UPON ARRIVAL TO Brave Horse, 1029 South Countyline Rd , Johnstown, OH 43031, ALL HORSES MUST HAVE AND PROVIDE:
HEALTH CERTIFICATE
REQUIRED
The Ohio Department of Agriculture requires each horse coming from out of state to have a valid Certificate of Veterinary Inspection (Health Certificate) dated within the prior 30 days of entry into Ohio with the following address listed as the Premises of Destination: Brave Horse, 1029 South Countyline Rd., Johnstown, OH 43031.

REQUIRED
Dated within twelve months (365 days) of the ending date of each show. This includes horses that originate from the state where the event is being held
All horses entering the Show Grounds must have a Negative Coggins on file. Coggins must reflect the horse's SHOW NAME.
EVENT PARTICIPATION DECLARATION
REQUIRED
Signed by the owner/agent/trainer verifying that the horse has been healthy with no sign of infectious disease and has not had a fever above 102°F within 72 hours (3 days) of arrival Event Participation Declaration Form available in this prize list and at Brave-Horse com
A new Event Participation Declaration Form must be filed each time the horse enters the property Horses stabled off the property that "ship-in" on a regular basis must comply with this requirement as well as additional requirements noted on the following pages
PROOF OF VACCINATION REQUIRED
Via a signed statement from the attending veterinarian that the horse has been vaccinated against the following:
Within 6 months (180 days) of each show: Equine Influenza (EIV)
Within 6 months (180 days) of each show: Rhinopneumonitis (EHV 1 and EHV 4)
If a vaccination will expire during one of the shows, the horse must receive its new vaccination prior to being issued a competition number for that show.
PRESENTATION OF HEALTH REQUIREMENTS
The Equine Health Requirements are Required for all horses entering the Show Grounds both Showing and Non-Showing. These documents must be filed in the Show Office before any competition numbers will be issued In addition, random checks by Ohio State Veterinarians for Equine Health Requirements will occur. Be sure to keep a copy of all of your horse’s Equine Health Requirements in an easily accessible spot (in your tack room or tack trunk, truck or trailer) to ensure you can comply with this requirement The results on all required papers must indicate the horse’s registered (show)name
Any horse not accompanied by these documents will be directed to the Show Veterinarian to obtain the required documents and/or vaccinations, and/or will be placed in quarantined stabling until the proper documents are obtained Any horse showing signs of fever/illness/stress is subject to examination by Brave Horse Officials and/or the Show Veterinarian, who may at their sole discretion, place the horse in quarantined stabling or take further action if deemed necessary
SHIP - INS
Any Horse entering the Brave Horse Show Grounds property, showing or non-showing, must abide by the Equine Health Requirements as set forth in the prize list. In addition, horses that ship-in daily for their classes, for warm-ups, or that are on the grounds for any reason and do not reside in a stall on the show grounds must also abide by the following: Health Documentation: All haul in horses (showing or non-showing) must file the proper Health Requirements in the show office, and must be prepared to present those health records at any time while on the show grounds Ship-ins/day trippers will be subject to random checks by Brave Horse Officials and/or Staff.
Keep a copy of all of your horse’s records in your truck or trailer to ensure you can comply with this requirement If proper records are not immediately available, the horse(s) a ) must remain under Brave Horse Staff observation until the paperwork is presented, or b ) will be asked to leave the grounds See Rules and Regulations for more information

• Riders must be a USHJA member at the free Outreach level (or higher). Rider may complete an application at the show or join online via the USHJA website.

• Horses do not need to be registered or microchipped in order to participate. Those wishing to have their horses earn Outreach points must record them with USHJA ($20/annual or $75/lifetime).

