REQUIREMENTS • Achieve & fulfill all requirements in points for 2015/2016 Shaklee US Global Conference (or Umrah) and/or National Conference. • Points awarded to effective rank Supervisor and above. POINTS CALCULATION • Every 300PGUV excluding new enrollment PGUV = 1point • Every New Enrollment with purchase of 200PUV = 1point (Points can be earned by multiplying 200PUV applicable to single distributorship, excluding new business leader breakout) • Each time a break out of a downline Business Leader is achieved, you will earn 20 Points each.

• Consistency Point - Be effective at rank of Coordinator and above for 6 consecutive months during the qualifying period, to earn extra 10% effective rank points. FAMILY PARTICIPATION To qualify your spouse and children for the 2015/2016 Shaklee US Global Conference (or Umrah) and National Conference, the additional point requirements are as follows: • Spouse (85% of your Points Requirement) - SHAKLEE US GLOBAL CONFERENCE - 842points OR - UMRAH (optional for Muslim BLs) - 842points - NATIONAL CONFERENCE - 323points • Children aged 4-17 years old (85% of your points requirement) • Children aged 18 years old & above (100% of your points requirement) • Parent(s) or Parent(s)-in-law (100% of your points requirement for married Business Leaders) • Sibling(s)or Parent(s) (100% of your points requirement for single Business Leader)
REGISTRATION FORM Once you fulfill the Conference Points requirements for any of the 2015/2016 Shaklee US Global Conference (or UMRAH) and /or National Conference, a Registration Form is issued. If your spouse qualifies to participate in the Conference, please ensure he/she becomes a registered Distributor with Shaklee Products (Malaysia) Sdn Bhd. (Shaklee) NON-TRANSFERABLE, NON-REDEEMABLE Shaklee US Global Conference (or UMRAH) and /or National Conference are exclusively accorded to outstanding Business Leaders who fulfil the requirements for completion. In the event a qualifier is unable to attend any of the conferences, he/she will not be allowed to: • Appoint a representative or transfer the Conference Points to the subsequent year or ask for any set compensation in lieu of the accumulated points. • Appeal for pro-rated qualifications or buy-ups for non-qualifiers to attend these conventions. • Appeal for buy-in or pay pro-rated for the difference of the points to attend additional conferences other than the conference for which he / she qualifies. In the event a qualifier is unable to attend the Conferences, his/her spouse or immediate family member (parent, parent in-law or one child above 18 years old) will be allowed to attend. For Business Leaders (who are single, unmarried), who qualify for family participation, they are allowed to nominate their respective sibling(s) or parents to attend. Terms & Conditions apply.

Accompanying Children – Children accompanying their parents must not be below 4 years old. Compulsory Participation – Participants and their accompanying spouses are to take part in all conference functions and activities, including the Business Sessions (if any). Deviation/ Extension – Deviations during the conference are strictly prohibited. Deviation from or extension of stay after the conference, can be considered on a case-by-case review. Visa – The qualifier has to arrange for his / her own visa application. If the need arises, Shaklee Products (Malaysia) Sdn Bhd may assist by endorsing the application and getting the help of its appointed travel agent. Shaklee or its appointed travel agent is not be responsible for any delay or rejection by the relevant embassy. Entry / Exit Rejection – Shaklee is be responsible or liable, in any way, for any entry / exit refusal by the Malaysia Immigration, or any Government Authorities or Airline Companies and their counterparts abroad. Medical Information – Conference participants are to provide Shaklee with information concerning their state of health. In cases of chronic health problems, including pregnancies and confinements, participants are required to get medical clearance from their doctors to certify that they are fit to travel. Travel Schedule – Possible Changes in travel schedule due to unforeseen circumstances such as non-availability of airline seats, hotel accommodation, political situation, climatic conditions and other acts of God. Travel Arrangements – Shaklee Conference (or UMRAH) and National Conference are ex-KLIA, where participants will meet at KLIA for departure to the Conference/UMRAH destinations. Any deviation from the direct air connections (i.e. stopovers) will be the individual’s own decision, and all costs incurred in this situation will not be borne by Shaklee. Participants in the Shaklee Conference are encouraged to return with the main group. Deviation from or extension of stay after the Shaklee Conference are subjected to case-by-case review and an administrative charge of RM150.00 per pax. Any requests must be clearly stated on the Shaklee Conference Registration Form. Last minute requests will not be entertained. Accommodation Arrangement – All Shaklee Conferences participants are to reside at the accommodation provided by Shaklee. Hotel accommodation based on prior room arrangement is on a twin-sharing basis. Under special circumstances, triple-sharing may apply. Children are to share the same hotel accommodation with their parents. The parents are responsible for the cost of acquiring any additional accommodation not provided by Shaklee. Insurance & Personal Indemnity – Shaklee Conferences is packaged with various activities. As such, it is deem compulsory for all participants to arrange for insurance coverage for themselves, spouses and children. Before departure, all participants are required to submit a signed Letter of Indemnity to indemnify possible accidents, injuries etc. Changes – Shaklee reserves the right to change the Shaklee Conference rules, venues, hotels, programme and schedules if the need arises.Shaklee also reserves the right to invite only qualifiers who are in good standing with the Company, and who do not breach the Conference Rules and Regulations and/or the Privileges & Responsibilities (P&R). * Travel reward points are subject to the Sales Plan of Shaklee Products (Malaysia) Sdn Bhd

