Meet John Connor & Family John Connor is a fifteen-year-old white male foster child, living with foster parents, Todd and Janelle Voight in the San Fernando Valley region of Los Angeles. John’s estranged biological mother, Sarah Connor, is being held at a psychiatric detention center called Pescadero State. John’s foster parents are of middle to working class socioeconomic background. John has lived with the Voights since the age of nine years old, but addresses both parents by their first names (Kassar, Hurd, & Cameron, 1991).

John is in generally good health. He is slender, at 173 cm and 62 kg, which place him between the 50th and 75th pencentiles on the CDC growth curve (CDC, 2017). He has a fair complexion, with brown hair and eyes.

John Connor is an only child. He is in 9th grade, and attends Van Nuys High School, but has few friends and “ditches” school often. He is known to frequent the Galleria Mall near his high school where he spends a lot of time at the video game arcade. He has neither cell phone nor pager. He is highly independent for his age, riding a dirt bike helmetless as his primary mode of transportation (Kassar, et al., 1991). He has no job, receives only a $10 allowance per month from his foster parents, and engages in illicit activity to acquire spending money.

*Final 20-25% height achieved during puberty. *Acne can appear during adolescence. *Boys grow in stature until around 18-20 years old, growing 10-30 cm and 7-30 kg over course of adolescence. *Sexual maturation occurs in following order: -testes enlargement -pubic hair growth -penile enlargement -growth of axillary hair -facial hair growth -vocal changes *Developing capacity for abstract thinking. *VS approximate those of adults. *Fine and gross motor skill levels reach those of adults.

*Weight: 62kg (136 lb) *Height: 173cm (68 in) *Sexual maturation evident by vocal changes, stature reaching adult averages. *Motor skills, physical abilities fully developed, specialized, as evidenced by ability to ride motorcycle independently in high-pressure circumstances.

*Piaget stage of formal operations; Erikson stage of identity vs. role confusion (ATI, 2016) (see next slide). *Close, same-sex friendships. *Transition from friendships to intimate relationships. *May view selves as invincible to bad health outcomes (ATI, 2016). *View selves in relation to similarities with peers (ATI, 2016).

*John’s problem-solving abilities and strong personality are consistent with the appropriate Piaget and Erikson stages. *Exhibits close, same-sex friendship with “Tim� a friend who rides his dirt-bike and ditches school with him regularly. *Intimate opposite-sex relationships are not represented in the film but are consistent with his character. *Helmetless motorcycle riding consistent with presumed invincibility to MVAs.

*Cognitive abilities, technological skill sets have reached adult levels as evidenced by ability to hack ATM machines as source of income.


Developmental stages and levels Piaget: Formal Operations Stage Adolescents in this stage are able to think through more than two categories of variables concurrently, use logic to inform decisions and are able to understand how the actions of an individual influence others (ATI, 2016). John demonstrates these capacities in his ability to commandeer the Terminator to apprehend his mother on two different occasions in te film – first, from the detention center where she is being held, and later, at the private residence of the Cyberdyne Systems engineer she is planning to assassinate.

Kohlberg: Conventional Stage Adolescents substitute blind acceptance of decisions made by adults for their own emergent set of morals and values. Their decisions involving moral dilemmas are increasingly based on an internal set of moral principles (Perry, Hockenberry, Lowdermilk, & Wilson, 2014). John demonstrates this development in his explanation to the Terminator that he (the Terminator) cannot kill people for no justifiable reason (Kassa, et al., 1991).

Erikson: Identity vs. Role Confusion Adolescents develop a sense of personal identity and come to view themselves as unique individuals; at the same time, they also become part of a peer group that greatly influences behavior (ATI, 2016). The only friend that is part of John’s peer featured in the film is his friend Tim, who also rides helmetless on John’s motorcycle, and ditches school with John to play arcade games. Both dress similarly in 90s “grunge” clothing, reflecting shared influences in style of clothing.


Nutrition Status John Connor’s current BMI adjusted for weight at 62 kg and height of 173 cm is 20.7, placing him in the normal weight category (NIH, n.d.). According to the Center for Disease Control and Prevention clinical growth charts (2017), John’s height and weight place him above the 50th percentile for his age. However his heavy reliance on fast and convenience store food offerings places him at high risk for weight gain and nutritional deficits in his adult years, as illustrated in below depiction of John in his adult years should he maintain his current habits.

