now considered mainstream, largely affordable and socially acceptable, many people still prefer to keep them a secret, even from family and friends. “The use of fillers has probably increased 300 to 400 per cent over the past five years and now rivals the frequency of

muscle-relaxing injections,” says associate professor Greg Goodman, of the Dermatology Institute of Victoria. “Lasers have probably increased about 100 per cent over the same time period. THE QUIET

REVOLUTION CONTINUED The Cosmetic Physicians Society of Australasia estimates that Australians spent nearly $450 million on non-invasive and minimally invasive treatments in year to March 2010, representing an increase of 30 per cent on the previous year. “We believe this growth can be attributed in part to the increase in choice for dermal fillers and an evolutionary leap in laser technology,” says CPSA president Dr Gabrielle Caswell. “The most popular non-surgical treatments are proving to be medical-grade skin care, dermal fillers (for full facial sculpturing), anti-wrinkle treatments with Botox and laser resurfacing. “Dermal fillers have grown enormously in popularity. For a long time there was only one brand of hyaluronic-acid filler. Now there are competing brands that are gel-like and pain-free as well as longer-lasting, with very low risk of adverse events.” These new-generation fillers — that come with names like Juvéderm, Restalyne and Esthélis — allow doctors to choose different thicknesses according to what part of the face is being treated, giving more natural results. “Australians have a large amount of sun damage. The new laser technology delivers good results with little downtime and now gives the doctor great tools to tackle deeper wrinkles and stubborn problems such as previously permanent pigmentation (melasma),” says Dr Caswell. “I find that my patients are outcome-driven and therefore unlikely to spend money on ‘fluff and buff’ but prefer proven medical technology,” she adds. “They’re strategic in their spending and want outcomes, not promises.” Leading plastic and cosmetic surgeon Dr Warwick Nettle,

from Sydney’s Silkwood Medical, says Australians’ uptake of cosmetic medicine has been “slowly increasing over the past 10 years, with a major increase over the past five years. Our business has more than doubled and that’s against a background of an explosion in providers of these services.” Dr Nettle attributes this boom to several factors. “Surgical and non-surgical rejuvenation is becoming more acceptable as baby boomers are ageing and have more disposable income,” he says. “Baby boomer women are increasingly in the workforce, especially in senior professional and management roles. They’re often working with people many years junior to them, and so feel the need to look fit and well. “The market for well-paid jobs is very competitive — people need to look good. And more women are divorcing and feel they need to freshen up to be attractive for the next phase of their life — competition is tough. “On top of that, procedures are getting better and better. Compare today’s injectables (biocompatible hyaluronic acids lasting nine or more months) with yesterday’s offering (potentially allergy-causing collagen lasting three months, with luck). “There is an explosion in what can be done, both surgically and non-surgically, from lasers and injectables to minor and major surgery. And the internet has made this explosion immediately available to people.” The phenomenon is not restricted to the capital cities, either. “When I first opened my practice (Eyra Medical in regional NSW), my colleagues thought I was quite mad and that country women ‘liked things natural’,” says Dr Caswell. “As one of my patients once put it, ‘It’s about trust (in your doctor) and access. We may be in the country, but we use the internet and read the world’s media. We know what’s available, but we don’t want to risk a bad job.’ That just about says it all.”

No matter what our skin type, we’re all prone to dehydration, especially during the winter months. “About 20 per cent of the skin’s moisture sits above the epidermis,” says Dee Davies from the Clear Skin Clinic in Double Bay, Sydney. “This is where most moisture-loss occurs, but in winter moisture loss can occur below the epidermis, too. The result is severe dehydration, which can make lines and wrinkles not only look more pronounced, it can actually cause them to deepen at a faster rate.”

“Eczema and dermatitis are interchangeable terms for itchy, scaly and inflamed skin,” says Dee. “There are many forms of the condition, most of which worsen in winter due to lack of humidity, a reduction in skin hydration and an upset barrier function. Anything that changes the lipid layer will aggravate the problem, such as overuse of soaps or over-cleansing the skin.”

WHAT TO DO ABOUT IT “Cut back on those long, hot showers and baths,” recommends Dee, “and increase humidity at home with a bowl of water in each room and by lowering heat levels. During the cooler months, skin can become prone to a red, ruddy appearance. This is likely to be due to sensitive conditions, like rosacea, or small, superficial broken capillaries on the surface of the skin. They tend to flare up because of extreme temperatures and other triggers, such as central heating, hot showers, alcohol, spicy foods and hot drinks.

