RECENT BAD WEATHER COULD IMPACT IRISH RETAIL SECTOR The retail sector in Ireland is counting the cost of the recent bad weather as many outlets were forced to close because of the dangerous conditions a couple of weeks ago. It was predicted in advance of the snowfall by Retail Excellence Ireland that ‘Storm Emma’ and the ‘Beast from the East’ could mean hundreds of millions of euro lost to the industry between revenue, repairs and employee costs. The lost trade equalled a considerable amount of money, said Lorraine Higgins, deputy CEO of the business group. “Those that are e-commerce enabled are certainly used this as an opportunity to try and drive traffic, however challenges remain with broadband for rural retailers and the overall lack of web presence by Irish retailers,” she added.

Retail Excellence Ireland along with Retail Ireland urged retailers in advance of the bad weather to prioritise the safety of their staff and customers and to heed the advice of national bodies like Met Éireann. “Safety is paramount, of customers and staff. Members are taking that quite seriously,” Higgins said. Meanwhile as the country braced itself for the worst pharmacists advised people to ensure they had an adequate supply of essential medicines before the very cold weather hit. The Irish Pharmacy Union warned that the forecast meant that pharmacies and patients could experience significant disruption for several days.

SANFORD HEALTH INCREASES GLOBAL PRESENCE WITH EXPANSION IN SEVEN COUNTRIES, INCLUDING IRELAND

Hermitage Medical Clinic in Dublin Sanford Health is expanding its presence in international healthcare by extending services to seven countries, including Ireland. This expansion follows last year’s acquisition of a minority stake in ISAR Klinikum, a hospital leader in stem cell therapies, located in Munich, Germany. Sanford Health’s international healthcare arm Sandford World Clinic will now be in nine countries with more than 30 locations. “With these partnerships, we are creating unique opportunities for shared learning,” said Kelby Krabbenhoft, president and CEO of Sanford Health. “This is not something we are pursuing for financial gain, but we believe this type of collaboration will help further our mission of health and healing.”

NEUROMOD ANNOUNCES APPOINTMENT OF CHIEF SCIENTIFIC OFFICER Neuromod Devices Ltd, an Irish medical technology company specialising in non-invasive neuromodulation technologies, has announced the appointment of Dr Hubert Lim, a world-renowned scientist and thought leader in auditory neuroscience, neuroplasticity and neuromostimulation, as chief scientific officer. Dr Lim is a widely-published leader in the neural engineering field, and has been at the forefront of the development and translation of brain-machine interfaces from scientific concept into clinical application. “Dr Lim will continue to expand the therapeutic applications of Dr Huber Lim, Neuromod’s non-invasive neuromodulation platform to additional Neuromod’s new chief indications to meet the unmet needs of many patients. In addition, scientific officer his strong ties to the global neuroscience and neuromodulation communities will be important to ensure that Neuromod’s clinical and scientific approach continues to robustly and conclusively build the evidence base to support widespread acceptance,” said Dr Ross O’Neill, CEO of Neuromod. SMALL FIRMS CAUTIOUSLY OPTIMISTIC FOR 2018 The mood amongst the small business community at the start of 2018 was cautiously optimistic, with nearly two-thirds of owner-managers believing that the business environment is improving, up from 50pc one year ago and 61pc in May 2017, according to the Small Firms Association (SFA) in its ‘Small Firms Outlook 2018’ survey report. SFA director Sven Spollen-Behrens said that 2017 has been a challenging year for small business. “Whilst cautious optimism seems to be returning amid emerging wage demands, the increasing cost of doing business and Brexit dampens the mood. However, nearly 60pc of SFA members say their businesses are growing, with only 4pc declining. This shows that 2018 has the potential to be a strong year, if the risks are managed effectively at firm level and Government level.” Nearly three-quarters (71pc) of survey respondents indicated their intention to recruit over the coming year, up substantially from the SFA survey in May 2017. “Small

firms already employ half of the private sector workforce and nearly three-quarters of our members will be hiring in 2018. Small firms have a crucial role to play in job creation around the country, reducing unemployment and attracting emigrants home to work,” continued Spollen-Behrens. The survey results, however, highlight a number of areas of concern for small firms. “A number of risk factors for 2018 have been highlighted, such as wage inflation, the ability to attract talent, legislative and regulatory burdens, increasing business costs as well as Brexit/Sterling exchange rate,” he added. “Many of these require decisive measures at Government level and the SFA will work with the Department of Business, Enterprise and Innovation and other departments to ensure the appropriate actions are taken. The fundamentals of the Irish economy are strong and economic growth and job creation are forecast to continue in 2018. If the specific concerns of small businesses are addressed, 2018 will be a very positive year for the sector.”

