Specialized heart care … without skipping a beat Electrophysiology keeps heart in rhythm By Kim Hasty
S
irisha Reddy, MD, had already put in a long day’s work as a cardiologist at Fayetteville Heart Center. But as evening turned to night, Reddy nevertheless took the time to pull up a chair next to Brenda Melvin’s bedside at Cape Fear Valley Medical Center and patiently answer questions. “I thought to myself, ‘I know she has to be tired,’” said Melvin, a resident of Elizabethtown who had come to Dr. Reddy in need of treatment for atrial fibrillation. Her atrial fibrillation, which is an irregular and often rapid heart rhythm, followed a transient ischemic attack, commonly referred to as TIA. “She sat right down like she didn’t have anything else to do but talk to me. She listened.” Though she was facing a daunting medical procedure – cardiac ablation – Melvin said the care she received at the center helped put her mind at ease. Dr. Reddy, who specializes in electrophysiology, performed the procedure in June. “I think it’s very important to take the time to explain the procedure,” Dr. Reddy said. “It’s also helpful, with the patient’s consent, to involve someone other than the patient in the discussion.”
“Our hospital administration has been very supportive,” she said. “The community needs to know they can get this kind of care and that they need not drive 100 miles from home to get it.” Though about 30 percent of people with atrial fibrillation don’t experience symptoms, Dr. Reddy said early diagnosis is critical. In fact, Dr. Reddy said, the problem is sometimes discovered while a patient is being prepared for an important diagnostic or preventive procedure, such as a colonoscopy. “That’s one reason you have to have your regular medical checkups by your primary care physician,” she said. “With early treatment, the success rate of ablation is close to 80 percent. Word needs to get out about the importance of early referral.” Ablation, however, is not always required to treat electrical problems in the heart. “The first step is usually medication,” Dr. Reddy said. “It’s not about one treatment. It’s a process.” Certain factors increase the risk of atrial fibrillation, which can lead to stroke. The older a person is, for instance, the greater the risk of developing atrial fibrillation. People with heart disease — such as heart valve problems, congenital heart disease, congestive heart failure, coronary artery disease or a history of heart attack or heart surgery — also have an

TIA is a temporary blockage of blood flow to the brain, and while it doesn’t cause permanent damage, it is often a warning sign of a stroke.

Melvin, a graduate of Fayetteville State University and a retired mental health social worker, considers herself blessed that she was able to receive excellent care and treatment and that she can again enjoy spending time with family and friends, including her supportive fellow members of Baldwin Branch Missionary Baptist Church.
“I had a knee replacement in March, and Brenda was driving me to physical therapy,” Locklear said. “We made it back home, but our cousin immediately took her to the hospital.”
“I feel pretty good,” Melvin said. “I used to stay active all the time, and I was always on the go, and now I can still go.”
From Bladen County, Melvin was transferred to Cape Fear Valley Heart & Vascular Center and to Dr. Reddy’s care. After the cardiac ablation procedure, which involves using heat or cold energy to block irregular electrical signals and restore a typical heartbeat, Melvin was able to return to her normal active lifestyle. Dr. Reddy is pleased that Cape Fear Valley is able to offer such specialized cardiac care.
“We’ve always been a close class,” she said. “We all have one ailment or another, but we’re hanging in there.”
She recently went on a trip to Myrtle Beach, South Carolina, with several of her classmates from Bladen Central High School.

‘Take back your health’ Kristen Wilkey is more active, regaining confidence after gastric sleeve surgery By Jami McLaughlin
I
n 2004, Kristen Wilkey was diagnosed with a medical condition that caused her to gain about 100 pounds in a year. To lose weight, she tried dieting, fasting, clean eating, the keto diet, and working out five to six days a week. Nothing worked.
weight,’’ Wilkey says. “After 15 years of fighting, I saw how wrong I was.’’ Originally from Columbia, South Carolina, Wilkey has lived in Fayetteville since 2008, when the Army brought her family here so she could be closer to her parents.
But she was hesitant to consider weight loss surgery.
She has three children. Her oldest daughter is 24 and an orthodontic assistant in Myrtle Beach. Her son is 22 and will graduate from the military’s Advanced Individual Training in January after which he will be stationed at Fort Meade, Maryland. Her younger daughter just turned 12.
“Before my weight struggles, I thought gastric surgery was the ‘easy’ way out and also had the misconception that it was for people who were too lazy to lose