WHAT TO DO ABOUT IT Clearly, it’s important to avoid the triggers listed above, but using calming, strengthening and healing skin care will work, too. “Avoid anything with active ingredients such as acids or vitamin A,” advises Associate Professor Greg Goodman, who’s a practising dermatologist. “Ingredients including Azelaic acid, green tea and other antioxidants are ideal. Always use non-irritating cleansers and moisturisers, and a water-based or light liquid sunscreen applied daily is essential.” He adds that any product used on the skin should be able to be easily removed. “You don’t want to apply anything that is too claggy or thick and requires a lot of rubbing when cleansing,” he says. When trying to soothe irritated, dry skin, you need to be an educated consumer. WHAT TO DO ABOUT IT “Anything that stimulates the skin is to blame,” says Dee, “so you need to keep it cool and calm. Steer clear of alcohol, spicy foods and caffeine and wear moisturiser with sunscreen. Avoid harsh cleansing and over-exfoliating. The colder, drier air of winter draws so much moisture from the skin that the hands may become cracked, chapped, sore and red, and the nails more brittle and prone to splitting. This can be aggravated by bathing in hotter water than usual. “Nail infections and hangnails can result from as little as one neglected cuticle,” warns beauty therapist Liza CurwenBegg, founder of Skinlab in South Yarra, Victoria. Avoid further irritation by minimising exposure to hot water.

The fragile skin around the eyes comes under greater fire from winter’s icy winds and chilly temperatures and can quickly become dry. If left untreated, this will lead to thinning and flaking and take on a crepey appearance. Crow’s feet may also worsen if not treated properly. Other causes for dry skin around the eyes include excessive washing in hot water or an allergic reaction to cosmetics and make-up ingredients.

WHAT TO DO ABOUT IT Don’t simply swap to a richer eye cream or moisturiser thinking it will ward off crow’s feet. “The skin here is finer than on the rest of the face and it can’t absorb heavy, oil-based formulas, and this can lead to millia, the little white bumps that sit under the skin,” says Liza. Legs, elbows, knees and heels can suffer in winter when humidity is low, as can shins and upper arms, which may experience dry, small bumps. Being wrapped up in woollen and synthetic fabrics might also lead to irritation. Dry skin often produces itching, and repeated scratching can make the area rough and thickened. This can then crack or cause dermatitis when the skin becomes red and inflamed.

WHAT TO DO ABOUT IT Keep skin smooth with daily dry body brushing, which will not only slough away dead cells, it will also boost lymphatic drainage, helping to keep mottled, dimpled skin at bay. Or exfoliate with a body scrub two or three times a week, being careful not to overdo it, or you may ultimately make dryness worse by stripping the skin. Similarly, too much hot, soapy water – a “treat” we can indulge in too much during the cold weather – depletes our natural oils. Also known as the traditional lift, this is major surgery that separates the skin, pulling it back and making it look tighter. On the day of surgery, incision lines are marked, as is the “vector” – or direction – in which the deeper tissue needs to be lifted. The patient is anaesthetised (general or twilight) and the face is infiltrated with a large volume of local anaesthetic and adrenalin to reduce bleeding. The skin is lifted and the deeper (or SMAS) fibro-fatty muscular layer is shifted to the right vector and suspended. The excess skin is then trimmed and the wound is closed meticulously to ensure minimal tension or distortion to the ear. Some structural fat graft may also be added to the cheeks to further enhance results.

WHO IT’S FOR People who have severe laxity and skin excess in the face and neck (the latter known as the “turkey gobbler”). The aim is a natural, refreshed look, with a fuller cheek, defined jawline and neck without the over-pulled “wind tunnel” look. Scars will be well hidden when fully healed, so you can wear your hair however you like.

FULL FACELIFT RECUPERATION Downtime is usually two weeks, with sutures out after a week and no strenuous exercise for four. Pain can normally be managed with Panadol or Panadeine. The S-lift gets its name from the smaller (than the traditional facelift’s) S-shaped incisions made in front of the ear. Through this incision, the surgeon can lift skin and underlying tissues. Using either general anaesthetic or twilight anaesthesia, an incision is made in the creases in front of the ear and curled behind the ear lobe. The skin of the cheek is elevated for whatever distance is necessary to move the fibrous/muscular layer beneath that gives the strength to the lift. The skin is trimmed and closed. As we age, the tissues of the cheek and middle part of the face, which give a youthful healthy appearance when we are young, descend “southward”. The mid-facelift procedure repositions the mid-face fat-pad to a more youthful plane. Usually performed under general or twilight anaesthetic, it lifts the cheek fat back up onto the cheekbones, raises the outer eyebrows and lifts the corners of the mouth. Incisions are hidden in the temple hair. It may often be performed in conjunction with a necklift or browlift.

Rejuvenates the bottom third of the face by tightening the face’s underlying structures that slacken with age, forming jowls and a fleshy neck. Aquamid, which lasts about seven years, could be used when both the doctor and patient have seen results with shorter-lasting fillers and are confident of the outcome. This means the injected muscle can no longer contract, allowing wrinkles to relax and soften. It can treat crow’s feet and creases under the eyes. Placed strategically by an experienced injector in vertical “glabellar” lines between the eyebrows and at points around the brow, Botox can also make the eye area look more “open” and youthful. You may opt for surgery if you have weakened or loose neck muscles, sagging skin as the result of the ageing process or weight loss, or localised deposits of fat in the front part of the neck (the “double chin”). Weak muscles contribute to the “turkey gobbler” or appearance of neck bands. Necklifts are typically combined with other surgery such as S-lifts or browlifts, but can be performed alone as some people notice a hereditary chubby neck even from their late teens.