SHOULD FLU SYMPTOMS BE TREATED WITH ANTIVIRALS IN PRIMARY CARE?

The HRB Primary Care Clinical Trials Network Ireland, based at NUI Galway, are working with researchers in Oxford University’s Nuffield Department of Primary Care Health Sciences, on the ALIC4E trial, which investigates whether the flu drug oseltamivir (Tamiflu) is cost effective and beneficial to patients consulting their GPs with flu symptoms. In particular, the study aims to understand if older people, infants, people with other health conditions, those treated early, or those with particularly severe flu can benefit from the treatment. Over 2,000 people have been recruited into the world’s largest clinical trial to test the clinical and cost effectiveness of the flu drug oseltamivir in primary care. GPs and practice nurses in five practices in Ireland are currently recruiting patients at Turloughmore Medical Centre and Main Street Clinic Loughrea in Co Galway, and Belgrave

Clinic, Tallaght Cross and Crumlin Medical Clinic in Dublin. “We don’t know for sure which people with symptoms of the flu should be prescribed antiviral drugs, and nor do we know the cost-effectiveness of antivirals in terms of helping people return to normal activity levels. The ALIC4E trial aims to answer these important questions,” said network collaborator Professor Tom Fahey of the Royal College of Surgeons in Ireland. ALIC4E is an initiative of the Platform for European Preparedness Against (Re-)emerging Epidemics (PREPARE) consortium. Funded by the European Commission’s FP7 Programme, PREPARE was set up to support research organisations to respond rapidly to pandemics with clinical studies that can provide real-time evidence to inform the public health response.

PHARMACISTS CALL FOR INTRODUCTION OF PHARMACY-BASED MINOR AILMENT SCHEME

In response to the worsening healthcare crisis, the Irish Pharmacy Union (IPU) has called on the Government to immediately implement a pharmacy-based Minor Ailment Scheme, which will allow Medical Card patients to access treatments for common illnesses directly from their pharmacist without having to attend their GP to obtain a prescription. This would alleviate pressure on overworked GPs, who could then see more seriously ill patients who may otherwise find themselves in hospital emergency departments. “Community pharmacists can play a greater role in patient care, particularly at times such as now when the rest of the healthcare system is under extreme pressure. A Minor Ailment Scheme could divert patients away from busy GP surgeries when their ailment is one that can be treated by a pharmacist, allowing GPs more time for treating patients with more complex

illnesses,” said IPU secretary general Darragh O’ Loughlin. “Similar schemes already operate very successfully in other jurisdictions including the UK. The IPU, together with the HSE, successfully piloted the service here in Ireland in 2016 but for some unknown reason, it has not been implemented yet, even though four out of five members of the public have said they would welcome such an initiative.”

SELF CARE REPORT LAUNCHED BY IPU AND IPHA Promoting increased levels of self care in Ireland would lead to a far more efficient and effective system of healthcare, according to a new report published by the Irish Pharmacy Union (IPU) and the Irish Pharmaceutical Healthcare Association (IPHA). ‘Self Care – taking charge of your health’ sets out a range of proposals for a more focused approach to self care which could lead to better patient outcomes and considerable savings for the State. Self care is a healthcare philosophy that emphasises the role of ordinary people in taking responsibility for their health and well-being. There is a focus on empowering people to take a more active involvement in looking after themselves through greater health literacy and greater access to information. As demonstrated by the publication, which is based on research from Behaviour & Attitudes, there is a clear desire from the majority of people (92pc) to be involved in their own self care. The report found that 92pc of people like to be involved in decisions about their own health and medication; 94pc reported that they have a mature and responsible attitude to maintaining their own health; nine out of 10 pharmacy customers indicate that the quality of medical advice they get at their regular pharmacy is

either very good or good; and 80pc view the pharmacist as a key partner in maintaining their health. The role of community pharmacists is pivotal to the successful development of self care and its more widespread adoption. A number of recommendations are made within the report to help achieve this. These include expanding the role of the pharmacist including the free treatment of common illnesses to medical card patients through the establishment of a pharmacy-based minor ailment scheme; the range of medicines made available to patients should be expanded through switching; the concept of self care should be actively promoted among healthcare professionals; patients should have access to good quality, trustworthy information so that they can seek care at the appropriate level and, thus, enhance their independence within the healthcare system; and patients should know that self care means access to high quality, safe and effective products.

