Country SA News October 2021 Edition Message from the Chair Dr Simon Lockwood

It can feel a little frightening as a remote rural GP with the approach of our borders opening in South Australia on 23 November. Like many of you, I worry that COVID-19 will sneak into our communities with the potential to impact the way that we practice in primary care, as well as impact on our local hospitals. For those of us that provide support to local hospitals there is the fear of prolonged times to retrieval of critically ill patients, and overwhelming of local hospital capacity. In my days as a medical student some time ago I was taught the Five ‘Ps’ at medical school Piss Poor Planning leads to Poor Performance. So, what are we, at Country SA PHN, doing to prepare? As a PHN we can't do much about the approach of our local hospital. We trust that they will put the appropriate measures in place. Our focus, as a primary health network, is on how we can assist primary care. We are working collaboratively with multiple stakeholders at a Federal and State level to ensure that country SA primary health care providers will be fully prepared when SA re-opens domestic borders, eases restrictions and we adapt to ‘living with COVID-19’. This preparation includes representing primary care providers in country SA on:

the Primary Care Taskforce the Rural Health System Preparedness Roundtable the SA Living with COVID19 Mental Health Support ensuring solutions are found for COVID19 vaccination of our most vulnerable populations continuing to support COVID19 vaccination providers rurally providing PPE to primary care developing clinical pathways to help guide primary care in how to care for COVID19 positive patients in our communities and in the home by commissioning the development of these guidelines through the HealthPathways project.

For those not registered for HealthPathways I implore you to do so for a range of COVID-19 links and guidelines. An important element in Country SA PHN’s participation, is planning for the sustainability of general practice as a critical element of the primary health infrastructure of small rural communities. What are you doing in your daily work to prepare? Ultimately every service provider needs to look critically at their preparedness and review their pandemic plans and ensure they are ready. For those of you who have not taken the opportunity to look at your emergency planning for some time, we are providing funding for general practices to access an Emergency Response Planning Tool (ERPT) which can be useful in preparation.


The Tool is an up-to-date cloud-based plan, developed in collaboration with the Royal Australian College of General Practitioners. It provides practices with individually tailored information to appropriately prepare for, respond to and recover from, the impacts of emergencies and pandemics in your community – ensuring your staff safety, reducing the risk of financial loss and assisting with the smooth recovery of your practice. Our overall role in this preparation and planning ensures the country voice is heard and that the unique challenges facing you, our rural and remote primary health care providers and your communities, are met as we move into this next phase of the pandemic. Free Cultural Awareness Online Training Available Now Country SA PHN (CSAPHN) has purchased subscriptions to the RACGP Cultural Awareness Active Learning Module (ALM) for Non-Member GPs, general practice staff and health professionals in country SA. RACGP Members have access to the Active Learning Module via the RACGP GP Learning Website. The Introduction to Aboriginal and Torres Strait Islander Cultural Awareness in General Practice aims to improve the health of Aboriginal and Torres Strait Islander peoples by enhancing the cultural awareness of GPs, general practice staff, medical students, primary health care staff, and other health care providers. This 6 hour Active Learning Module aims to help participants to:

extend their knowledge about Aboriginal and Torres Strait Islander history and culture, explore how attitudes and values can influence perceptions, assumptions and behaviours in a clinical setting, and discuss key ways the practice team can be more culturally aware.

The module is accredited for 40 Category 1 QI & CPD points. The value of the subscription for Non-Members is $179, however CSAPHN is offering this free to health professionals working in country SA. The Lam Brothers – Supporting Workforce Shortage in Rural & Remote Areas Brothers Dr David and Nathan Lam are providing locum services in Port Lincoln and Streaky Bay while splitting their time as DJs across Australia. David was named South Australian/Northern Territory GP of 2019 by the Royal Australian College of General Practitioners while both David and Nathan were recently awarded the Fellowships in Advanced Rural General Practice, an accreditation David said was held by about 700 of 40,000 GPs across the country. In 2018, David also won the Medical School Award for Best Regional Teacher as a senior lecturer and Rural Medicine Coordinator for the University of Adelaide. The siblings graduated from the regional medical student program at the University of Adelaide and then developed their skills by working in regional South Australia. The experience highlighted to them that with Adelaide being the only major city in South Australia, there was a lack of resources throughout regional towns to provide the appropriate measures for health care systems. During the pandemic, the siblings created an educational podcast called "GP Lyf Hacks" to assist young practitioners and medical interns lacking face-to-face teaching. David said social media platforms also helped regional doctors communicate with patients located hundreds of kilometres away and that they had used them to mentor students interested in regional medicine. Both David and Nathan work alternative weeks in Streaky Bay providing support for Dr Victoria Bradley. In Adelaide both David & Nathan teach at the University of Adelaide, David as the Senior Lecturer for Adelaide Health Simulation and Nathan as the Coordinator of the Transition to Internship Program for final year medical students. As a Board Director of Country SA PHN, David is passionate about rural practice. He believes one solution was for state and educational policies to target younger doctors and entice them into the benefits and growth in regional health care, but he was not waiting around for this policy shift. In November David will be working at the Practice, the Streaky Bay Hospital and in his spare time DJing at the Streaky Bay area school formal.