If surgery isn’t needed (or desired), non-invasive skin-tightening procedures such as Ultherapy can assist with firming moderately saggy skin. The full face and neck are generally treated but the neck can be done separately. A handpiece delivers “passes” of acoustic sound energy (sound waves) section by section across the treatment area. More youthful lip contours are easily achieved with injectable dermal fillers like Juvéderm and Esthélis. They can add definition to a softened lip border and enhance the cupid’s bow. Which injectables are used in a non-surgical nose job? The injectable fillers used to build up the nose can be temporary hyaluronic acid fillers, such as Juvéderm, Restylane, Fortélis or Esthélis, or Radiesse, which is a filler comprised of calcium-based microspheres suspended in a water-based gel. These last between 12 months to two years. Because the nose is relatively static in comparison to other areas of the face where fillers are used, they tend to last longer in this area. A longer-lasting alternative is the filler Aquamid, which is a sterile hydrogel consisting of 97.5 per cent water and 2.5 per cent cross-linked polyacrylamide. It will last up to 10 years or more. A surgeon will decide on the filler to use on an individual basis, taking into account skin type and the irregularity that needs to be addressed. During the first consultation, a saline solution is injected into the nose to allow for a preview of how the nose will look. Pictures are taken and the patient can then use the time until the next appointment (usually one week) to decide if that’s the look they want. Then, on the second appointment, the appropriate filler will be injected. This takes approximately one hour, and the patient can return to work immediately afterwards. There is no downtime, although some patients may experience slight swelling and tenderness.

This would lessen the visual impact of the tip,” says Dr Caswell. How to gain balanced features “A balanced nose on a balanced face is the epitome of facial beauty,” says Dr Nettle. “The bridge and the tip need to be related in the golden ratio of tip projection to nose length of 1:1.618. This is a recurring pattern in nature. “Harmony is enhanced by not making the nose too big or too small for the face, by not being too broad or too narrow. For instance, in thin-faced, thin-skinned patients it’s good to have a fine rather than broad nose and vice versa for the broader-faced, thicker-skinned patient.” Dr Marcells adds, “The angle of the nose tip can vary – shorter people look better with a slightly upturned nose, while taller people look better with a less upturned nose. This has to do with the angle we look at them from – on shorter people, we look at them from above, and if their nose is downturned, we won’t see their nostrils. Similarly, we look up at taller people, so an upturned nose means we’re looking straight into their nostrils.”

When is a nose job medically necessary? In some cases of nasal irregularity, there will be no option but to have surgery. “If you want to improve the function of your nose, surgery is required,” says Dr Gerarchi. “And if you wish to improve the shape and definition of the nose, particularly the tip of the nose, you need a rhinoplasty procedure. “Likewise, fillers can’t reduce the size of the nose or make it narrower, and surgery is also your best option for things like making significant changes to the angle of the tip, or correcting a droopy tip.”

RHINOPLASTY OFFERS PERMANENT IMPROVEMENT FOR NASAL FLAWS > THE PROCEDURE Surgery takes place after careful assessment to determine the correct size and shape of nose for the individual and a full functional assessment. A general anaesthetic is administered and the patient must spend the night in hospital. Most rhinoplasties today are performed using an open-structure procedure, which affords better access to the tip of the nose. This begins with a small incision across the skin between the nostrils and then involves reshaping the bone and cartilage skeleton of the nose. A dorsal hump can be removed, a nose straightened and a tip refined. Internal work then addresses any functional issues. There may be a tiny scar, but this fades as the nose heals.

Initially, there will be swelling and bruising, with most patients sporting black eyes. A cast is worn over the nose and there may be a small stitch under the nose. The nose will feel blocked, but this usually subsides after a week. Patients can expect to return to work within two weeks, and the best results can be seen six to 12 months after surgery.

SUN UNDONE There’s no better time than winter to tackle the pigmentation, sunspots, UV damage and general lacklustre skin that come about from overexposure to the elements. For one thing, the UV onslaught is less than in summer because we spend less time outdoors (although daily use of a high-SPF sunscreen is still essential). Also, no-one expects you to be as social in winter, so a few days of downtime after more invasive age-reversing procedures is likely to go unnoticed by friends. What’s more, the healing process from resurfacing treatments will be far more comfortable when the mercury isn’t soaring, because we sweat less in winter. “Perspiration contains salt, which can cause further irritation to skin,” says Lisa Sullivan-Smith from The Clinic in Bondi Junction, Sydney. “You don’t want to expose deeper layers of skin in hotter months for this reason alone. “There’s also a chance that if you have treatments in the summer, your pigmentation will just bounce straight back. You can’t escape the summer sun in Australia, so winter is by far the best time to start undoing your sun damage.”