Rapid growth and metabolism at John’s age call for increases in quality nutrients, including additional calcium, iron, protein, and zinc (ATI, 2016). Inadequate inake of folic acid, vitamin A and B6, iron, calcium and zinc is common (ATI, 2016). The USDA’s 2015-2020 dietary guidelines (2015) call for limitations on foods with added sugars to less than 10% of calories per day, less than 10% of daily calories from saturated fat, and less than 2,300mg per day of sodium. Key recommendations include at least five servings of whole fruits and vegetables per day, with the vegetables coming from a variety of sources including dark green, red and orange, and legumes; whole grain sources of carbohydrates; low fat dairy or fortified soy products including milk, cheese, and yogurt; and variegated sources of proteins such as seafood, lean poutry and meats, eggs, legumes and nuts (USDA, 2015).


Orem’s Self-Care Requisites (SCRs) Universal SCRs: Partially Met John Connor clearly maintains sufficient intake of air, food and water and has no deficits in elimination or excrement care. His balance between activity and rest is met to the extent that he is virtually always on the move, including physically taxing motorcycle rides. He does not reasonably meet SCRs for hazard prevention AEB his habit of riding his motorcycle at high speeds and off designated roadways helmetless. His habitual truancy from school, lack of welladjusted friends, and engagement in criminal activity (stealing from ATM machines) represent aberrations in normal development for his age (Kassar et al., 1991).

Developmental: Maturational Partially Met As discussed in Piaget and Erikson staging, John Connor’s cognitive and psychosocial milestones appear on time for his age AEB his strong personality and future-oriented thinking. However, his social development is compromised by his lack of school attendance, lack of involvement in ageappropriate peer activities such as work or career-training programs, sports or social organizations (ATI, 2016)

Developmental: Situational Met

Health Deviation SCRs: Not Applicable Health deviation SCRs refer to measures taken to secure medical treatment for a pathologic condition, and effective management of therapeutic and rehabilataitive measures for said condition (Dennis, 1996). John is free of pathologic conditions at this stage of his life.

John Connor finds himself in the profoundly unique circumstance of being physically hunted by a shapeshifting cyborg-like assassin from the future referred to as the T-1000 (Kassar et al., 1991). Defended by the Terminator and his biological mother in tow, he successfully prevents deleterious effects on his health to the extent he is alive and unscathed after a number of harrowing encounters with the T-1000. The T-1000, in contrast, is destroyed after successful defensive maneuvering by the Terminator and Sarah Connor.


Anticipatory Guidance Age-Appropriate Activities Age-appropriate activities for a healthy 15-year-old male include nonviolent video games, nonviolent music, sports, caring for a pet, career-training programs, reading, and social events such as going to movies and school dances (ATI, 2016). John engages in both violent and non-violent video games, listens to nonviolent punk-rock music, and has a dog “Wolfie” in whose care he is marginally involved. He could benefit highly careertraining and greater involvement with a more diverse circle of friends from school.

Immunizations The CDC recommends catch-up doses of the following immunizations not received at 1112 years old: tetanus diphtheria toxoids and pertussis (Tdap); HPV4; and meningococcal MCV4 vaccines (ATI, 2016). Annual influenza shots are also advisable (ATI, 2016). John lacks all of these.

Nutrition, Dental Hygiene, Sleep & Rest Rapid growth and high metabolism at his age require increases in quality nutrients, with special emphasis on addition calcium, iron, protein zinc, folic acid, vitamin A and B6 (ATI, 2016). Brush and floss after meals, snacks, and at bedtime and get regular dental checkups. Sleep needs are highly variable at this age however needs are generally increased during period

Safety Teaching (ATI, 2016) Firearms should be kept locked away; and teens should be monitored for self-harm risk. Teach proper use of helmets for skating, biking, riding motorized vehicles, and snow sports. Fire, sun exposure, and swim safety should be taught. Drivers’ education and seat belt use and discouraging cell phone use prevent MVA-related injury. Present a notolerance stance towards substance abuse. John could tremendously benefit from using a helmet, adopting seat belt adherence, and taking up some sort of vocational hobby to discourage him from lapsing into recreational drug use.

QSEN Practices in Educating Adolescents Respecting privacy is key for John Connor’s age group (Perry, Hockenberry, Lowdermilk & Wilson, 2014). In practice this entails conduction health interviews without the parents present; showing genuine appreciation of the adolescent patient’s perspective, e.g. “tell me what you think is happening”; and maintaining objectivity, i.e. a nonjudgmental stance (Perry et al., 2014). Keeping current on the AMA’s GAPS framework (Guidelines for Adolescent Preventive Services) – covering immunizations, nutrtion, sleep and rest, dental health, personal care, and stress reduction,and injury prevention – is vital to best practices in nursing for this demographic (Perry et al., 2014).
