STRATEGY CATALYSTS: TRANSFORMATIONAL LEADERS WHO RECRUIT, ENGAGE AND INFLUENCE PUBLIC AND PRIVATE PARTNERS TO INITIATE CHANGE This past year has been another busy one for Common Table Health Alliance. The list of accomplishments presented here is but a snapshot of the many activities to improve the multiple aspects of health that affect our local, state and regional communities. Sustainability for our multi-faceted mission is a complex task. We rely on grants and personal contributions to support many aspects of the REAL Community Goals we have set for ourselves and our communities of interest. The continued growth of primary care and prevention activities led by Project Better Care and the Healthy Shelby Hypertension Collaborative are particularly exciting to me as a family nurse practitioner. Project Better Care has guided changes within healthcare practices to improve patient care. Healthy Shelby’s focus on patient outcomes gives objective evidence of the degree of blood pressure control present in the county. CTHA provides both specific analysis and related guidance to move healthcare practices beyond good intentions and best guesses to solid evidence of areas of excellence and areas for improvement. In 2014, our Alliance aggressively focused on: advancing health literacy, advocating for fresh, healthy affordable food, eliminating disparities and improving preventive and primary care. Much of the work is driven by community spirit which engages multi-stakeholders across the community. CTHA is known for our broad reach and the many partners who support the work in various ways. In this report, we highlight 12 of the top accomplishments of Common Table Health Alliance, Aligning Forces for Quality, Project Better Care and Healthy Shelby team members. We also showcase the impact of our work on thousands of people and their families to address the improvement of health and healthcare services received. The key to 2014 has been a continued focus on addressing the needs of all of the community. This provides a common good, interest and commitment highlighted by the results noted on page three of our report. I want to thank the board, staff, interns, volunteers, donors, sponsors, neighborhood partners, our Mayors and the entire community who continue to embrace the work of Common Table. Since 2003 we have strived to be innovative in our work, and we will continue to be a strong voice and catalyst for change!