THE TEAM CONSISTS OF 3 MEMBERS PER TEAM HORSE/ RIDER COMBINATIONS MAY NOT COMPETE AT HEIGHTS OVER 1.15M AT THE SAME EVENT. DECLARATIONS: TEAM DECLARATIONS MUST BE MADE NO LATER THAN 4PM ONE DAY PRIOR TO THE DAY OF THE COMPETITION AND MUST INCLUDE THE TEAM ORDER OF GO AND THE TEAM NAME. THE TEAM WILL ENTER THE COMPETITION ARENA TOGETHER. EACH TEAM MEMBER WILL NAVIGATE THE COURSE ONE AFTER THE OTHER, SCORES ARE ADDED TOGETHER TO DETERMINE THE PLACINGS. AT THE CONCLUSION OF THE FIRST ROUND THE ORDER OF GO WILL BE RESET AND THE TOP SIX (6) TEAMS WILL RETURN IN REVERSE ORDER OF QUALIFICATION FOR THE SECOND ROUND, THE BEST QUALIFIER COMING IN LAST. EACH ROUND WILL BE JUDGED INDEPENDENTLY. REGULAR JUMPING RULES ARE IN PLACE. EACH ATHLETE MUST WAIT FOR THE TONE FOR THEIR RESPECTIVE ROUND.
PLACINGS: FAULTS AND TIME ARE ADDED TOGETHER TO CREATE A TOTAL TEAM SCORE. THE WINNING TEAM WILL BE THE TEAM WITH THE FEWEST FAULTS AND LOWEST TEAM TIME OVER THE TWO ROUNDS. ALL OTHER PLACINGS WILL FOLLOW THE SAME. IN THE EVENT OF A TIE FOR ANY PLACING, THE TOTAL TEAM TIME OF THE SECOND ROUND WILL BREAK THE TIE. IN THE EVENT THE TEAMS ARE STILL TIED, THE TIME FROM THE SECOND ROUND OF THE QUICKEST RIDER WILL BREAK THE TIE(S). 1) The customer represents that only licensed drivers will operate equipment and that the vehicle will be used in a safe and prudent manner in accordance with all instructions on equipment.
2) Customer agrees to carefully use said equipment, and returns the equipment in the same condition as when Customer received it. In the event that the equipment is lost, stolen, damaged or in disrepair regardless of fault, customer agrees to reimburse Brave Horse for all necessary repairs, fees including replacement of equipment. Equipment Value may be up to $10.000.00 ea. Customer agrees not to sell or attempt to sell, remove or attempt to remove the same or any part thereof, from the place of business listed above without the written consent of rental company.
3) Customer agrees not to tamper with any part of the throttle assembly, linkage or governor. Customer also agrees not to place any type of stickers or advertising on said equipment, other than on the information holder provided, that may damage or leave any glue residue on the body material or any other part of the equipment and to return equipment in reasonably clean condition, free of trash. A charge for each item may be added to the rental bill for damage. $80.00 per hour plus parts will be charged to cover the expense of readjusting linkages, removal of trash, stickers or glue and the pressure washing of excessive dirt, mud or other debris. The customer will be charged $5.00 per key for all non returned keys.
4) It is expressly understood that the title to the equipment shall be and remain in rental company name, free and clear of all claims of the Customer or others.
5) Customer agrees that it shall not pledge, or part with possession of the equipment, or mortgage it, or suffer any claims or encumbrances to be made thereon, it is further agreed that this rental agreement may not be assigned except with the consent of rental company first being obtained in writing.
6) Brave Horse reserves the right to inspect the equipment at any time during the contract.
7) Brave Horse and its employees retain the right to take possession of any equipment at any time for default in payment or upon the breach of any of the conditions in this contract. Customer on demand by Brave Horse shall return the equipment in as good condition as when received by customer. And should customer fail or refuse so deliver equipment. Customer hereby authorizes Brave Horse to enter any premises in which the equipment may be found with or without force or process of law, and forthwith take possession of equipment.
9) Customer agrees to indemnify and hold Brave Horse, Mid Ohio Golf Cars, and employees and assignees harmless from any and all losses, damage injuries, claims, demands and expenses by (including but not limited to attorneys fees, court costs and any claim made under strict liability) arising out of use or operation of the equipment by Customer from and after delivery of the equipment. In the event customer is a party to proceedings subject to this paragraph, at the option of Brave Horse and/or MOGC Customer shall receive such counsel and other third parties as are reasonably necessary and acceptable to defend such action. The provisions of this paragraph shall not apply to claims resulting from the willful negligence of Brave Horse and/or MOGC in maintaining and/or repairing equipment during the term of this agreement
10) Customer agrees to allow Brave Horse or Mid Ohio Golf Car to charge the following credit card: Card________ #_________-_________-_________-_________ Exp. MOGC will also charge for any damages it suffers as a result of customer’s breach of any provision of this agreement.

Return this form via email to STALLS@BRAVE-HORSE COM in this way, we hope to accommodate everyone’s wishes and stabling requirements Stalls and pre-ordered shavings MUST be preordered 14 days prior to the start of the show. Shavings can be ordered in the office upon arrival and wait for delivery No stall will be reserved without a check or credit card authorization. No refunds after the closing date.