THE SIGNS OF SUN DAMAGE Sun exposure and the resulting sun damage manifest in many ways, which often dictate the type of treatment or procedure required to address the issue.

pigmentation Solar pigmentation is a direct result of sun exposure. “In youth, we call it freckles; as we mature, we refer to it as sun spots,” says Lisa. “The skin is under attack from UV rays and it produces pigment to defend itself. With age, this reaction becomes worse.” And how do we deal with it? “Fraxel Re:Store Dual is the gold-standard treatment for removing pigmentation,” adds Dr Jeremy Cumpston, from Ageless Clinics in Randwick, Sydney.

surface veins and broken capillaries Heat generated from the sun’s UV rays causes blood to rush to the skin’s surface in an effort to cool it down. As a result, the veins or capillaries swell, their walls burst and a cluster of broken capillaries or veins may appear on the surface of your skin – most commonly around the nose and across the cheeks, but sometimes on the chin. Cutera Pearl Fusion This treatment combines both Pearl and Pearl Fractional lasers, for full facial epidermal skin renewal. The Pearl laser treats the skin’s top layer with pulses of light to remove sun damage and brown spots, while the Pearl Fractional laser uses heat to create tiny holes in the skin to remove sundamaged tissue and stimulate new skin and fresh collagen. The treatment is ideal for people with light to medium skin tones with mild to moderate sun damage, and can address pigmentation on the face, neck, décolletage and hands. It also helps to firm the skin and minimise the appearance of lines and wrinkles. It works by affecting only a fraction of the skin at a time with thousands of microscopic RF pixel spots – a bit like Photoshop editing software for the face! It can be used to improve pigmented lesions such as age spots, sun spots and melasma (skin discolouration). Pixel RF is “colour blind”, so sun exposure or skin type are not such important issues as with other technologies – although of course it’s best not to tempt fate by overdoing UV exposure or neglecting sun protection! After treatment, skin generally feels softer, looks smoother and tighter, and pores are usually smaller. Skin may look pink for a couple of days before results begin to show, with further improvement over the following weeks. Two to four treatments are recommended, at fourweekly intervals. Fraxel Re:Store Dual This new-generation fractional laser is designed specifically for pigmentation and sun damage; it’s good for the face, neck, décolletage and hands. It boasts two unique heads – a thulium laser and an erbium laser. Practitioners can swap between the two, depending on individual skin issues. The erbium laser is deeper and causes formation of collagen and elastin, addressing wrinkles and sagging skin. Age spots, pigmentation and surface veins are then blasted away with a thulium treatment. The procedure requires multiple laser “passes” over the treatment area. A prickling sensation is felt initially, then slight heat. There will be one or two days of redness and mild swelling, followed by five to seven days of light peeling, after which skin will appear softer and brighter. Expect to pay $150 to $165 for one treatment, with treatment packages available.

LimeLight Photofacial LimeLight is a programmable IPL treatment that can be dialled up or down according to the issues being addressed. “Different programs are used, labelled A to C, “says Dr Gabrielle Caswell, president of the Cosmetic Physicians Society of Australasia. “Program A works on redness and blushing, B is for vascular issues and brown spots, dyschromia or skin yellowing due to sun damage, and C is for really freckly and pigmented faces that have little redness, and is also great for the backs for hands.” A beneficial side-effect is that collagen production is triggered. Patients can have one, more powerful treatment, which will involve some post-procedure swelling and downtime, or a series of gentler treatments. The Herbal Aktiv Peel This relies on a solution of natural stimulants, exfoliants and regenerating and soothing herbs, which is massaged into the skin to trigger a peeling, resurfacing effect. HOW IT WORKS

Anyone can choose to undergo breast augmentation, although the most common patients are young women whose breasts have never grown, or women who have breastfed their children and have consequently experienced sagging or “empty” breasts. Remember, the size and shape of the implant you have must be in proportion to your body for the best and most natural-looking results. A very thin woman with a natural size A cup would not realistically be able to go up to a D or E cup. The skin is very elastic and will stretch, so if the implants are too big, the skin will not be able to hold them up and they will appear to sag.

Breast augmentation is typically performed under general anaesthetic and is approximately a two-hour operation. Some surgeons will perform it using twilight sedation and local anaesthetic, which can reduce costs and sometimes recovery time, as you’re not affected by the general anaesthetic. The procedure, which is also referred to as ”augmentation mammoplasty”, involves making an incision and placing an implant, usually made from a cohesive silicone gel, into a pocket either behind the chest muscle or between the muscle and the breast itself, although variations and combinations of these can be performed to suit an individual’s anatomy (see opposite).

Typically women who are unconcerned about their breast size, but instead have issues with their placement and loss of firmness, are ideal candidates for a breast-lift procedure (which is also known as mastopexy). Their breasts are beginning to sag, or indeed have already sagged and are pointed downwards. Women whose breasts have enlarged and consequently sagged due to gravity, through weight gain or loss, or through pregnancy or hormonal changes associated with menopause, may be good candidates for the procedure, too. The ability to breastfeed is very unlikely to be affected by having had a breast lift.

HOW IT WORKS

A few incision techniques can be used, which your surgeon will determine. Surplus skin, tissue and fat are removed and the surgeon sometimes performs liposuction to remove excess fatty tissue. The nipple and areola are then lifted to a higher position and the skin and tissue are stitched into place.