“I was petrified about having surgery, but the more I read and researched, the more convinced I was that the procedure was safe and maybe my only hope,” Wilkey says. She consulted Ijeoma Acholonu Ejeh, a general surgeon with Cape Fear Valley General Surgery and a fellowship-trained bariatric surgeon. They determined the gastric sleeve was the best option for her since she did not have an excessive amount of weight to lose. “For my personal circumstances, the sleeve was the better route,’’ Wilkey says. “I take a daily thyroid medication, and the incidence of malabsorption is much less with the sleeve.’’ “Dr. Ejeh made sure I understood all of the pros and cons to having the gastric sleeve and allowed me to actively participate in the decision-making.” According to Dr. Ejeh, bariatric surgery offers the longest sustained weight loss when other methods have failed. The sleeve can be a good option for those who fit the criteria.
“Choosing the sleeve is first patient preference followed by proper evaluation of their medical conditions and determining if the sleeve is the best option to lose their excess weight and resolve their associated medical conditions,” says Ejeh, who has performed bariatric surgery for the past 20 years along with minimally invasive general and advanced surgery. Ejeh says there are several things to be considered when trying to determine which option is best for a patient. Criteria are set by the national bariatric society, and insurance companies determine whether a sleeve or full bypass surgery is covered. “There are several ‘hoops’ the surgeons and the patients must jump through before approval for surgery,’’ Ejeh says. “This is inevitable because this is not emergency surgery; it is elective surgery.” Wilkey said Ejeh was amazing to work with. She also credited Dr. Kelly Van Fossen, another general surgeon, who helped with her surgery. “My entire experience at Cape Fear Valley was exceptional, from pre-op testing to discharge,’’ Wilkey says. “The nurses, CNAs, EVS and transporters were incredibly professional, and my care was top-notch. I also never felt rushed during appointments and cannot say enough about the staff.” Ejeh says patients should expect followup appointments and assistance when it comes to the surgeons and their staff. She says patients are fully evaluated, including consulting with a psychologist before surgery. “More importantly, we work to evaluate that there are no latent conditions that could be exacerbated after surgery and the challenging diet and exercise regimen,” Ejeh says.

In Wilkey’s case, she had done everything she could to lose weight before undergoing gastric sleeve surgery.
“I like to think that bariatric surgery is a proven weight-loss option with a lasting positive effect,’’ Ejeh says. “Several patients may view it as a ‘last resort’ after all other methods have failed.”
A little over a year after her surgery, Wilkey says she has lost 80 pounds.

“I am able to walk without feeling like I have to stop or getting winded,’’ she says. “I am able to enjoy activities with my daughter again and just enjoy living life. Now, I’m usually the one wearing

see. Having the sleeve has been a great experience, and I only wish I had done it earlier.” She has even discussed future surgery with Fayetteville plastic surgeon Dr. Leif Nordberg. “Dr. Nordberg does excellent work, and I’ve discussed the possibility of having plastic surgery but want to lose as much as I can before I do that,” Wilkey says. Care when — and where — it’s needed Cape Fear Valley’s new orthopedics residency program boosts care in the community. By Jami McLaughlin

laying pool and shooting darts were favorite pastimes for Jeremy Perry, who moved to Fayetteville six years ago from upstate New York for the warmer weather and to reunite with family. He bought a house and was setting down roots when he suffered an injury at work. 22 | 2024 Medical & Dental Directory
“I was hauling fuel in my tractor-trailer, and when I went to pull the hose off, I felt a snap in my arm,” says Perry. “I started losing feeling in my fingers and hand, up my right arm.” Perry said his fingers and hand began to curl up, which led him to Ben Levine, MD, an orthopedic surgeon in Fayetteville.
“I had to stop doing some of the things that I loved to do or try to do them with my left hand. Dr. Levine said that it was my ulnar nerve, and so we started down the road of surgery and therapy,” Perry says. At age 33, Perry had developed ulnar nerve palsy, a condition that can affect the ability to make fine movements or perform routine tasks. He had four surgeries and worked with a therapist multiple times a week to repair the nerve damage in his hand and arm. Now that Perry has regained feeling and function in his hand and arm, he has been able to open his own business in construction land management, working with heavy equipment. He says being able to see a local
physician was “absolutely amazing.” “The procedures were here, the appointments were easy to make, and I didn’t have to travel,” Perry says. “Plus, I can’t say enough about Dr. Levine. His bedside manner is amazing. You can tell when he talks to you, he cares about you. He wanted to know more about me, so he knew how to help me. Getting that function back in my hand and arm was life changing.”
Underserved specialty Levine, a hand and upper-extremity orthopedic surgeon, moved to Fayetteville in 2019. He and Dr. Stephen Kouba will be the two doctors on the staff of the new Valley Orthopedics and Sports Medicine, part of Cape Fear Valley Health, when it opens this month. The clinic is at 1219 Walter Reed Road. Dr. Levine says that orthopedics is an underserved specialty in the Fayetteville area. The goal is to bring in more doctors to serve the community through the hospital-based clinic. “We hope to expand over the next few years. The vision is to attract physicians in all the orthopedic subspecialties including sports; hand and upper extremities; foot and ankle; spine; oncology; and pediatrics,” says Dr. Levine. Dr. Levine is also the new associate program director of the Cape Fear Valley orthopedics residency program, which was accredited in 2022 and will welcome its first class starting in July. “We will be attracting new doctors through the program. It will be a draw,” Dr. Levine predicts. That is where Dr. Levine, Dr. Kouba and six other doctors will teach orthopedics residents. Michael Nagowski, CEO of Cape Fear Valley Health, said at the ribbon-cutting on Jan. 13 that the residency programs serve to bring in more doctors and that already 50% of their graduates have decided to practice in the community.

