What is a Preferred Provider Network (PPN)? The PPN is the care continuum outside of the acute care setting, including ambulatory. It requires AMITA Health and the post-acute providers work together to supply a seamless, high quality care, low cost transition. It provides discharge collaboration, leverages clinical expertise, provides oversight to improve efficiency, and improves patient outcomes and patient experiences. In addition, the PPN works to decrease unnecessary readmissions, decreases length of stay and applies best practices in clinical care. Choosing a post-acute provider can be hard for many patients and that is why AMITA Health is creating a PPN to help our patients make the choice that is right for them. The AMITA Health PPN focuses on providing a high quality, seamless and cost-effective experience at all levels of care. We are committed to putting our patients first! To do this, we want to partner with likeminded post-acute care providers that will align with our goals and values. The Preferred Provider Network is a select group of post-acute providers from the PAN that have met our criteria. Providers in the PPN are selected with the help of physicians, nurses, social workers, case managers and other quality experts. Each network provider meets and maintains high standards for quality care. The criteria that AMITA Health will be using as a guideline to select these partners are below: Requirements: • • •

Current referral volume AMITA Health affiliation Variety of clinical programs & services

Maintain a minimum of 3 star based on CMS 5-star Rating Appropriate length of stay Improve all cause readmission rate Manage overall cost of care

This is a bigger network where AMITA Health will share data, quality, best practices and education to partners within our geographic area by region. There will be two PAN forums a year. There are three regions for the AMITA Health PPN. Below are their names and the facilities that are within each region: •
