DFA cares about your health and wellbeing and wants to ensure we are providing support and assistance in every way possible. We are committed to helping make your health a priority and continue to team up with MyEvive to administer the Happy. Healthy. Safe. program designed to incorporate a way of living that encourages improved overall health and provides the tools to do so.

MyEvive also shows you all of your benefits resources in one place, helping you discover, connect with and get answers about your DFA benefits.

PROGRAM OVERVIEW Medically enrolled employees and spouses can avoid the monthly $30 surcharge in 2023 by completing the below steps by the deadline provided. Plus, medically enrolled employees can earn a monthly $30 incentive.*

If your benefits effective date is after Friday, July 1, 2022, you are not required to participate in the Happy. Healthy. Safe. program.

If you are not tobacco free, you must sign up for the MyEvive Cold Turkey HealthQuest or Lockton Nurse Advocate tobacco cessation program.

If you did not meet three of five biometrics: Register for Lockton Nurse Advocate by Friday, Sept. 30, and complete two coaching calls by Wednesday, Nov. 30.

Note: It is recommended that calls be completed at least two weeks apart.

Employees or spouses who need a reasonable alternative or a medical exemption for the above activities may contact Lockton Nurse Advocate at 1-888-251-2260. *The $30 incentive for employees enrolled in a High Deductible Health Plan (HDHP) is a contribution to their Health Savings Account (HSA), and employees enrolled in a Preferred Provider Organization (PPO) will receive a reduced premium.

FORGOT YOUR PASSWORD? Follow the steps to log in to MyEvive on the web or app, then when asked for your login credentials, click “Forgot password?” You will then receive an email to reset your password.

COMPLETING YOUR HEALTH ASSESSMENT For medically enrolled employees and spouses with a benefits effective date prior to Saturday, July 2, 2022: Log in to MyEvive, click the “Happy. Healthy. Safe.” card under “My Benefits,” and complete the “Health Assessment” activity. Once you finish the short quiz, this will be marked as complete.

Non-medically enrolled employees are welcome to complete the health assessment to learn more about ways to improve your well-being.

COMPLETING YOUR BIOMETRIC SCREENING QUEST DIAGNOSTICS APPOINTMENT • Click the “Happy. Healthy. Safe.” card under “My Benefits” • Click on the “Annual Biometric Screening” activity. This will direct you to Quest Diagnostics where you can schedule your screening at a Quest facility Scan to go to my.questforhealth.com

• Spouses go to my.questforhealth.com and use registration key: DFA2022, or call 1-855-623-9355 • Log in with your Unique ID and date of birth (DOB). Your Unique ID is the last 5 digits of your SSN followed by your 6 digit DOB in MMDDYY format. If you’re a dependent and having trouble logging in, try null+six digit DOB (ex: null010160)

ANNUAL PHYSICAL WITH YOUR PHYSICIAN: • Click the “Happy. Healthy. Safe.” card under “Featured Benefits”

• Have your physician complete the Healthcare Provider section of the form, including signature. Quest will not be able • Click on the “Annual Biometrics Screening” to process incomplete forms activity • Download your personalized Physician Results Form: In the Annual Biometrics Screening section, click “Take Action.”

You will be taken to Quest Diagnostic’s dashboard, where you can download the Physician Results Form. Click on “Order Form” then click on “Download Form”

• After your doctor completes the form, there are two options to submit: 1. Have your doctor fax the completed form to the fax number indicated on the form.

2. You may upload the completed form to Quest Diagnostics electronically using the Upload Form button on my.questforhealth.com

Note: Annual physicals to have the Physicians Results Form completed are 100% covered by your medical plan and do not require a co-pay on any of our plans. Additional testing and care for conditions found during your exam are subject to your policy. Please allow approximately 14 days for this activity to be marked complete. It will be marked as done once MyEvive receives your completion information from Quest.

ON-SITE BIOMETRIC SCREENING Your location may be offering on-site screenings in partnership with Quest Diagnostics as an option to complete your biometric screening in 2022. Please check with your local human resources representative for more information.

Can I access MyEvive in Spanish?

Currently, you may only access MyEvive in English.

How does my spouse access MyEvive?

Spouses should go to MyEvive on the web (dfa.myevive.com) or app, then create their own login credentials (they will need your employee ID). Your employee ID can be found in ADP. On your dashboard in the top right corner is your picture or initials. Click on your profile and you will find your employee ID. How do I determine if I’ve met all the Happy. Healthy.

Safe. program requirements?

Log in to MyEvive, then click on the “Happy. Healthy. Safe.” card to check on activity and program completion status.

You’ve completed the program when all your activities are marked as complete.

What does my spouse need to complete to avoid the surcharge?

Medically enrolled spouses with a benefits effective date prior to Saturday, July 2, 2022, must complete both the health assessment and biometric screening by Wednesday, Aug. 31, 2022, to avoid their monthly $30 surcharge in 2023.