报名表格 一旦符合任何2015/2016年美国国际大会旅行资格后，您将获得一份表格供填写。如果您的配偶也达 到参加大会资格，确定他/她已注册成为马来西亚嘉康利的传销商。 不得转让，不得兑换 嘉康利美国国际大会（或UMRAH）及/或全国大会是一项特殊利益，只特别让同表现卓越的传销领 袖，并为达致所有要求分数的领袖而设。因此，下述情况将不为嘉康利激励旅行采纳，即使一位合 格者因故无法参加任何一项激励旅行，也不被考虑: • 委任一位代表或将研讨会转移至下一年，或要求按照所累积的分数作出任何补偿。 • 建议不合格者透过按比例分配(pro-rated qualifications)或累积(buy-ups)方式以达到参加研讨格。 • 建议以补购(buy-in)或支付分数差额以达到参加已符合资格的大会以外的其它大会。合格資格 只可轉讓於配偶或亲属(父母,岳父母或子女必须18岁以上)。 事业领袖（单身，未婚）家庭成员参与合格可让兄弟姐妹或父母参与。 需符合条件与条规。
其它重要资料 • 随行的孩子 - 跟随父母参与激励旅行的儿童必须年介于4岁以上。 • 激励旅行强制活动 – 参加者与同行的配偶必须参与激励旅行的所有宴会及活动,包括商业 研讨项目。 • 离队/延长逗留 – 严格禁止在研讨会中途离队。我们只按个别桉例处理研讨会结束以后的离 队或延长逗留。 • 签证 – 合格者必须亲自处理他/ 她的签证申请。假若必要，马来西亚嘉康利可认同有关申 请，并要求指定的流行代理于以协助。然而，假若大使馆以任何理由延慢或拒绝，嘉康利或 其代理概不负责。 • 入境出境遭拒 – 嘉康利将不会对任何入境或出境被马来西亚移民厅，任何政府当局，航空 公司及其海外伙伴阻止之事负责。 • 医药资料 – 大会参与者有须向嘉康利提供有关本身健康状况的资料。如果本身面对 慢性健康问题如怀孕及处于分娩期，参与者有须获得医生证明其身体状况 适宜旅行。 • 旅行时间安排 – 为方便合格者参与，嘉康利将尽可能在公共/ 学校假期主办激励旅行。基 于不可预见的情况，如订不到机位，酒店房间，天气状况，政治情势或不能抵抗的天灾，旅 行时间有可能有所更动。 •
流程安排 –国际大会及UMRAH是从吉隆坡国际机场出发，所有参与者将在该机场集合以 飞往激励旅行/UMRAH 目的地。建议嘉康利激励旅行的参与者与出发团一起回国。在大会 过后飞往其它地点或延长在当地的逗留的要求，将视为个别情况斟酌，并且每人有须缴付150 零吉的行政费用。任何要求必须清楚列明在研讨会登记表格上。临时提出的要求将不受理。
• 住宿安排 – 所有激励旅行参与者将住在嘉康利提供的住宿。如果两人同房的基础,酒店的住 宿是根据公司预定分配为准。在特别情况下，三人同房也有可能。子女将同他们父母同房， 如果不在嘉康利提供范围的，父母将须负责承担额外的住宿费。 • 保险与个人保险 – 嘉康利美国国际大会设有多项室内与户外活动。参与者必须为本身，配偶和 孩子购买保险。离境之，所有参与者受促向嘉康利呈交一份个人意外，受伤等的保单。 • 更改 – 如果有必要，嘉康利有权修改激励旅行条例，地点，酒店，活动项目与时 间表。此外，嘉康利也有权邀请与公司保持良好关系，以及不曾违反激励旅行的条 例规则，或特权与职责声明(P&R)合格者参与。

