MESSAGE TO THE COMMUNITY When we produced this community report last year, we celebrated the addition of several new providers to our CFVH family and looked ahead to expanding our services to allow us to better meet the needs of our patients. It has been rewarding to see those efforts come to fruition and it is just as exciting to contemplate what is to come. We hired two behavioral health specialists who see patients from each of our three Family Medicine Network offices across the county. We also partnered with the International Heart Institute of Montana in Missoula and Rocky Mountain Heart and Lung in Kalispell to offer cardiology services in Plains three days per week. There has been a continuous trend over the last few years to find creative and innovative ways to ensure our patients get the care they need close to home and we feel fortunate to have a great network of specialty care providers to allow us to do just that. Additionally, CFVH joined the Veterans Choice Program earlier this year so local veterans can get their care at CFVH instead of traveling to the closest VA healthcare facility. We remain committed to delivering the safest and most effective care possible. With that in mind, I am proud to share that CFVH recently purchased a new stateof-the-art CT machine to allow for quicker results in trauma situations as well as reduced radiation exposure to patients during their exams. Our providers also worked together to develop a consistent, comprehensive approach to chronic pain management in collaboration with specialists from Kalispell. Community outreach efforts have been in full swing this last year and have included a number of courses, seminars and speaking engagements related to topics like diabetes prevention, chronic disease self-management and overall wellness. These were developed in response to the 2012 Community Health Needs Assessment nearly 200 Sanders County residents participated in to help us engage our community and assist in providing the necessary tools to live healthier, happier lives. Two final things as we look ahead: As payment methods for healthcare rapidly change, we will participate in an Accountable Care Organization this year to be able to continue to serve you close to home. This will be a new experience for us as it is for all rural healthcare facilities, but we look forward to the challenge. We have also entered into a partnership with Studer Group to further improve our quality and service performance. They have an outstanding national reputation and we believe this partnership will play a significant role in our ability to serve you for years to come. AWARDS AND RECOGNITION FOR QUALITY CARE Montana Rural Healthcare PIN Quality Improvement Award - September 2014 The Montana Performance Improvement Network (PIN) recognized CFVH for their quality improvement in Emergency Department transfer communication.

iVantage Health Excellence in Quality Award - November 2014 iVantage Health Analytics recognized CFVH for overall excellence in quality, reflecting a standing in the top 25 percent of all acute care hospitals in the nation. The metrics used were based on quality indicators for patient care in acute heart attacks, heart failure, pneumonia, surgical care infection prevention and outpatient process of care scores.

CMS 5 Star Quality Rating - February 2015 CFVH’s Long Term Care received an Overall 5 Star Rating from the Centers for Medicare and Medicaid based on health inspections, staffing and quality measures.

Andean Health 1500 Rural Health Award - June 2015 Andean Health works to improve rural health in the Americas and chose to recognize CFVH for its innovative approaches to rural healthcare. CFVH FOUNDATION The Clark Fork Valley Hospital Foundation is a charitable organization whose sole purpose is to support CFVH and its related entities fulfil their mission to improve the health of Sanders County residents. The Foundation has provided nearly $240,000 in donations over the past seven years. In 2014 the Foundation donated funds to enhance the hospital’s conference facilities which are open to the public and utilized regularly by a variety of local organizations. The hospital primarily uses them to enhance patient care through presentations and ongoing training. To facilitate projects like this, the Foundation conducted two fundraising efforts in 2014; a Gala event in June and the Golf Classic in September. The Gala featured musical entertainment by Montana-based Montana Rose and an evening of food and fun at the Sanders County Pavilion for almost 50 attendees. The Golf Classic, which was in its 7th year, was held on Sunday of Labor Day weekend and hosted 42 golfers for the day. Tremendous support from local and regional businesses helped make it successful and over $10,000 was raised. Each year the Foundation is asked to fund hospital projects and services that are not covered in the budget for the year but directly benefit the patients and guests to the facility. Your tax-deductible donation does make a difference. Past purchases continue to save lives and enhance the care the hospital can provide.