How do I/my spouse access the health assessment on MyEvive?

Does my spouse need to complete the health assessment and biometric screening?

Medically enrolled spouses with a benefits effective date prior to Saturday, July 2, 2022, must complete both the health assessment and biometric screening by Wednesday, Aug. 31, 2022, to avoid their monthly $30 surcharge in 2023.

Does my spouse need to complete the tobacco cessation or health coaching calls to avoid the surcharge?

No, your spouse does not need to complete tobacco cessation or health coaching calls to avoid the surcharge. To avoid the surcharge, your spouse only needs to complete the health assessment and biometric screening. Spouses are not eligible for the incentive and do not need to complete the health coaching calls or tobacco cessation for any part of the program.

How long will it take to get my biometric screening results?

If your Physician Results Form is complete with the required risk factors, you will receive your results online within 10 days and a paper report in the mail within three weeks of submitting the form. Please allow 14 days for your biometric screening activity to be marked complete in your Happy. Healthy. Safe. program on MyEvive.

If it’s been over 10 days since you uploaded or faxed your form and you haven’t received a “Results Are Ready” email (indicating your form is processed) or a Physician Form Rejected email (indicating there is an issue with your form), call the Quest Diagnostics’ Service Center at 1-855-623-9355 for a status update.

What sections must my doctor fill out on my Physician Results Form?

Your doctor must complete the health care provider section of the form, including signature, date, test(s) performed and UPIN/NPI. Your doctor will need to fax the completed form to 1-844-560-5221, or you may upload the completed form to my.questforhealth.com using the Upload Form button on the dashboard.

When do I need to submit my Physician Results Form to Quest?

You are responsible for ensuring your form is returned to Quest Diagnostics by Wednesday, Aug. 31, 2022.

My Physician Results Form was rejected.

What do I do now?

If you submit an incomplete form and your email address is on file, you will receive an email notifying you that your Physician Results Form is incomplete. If there is no email address on file, you will receive a notification letter in the mail. Then, you will need to re-submit the completed form following the same submission steps again.

How do I determine if I’ve met three out of the five biometric requirements?

Check your biometrics results to see whether you meet three of five biometric healthy ranges. See page three to see the biometric targets.

How do I know if I need to schedule coaching calls with Lockton Nurse Advocate?

Calls with Lockton Nurse Advocate are an alternative to meeting three out of five biometrics. Employees who did not meet three out of five biometrics may contact Lockton Nurse Advocate at 1-888-251-2260 to register. It is recommended that you schedule your calls at least two weeks apart.

When do I need to schedule my coaching calls with the Lockton Nurse Advocate?

Coaching calls with Lockton Nurse Advocate are an alternative to meeting three out of five biometrics.

Employees who did not meet three out of five biometrics need to register for Lockton Nurse Advocate calls by Friday, Sept. 30, 2022, and complete two coaching calls by Wednesday, Nov. 30, 2022, in order to earn the wellness incentive in 2023. It is recommended that you schedule your calls at least two weeks apart.

What happens if I am unable to make my scheduled Lockton Nurse Advocate call?

If you are unable to make your scheduled appointment, you must notify Lockton Nurse Advocate at least 24 hours prior to the scheduled call. If there are no appointments available, you will be prioritized as soon as there are new appointments available. If you do not show for your scheduled appointment, you will receive an automated email letting you know that you will need to reschedule your appointment. If there are no available appointments and you did not provide the necessary 24 hours notice, you will not be prioritized once new appointments are available.

Can I schedule an appointment with the Lockton Nurse Advocate if my biometrics are not appearing in MyEvive yet?

Yes, anyone is welcome to participate on a voluntary basis even if you aren’t eligible for health coaching because of your biometric screening results. You can schedule an appointment with a Lockton Nurse any time by calling 1-888-251-2260, through the resource card on MyEvive, or by scanning the QR code below.

Can I complete Lockton Nurse Advocate coaching calls instead of completing the biometric screening?

No, you must complete the biometric screening in order to avoid the surcharge in 2023. If you do not meet three of five biometrics, you may register for and complete Lockton Nurse Advocate coaching calls in order to earn the wellness incentive in 2023.

What are my options for completing the tobacco-free program?

You may register for calls with a Lockton Nurse Advocate or the Cold Turkey HealthQuest on MyEvive by Friday, Sept. 30, 2022, and complete one of these Tobacco-Free Programs by Wednesday, Nov. 30, 2022.

The Lockton Nurse Advocate program consists of three calls and six modules. The Evive HealthQuest is an online program and takes four to six weeks to complete.

Why do I see that my incentive activities are worth points?

When you complete an activity like the health assessment or biometric screening, you will see your status update from 0/2 points to 1/2 points. This is a way to track your progress toward avoiding the surcharge or earning an incentive.

The requirements to either avoid the surcharge or earn the incentive remain the same and are based on which activities you complete, not a certain number of points earned.
