DEAR COSTCO,

2014 has been a year of remarkable growth for Costco and Children’s Miracle Network . Because of Costco employees’ dedication and passion, children’s hospitals in Canada will receive more than $8.7 million in much-needed support. ®

Costco’s continued commitment to supporting children’s health care in Canada has meant that 14 children’s hospitals across the country are able to provide the very best health care to more than 2 million children each year. As Children’s Miracle Network positions for growth to meet the needs of our member hospitals, we know that dedicated partners like Costco will propel us toward new heights. We are continually inspired by Costco employees’ enthusiasm and connection to their local Children’s Miracle Network member hospitals. Having now raised in excess of $60 million over the last 22 years, Costco is leaving an enduring legacy on the landscape of pediatric health care in our country. In this report, you will find a summary of the activation and recognition received from each of the 12 hospital foundations, as well as examples of the impact funds are having on children’s lives across the country. Mateo’s fun personality has helped him endure two serious diseases: Hirschsprung’s Disease and short bowel syndrome. At birth he had a 30 percent chance to live but, thanks to his Children’s Miracle Network® member hospital, Mateo is now beating the odds and inspiring everyone he meets.

MEET REBEKAH WINSOR JANEWAY CHILDREN’S HOSPITAL FOUNDATION BRAIN BLEED AGE 7

It started with a simple headache. When the pain and duration increased, Rebekah’s parents knew something was terribly wrong. Tests at a local hospital were inconclusive and she was airlifted immediately to Janeway Children’s Health and Rehabilitation Centre. Her parents’ greatest fear was confirmed: Rebekah was experiencing a spontaneous brain bleed. Specialists and surgeons took 5-year-old Rebekah into their care for emergency brain surgery. Post-surgery and in critical condition, Rebekah was kept in an induced coma to stabilize her. It took over a week for healthcare professionals to slowly wake her. Once awake, she had to relearn how to do everything: how to walk, dress herself and even how to swallow.

MEET DAVID CLOUTIER OPÉRATION ENFANT SOLEIL BURKITT’S LYMPHOMA AGE 14

David was only 10 years old when he was diagnosed with Burkitt’s lymphoma. While some children might fear such a scary diagnosis, inquisitive David sought out to learn more. He impressed Opération Enfant Soleil staff with his maturity and confidence when he began asking the doctors and nurses earnest questions about his condition. David’s treatment was not easy. He received catheters for his heart’s vein dialysis, faced complications, infections and endless other tests, and underwent surgery to remove the rest of the cancer mass and part of his intestine. Eventually David’s treatment ended and back was the jovial, upbeat child everyone knew. Now that the Cloutier family has their son back, they are able to resume their favorite pastime: biking. Spending that time together helps them appreciate life and David’s health.

The team at Janeway was committed to providing the excellent care, resources and treatment necessary to result in a full recovery. Rebekah beat the odds and is now happy to be singing and playing soccer again.

FUNDS IN ACTION

FUNDS IN ACTION Children’s Miracle Network funds help the Janeway purchase vital medical equipment like the therapeutic pool lift. This helped Rebekah get in and out of the pool with ease during water therapy.

Montreal Children’s Hospital has been able to improve its quality of care thanks to funds raised by Opération Enfant Soleil and Children’s Miracle Network. David’s treatment at The Children’s included many scans, MRIs, spinal taps, X-rays and blood tests.

MEET RYAN VELDKAMP CHILDREN’S HOSPITAL FOUNDATION OF MANITOBA T-CELL ACUTE LYMPHOBLASTIC LEUKEMIA AGE 17

“Hey, listen here Jack. If you aren’t here to compete, you’re going to get beat,” is Ryan’s favourite line. He can often be heard reciting it on visits to Children’s Hospital of Manitoba, where he continues to have his health monitored. Diagnosed with T-cell acute lymphoblastic leukemia at 11 years old, Ryan can match almost any calledout number to his treatments. First are the “initials” treatments: two EEGs, three MRIs, four EKGs, eight CT scans and 10 IVIG treatments. Add to that two lung tests, three echocardiograms and equal bags of platelets, four bone marrow tests, seven blood transfusions, eight rounds of radiation, 27 spinal taps, 50 X-rays and numerous chemo treatments, shots and blood tests. To cite his go-to quote, Ryan’s competed with an aggressive form of cancer and to date has “got it beat”! Powering through his challenges has allowed him to focus more on his many hobbies and volunteering at his member hospital.

MEET ASAIAH EDWARDS THE HOSPITAL FOR SICK CHILDREN (SICKKIDS) SICKLE CELL ANEMIA AGE 11

Asaiah is an avid basketball and soccer player. He has a keen interest in science experiments and is constantly on the lookout for new opportunities to learn more about the world he lives in. Asaiah was born with a genetic condition called sickle cell anemia, which causes pain in various locations of the body and has no known cure. He’s been admitted to SickKids more than 30 times, has received over 50 blood transfusions and attends frequent clinic visits. As a patient for more than a decade, Asaiah and his family have witnessed firsthand how research has improved treatment and provided options for less-invasive procedures and tests. Although Asaiah’s condition can cause him to tire easily, it is important for him to be active within his personal limitations. His family is proud of his resolution to play, learn and overcome his health challenges.

FUNDS IN ACTION Children’s Miracle Network funds helped make Ryan’s hospital stay less difficult by providing a pain medication pump. Donations also support child life specialists to keep his kids’ minds off chemotherapy and cancer treatment.

Children’s Miracle Network’s support helps fund SickKids Research Institute’s needed research activities. This investment helps lead to new discoveries and treatments for a range of child health conditions and diseases, such as sickle cell anemia for patients like Asaiah.

What is Children’s Miracle Network ? ®

Children’s Miracle Network raises funds and awareness for 170 member hospitals, 14 of which are in Canada. Donations stay local to fund critical treatments and healthcare services, pediatric medical equipment and research. Its various fundraising partners and programs support the nonprofit’s mission to save and improve the lives of as many children as possible.

Why is support important? Each day, over 4,900 children will receive medical treatment for variable diseases, sickness and injuries at Children’s Miracle Network member hospitals across Canada. These children’s hospitals depend on financial support to help fund critical priorities such as: Mental Health - 70% of mental health issues have their onset during childhood or adolescence. Pediatric Health Research - Research has the greatest potential to transform child health. Emergency and Urgent Care - Enhancing patient flow, care, privacy and safety is vital to saving kids lives. Healthy Living - Children’s hospitals research, educate and advocate proper nutrition, regular physical activity, and positive body image. Cancer - is the single most common cause of disease-related death in children and adolescents. Fifty years ago we only hoped for the treatment and cures we have today. Child Life Programs - are devoted to helping kids be kids, even in the midst of illness and hospitalization. Children’s Miracle Network provides additional funding to ensure that vital programs, equipment and research devoted to children’s health continue to thrive.