Strong growth online within the pharmacy sector Retail Excellence Ireland’s lastest eCommerce Dashboard Report, a monthly publication that provides a measurement that allows retailers to benchmark their online performance within their individual sector, has found that the pharmacy sector continues to perform strongly. The sector recorded the highest desktop computer conversion rate of over 3pc and an increase of almost 2pc across mobile devices. Retail Excellence’s ecommerce manager David Campbell said the report showed the current state of play in the Irish online marketplace. “We are seeing strong performances within pharmacy that have continually highlighted strong growth online over the past 24 months. It is clear from our research that Irish

consumers’ buying habits are evolving and it is important for retailers to recognise this.” Most consumers did their research online before purchasing, and this was corroborated by the figures on mobile browsing activity. “It is clear that retail is now boundary-less and there is a huge global opportunity for Irish retailers to capitalise on,” added Campbell.

SURVEY FINDS SHOPLIFTING MOST COMMON CRIME IN PHARMACIES Almost 80pc of pharmacies have been the victims of crime within the last year, a survey by Irish Pharmacy Union (IPU) has revealed. A total of 79pc of pharmacies have been affected in the last 12 months by crimes such as shoplifting, break-ins and raids. Almost one-third of these cases were described as violent and/or included a weapon. The survey revealed that while shoplifting was the most common crime in pharmacies, the number of pharmacists who experienced a raid jumped to 13pc, compared to 6pc the year before. "Almost one in three crimes against pharmacies is violent in nature, involving not only a physical threat but also a substantial psychological threat to victims. It is difficult enough to run a pharmacy in the current environment without repeatedly being the target for criminal activity,” said IPU president Daragh Connolly. "Not only do these crimes have significant cost implications but more importantly, they have a very detrimental impact on the people working in the pharmacy. It is unacceptable that pharmacy owners and their staff are viewed as ‘soft targets', where the probability of repeat offences is high and the risk of apprehension and penalty is low.” Almost all pharmacists (97pc) had invested in CCTV to help protect their business and staff. Crimes against pharmacies have now reached “crisis levels”, noted Connolly. “Pharmacists who are victims of crime say they are sick and tired of the ‘revolving door approach', with many complaining that even when the criminals are caught, they are not sufficiently penalised and are allowed to continue with their criminal activities.”

REVOLUTIONARY EYE TECHNOLOGIES MANUFACTURED IN CORK BY ALCON TO BE LAUNCHED INTO EUROPE

New ground-breaking eye technologies were used in surgery recently to treat a patient with a cataract. Consultant ophthalmic surgeon and retina specialist at the Institute of Eye Surgery Eugene Ng performed the surgery, which involved an innovative lens implant procedure that uses a new biomaterial and unique delivery system to greatly enhance patient vision and clarity. Created by Alcon, a global leader in eye care, the surgery marks the beginning of a Europe-wide launch of the technologies after the award of the European CE Mark of regulatory approval. To become commercially available in Ireland soon, the futuristic lens implant system is manufactured in Cork and was invented by Alcon, a subsidiary of Novartis, which specialises in technology for cataract surgeries. Frank Doheny, general manager of the Institute of Eye Surgery, which provides ophthalmic surgical capability to Whitfield Clinic in Waterford and Clane General Hospital in Kildare, said the surgery represented a great win for healthcare innovation in Ireland. “This development is a huge boost for Ireland Inc and allows us to further progress up the global healthcare innovation food-chain. It builds on Ireland’s capacity to attract investment from global pharmaceutical giants due to the presence of a highly skilled workforce and favourable socio-economic conditions here. The Institute of Eye Surgery is extremely excited to play a part in this fantastic Irish success story.”