TEMPOH KELAYAKAN Mac 2015 hingga Februari 2016 PERSIDANGAN ANTARABANGSA SHAKLEE - 990mata ATAU UMRAH (pilihan bagi Pemimpin Perniagaan Muslim) - 990mata PERSIDANGAN NASIONAL - 380mata SYARAT KELAYAKAN • Mencapai dan memenuhi mata kelayakan yang ditetapkan bagi 2015/ 2016 Persidangan Antarabangsa Shaklee (atau Umrah) dan/atau Persidangan Nasional • Mata ganjaran hanya akan diberikan kepada para pencapai pangkat Supervisor dan ke atas yang efektif sahaja.
PENGIRAAN MATA GANJARAN • Setiap 300PGUV kecuali PGUV pendaftaran keahlian baru = 1mata • Setiap Pendaftaran Keahlian baru dengan belian 200PUV = 1mata (Mata-mata ganjaran boleh diperolehi dengan menggandakan belian 200PUV, sah bagi setiap pengedar sahaja tidak termasuk Pemimpin Perniagaan ‘breakout’ baru) • Setiap kali Pemimpin Perniagaan Bawahan (downline) anda mencapai pangkat yang lebih tinggi (breakout), anda akan memperolehi 20 Mata ganjaran setiap satu Efektif pada pangkat pencapaian Pangkat Efektif yang lebih rendah dari Pangkat tersebut: Pencapaian:

BORANG PENDAFTARAN Sebaik sahaja anda memenuhi syarat kelayakan Pelancongan bagi mana-mana Insentif Pelancongan 2015/2016, anda perlu mengisi Borang Pendaftaran yang diberikan. Jika suami/isteri anda turut layak menyertai Pelancongan tersebut, sila pastikan beliau telah menjadi Pengedar Berdaftar Shaklee Products Malaysia. (Shaklee) TIDAK BOLEH DIPINDAH MILIK, TIDAK BOLEH DITUKAR GANTI Persidangan Antarabangsa Shaklee (atau UMRAH) dan/atau Persidangan Nasional merupakan pengiktirafan khas kepada Pemimpin-pemimpin Jualan yang cemerlang serta memenuhi syarat kelayakan pelancongan tersebut. Oleh itu, seandainya peserta yang layak itu tidak dapat menyertai mana-mana pelancongan Shaklee tersebut, beliau tidak dibenarkan untuk: • Melantik wakil atau memindahkan mata ganjaran pelancongan tersebut ke tahun berikutnya atau minta sebarang pampasan bagi gantian mata terkumpul. • Merayu untuk kelayakan pro-rata atau merayu untuk kadar belian bagi mereka yang tidak layak menghadiri pelancongan ini. • Merayu untuk kadar belian atau membayar secara pro-rata bagi perbezaan mata ganjaran bagi menghadiri pelancongan- pelancongan yang lain sebagai tambahan bagi pelancongan yang mereka telah layak sertai. Kelayakan hanya dibenarkan untuk dipindahkan kepada suami isteri atau keluarga terdekat (ibu bapa, mertua atau anak yang berumur 18 tahun keatas). Untuk Pemimpin Perniagaan (bujang, belum kahwin) yang telah layak untuk penyertaan keluarga boleh mencalonkan adik-beradik atau ibu bapa untuk menyertai. Tertakluk kepada terma dan syarat. MAKLUMAT PENTING YANG LAIN: • Anak-anak yang menemani – anak–anak yang turut serta menemani ibu bapa mestilah berumur 4 tahun keatas. • Penyertaan Wajib – Peserta dan suami/isteri mesti mengambil bahagian dalam semua aturcara dan aktiviti Pelancongan Shaklee, termasuklah Sesi Perniagaan (jika ada). • Lencongan/Sambungan – Lencongan semasa Pelancongan adalah dilarang sama sekali. Lencongan/Sambungan selepas pelancongan akan dilayan mengikut kes demi kes. •
Visa – Pengedar layak perlu menguruskan permohonan visa. Jika perlu, pihak Shaklee boleh membantu dengan mengesahkan permohonannya dan mendapatkan bantuan daripada ejen pelancongan yang dilantik Shaklee. Namun, pihak Shaklee dan ejen pelancongan yang dilantik tidak bertanggungjawab terhadap sebarang kelewatan atau urusan kelulusan oleh pihak Kedutaan berkenaan.
• Penolakan Keluar/Masuk – Shaklee tidak bertanggungjwab seandainya Jabatan Imigresen Malaysia, atau mana-mana Pihak Berkuasa Kerajaan atau Syarikat Penerbangan, atau cawangan mereka di seberang laut, tidak membenarkan urusan keluar/masuk. • Maklumat Perubatan – Para peserta Persidangan perlu memberitahu pihak Shaklee tentang keadaan kesihatan mereka. Dalam kes masalah kesihatan yang kronik termasuklah kehamilan dan dalam pantang, peserta perlu mendapatkan perakuan daripada doktor atau pakar kesihatan yang mereka sihat untuk melancong. • Jadual Perjalanan – Perubahan jadual perjalanan mungkin berlaku disebabkan keadaan-keadaan yang tidak diduga seperti ketiadaan tempat duduk penerbangan dan penginapan di hotel, keadaan iklim, keadaan politik atau bencana alam. •
Urusan Perjalanan – Persidangan Antarabangsa dan Umrah adalah menerusi KLIA, di mana para peserta akan bertemu di KLIA untuk berlepas ke destinasi Pelancongan/Umrah. Para peserta Insentif Pelancongan digalakkan untuk pulang bersama dalam satu kumpulan. Lencongan atau lanjutan selepas Konvensyen akan dilayan mengikut kes demi kes dan dikenakan caj pentadbiran sebanyak RM150.00 seorang. Sebarang permintaan hendaklah dinyatakan di dalam Borang Pendaftaran Persidangan dan sebarang permintaan pada saat-saat akhir tidak akan dilayan.

Urusan Penginapan – Kesemua peserta Persidangan Shaklee hendaklah tinggal di tempat penginapan yang telah disediakan. Penginapan di hotel adalah berdasarkan aturan bilik terdahulu iaitu secara berkongsi dua orang sebilik. Dalam keadaan tertentu, berkongsi tiga orang sebilik adalah dibenarkan. Anak-anak yang turut serta hendaklah menginap di hotel yang sama dengan ibu bapa mereka. Ibu bapa bertanggungjawab terhadap sebarang kos penginapan tambahan yang tidak disediakan oleh Shaklee.

Insurans dan Indemniti Diri – Persidangan-Persidangan Shaklee diselenggarakan secara pakej meliputi pelbagai aktiviti dalaman dan luaran. Para peserta mesti menguruskan perlindungan insuran untuk diri sendiri, suami/isteri dan anak-anak. Sebelum berangkat, semua peserta dikehendaki mengemukakan surat indemnity (tanggung rugi) yang telah ditandatangani kepada Shaklee.
• Perubahan – Shaklee berhak mengubah peraturan, destinasi, hotel, program dan jadual persidangan sekiranya perlu. Shaklee juga berhak menjemput hanya mereka yang layak dan berkedudukan baik dengan syarikat, tidak melanggar kaedah & peraturan Pelancongan, dan/atau P&R.