Expect to take at least two weeks off work after a breast lift. There’ll be pain and discomfort, so it’s important to take things easy – even coughing can hurt. Lifting anything – even your kids – is off limits, as are bending down and driving. Expect to see best results several months later, when swelling subsides and your breasts have settled into their new shape and position.

liposuction THE IDEAL CANDIDATE People who have tried to lose weight and have stubborn fatty deposits that won’t shift are suited to this procedure, as is anyone who has disproportionate collections of fat on the tummy. It has to be noted, however, that liposuction is not a weight-loss alternative, but rather a technique used to shape the body and create a sleeker contour. The patient should also have a good quality of skin, as it needs to be able to spring back into shape, without looking lax or sagging.

HOW IT WORKS

A liposuction procedure is performed under general anaesthetic, although some surgeons are using twilight sedation in a sterile environment, which allows the patient to move around for better access and results. It generally takes about two hours. Small incisions are made in the groin and around the belly button, and a metal cannula is inserted to remove fat.

AFTER THE PROCEDURE Patients should be prepared to take a few days off work and will need to wear compression garments for up to six weeks. There will be swelling, bruising and some discomfort that can be managed with oral pain relief. Strenuous exercise should be avoided for six weeks or more, although a gentle power walk is fine after seven to 10 days. Best results will become apparent about three months after treatment, although you’re likely to see a noticeable difference within days or weeks, as swelling subsides. Few people have cash on tap to pay their doctor upfront, or even on the day of the surgery. Many people are turning to specific health loans to have a procedure done as soon as possible, and according to Jodie Laing, CEO of Mediplan Healthcare Finance, people no longer need to wait for their cosmetic procedures. Before surgery, she gave up smoking and has been taking vitamin C and zinc supplements to aid her recovery, and she feels they’ve significantly improved her skin texture. She disliked their droopiness, which she’d experienced for many years, and this problem had worsened recently as a result of moderate weight loss through an exercise program she’d been undertaking. Helen felt that her breasts were too big, with all her breast tissue falling down the bottom and out the sides. This was making her embarrassed, with a loss of confidence in her appearance, as well as causing her physical discomfort, particularly when exercising. She’s in good health and had recently stopped smoking. I saw her for two pre-operative consultations to assess and advise her of her best options. I recommended she have a “short scar” breast reduction and lift, and explained the surgery to her extensively, including the potential risks and the recovery process. A lesser concern for Helen were early signs of ageing of her face, with frown lines around her eyes, and for this we performed musclerelaxing injections in my office before her surgery, as well as starting a skin-care program. Helen’s surgery took place under a general anaesthetic and took about two hours. It went exactly as planned, and I was very pleased with the result. She stayed one night in hospital and then went home. She’s making excellent progress, has had very little pain and her breasts are healing very well. EYES BRIGHTER SICK OF BEING TOLD SHE LOOKED TIRED, TRACY HARRIS TRIED TEAR-TROUGH REJUVENATION AND SHED YEARS FROM HER FACE > THE PROBLEM “No matter how much I spent on hair, clothes or make-up, every time I went out I’d hear repeatedly how worn out I looked,” says Tracy, a publishing professional in her 40s. “As a busy person in daily contact with others, that irked me – especially if I’d slept for more than seven hours the night before. No matter how much corrective make-up I applied, the under-eye darkness and hollowness made me look sleep-deprived.”

the treatment regimen After a 20-minute consultation, in which Dr Calfas clearly outlined the tear-trough rejuvenation procedure to Tracy, the method and risks involved, he started injecting the area where her cheekbone area merged with the softer tissue beneath the eyes. She opted not to use Emla, which is a numbing cream normally offered for these procedures and is worn for up to an hour before injecting begins. Tracy recalls that the level of pain she experienced was a “six out of 10 on my personal Richter scale” but that, even so, the sensation was manageable. Her entire eye area was completed within 40 minutes and the results were visible immediately. A softening of the area was apparent as well as a reduction in depth under the eyes, with the darkened “cavities” restored to a plumper and more rested look. Tracy says the area felt sensitive for the remainder of the day and she was advised not to rub or touch it if possible, so that the filler could “assemble” itself naturally, but she had no problem with following that suggestion.

> WHAT I DID AND WHY I wanted surgery to regain my pre-baby body and give my confidence a boost. In late 2010, Sydney plastic surgeon Dr Robert Drielsma performed an abdominoplasty (tummy tuck), mastopexy (breast lift) with implants and some liposuction around my hips and thighs. At first it’s a little daunting when you wake up with all these major changes and you’re in discomfort at the same time. treatment Gael Donaldson, 42, is director of marketing and PR at a leading Sydney hair salon, RAW Anthony Nader. Having been overweight since she was a child, Gael had become morbidly obese by her 30s, plagued by emotional and physical issues. “I’d tried every single diet known to man,” she says. “Nothing could beat my lack of willpower.” In March 2010, Gael decided to have gastric (or lap) banding surgery. Her journey started at 116kg. She now weighs 84kg. Her goal weight is 75kg, and she’s well on her way to achieving it. Around 11,000 Australians a year undergo gastric banding.