PLAIN CIGARETTE PACKAGING NOW ON IRISH SHELVES Plain cigarette packaging with prominent health warnings is now on shelves in the Republic as part of the State’s Tobacco Free Ireland programme. It aims to make tobacco packs less attractive to consumers and also features graphic photographs of illnesses and diseases people develop as a result of smoking. Ireland is the fourth country in the world to introduce plain cigarette packaging. “Our aim is to decrease the appeal of tobacco products, to increase the effectiveness of health warnings and to reduce the chances of consumers being misled about the harmful effects of smok-

SEXUAL HEALTH T

he pharmacy can play a key role in advising the public about sexual health and contraceptions.

The local pharmacy is often the first port of call for people looking for advice about their sexual health and contraceptive needs. Indeed less than two years ago the Irish Pharmacy Union (IPU), supported by the IFPA and the HSE Sexual Health and Crisis Pregnancy Programme, launched a campaign to encourage people to ask their pharmacist for advice about their sexual health and contraceptive needs. The campaign complements the Government’s Sexual Health Strategy 2015-2020, which is Ireland’s first national framework for sexual health and well-being and was launched the previous year. With Ireland witnessing a surge in sexually transmitted infections (STIs) – the latest Health Protection Surveillance Centre data confirming at the beginning of the year that rates of all major STIs have dramatically increased from 2012 to 2016 – the pharmacy sector is continuing to take an active role in improving sexual health and combating the rise in STIs. If any of your customers have sexual health queries at the counter you can invite them to have a private and confidential consultation with the pharmacist, however you should also have some knowledge yourself.

They can be spread through sexual contact whether it is oral, anal or vaginal and can spread quickly from one person to another especially if they are not practising safe sex. Although many STIs are transmitted through sexual penetration certain STIs can also be spread through skin to skin contact with the genitals or mouth. Symptoms of STIs include, but are certainly not limited to, genital rashes, pain during and after intercourse, and redness or discharge in the genital region. It’s important to note that 80pc of STIs are symptomless, however a person can have more than one STI at once. One of the best ways of STI prevention is condom use – so don’t just encourage customers to buy them to prevent pregnancy. Make sure to explain the importance of having an STI screening to customers. Early diagnosis is critical in helping to prevent the spread of STIs, as well as maximising the health outcomes of the infected person. A list of free STI testing clinics can be found on www.hivireland.ie and many pharmacies now stock STI home testing kits – so if your pharmacy is one of them explain how to use it.

Emergency contraception

The emergency contraceptive pill (ECP) is available now directly from pharmacies without a prescription. Emergency contraception is a safe, effective and responsible method of preventing pregnancy when regular contraception has failed, no contraception was used, and/or in the case of sexual assault. If a person acts quickly, emergency contraception will usually prevent pregnancy. Anyone coming into your pharmacy looking for emergency

IRRITABLE BOWEL SYNDROME (IBS) EXPLAINED

Irritable bowel syndrome (IBS) is a disorder of the large bowel (colon) in which the bowel overreacts to a mild stimulus – such as eating or the presence of gas – by going into spasm. It is also known as spastic colon. IBS is characterised by abdominal pain, bloating and irregular bowel habits – including alternating diarrhoea and constipation. IBS is extremely common (One to seven adults). It is most common in people in their 20s and 30s, and affects more women than men. In

some people irritable bowel syndrome is a mild annoyance, while in others it can have a tremendously debilitating effect. Some symptoms of IBS may include:

Piles Also called haemorrhoids, piles are the result of increased pressure on the blood vessels in your bottom, which become swollen and infl amed. Internal piles, which are the most common form, are tucked up and away; external piles protrude from the back passage and may look or feel like hard, painful lumps.

Factors that increase the risk of developing piles: • Prolonged constipation, due to the extra effort required at the toilet • Pregnancy, due to extra weight adding pressure to pelvic blood vessels • Aging, as our supporting tissues weaken • Being overweight or obese

