Luke received his bachelor’s degree in business administration from Abilene Christian University in Texas.

Palmetto GBA 2019 Workshop Series for Hospice and Home Health: Tools for Medicare Success Palmetto GBA is pleased to announce our 2019 Hospice and Home Health Workshop Series. These workshops are designed for providers and their staff to equip them with the tools they need to be successful with Medicare billing, coverage and documentation requirements.

These workshops will provide insight for hospice and home health agency staff at all levels; however, we suggest that providers who are new to Medicare or have new staff attend online learning courses for beginners offered at www.PalmettoGBA.com/hhh. Basic billing and other online educational resources can be found in the Self-Paced Learning section by selecting the Learning and Education link under the Browse Topics option at the top of the page. During the workshop series, Palmetto GBA will provide information related to the most common errors identified through a variety of data analysis and some hints and tips on the reasons why these errors occur. Palmetto GBA’s ultimate goal is to have educated and astute providers who know how to accurately and skillfully apply the information they learn to their documentation and billing practices.

A1 - How Agency Outcomes Affect Star Ratings Under PDGM Sue Payne, MBA, RN, CHCE VP & Chief Clinical Officer, Corridor Christopher Attaya, MBA VP Product Strategy, SHP Data This session will review how agency outcomes affect star ratings under PDGM. A case study will be presented focusing on care of the patient with a specific primary diagnosis and differences between caring for the patient under PPS versus PDGM. Key areas of focus will be managing inappropriate LUPAs under PDGM, cost effective approaches for improving functional status of patients under PDGM, and recommended metrics to track under PDGM.

A2 - The Top 5 Hospice Medical Review Denials and How to Avoid Them Melinda Gaboury, COS-C, Co-Founder/CEO Healthcare Provider Solutions In the current environment of targeted and third party reviews, there is a constant concern of reviews that could result in recoupment of payments already made to hospices. This session will narrow the focus to the top five denial reasons, go into depth on each one and provide broader concepts. This session will highlight the documentation requirements to avoid the denials and the regulations that support. If your hospice is currently in a review or is just looking to improve the quality review standards, this session is a must.

A3 - Leadership and Supervision: Key Skills for Home Care Supervisors Kathie Smith, BSN, RN, VP of State Relations, Home and Community Based Care AHHC of NC/SCHCHA Program and staff supervision and oversight are key components in ensuring quality home care services and retention of staff. Home Care supervisors provide leadership and supervision to staff providing direct care services to home care clients/patients. This session will cover components of leadership and supervision for Home Care supervisors and will provide information supervisors can add to their “toolkit” of resources for effective supervision.

B1 - PDGM - A Deeper Dive Into the Numbers!

Nick Seabrook, Managing Principal, Founder Blacktree Healthcare Consulting With less than nine months until PDGM takes effect, home health agencies must start taking a deeper dive into the numbers and fully understand the details of PDGM and how they will impact their agency. In this presentation, we’ll examine the numbers of PDGM using national data from CMS’ Limited Data Set (LDS) which details 2017 PPS payments, converting to payments if PDGM had existed during that time.

Following B1 from 12:15-12:45pm Mary Jo Roue with SC DHEC will be providing a licensure update for Medicare-certified agencies.

B2 - An In-Depth Review of the Hospice Regulations: From Eligibility to Discharge and Everything in Between Lisa Meadows, MSW, Clinical Compliance Educator Accreditation Commission for Health Care Understanding the hospice regulations from eligibility to discharge can be quite overwhelming. Often clinical staff has a limited understanding of the regulations and the requirements needed to be compliant with the regulations. A lack of understanding of the Hospice Medicare Benefit in addition to a lack of understanding of the Medicare Conditions of participation can lead to patients and families missing out on the required benefits of hospice care. This presentation is intended for anyone who is responsible for the delivery of care to hospice patients and families.

B3 - Update from the South Carolina Department of Health Control Marcus M. Jackson, Field Manager Facility Services Oversight Division SC DHEC The presentation will cover the following sections of Regulation 61-112 “Standards for Licensing In-Home Care Providers”: Requirements for Licensure, Hours of Operation, In-Home Caregivers, Background Checks, Health Assessment, Tuberculin Skin Test, Staff Training, and Incident Reporting.

C1 - PDGM Changes Cannot Start Soon Enough

D1 - Home Health Financial Management and PDGM

Melinda Gaboury, COS-C, Co-Founder/CEO Healthcare Provider Solutions The most significant change in Homecare Reimbursement in 20 years will go into effect January 2020. The Patient Driven Groupings Model (PDGM) is complicated, confusing, and overwhelming. CMS has provided some data on the revenue impacts and new PDGM components but there is more to be considered.

Operational areas likely impacted under the new payment model include referral and sales management, intake, revenue cycle, operational reporting, order and supply management, and much more. This session will highlight the areas impacted and outline potential process changes to assist in optimizing operations performance under PDGM.

C2 - The Service Intensity Add-On and End-of-Life Care: Impact to Clinical, Financial and Quality Operations Catherine Dehlin, BSN, CHPN Director of Hospice Services Fazzi Associates The national hospice provider market has consistently revealed a predictable and often unintentional practice directly impacting the patient experience in the last days of life; negatively impacting quality, satisfaction and financial performance under the Service Intensity Add-On (SIA). How providers address care planning as end of life approaches is important not only to patients and their families, but is of increasing interest to CMS and the OIG. Clear guidance for staffing and a standardized, best practice, interdisciplinary and operational approach will give attendees tools to integrate into practice for an immediate impact on quality, patient and family experience and on the organization’s financial performance.

C3 - Creating Revenue Streams Through Specialty Programs Tammy Ross, Senior Vice President of Operations Professional Services, Axxess As a home care agency, how can you diversify your revenue streams and remain a competitive/valuable link in the healthcare chain? This session will cover the following objectives: private duty partners in care for disease management; training caregivers; marketing the program; and a deeper dive into dementia programs.

Michael Simione, MBA, Manager Simione Healthcare Consultants With PDGM set to begin January 1, 2020, now is the time to look inward at your agency’s financial management.

With the change from PPS to PDGM, agencies must look at their processes to ensure they have most efficient financial workflows. In this session we will explore the potential impact of PDGM on the financial operations of an agency and discuss financial management strategies to help an agency manage under the new payment model. We will provide benchmark data, key metrics and strategies for engaging all staff in helping the agency with successful financial management.

D2 - Deprescribing Medications for Hospice Patients Tiffany Richter, MD, Medical Director, Roper Hospice Hospice requires a reconciliation of all medications and an assessment for appropriateness and the impact they have at the end of life. In this session, Dr. Tiffany Richter will discuss deprescribing, review medical questions, ethical issues and financial factors involved in withdrawing or continuing medications at the end of life.