I’d been overweight since childhood and it’s affected my whole life. It’s made me have to think about things that other people take for granted: How will I keep up with the others? Is everyone laughing at me? Are my friends ashamed of me? If I order that food will people think “look at the fat chick gorging herself”? I had a loving family but I was ridiculed at school and would hide the hurt with humour. I ate secretly, using food to compensate for my feelings of low self-esteem and loneliness. You could say I “ate” my feelings. I pretty much did whatever I could to try to feel accepted as a teenager, and I succumbed to peer pressure. I’m not proud of sacrificing my values along the way. I can see that I’ve craved reassurance in present and past relationships and friendships. I have to learn that I do deserve good things and that I am a good person. Having surgery to improve my appearance and my life has been part of that learning process.

> TREATMENT PLAN Five years ago, at the age of 37, I started investigating the idea of having a gastric-banding procedure. A friend had had one and was dropping a lot of weight. Mine was performed by Dr Craig Taylor, of Sydney’s OClinic. It cost a lot of money for me (around $10,000) but my lovely mother helped me out. I’m so happy with the results. treatment The gastric band creates a smaller stomach pouch, so that less food is required to feel satisfied. Because the band has reduced the size of the passage through which food passes, it also takes longer to digest. “My procedure was performed laparoscopically and there was hardly any pain, and barely any scarring,” says Gael. “I took a week off work but I really didn’t need all that time. Not once have I felt any regret. After the surgery I was on a fluid diet for about two weeks while I healed. In weeks three and four you go through a soft, mushy food phase and then at week five you graduate to eating a wide range of everyday solid foods such as chicken, fish, soft meats, pasta, rice, fruit and vegetables. Now I never feel hungry. I’ve learnt only to eat and drink when my body tells me to and not at dedicated meal times. Life is easier in that I can walk upstairs without huffing and puffing, I walk further and more often, and I now feel more confident – I don’t feel people are sniggering at me.”

I think I’m ready for a facelift, but I’m scared that I’ll look like I’ve had work done. Are there new techniques that deliver a natural, non-‘lifted’ result? There are indeed new techniques that can ensure a natural look – many people have had facelifts these days and you’d never know. Modern techniques reshape the underlying layer of connective tissue, unlike the facelift procedures of the 1970s and 1980s, which stretched only the wrinkled surface of the skin. The main determinants of a natural look rely on tightening the underlying muscles of the face in the appropriate vertical oblique direction, and not pulling the skin too tight, as well as paying close attention to a myriad of details such as avoiding hairline alteration and pixie ear.

I really need a nose job to correct the large hook in my nose, but my friends tell me it will affect my breathing. Is this true? Breathing is evaluated in all cases prior to nose surgery. One of the big changes in rhinoplasty procedures today is that almost all patients have internal nasal surgery performed to improve breathing, as well as external appearance surgery. So, in fact, the opposite of what your friends tell you is usually true. Cosmetic rhinoplasty tends to improve rather than compromise breathing.

Having children has left me with a lax tummy and stubborn areas of fat on my core and around my bottom. Is it safe to team a tummy tuck with liposculpture, or is it best to have these done separately? Safety has nothing to do with the operation itself or combination of procedures, but rather with patient selection and the surgeon’s skill. It’s perfectly safe to team an abdominoplasty with liposculptural procedures and I generally perform them both at the same time, as I believe liposculpture further perfects the result of the tummy tuck. My only rule is that patients should not be on an operating table for this type of elective surgery for longer than eight hours, if possible.

I have large, heavy breasts that cause me a lot of back pain. How simple is a breast-reduction procedure? Will it leave scarring? And do they regain their size? I broke my nose in a sporting accident some years ago. Aside from it now being misshapen, I also experience breathing problems. Can rhinoplasty fix the way my nose works, as well as the way it looks? As an ear, nose and throat specialist as well as a facial plastic surgeon, I’m able to deal with both functional breathing problems and the aesthetic appearance of your nose. I use techniques based on openstructure rhinoplasty. These are more demanding on the surgeon and take longer than other methods to perform. However, I believe they help to ensure the basic foundation of the nasal structure is kept strong. People tell me I look tired or unhappy a lot. I think it’s because my brow is lined and my eyelids are heavy. How can this be improved? Rejuvenation of the upper face can restore a younger and fresher appearance to tired-looking eyes and the ageing face. Drooping eyebrows, excess skin and fat in the upper eyelids, and bags under the eyes are signs of upper-face ageing. Browlift surgery is performed to re-suspend the eyebrows to their natural and more youthful position. The female eyebrow usually sits just above the bony upper rim of the eye, whereas the male eyebrow usually sits on the rim. This surgery can help remove “hooding” over the upper eyelids. I most commonly use the endoscopic (small incision) browlift technique, although I may use other techniques if they’re more appropriate. Surgery is performed to remove excess skin and fat in the upper lids and also correct bags and sagging under the eyes. Small incisions around the eyelids or behind the lower eyelids heal very well. Eyelid surgery may be performed alone but is often performed with browlift surgery, and also with other procedures BEFORE such as facelift and necklift surgery. The lady on the right had left-sided browlift and upper lid blepharoplasty to restore symmetry to the eyebrows and remove excess hooding of the lids. The post-op view shows a natural, refreshed look with improved symmetry. Before and after browlift and eyelid surgery, the skin of the upper face can be kept youthful by non-surgical procedures such as laser and light treatments, microdermabrasion, micropeels, soft-tissue fillers and muscle relaxants.