Preventing piles: Simple lifestyle changes can reduce the strain on the blood vessels around your bottom, and lower your risk of developing piles: • Eat more high-fi bre foods like whole grains, fruits and vegetables, and avoid refined and processed foods like cakes and biscuits. • Drink plenty of fl uids to stay hydrated and prevent constipation. The important task of writing pharmacy policies and procedures rarely gets the respect it deserves. Most business owners recognise the need for having Standard Operating Procedures (SOP Documents) but rarely have the time to develop such lengthy documentation. However, good procedures and work instructions provide a way to communicate and apply consistent standards and practices within your Pharmacy business. An SOP manual is one of the most useful systems to streamline your pharmacy business. The manual lists all the tasks that are essential for business success, how to do these tasks and who is responsible for all the tasks listed. Your productivity improves, and as a small/ medium business owner you need to be highly productive to succeed. The five main benefits of an SOP manual are: 1. Reduced training time for new staff members: When a team member is new on the job, your well-written SOP will be a lifeline to them to be able to know how everything works in your business. 2. Ensure business continuity: When one of your key players is absent, the pharmacy doesn’t have to come to a standstill. By simply referring to the SOP

Standardise processes: The SOP makes it easy to find out what company policies and procedures are in place to handle repetitive pharmacy tasks. 4. Put a pension plan in place for both you and your employees, advises Ted Dwyer

I

recently attended a seminar hosted by one of Ireland’s largest pension companies in the new Pairc Ui Chaoimh stadium in Blackrock, Cork. One of the topics covered was the well-documented lack of interest that many young Irish people have in setting up a pension plan. In the UK it is now compulsory that when anyone enters the workforce they automatically become a member of a pension plan. Yes, they can opt out if they want to afterwards, and some do. In Australia, contributing to pension has been compulsory for years and the value of their individual pension pots is often a topic of conversation amongst young Australians. You might wonder why? Well, it is now compulsory for an Australian employer to contribute a

staggering (to us) amount each year of 9.5pc of the salary being paid to each employee. Each employee must also contribute 3.5pc of their salary. Therefore, a total of 12.5pc of each employee’s salary is invested in their individual pension pot each year. If the salary paid is €30,000 then every year €3,750 is being invested in that person’s pension fund. It’s no wonder the young Australians are so interested and involved in their pension planning, and why they are constantly checking the value of their pension funds online! It is assumed that Ireland will eventually go down a similar road. My advice to employers and employees is not to wait until then. Get some advice and put your own plan in

place for you (the employer) and for your employees. You will enjoy the peace of mind and the generous tax reliefs.

STRESS AT Within the workplace, an employee will often be confronted by a number of different challenges. These challenges or factors can also have a negative effect on the employee’s ability to deal with stress, or be the cause of stress. An organisation’s culture can be the key in determining the factors of work related stress. This particular element can highlight whether working long hours is affecting employees. Poor communication and management style also contribute and will indicate the organisation’s attitude towards stress. Working demands Demands on the employees can result in elevated stress levels. This may occur if too much work is expected and there is insufficient resources provided. The work may be too difficult, or management may have inadequately trained the employee to complete the task. Control of work The lack of control over the

job can result in heightened stress levels, this may happen when the pace of the work is set outside of the worker’s environment. Relationships within the workplace Some employees may have difficulty interacting or building relationships with existing staff. In some situations this can lead to interpersonal conflicts, which could just be

down to a lack of communication. Position and training When an employee begins a new job, stress can develop because their position is not clearly defined or they do not fully understand what is expected of them. They may not be completely confident of the scope and requirements of the position. There is a lack of support from their

AT WORK surrounding colleagues, as well as from the management. Training can also be a major factor, as a lack of training at induction or when a working task has changed can result in stress. Additionally, the training may not be adequate and result in the employee feeling worried or anxious. What does this mean for employers? If stress is allowed to embed itself within the workplace, it can have a number of negative effects on the business. An increased number of employees calling in sick will

reduce output and productivity. It will affect the workforce’s morale and motivation. High staff turnover and recruitment problems indicate a lack of control by management. This can cost organisations. How employers should manage stress Employers should adopt a prevention, management and minimisation policy. This approach will look at the issues of stress, what is the source of stress and will help stop its reoccurrence.

Management of the employee through their career. This is finding the fine line between micro-management and a lack of engagement with employees. Finally, the minimisation of stress can occur through counselling of employees or assistance programmes to provide extra support if the employee feels it necessary. HEART OF THE MATTER The role of the pharmacist is an important one when it comes to helping people maintain good heart health.