Dr. Levine is certain that the orthopedics program will succeed. “Orthopedics is competitive as a residency, so we will be able to attract high-quality surgeons to Cape Fear Valley,” he adds.
Army, health system work together The residency program is a joint effort between Womack Army Medical Center and Cape Fear Valley Health. “The residency program started with Dr. Ken Nelson at Womack in 2017. He brought me on board and, as I got more involved, I got more excited and wanted to be a part of it,” says Levine. “When he asked if I would lead as an associate director, I jumped at it.” He and Dr. Nelson decided to bring the military and civilian sides together to create a stronger program. “Where the Army and Cape Fear Valley
can do good residency programs on their own, together we can create a stronger, more superior residency program,” Dr. Levine reasons. The program will train four residents over a five-year residency period, two civilian doctors and two Army doctors. “It is a five-year residency program, so we will have 20 residents at the end,” Dr. Levine explains. “We want them to stay here and get involved in the community as well. Teaching to give back is also important.” Bringing the physicians to the community will have a major impact over the next five years, but in the meantime, Dr. Levine and other orthopedic surgeons are making a difference. Jeremy Perry can attest to that. “I can do things again,” Perry says. “(Dr. Levine) brought the use of my hard and arm back, and I’m thankful.” 2024 Medical & Dental Directory | 23

Confidence restored Reconstructive surgery helps patients dealing with traumatic disease or injury regain a positive outlook on body image. By Jami McLaughlin
A
boost in self-confidence and positive body image can make plastic surgery a sought-after procedure, especially after undergoing breast cancer treatment or suffering traumatic injuries in accidents. Dr. Leif Nordberg opened Cape Fear Valley Plastic Surgery almost three years ago as part of Cape Fear Valley Health to address the needs of breast cancer patients, including breast reconstructive surgery. “I see a lot of breast cancer patients. There are no other plastic surgeons in this area, and Cape Fear Valley recognized the need,” Dr. Nordberg says. Breast cancer affects one in eight women and continues to be one of the major health concerns in the nation, according to the American Cancer Society. “We try to turn a terrible, scary situation into something more positive,” Dr. Nordberg says. Losing a breast in a mastectomy or part of a breast in a lumpectomy can be traumatic, according to Dr. Nordberg. He offers two main methods of breast reconstruction: the use of implants or creating a new breast using the patient’s own tissue with a transverse rectus abdominis, or TRAM, flap. Breast augmentation, one of the more sought-after surgeries Dr. Nordberg performs, can help patients address cup size and projection; improve symmetry; and restore volume. The office can also perform breast lifts and reductions. “Seventy percent of what we are looking at in our office is reconstructive. It’s an important part of our practice,” Dr. Nordberg says.

In other cases, a patient just wants to right something that has felt wrong to them. Dr. Nordberg says body contouring can address those issues. Brachioplasty, or arm lift surgery; liposuction; tummy tucks; body lift procedures; body contouring after weight loss; and the “mommy makeover” are other options. Dr. Nordberg says diet and exercise cannot always adequately address problems associated with pregnancy, weight gain or loss, and breastfeeding, which can affect confidence and body image. “The ‘mommy makeover’ is a comprehensive procedure that can include a tummy tuck, liposuction and a breast lift with implants. It helps our patients feel like themselves again,” Dr. Nordberg says.
New procedures The office has continued to grow, recently adding nonsurgical procedures such as skin care, neurotoxins, fillers and other treatments that complement surgery options. Tabitha Strickland, a registered nurse specializing in injections such as Botox and Juvéderm, was added to the staff for such procedures. “Tabitha is doing Botox and similar injections, and it’s becoming a growing part of our practice. She has had rave reviews on her bedside manner and techniques,” Dr. Nordberg says. The nonsurgical procedures also include CoolSculpting, which helps freeze unwanted fat cells for a nonsurgical body sculpting, and skin lesion treatment, which deals with skin cancer, sun damage, brown spots, moles and other skin issues. “There is something for everyone with our skincare line. Our topical treatments are suggested after an in-depth skin analysis,” Strickland says.