FIRST HAND EXPERIENCE Different Time Different Outcome Patient Testimonial

After returning home from some morning chores in March, my husband Don, 62 years old, collapsed to the floor struggling to speak. I rushed him to the ER at CFVH where we later found out he was having a bradycardia, slower than normal heart rate. As soon as we arrived, we were greeted with kindness and compassion. It wasn’t long before Don began passing out for the second time. Emergency room providers assessed the situation immediately so they could begin treating him. Having just moved here in December from the Seattle area, we knew how long we might have waited on the coast before being seen in the ER. We also knew that the time it would have taken to get to a quality care facility may have caused us to still be in-route to the hospital when Don suffered his second spell. We know now that this could have been a life ending event but the quality doctors and nursing staff at CFVH saved Don’s life. At one point, Don’s heartbeat slowed to the point CPR was required and staff made sure to check if I was ok after it happened. Their genuine concern wasn’t only exhibited to my husband in his time of need, but for me as well which I thought was exceptional. Don was transported by helicopter to St. Patrick Hospital for continued treatment after he was stabilized, but I remember the staff communicating with us every step of the way so we knew what was happening, why we were going and what to expect when we got to Missoula. The transition of our care couldn’t have gone any smoother. Words can’t express how fortunate I feel to have a healthcare facility so close to our home that provides such quality care. We have access to more than just emergent care, but preventative and critical care as well and that means the world to us living in this small community. All of the staff that we interacted with during this traumatic visit were beyond competent in our time of crisis. It was extraordinary how sympathetic they were to our stress and emotions. Orthopedic Surgical Services Close to Home Patient Testimonial

After five years of really feeling the affects and pain of my osteoarthritis and knee joint degeneration, I chose to undergo total knee replacement surgeries on both of my knees. I had the choice to receive my care at many hospitals due to the fact that Dr. Jeffery LaPorte was my orthopedic surgeon. Choosing CFVH allowed my family and I to save a lot of mileage and time, not only for my surgical services, but for pre and post-op and rehabilitation appointments. Most importantly for me, using CFVH allowed me to minimize the impact upon my business and lost time during recovery. I am the owner/operator of Able Wright, Inc. and we do specialty disinfection and chemical feed work across many northwestern states which requires me to travel often. My care providers were sensitive to this and helpful when scheduling appointments for me. I have enjoyed the familiarity and consistency of all CFVH departmental teams. Clinical, surgical, acute care, nursing, imaging, housekeeping and rehabilitation are all top notch. My surgeries and recovery were not without complication, but my concerns and problems were always dealt with promptly and I never had a reason to regret my hospital choice. Community Benefit

As a tax-exempt non-profit healthcare provider, it is an important part of our mission to maximize and measure the benefits we provide to the communities we serve. Some of the benefits we provide are more obvious in nature, such as charity assistance and sliding scale discounts to low income patients which amounted to $647,827 in 2014. Other community benefits are not as well known, but are nonetheless diverse and include: Community Benefit Operations Community Building Activities Community Health Improvement Services Financial and In-Kind Contributions During 2014, CFVH employees tracked 475 hours of work time dedicated to various community activities. Student Sports Physicals Program Encourages physical activity and wellness through youth athletics 2014 marked the third year CFVH donated 100% of its proceeds from the Student Sports Physicals Program to local Sanders County Booster Clubs. Clubs benefitted from donations anywhere from $450 - $1,700, with total donations amounting to over $4,300 across the county. With childhood obesity and other serious health issues on the rise, CFVH wants to encourage community youth to get moving and make physical activity and wellness a focus in their lives earlier rather than later.

CFVH has donated over $12,000 to local Booster Clubs through the Student Sports Physicals Program since it began in 2012. Please note: The $12,000 figure includes 2015 donations for the 2014 - 2015 school year which are not represented in the 2014 Community Benefit Chart to the left.