P

harmacists, dieticians and pharmacy staff all play important roles in helping people to maintain their heart health, but their efficacy increases when they work together. Being part of a team means filling a valuable role. The best teams have people who complement each other and push the group’s performance beyond that of the individual. In a pharmacy setting, this concept is especially important. A healthy diet and lifestyle are the best weapons to fight cardiovascular disease. Healthy eating and an active lifestyle are proven methods to reduce the risk of cardiovascular disease. Dieticians work in a variety of clinical and community settings leading nutrition interventions to reduce cardiovascular disease. Coronary heart disease is one of the leading causes of death in Ireland. One of the risk factors is high cholesterol, as it causes fatty deposits to build up in the arteries, making it harder for the heart to pump blood around the body. Cholesterol is a fatty substance required for many vital body processes. As well as being a structural component of all cell membranes, it is used by the body to produce some hormones, and it is a precursor to bile acids, which are vital for fat digestion. A dietician can assess the patient’s diet and implement cholesterol lowering advice where appropriate. Reducing cholesterol levels by 1pc can lower the risk of heart disease by 2pc. Also, by just eating healthy it is possible to

reduce cholesterol levels by 10pc! The main focus for a cholesterol lowering diet is reducing the total amount of fat a person eats, as well as the saturated and trans-saturated fat. Replacing saturated fats (eg fatty and processed meats, full fat dairy products, cakes, biscuits,etc.) with a small amount of polyunsaturated fats (eg vegetable oils) and monounsaturated fats (eg rapeseed oil, nuts etc) will lower LDL and increase HDL cholesterol. Diet plays an important role in maintaining a healthy heart, but there are many other factors that are linked with heart health. Other factors include smoking, being overweight and high blood pressure. It is also recommended to increase omega-3 in the diet. Oily fish (eg salmon, trout, mackerel, herring and sardines) is the richest form of omega-3 and can help to reduce cholesterol. Fibre has been shown to reduce LDL cholesterol by decreasing the amount of cholesterol absorbed by the intestine. It is also recommended to increase fruit and vegetables to at least five-a-day and have a handful of unsalted, uncoated nuts per day, eg walnuts, pecans, almonds, brazil nuts etc. Plant sterol and stanol esters are also regularly promoted as cholesterol lowering products. They compete with cholesterol in the gut and inhibit the absorption of cholesterol. They are effective in lowering cholesterol levels. Another factor to consider in relation to coronary heart disease is high blood pressure. High blood pressure has been linked with high salt intake. Therefore, it is recommended to avoid adding salt to foods, and reduce the consumption of foods that contain a lot of salt, eg bread, packet soups, processed meats and cheese. A high salt intake translates to >1.5g per 100g.

IRON NEEDS FOR VEGETARIANS Whatever the reason your customer has decided to become a vegetarian, a diet without meat or fish can be healthy, but because you're eliminating certain food groups from your diet you need to make sure that you get all your key nutrients.

O

ne of the biggest concerns when you are a vegetarian is ensuring that you're getting enough iron in your diet, which is an important mineral because it contributes to the production of blood cells.

For an iron-rich, nutrient-dense meal, add chopped red peppers, lightly steamed broccoli and fresh lemon juice to a portion of quinoa. Kale and broccoli naturally contain vitamin C so consuming these ingredients would be a bonus.

The World Health Organisation currently considers iron deficiency as the number one nutritional disorder in the world and as many as 2 billion (over 30% of the world's population) are anaemic.

Just as certain foods can enhance iron absorption, there are others that can inhibit its absorption.

Although meat may be one of the best sources of dietary iron, there are also a wide range of vegetarian foods that can provide you with all the iron you need. There are two types of dietary iron. 1. Iron found in lean red meat and 2. iron found in plants, grains and nuts. How to get more iron from your diet. When iron is consumed with vitamin C, the body absorbs it much better. It is therefore more beneficial for vegetarians to consume food with vitamin C. You can do this by adding fruit to your cereals, or consuming a glass of fresh orange juice. Similarly, adding vegetables such as tomatoes, red or green peppers, cauliflower or courgettes to your grains, pulses or brown rice or salads will have a similar effect.

Avoid caffeine, this will inhibit iron being absorbed. Tea, coffee, chocolate and cola-based drinks - which bind to iron and hinder the absorption of this important mineral. Try not to have a cuppa with your main meal and wait at least an hour after you've eaten.