How can I look younger and fresher without resorting to plastic surgery? I’d suggest using a combination of laser or IPL (light) treatment and dermal fillers, which come in a range of consistencies and can be used in different parts of the face for different effects. Thicker fillers such as Juvéderm Voluma can correct “sag and bag”, while lip augmentation doesn’t need such a thick substance, so I might choose Juvéderm Ultra XC – there’s no pain and it’s a very flexible gel for soft, poutable lips. To remove redness from the face, we can use a vascular device such as Dual Yellow (for larger vessels) and then IPL LimeLight. Fractional laser resurfacing is great for reducing deeper lines and wrinkles and dealing with sun-damaged skin. The results last five to ten years and the treatment can be repeated. I’ve heard a lot about fractional laser treatments and how they can give a smooth, even skintone and firmer skin. How do they work? The patient above has had dermal fillers, anti-wrinkle and Cutera Pearl Fusion (a combination of Pearl and Pearl Fractional) laser resurfacing treatment. Fractional laser devices work by causing ablation of small spots on the skin. For example, when using the Pearl Fractional device with a single pass, about 76 per cent of the skin remains intact. I want to have some fillers done, but what if I’m unhappy with the results? Is there anything I can do to correct them? New-generation fillers are based on hyaluronic acid, which occurs naturally in the body so there’s no need for allergy testing (unlike with collagen-based products). They swell less once injected and some are packaged with pain relief, making the procedure truly walk-in walk-out. They can be dissolved with hylase, an enzyme that’s injected to dissolve the filler, providing the patient with ultimate flexibility with their total look.

Is it possible to have a necklift as a single procedure? My face is relatively line-free and not really sagging, so I don’t require a surgical procedure, but my neck is quite crepey and is showing some sag. A necklift can be performed as stand-alone surgery or in combination with other facial cosmetic procedures. It’s very common to feel your neck has aged out of proportion with the rest of your face; people are often bothered by a pouch or sagginess that develops under the chin while the rest of their face appears relatively beautiful. A necklift is an excellent procedure for removing this fatty deposit and loose skin and creating a more defined cervicomental (neck to chin) angle. However, a necklift has only a limited benefit in improving the jowls that form along the jawline. If these are an issue for the patient, a facelift as well as a necklift is likely to be more beneficial.

My teenage daughter has a bump in her nose. What age is best for her to consider rhinoplasty? I’m worried about her doing it too soon in case her face is still changing as she matures. Traditionally, nasal surgery is delayed until most facial growth has occurred, commonly agreed to be at about 16 years old. This is because of concerns about interfering with facial growth, although evidence for this is limited. In reality, if there are reconstructive or significant functional concerns, we’d generally proceed with surgery. By taking time during the initial consultation, being a good communicator and therefore allowing the patient to freely discuss all of their likes, dislikes and desired goals, I can very quickly gain a grasp of whether the patient is realistic about what they expect the outcome of any surgery to be.

What does a consultation with you entail? I treat the initial consultation like a chat, to make the patient feel comfortable in a nonjudgemental environment. But it’s designed so that the basic information – general medical background, family medical history, previous surgery and the like – is gleaned.

No-one is immune to frizz, and it can get out of control in the Australian winter, due to high levels of rain and humidity. Dry and dehydrated hair and a compromised hair cuticle – often the result of bleaching, chemical processing and over-styling – intensify the problem, drawing in moisture from the humid environment, causing hair to frizz. “The classic trick that we hairdressers adore is to rinse hair in cold water. It closes the cuticle and imparts shine and manageability,” explains Anthony Nader, from Raw Anthony Nader in Darlinghurst, Sydney. “Of course, you should also always use a creamy shampoo and conditioner with a frizz-defence formula to rehydrate the fragile hair shaft.” In terms of your haircut, look towards stylish, long layers, says Anthony. “They’ll give the hair weight and fullness and make any bouts of frizz less obvious. Keep the ends of your hair blunt, rather than flyaway, and avoid razor cuts and thinning scissors, which will simply make frizz seem even worse.” Cut back on heated styling, too. If you must use straightening irons, iron the outer layers of the hair only, and if you can’t go a day without blow-drying, use one that has ionic technology, such as the Parlux 3500 Ceramic & Ionic Dryer, $184.95. “Ionic technology uses negative ions to strengthen and smooth the hair strand from within, leaving it shiny and glossy,” explains Anthony. “The negative ions work cleverly to reduce the size of water molecules while styling.” Always angle the nozzle of your hairdryer down, so that the heat is directed along the hair shaft as opposed to directly at it, or upwards. Use a low-speed, low-heat setting and dry hair slowly in small sections.