REACH Students Exploring Careers in Healthcare CFVH has a long history of supporting healthcare career exploration amongst local Sanders County youth. In Spring 2015, students from across the county joined CFVH employees for a day on the job as part of the Montana Area Health Education Center (AHEC) REACH program. Students were given the chance to explore a number of fields in healthcare through a variety of hands on activities including intubation and IV placement of a dummy patient in the ER and sutures of chicken thighs in the clinic setting. We hope experiences like these will spark an interest in local youth to pursue careers in the industry and possibly return to the area to work one day.

Shelby Zenahlik, RN, BSN, Acute Care Clinical Coordinator As a nurse caring for patients in a rural hospital setting, I have the unique opportunity to strengthen my skills in all areas of nursing. One day I am needed in the ER, the next in OB or with a patient in the ICU. You never know where your day will take you and I appreciate that my work is never boring and I am always learning. The size of our hospital allows our care team to develop a unique familiarity with each other that allows us to care for patients in a way larger hospitals can’t. We work closer as a team and our patients get the benefit of that in their care. Providing quality patient care for me means acknowledging the person I am caring for, not just the patient. Here at CFVH, we truly are members of a community sharing a community and each person who walks through our


doors is our friend, neighbor or loved one. I feel very blessed to work with so many people who truly care about, care for and advocate for our patients. It makes me proud to be part of the CFVH family.

Rusty Kinkade, Acute Care/Emergency Dept. Unit Secretary Healthcare has always been important to me, whether I was coaching, teaching, working in the emergency room, Home Health and Hospice or practicing as an EMT or Radiological Technician. Individuals are always looking for treatment, education and support to improve their physical, emotional and spiritual health and I find being able to assist in those areas is a very rewarding experience. Recently, I served as manager for the CFVH Home Health and Hospice Department. This service is a unique aspect of medicine that is vital to our area. Having the opportunity to assist a patient that is restricted to their home or is receiving end-of-life care is truly a blessing. I believe this is where we can make a tremendous difference and impact in a patient’s life; sharing everyday life-on-life experiences, whether it be smiles or tears, with our patients during this time can be very positive. My mother received services from our hospice program and I will be forever grateful for the care and compassion she received. Each person of our care team contributes their individual expertise to manage patient care specific to the individual patient needs. Personally, I have chosen to approach each daily encounter with the heart of a servant as do many of my co-workers. I believe this attitude has become a distinct philosophy with many of the members of the care team here. We have an excellent and experienced staff that works diligently to provide positive outcomes for each and every patient that comes through our door.

TaLoni DuBois, RT, Cardiopulmonary Services Manager After working at larger hospitals for most of my career, I see the unique opportunities a smaller hospital setting can offer healthcare professionals. Throughout my eight years at CFVH, I have been able to apply all of my respiratory therapy skills and knowledge as I care for patients in not just one, but a variety of service areas. I feel challenged, valuable and like I am truly making a difference in the lives of my patients. I feel so blessed to have the job I have. I get to be a part of a team that helps save lives, teach patients about lung disease, assist new moms with their babies, comfort family members during difficult health challenges and encourage patients to accomplish small victories while fighting their illnesses. I get to be a cheerleader, teacher and a lifesaver all in one. At CFVH, we love our patients...plain and simple. They are like family to us and we make decisions based on what is best for them. I recently had the chance to work with a patient in one of our Pulmonary Rehabilitation Program courses who came to me with very little knowledge about his lung disease and the effects food, not eating and lack of exercise had on his body and ability to breath. Our team cheered him on to work hard and be proud of small victories. He lost 30 pounds and gained a new freedom in life. He told me at his program graduation that my team saved his life. To hear that touched my heart and was evidence I am fulfilling my purpose and making a difference in my community.

Specialty Care Services offered from CFVH and the Plains Family Medicine Office include Audiology, Cardiology, Neurology, Ophthalmology and Orthopedics. Have you established care with a Primary Care Provider? Identifying and successfully treating medical conditions before they become serious enough to require hospitalization or emergency treatment is an essential reason to establish care with a Primary Care Provider (PCP). Regular scheduled visits with your PCP will provide an understanding of potential health risks, continuity of care, pharmacy assistance and access to additional health resources.