her legs without evasive vascular surgery. She had a 100% blockage, but we were able to take care of her. And if this wasn’t taken care of, she would have lost her leg,” says Bahekar.

He says peripheral vascular disease is most often a complication of uncontrolled diabetes, hypertension and smoking, which constricts blood vessels in the heart and legs. “If you don’t get those issues under control early on, it will develop and it’s a serious disease,” Bahekar says. Rojas is not diabetic, and she quit smoking seven years ago. She does have high blood pressure and she was diagnosed with chronic obstructive pulmonary disease before doctors discovered the blockage in her arteries. She quit smoking when her doctor to her that if she continued, she was at risk of having a heart attack. “I started cutting down until I finally said, ‘No more,’” Rojas says. But some of the damage had been done, and Rojas started feeling pain in her legs. “It started out with feeling numb, and my feet were always cold. Then it started getting painful to walk. Now I know there was no blood running through my legs. I had a full blockage,” Rojas says.

hen Ruth Pacheco Rojas started having leg pain when she walked, she sought help.
“Two years ago, I started running out of breath when I walked. Then I couldn’t walk much because not only did I need air, but it hurt,” Rojas says, 68. She was diagnosed with peripheral vascular disease and referred to a hospital in Raleigh. But her doctor there could not help, so she was referred back 34 | 2024 Medical & Dental Directory
to Fayetteville. That’s when she met Dr. Amol Bahekar. Bahekar has practiced interventional cardiology at Cape Fear Valley Health for 10 years. He also is the program director for cardiovascular fellowship at Campbell University. He knew immediately what Rojas needed and put two stents in her legs in midJanuary. That helped relieve Rojas’ pain and restore the blood flow in her legs. “I was able to open the blood vessels in

It felt like I was constantly scared, so I would pray nothing would happen.” Numbness is one of the first warning signs of peripheral vascular disease. “In the early stages, a person might feel numbness and some pain, but if left undiagnosed or untreated, it can develop into gangrene or result in an amputation or loss of legs,” Bahekar says. Bahekar says studies have shown that patients who undergo an amputation have a life expectancy of five years or less. It can also lead to heart disease. “Our goal in our practice is to screen and catch the signs early on so we can prevent this from happening and try to prevent amputations and bad outcomes,” Bahekar says. He says the biggest challenge is a lack of awareness about peripheral vascular

The leg pain they are experiencing is actually a problem with blood flow and not arthritis or nerve pain,” Bahekar says. The disease is becoming more common as 10% of Americans over 40 and onethird over 50 with Type 2 diabetes are diagnosed with peripheral vascular disease. The strongest way to prevent the disease is to take medications prescribed by a doctor to address diabetes and high blood pressure; to stop smoking immediately; and to make lifestyle changes such as eating healthy and exercising daily. “The more walking, the better for circulation. Exercise is crucial,” Bahekar says. She plans to keep all follow-up appointments and heed any advice her doctor gives. “Everyone was really friendly before and after my procedure,” Rojas says. The hospital also helped accommodate her primary language, Spanish. Born in Puerto Rico, she moved to Fayetteville three years ago to be near a close friend she considers family. She says the bilingual nurse who assisted with her surgery was a relief to her. “She explained everything to me and gave me the directions in Spanish. She talked me through it all. It was a huge

comfort to me and so much easier to hear everything in Spanish,” Rojas says. She also was grateful to be able to get care in Fayetteville as opposed to Raleigh. “I don’t drive, so I would need to rely on friends to take me to Raleigh. It was hard. But in Fayetteville, it’s a lot easier. They also have transportation you can use where they come and get you, then pick you back up to take you home,” Rojas says.
Bahekar agrees that patients do not need to go out of town for treatment of peripheral vascular disease. Rojas says she is feeling better by the day. “I’ve noticed a huge difference already. I’m walking, and it’s not hurting,” Rojas says. Her immediate plans are to visit her children and five grandchildren in Florida. “Now, I can walk without pain,” Rojas says. 2024 Medical & Dental Directory | 35