We all have a role to play. In 2021 Mental Health Awareness is expanding from a week to a month. The theme for SA this year is, ‘We all Have a Role to Play.’ With almost 50% of Australians likely to be diagnosed with a mental illness in their lifetime, this leaves 50% more who know someone living with, or have experienced, a mental health issue.(1) If you are looking for information to support assessment, management and care for your patients experiencing mental health issues, HealthPathways SA has information for adults on Depression and Anxiety (including Perinatal), Suicide Risk, Psychosis, Deliberate Self-harm, Eating Disorders, Bipolar Disorder and Borderline Personality Disorder.


Recently the HealthPathways SA team of GP Editors and Clinical Coordinators worked alongside Lead Clinicians from Child and Adolescent Mental Health Services (CAMHS) and headspace to adapt pathways for children and youth. Pathways provide guidance to assess, manage and offer best practice treatment options. Importantly, they detail the most appropriate services and resources for use during consultations, that support and benefit your young patients and their family’s journey to recovery:

Support for aged care residents during COVID-19 Webinars GPEx and Country SA PHN recently delivered a three part webinar series to raise awareness of the importance of providing allied health services and support to people with dementia, and other residents with complex medical needs living in Residential Aged Care Facilities (RACFs) during COVID-19. A key aim of the education package is to inform health professionals of the temporary Medical Benefits Schedule (MBS) item numbers made available by the Commonwealth Department of Health to facilitate additional allied health and mental health services to aged care residents up to 30 June 2022:

Jane has 18 years of nursing experience, has postgraduate qualifications in Chronic Disease Management and was nominated in 2015 and 2017 for an Australian Primary Health Care Nurses Association (APNA) Best Practice Award in Chronic Disease Management for her accomplishments in Aboriginal Health. Jane’s unique nursing background, knowledge of primary health care funding mechanisms and Medicare Benefits Schedule (MBS) Item Number requirements is a valuable resource for general practices. The webinars include links to useful resources including pre-populated billing sheets that provide a detailed overview of applicable MBS items to facilitate and maximise the delivery of services to residents. Emergency Response Planning Tool (ERPT) Expression of Interest Country SA PHN is providing funding for general practices within country SA to access and use the ERPT. An up-to-date plan which best positions your practice to appropriately prepare for, respond to and recover from, the impacts of emergencies with your community. What is the ERPT? The Emergency Response Planning Tool (ERPT) is a cloud-based tool which assists general practices in better preparing for, responding to and recovering from the impacts of emergencies and pandemics. The ERPT is managed by Healthpoint ANZ and was developed in collaboration with the Royal Australian College of General Practitioners (RACGP). How does it work? The critical information entered into the ERPT is used to create an emergency response plan which is individually tailored to your practice, providing a roadmap to recovering from emergencies. Maintaining an up-to-date emergency plan helps ensure the safety of your staff, the smooth recovery of your practice and reduces the risk of financial loss. To ensure business continuity, the ERPT provides suggested contingency measures for staff so that, in the event of an emergency, they will know how to respond to:

a pandemic loss of power supply to the practice disruption to water and gas supplies disruption to telecommunications systems loss of IT systems or data loss of business records complete or partial loss of practice premises loss of medical supplies, equipment and furniture loss or non–availability of key staff Covid safety plan.