02 limp, oily locks with split ends HAIR S.O.S

Limp, lightweight hair is mostly genetic, but with winter’s damp conditions limpness can affect us all, even those with the thickest of hair. You can easily pump up the volume with a few simple tricks. “A shorter haircut looks fuller and provides manageability without weighing hair down,” says Anthony. If you must wear hair long, “be careful not to over-layer it, which causes wispiness,” advises Hamish Glianos, from People Hairdressing in Sydney. “A structured shape with longer layering works well.” Watch the oil balance in your hair, too. It can go into overdrive in winter to compensate for harsh temperatures that sap moisture from the hair. An oily scalp can often exacerbate limpness and goes hand in hand with dry, fragile ends. Always condition the ends of your hair well to prevent further splitting (apply product from mid-lengths down). Try De Lorenzo Instant Accentu8 Conditioner, $21.95. When styling, Hamish recommends using a high-hair-inducing mousse. “It will help build volume from within. Curls are by nature unruly. They’re tricky to manage and suffer dryness faster than any other hair type. “A great cut can help,” says Anthony, who has developed a unique cutting technique to deal with them. “I call it ‘Seamless Texture’. It re-proportions the pyramid effect of hair sitting flat at the top and wider at the bottom. If want to dry hair straight, you won’t get a layered feel, and if you want to wear it curly you get a beautiful shape all over.” Opt for a semi-permanent colour to help control the dryness and tame the curl, use curl-specific hair care and never rub the hair when towelling it. “Just blot it dry,” says Anthony. “Use a

super-absorbent towel designed specifically for hair.” Jayne Wild, from Wild Life Hair in Sydney, suggests using a cloth kitchen towel. “Don’t be afraid to overload product,” adds Hamish. “Curly hair needs the moisture that products can provide. Any kind of wet or shine wax will help, as will curl-smoothing lotions. Apply to hair then leave. Allowing hair to ‘air-dry’ can often be the best solution. Dry hair equals damaged hair and the culprits are varied – stepping outside with wet or damp hair (this can cause breakage in cold temperatures), over-styling, over-processing or simply infrequent trims. “The six-weekly trim is essential to prevent split ends and weak strands,” says Anthony. “This is especially so in winter, when cold winds and cool temperatures can cause hair stress.” Of course, if hair is damaged beyond repair, the only solution is to opt for a chic, short crop and begin growing your locks long again. However, if it’s not super-stressed there are ways to nurture it back to health. While summer is the worst time of year for leaching the pigment from your luminous locks, winter can be every bit as harsh for colour fade. There are some at-home tricks you can use, though, depending on the depth of colour you want. Rinsing hair with carrot juice can add vibrancy to copper locks, as can a beetroot juice rinse.” Brunettes should avoid too-light highlights, which can quickly turn brassy. “Instead, look towards caramel hues that will make the face seem candle-lit,” advises Anthony. “Blondes have brassiness and yellowing to contend with, too, so it pays to use a clarifying shampoo once a week for deep cleansing. Hair ages for two primary reasons – firstly, because you’re getting older (doh!) and also because of damage from chemical styling (such as colouring, straightening or perming) and physical styling (with heated appliances). Ironically, a woman of 20 who bleaches her hair and uses straightening irons regularly but neglects its condition can have “older” tresses than someone of 65 who lovingly tends them. The years do play a significant role, of course. Menopause can occur at any time during adulthood, but most commonly during the late 40s to mid-50s. Gently exfoliate the lips with a toothbrush or your facial exfoliant to remove any flaky skin. Apply a lip balm and blot away any excess. Then apply a light layer of foundation and allow to dry. Because of the strong pigments in red lipstick, it’s best to outline the lips with a lip pencil (to prevent the colour bleeding) and fill in using a lip brush, says Natasha Severino, make-up artist for YSL Beauté. “After lining the lips, apply one layer of lipstick and then dust with translucent powder to make the lipstick stay put,” she advises. “Finish by applying a second coat of lipstick with the lip brush.”

Nearly all make-up artists agree on the basic rules of matching a red lipstick to your skin tone. Michael Brown, Lancôme’s national make-up expert, says reds falls into three major categories: orange, pink and blue-based. “If a red lipstick looks out of place on your face, you’ve usually picked the wrong shade,” he explains. Women in their 30s look best in pink-based reds as they’re softer and more feminine, so the mouth doesn’t get all the attention. “These work well with most skin tones as they complement pink-toned skin and brighten yellow-toned skin,” says Michael. Orange-based reds are brighter and warmer. “These look really good on blondes, whether they’re pale-skinned or tanned,” Michael explains. “They’re also a great colour for women in their 50s who prefer to wear less eye make-up and are also finding their complexion has lost colour or has a tendency to flush.” Blue-based reds recall the perfectly painted lips of Hollywood’s golden era – elegant and sexy at the same time. “They particularly suit darker and brunette women with olive or yellow-based skin,” says Michael. “But women of any age can wear them if they have the defined features to carry it off. Chic women in their 40s wear blue-reds very well, and they’re a great choice for winter.” Even with all the best advice in the world, many women are still intimidated by red lipstick. According to Michael, though, there is another option: brown-based reds, which he calls the “I’m not sure” reds. “They’re more of a brick-style red and are a great compromise for women who don’t want to attract too much attention or feel too ‘done up’,” he says. “They’re a very flattering choice for women of all ages and skin tones. Brown-reds brighten skin tone, balance a strong eye look or offer a focal point for those who prefer softer eye make-up. A top choice for winter, too.” MORE ›
