25 years of Community Health Center. It hardly seems possible. Fueled originally by the love and passion of a few, in 25 years, Community Health Center has grown to become a medical home to so many on Cape Cod.

We provided medical, behavioral health and dental care to more than 20,000 patients, addressed the opioid overdose epidemic on the Cape, launched a remote patient monitoring program and provided critical assistance for patients facing food insecurity, housing insecurity and transportation challenges. We increased our ability to serve more patients where it’s needed with new locations and services. We helped thousands of individuals navigate applying for health insurance. And that’s just part of the story.
We received several quality recognition awards from the Health Resources and Services Administration (HRSA), along with a grant to expand our virtual care program. Cape & Plymouth Business Media named us one of the Best Places to Work, a testament to the culture of compassion and excellence we’ve created internally. Accolades like these underscore our team’s unwavering commitment to first-class health care. As we reflect on what we’ve achieved, it’s also a time to look ahead and plan for continued future success. Thank you so much for being a key part of that journey.
A Board Member once said that while the growth and success of CHC came through the endless generosity of CHC’s volunteers, so too are our donors prized members of our volunteer staff. FOUNDED IN 1997
In 1997, Community Health Center is founded by volunteer doctors, volunteer nurses and lay people as the Falmouth Free Clinic.
In 1998, CHC began seeing patients on the ground floor of Falmouth Hospital once a week, completing 600 patient visits in the first year.
In 1999, CHC volunteers started working out of Town Hall Square in Falmouth.

CONTINUED GROWTH
In 2008, CHC begins offering dental services.
In 2008, a 10,000 square-foot facility in Mashpee facility opens, and Community Health Center surpasses 5,000 patients.
In 2009, CHC opens a Falmouth office in Homeport for greater capacity.
In 2010, CHC starts an Office-Based Opioid Treatment (OBOT) program, offering Suboxone and Vivitrol.
In 2004, CHC opened a location in Mashpee Commons, and the number of patients grew to more than 3,000.
In 2005, CHC launched a Veterans Partnership and started implementing Electronic Health Records.
In 2006, CHC opened a satellite location in Bourne. CHC also received designation as a Federally Qualified Health Center (FQHC).
In 2007, the Town of Mashpee gave 4.2 acres to CHC for their new building.

WE’RE NOT DONE YET EXPANSIONS & ACHIEVEMENTS
In 2011, CHC enrollment staff assist more than 3,000 patients and members of the community to enroll for health insurance annually.
In 2012, a 22,000 square-foot extension in Mashpee opens, with expanded offerings of pharmacy, walk-in and imaging services.
In 2013, CHC is first awarded Patient Centered Medical Home Recognition at Level Three from the National Committee for Quality Assurance – the highest level possible, and has maintained this status in the years to follow.

NEW SERVICES AND OFFERINGS
In 2019, Community Health Center opened an office in Centerville which offers primary care and walk-in services for anyone.
CHC remained open during the COVID-19 pandemic, providing access to care when people needed it most, expanding telehealth services and leading testing and vaccination efforts.
In 2021, an office in Sandwich was opened that provides primary care, behavioral health and substance use disorder services.
In 2022, CHC opened a retail Pharmacy in Falmouth and an office in North Falmouth focusing on primary care, gynecology and lifestyle medicine.
Today, CHC cares for more than 23,000 individuals on the Upper Cape and beyond!

In 2015, optometry and nutrition education services are added to CHC’s continuum of care.
In 2016, CHC begins offering telehealth services, including remote patient monitoring, video interpreting services and telepsychiatry.
In 2017, 1 in 5 Mashpee residents and 1 in 6 Falmouth residents are CHC patients.
In 2017, CHC adds dental and pharmacy services to the Bourne office, and expands substance-abuse programming.