The response and uptake for the tool has been overwhelming and we have limited licences to offer. Shingles Recall Pilot Program Extended There are 22 practices in Country SA who are currently participating in the Shingles Recall Pilot program with over 2,000 people identified and contacted via SMS to receive the Zostavax vaccine free of charge. The participating practices have provided Country SA PHN with positive feedback about the Pilot with patients also receiving health literacy resources and education about the Shingles virus. Another benefit is the cost saving to the patient and preventable hospital admissions, saving the community thousands of dollars. The Department of Health recently announced Australians aged between 71-79 years will continue to be able to receive a free Zostavax® shingles vaccine, with the Morrison Government extending the program for another two years to 31 October 2023. With this announcement registration for the Shingles Recall Pilot has now been extended a further month and practises interested in registering in the Pilot can contact the Practice Support Team on support@countrysaphn.com.au.

National Cancer Screening Register Update The National Cancer Screening Register enables a single electronic record for each person in Australia participating in cervical and bowel screening. The Healthcare Provider Portal and integration with clinical software will allow healthcare providers to complete a range of activities, these include:

Accessing a patient’s bowel and cervical screening results and histories online, in realtime Submitting forms electronically Managing your patient’s participation Ordering a bowel test kit directly to a patient’s address

Healthcare Provider Portal Access Healthcare providers will be required to authenticate themselves through the creation of a PRODA account which is a secure online identity system used to enable access to government online services.


Healthcare providers will need to link their PRODA account to the portal upon initial registration. Linking accounts can be done with a Medicare Provider Number (MPN) or by requesting to be a delegate of a provider with a MPN who has already registered on the portal. Delegate access can be provided to practice staff. Nurse performing cervical screening tests can link their account using their existing Register Identifier Number or State and Territory Access Number. This is the number nurse cervical screening providers use when calling the NCSR to authenticate themselves. Clinical Information System Software Integration Currently, the Register is integrated with Clinical Information System, Best Practice and MedicalDirector. The integration with Communicare is currently in testing phase and aiming to go live later this year. Telstra Health will also engage other vendors with the aim of connecting with as many clinical information systems as possible to help us all reach the goal of creating a connected ecosystem. The integration essentially means the provider can access and submit register data from within a patient record from their desktop software, rather than accessing the portal.

First Annual Report on Quality Improvement Measures in General Practice General practitioners are the first point of contact for most Australians seeking healthcare, with 83.2% of the population seeing a GP in the previous 12 months. In 2018-19, nationally there were 158 million GP attendances, or 6.3 per person, up from 5.3 per person (113 million) in 2008-09 claimed through Medicare. The Practice Incentives Program (PIP) Quality Improvement (QI) Incentive is a payment to general practices for activities that support continuous data driven quality improvement in patient outcomes and the delivery of best practice care. The first annual report has been released by the Australian Institute of Health and Wellbeing (AHIW). The report on the 10 PIP QI measures shows data (2020-2021), shared at the community level, and collected through the PIP QI Incentive, has the potential to inform primary health providers how to improve care and services to clients within a population. Closing the Gap Pharmaceutical Benefit Scheme CoPayment Program Patient Registration A significant number of eligible Aboriginal and Torres Strait Islander patients have not been correctly registered for the Closing the Gap Pharmaceutical Benefit Scheme Co-payment Program (the Program). As of 1 February 2022, these patients will not have the applicable copayment reduction applied when their prescriptions are being dispensed. PBS General Schedule medicines prescribed under the Program have been issued for patients who are not formally registered for the Program with Services Australia. The Department is seeking your assistance to address this issue. To ensure eligible patients are formally registered for the Program and continue to receive their PBS medicines at the reduced PBS co-payment, we are asking that you review the registration status for any Aboriginal or Torres Strait Islander patients. Patient registration status can be checked using the Health Professional Online Services (HPOS). If the patient is not formally registered with Services Australia, HPOS will indicate that the person is “inactive”. In this case you will need to contact that patient to arrange to have them assessed by either a PBS Prescriber or an Aboriginal and Torres Strait Islander Health Practitioner. This issue must be rectified before 31 January 2022 to ensure all eligible Aboriginal or Torres Strait Islander patients who have previously received their PBS General Schedule medicines under the Program can continue to do so. Public consultation on the proposed changes to the Controlled Substances (Poisons) Regulations 2011 to mandate the use of Script Check SA close at 12 noon on Monday 1 November 2021. caring@home Indigenous Art Competition caring@home invites Aboriginal and Torres Strait Islanders to take part in a one-off, unique art competition, the caring@home Indigenous Art Competition, which is now open, with submissions closing on Wednesday, 9 February 2022. The theme of the competition is “Journey to Dreaming at Home,” which highlights an important aspect of palliative care for Aboriginal and Torres Strait Islander people – a preference for care during the end-of-life journey to be provided at home. Your entry can be drawn, painted or printed on paper, canvas, or other similar medium or created in digital form. There are a number of prizes on offer, including:

Linking GPs with Tertiary Renal Services – Improving communication to help patients with kidney disease The Central Northern Adelaide Renal and Transplantation Service (CNARTS) recognises the value of GPs and all primary care providers in kidney care, and the potential to improve integration between tertiary and primary care services. CNARTS is undertaking a research program to improve communication with primary care partners, while enhancing the patient health care experience and reducing unnecessary emergency department presentations and hospital admissions. The research program is underpinned by in-depth qualitative data provided by primary and tertiary sector stakeholders and patients, and is directed by a panel of primary and tertiary clinical Investigators. Initiatives being implemented include:

Consultation Sessions - Draft South Australia’s Rural Oral Health Workforce Plan 2021-2026 The Minister for Health and Wellbeing, the Hon Stephen Wade MLC, has announced the release of South Australia’s draft rural oral health workforce plan for public consultation. This plan is being developed as part of the State Government’s Rural Health Workforce Strategy. The plan can be accessed here. From October to November 2021, consultation sessions (online and face-to-face) are taking place in regional Local Health Networks to seek feedback on the draft plan. Mr Mark Chilvers, SA Dental, Executive Director and Mr Bradley Abraham, CEO, Australian Dental Association SA will be visiting the regional areas to consult with local clinicians, consumers, peak bodies, private and not-for-profit providers, and other interested parties


regarding the draft plan. Following consultation the final document will determine the strategy and guide investment in the rural health workforce for the coming years.

Changes to CAMHS referral process, since Monday 27 September 2021, The Child and Adolescent Mental Health Service (CAMHS) will

accept referrals from General Practitioners, relevant health professionals, school counsellors, or mental health community organisations, rather than directly from a parent or carer. This means that a parent or carer’s concerns will be heard and assessed for immediate risk as per the current management and escalation processes within their teams. However, any non-urgent enquiries, with some exceptions for vulnerable groups, will be redirected to their GP or a primary health care provider. Please be aware that this may have an impact on your practice. If you have any queries related to this change please contact CAMHS directly. New notifiable conditions in South Australia Changes have been made to the

South Australian Public Health (Notifiable and Controlled Notifiable Conditions) Regulations 2012 which affects medical practitioners, pathology services, registered nurses, midwives and pharmacists who are authorised immunisers as per the Vaccine Administration Code. The National Cancer Screening Register Communication Toolkit The Cancer Council has

developed a National Cancer Screening Communication Toolkit designed to ensure consistent messages are communicated about the three national cancer screening programs – BreastScreen Australia, the National Bowel Cancer Screening Program (NBCSP), and the National Cervical Screening Program (NCSP) to all stakeholders and relevant audiences. It contains key messages, social content, images and case studies that can be used directly, or adapted and edited for your channels and outlets such as Electronic Direct Mail (EDM) marketing and newsletters. RACGP Resource: Preventing and Managing Patient Aggression and Violence The RACGP has developed a brief guide to

help general practice staff identify and manage incidents of patient aggression and violence. The guide addresses common questions about dealing with aggressive or violent patients. The guide also includes editable templates for you to use when issuing warning letters, placing patients on acceptable behaviour agreements or discontinuing care for patients at your practice. The RACGP has also developed posters that can be display in your practice to inform everyone that aggression and violence will not be tolerated.

The practice supports GPs, GP Registrars, Interns as well as Flinders University medical students and offers a wide variety of scope for nurses working in rural general practice. There is an opportunity for 2 experienced nurses to join the team. Registered and Enrolled nurses are invited to apply. Position 1 – Nurse Manager; Position 2 – Treatment room Nurse. Both positions are full time; job sharing will be considered for suitable applicants. Are you a good communicator with a knowledge of triage, wound care and infection control, then this is the job for you. Support is available for necessary upskilling. For those who support and care for someone experiencing depression or anxiety Minds Together is an evidence-informed online program dedicated to educating and building the skills of people in the community who support a partner, family member, friend or colleague experiencing symptoms of depression or anxiety. The program aims to build confidence and capacity as well as promote positive mental health and wellbeing for those who provide care. As part of a national trial, Everymind is seeking people who live in Australia, are aged 16 years or older, and support someone with depression or anxiety to participate in the new Minds Together online program. Involvement for all participants will include completing the four-module online program and providing feedback through surveys.
