The majority of people are able to return home following a stay in hospital without any additional support. If you feel you or your family member will require support to return home, please speak with the ward team who will be able to discuss your options with you. Where necessary, the ward team will work with our Integrated Discharge Team to support you in developing a plan so you can be discharged home, as quickly and as smoothly as possible.
