Physical disability If you have a physical disability or a longterm illness, Adult Social Care and other organisations in the independent and notfor-profit sectors may be able to signpost you to, or provide you with, a range of services to help you live as independently as possible. There are also services available to support someone who may be your carer. As with all support from Adult Social Care, your needs must firstly be assessed (see page 15) to determine whether you are eligible for assistance and identify the best ways to support you, regardless of eligibility. You may wish to discuss your needs with your family doctor in the first instance.
Sensory services The rehabilitation service may provide specialist assessment, training, information and advice to people with visual impairments (and their carers). The service aims to provide training and advice about visual impairments, so social workers may also need to be involved to assess other social care related needs. Information can be given about mobility, communication, independent living skills, assessment and specialist environmental aids. Please contact your Adult Social Care department, see page 4 for details. They will be able to advise you on any sensory services in your area.
Advocacy
Advocates can give advice, support and information to people of any age. They help you to voice your concerns and guide you through difficult or challenging times. You should consider using the services of an advocate if you feel unsure or concerned
