explore health insurance options This year we saw a significant increase in claims experience which resulted in an increase in premiums. Plan changes and the addition of the SimplyBlue (high deductible health plan) with Health Savings Account (HSA) helped to mitigate this increase.

Health Savings Account (HSA) FAQs Who owns the HSA? The member owns the HSA as soon as funds are contributed Are members only allowed to withdraw from their HSA for eligible medical expenses?

Funds may be used for any reason, however only eligible medical expenses are tax-free. Non-qualifed expenses are subject to income tax. If a member uses funds for non-qualified expenses before age 65, they will have to pay an additional penalty tax - 20% of funds used.

Your HSA If you contribute at least $5 per pay to your HSA, Le Moyne will contribute a starting balance of $300 plus an additional $15 per pay

What happens when a member is enrolled in the HSA compatible plan and then changes coverage - whether it’s a different plan or they become eligible for Medicare?

HSA funds will remain in the account and can be used for eligible expenses. However, the account will no longer be permitted to receive contributions.

What forms will members need for tax season?

Members will receive Form 5498-SA that states all contributions for the previous calendar year - including employer and other contributions to the HSA. In addition, each member will receive a 1099-SA that shows the distribution from the HSA account.

Were qualified medical expenses impacted by health care reform?

Yes. Beginning January 1, 2011 you need a physician’s prescription for over-the-counter drugs in order to use funds from your HSA account

Can I use HSA funds for adult children covered on my health plan?

Yes, but only up to age 19 or 24 (for a full-time student). While adult children can be covered on your health plan up to age 26, the IRS definition of a “dependent” still applies to use of the HSA funds.

If you can’t claim a child as a dependent on your tax return, then HSA funds can’t be used on services provided to that child.

According to the IRS definition, a dependent is a qualifying child who: • Has the same principal place of abode as the covered employee for more than 1/2 of the taxable year • Has not provided more than 1/2 of his/her own support during the taxable year • Is not yet 19 (or, if a student, not yet 24) at the end of the tax year or is permanently and totally disabled.

What is covered under preventive care?

Preventive care includes routine physicals, routine GYN exams, mammograms, pap smears, well-child visits, immunizations (including flu shots) and prostate exams. Because these services are covered outside of the deductible, you do not need to meet your deductible before services are covered What do I do when I go to the doctor’s office?

Show them your member ID card and let them know you are using a Health Savings Account (HSA). The physician will bill Excellus BCBS and once the claim is processed an Explanation of Benefits (EOB) will be sent to you and your physician. This will show the expenses that contributed towards the deductible and what expenses you still owe.

Once the physician receives the EOB they will bill you for the balance.

What do I do when I need a prescription?

The pharmacy system processes in real-time so the pharmacist will be able to tell you exactly what you owe at the time of service What happens once I meet the out-of-pocket maximum?

Once you meet the out-of-pocket maximum, which includes medical services and pharmacy expenses, services will be covered in full (for any covered benefit) up to the allowable amount.

A: Yes. You must also meet the required age and points provisions of the retirement plan.

Q: Will I receive a new ID card, even if I don’t change my insurance plan?

A: Yes. All employees enrolled in a health insurance plan will receive new ID cards prior to January 1, 2014.

Q: When can I enroll in the Dental plan?

A: After 6 months of active fulltime employment or any 1st of the month thereafter. If you waive coverage when you are initially eligible (6 months after employment) and chose to enroll at a later date, waiting penalties will apply for certain services.

Q: If I enroll in a flexible spending account and my expenses are less than the amount in my account, what happens to the extra money in my account?

A: You have from January 1 to December 31 to incur expenses that you can submit for reimbursement. Any money left in the account is forfeited.

Q: Are there any changes to the dental plan or premiums for 2014?
